Evidence-based clinical practice guidelines for gastroesophageal reflux disease 2021.

In Japan, with the increasing prevalence of gastroesophageal reflux disease (GERD) and growing public interest, the Japanese Society of Gastroenterology issued Evidence-based Clinical Practice Guidelines for GERD (1st edition) in 2009 and a revised 2nd edition in 2015. A number of studies on GERD were subsequently conducted in Japan and abroad, and vonoprazan, a potassium-competitive acid blocker (P-CAB), became available for the first time in Japan in February 2015. The revised 3rd edition (Japanese edition), which incorporates new findings and information, was published in April 2021. These

Evidence-Based Policy in Practice: Management of Carcinoid Syndrome Diarrhea.

Carcinoid syndrome causes substantial morbidity and reduces quality of life and survival. In a recent clinical trial, 97% of patients reported bowel movement-related issues, abdominal pain, flushing, and low energy. Combining somatostatin analogs with elotristat ethyl provides a new option for managing refractory CS symptoms. Health care providers should consider strategies that take advantage of approved dosing for patients with CS-related diarrhea.

Evidence-based clinical practice guidelines for gastroesophageal reflux disease 2015.

As an increase in gastroesophageal reflux disease (GERD) has been reported in Japan, and public interest in GERD has been increasing, the Japanese Society of Gastroenterology published the Evidence-based Clinical Practice Guidelines for GERD (1st edition) in 2009. Six years have passed since its publication, and there have been a large number of reports in Japan concerning the epidemiology, pathophysiology, treatment, and Barrett's esophagus during this period. By incorporating the contents of these reports, the guidelines were completely revised, and a new edition was published in October 201

Recent developments in family psychoeducation as an evidence-based practice.

Among potential resources for people with serious mental illnesses (SMI) and their families, professionally delivered family psychoeducation (FPE) is designed to engage, inform, and educate family members, so that they can assist the person with SMI in managing their illness. In this article, we review research regarding FPE outcomes and implementation since 2001, updating the previous review in this journal (McFarlane, Dixon, Lukens, & Lucksted, Journal of Marital and Family Therapy 2003; 29, 223). Research on a range of FPE variations continues to return mostly positive effects for adults wi

Clinical decision support chairside tools for evidence-based dental practice.

Evidence-based clinical decision support (EB-CDS) tools designed for chairside use, help support the implementation of Evidence-Based Dentistry. EB-CDS tools organize available evidence and risk factors in order to facilitate clinical decision-making as well as to enhance rapid and effective transfer of knowledge to the patient at the point of care. Gingival recession, root exposure, caries, dental sealants, decay prevention and topical fluoride guides are presented and discussed. The Assess-Advise-Decide Approach, described in this article, better enables patients to determine which course of

The "evidence-based practice inventory": reliability and validity was demonstrated for a novel instrument to identify barriers and facilitators for Evidence Based Practice in health care.

OBJECTIVES: To design and validate a practical questionnaire for clinicians, to identify barriers and facilitators for evidence-based practice (EBP), that is, the use of research evidence in patient care. The inventory is ultimately intended for departments to assess local conditions for EBP, to aim and evaluate efforts at improving or maximizing EBP. STUDY DESIGN AND SETTING: We derived candidate items from existing EBP scales, psychology, and behavioral economics. In an online Delphi study, 537 international expert clinicians, researchers, teachers, and policymakers interested in EBP identi

Use of pimobendan in cats: a practical evidence-based review.

PRACTICAL RELEVANCE: Cardiomyopathies are a common condition and the leading cause of congestive heart failure (CHF) in cats; however, to date there have been limited therapeutic options available. Commonly used therapeutics include diuretics for CHF and antithrombotics 2 to reduce the risk of arterial thromboembolism (ATE), which do not directly affect myocardial function. This review summarizes the evidence for the use of pimobendan, a phosphodiesterase III inhibitor and calcium channel sensitizer, in feline cardiomyopathies. Potential benefits of pimobendan in cats include improved left ven

Dissemination of evidence-based practices for anxiety disorders in Wyoming: a survey of practicing psychotherapists.

Despite the well-established effectiveness of exposure-based cognitive-behavioral therapy (CBT) in the treatment of anxiety disorders, therapists have been slow to adopt CBT into their clinical practice. The present study was conducted to examine the utilization of psychotherapy techniques for anxiety disorders among community practitioners in a rural setting in order to determine the current status of the dissemination of CBT. A sample of 51 licensed psychotherapists from various mental health professions was recruited from online practice listings in the state of Wyoming. Participants comple

A community-based treatment for Native American historical trauma: prospects for evidence-based practice.

Nineteen staff and clients in a Native American healing lodge were interviewed regarding the therapeutic approach used to address the legacy of Native American historical trauma. On the basis of thematic content analysis of interviews, 4 components of healing discourse emerged. First, clients were understood by their counselors to carry pain, leading to adult dysfunction, including substance abuse. Second, counselors believed that such pain must be confessed in order to purge its deleterious influence. Third, the cathartic expression of such pain was said by counselors to inaugurate lifelong h

Integrating technology in leech therapy: Toward standardized evidence-based Ayurvedic practice.

Jalaukavacharana (leech therapy), an integral part of Ayurvedic medicine, is widely recognized for its benefits in managing chronic wounds, inflammatory conditions, and vascular disorders. However, its traditional application often lacks standardized procedures, real-time monitoring, and regulatory oversight, limiting its acceptance in modern clinical practice. While these challenges are notable in the Indian context, including species misidentification and inconsistent protocols, they are also encountered globally, even in countries where leech therapy is medically recognized. This narrative

Symphony of Success: Leader-Practitioner Reciprocity during Evidence-Based Practice Implementation.

This study aimed to explore the reciprocal relationships between implementation leadership and practitioner implementation citizenship behavior during the implementation of evidence-based practices (EBPs). Data were collected at two timepoints with a time lag of six months during a national implementation of evidence-based treatment for post-traumatic stress disorder in Norwegian mental health clinics. Data from 72 leaders and 346 practitioners were analyzed with a two-wave cross-lagged panel model, accounting for the nested structure and adjusting for demographic variables. Significant positi

New tools in wound care to support evidence-based best practice.

Throughout history, various wound healing and management concepts have been recorded, with some approaches such as honey, silver and larvae still in use and others such as blood-letting completely dismissed. In more recent times, dressing products have begun evolving, moving on from basic first-aid supplies to products that support positive healing by addressing the needs of the wound bed and considering underlying factors that impact healing. With an ageing population, the incidence of chronic wounds is predicted to rise, and chronic wounds can negatively impact the lives of patients physical

Evidence-Based Prevention of Preeclampsia: Commonly Asked Questions in Clinical Practice.

In this review, we discuss the recent literature regarding the prevention of preeclampsia and aim to answer common questions that arise in the routine antenatal care of pregnant women. Prescription of low-dose aspirin for high-risk patients has been shown to reduce the risk of preeclampsia (PE). A daily dose between 100 and 150 mg taken in the evening should be initiated prior to 16 weeks of gestation and can be continued until delivery. Calcium supplementation seems to be advantageous but currently it is only considered for patients with poor dietary intake and high risk for PE. Recent data a

Evidence-based practice: evaluation and management of unilateral vocal fold paralysis.

This article discusses the causes and symptoms, evaluation, and management of unilateral vocal fold paralysis (UVFP). Cross-sectional imaging is appropriate in the work-up of idiopathic UVFP, but the routine use of serology is not well supported. The usefulness of laryngeal electromyography has remained controversial. Predictors of poor prognosis for functionally meaningful recovery include fibrillation potentials, positive sharp waves, and absent/reduced voluntary motor unit potentials. Voice therapy may be helpful. Injection and laryngeal framework surgery (medialization thyroplasty) improve

Evidence-based practice implementation and staff emotional exhaustion in children's services.

Understanding the implementation of evidence-based practice (EBP) in community service settings is critical for the successful translation of research to practice. However, we have limited research evidence about the impact of EBP implementation on the mental health and social service workforce. In a previous study we demonstrated reduced staff turnover where an EBP was implemented with fidelity monitoring in the form of supportive ongoing supervision and consultation. Other research has shown that staff burnout and emotional exhaustion in particular is associated with poor quality of care and

Infusing evidence-based practice into interdisciplinary perinatal morbidity and mortality conferences.

Incorporating evidence-based practice into the hospital setting has been a challenge but is needed to deliver quality healthcare. Interdisciplinary morbidity and mortality conferences are used to discuss perinatal and neonatal care issues with high-risk and low-frequency cases, such as fetal demise, maternal death, or identified areas for improvement. By involving an interdisciplinary team to review the patient's case, a more holistic perspective of the patient's care will be achieved. The purpose of this article is to demonstrate how nurses can be an essential part of the interdisciplinary mo
