The enigma of the 'smoker's paradox': Results from a single-center registry of patients with STEMI undergoing primary percutaneous coronary intervention.

BACKGROUND: Smoker's paradox usually refers to the observation of a favorable outcome of smoking patients in acute myocardial infarction. METHODS: From April 2006 to December 2018 a population of 2456 patients with ST segment elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (pPCI) were prospectively enrolled in the MATRIX registry. Ischemic time, clinical, demographics, angiographic data, and 1-year follow-up were collected. RESULTS: Among 2546 patients admitted with STEMI, 1007 (41 %) were current smokers. Smokers were 10 years younger and had

Asthma and smoking-induced airway disease without spirometric COPD.

Due to the high prevalence rates of cigarette smoking and asthma, current and ex-smokers frequently develop chronic airway disease without spirometric evidence of chronic obstructive pulmonary disease (COPD), either alone or associated with asthma. This review considers the classification, clinical outcomes, inflammatory and imaging variables, phenotypes, and management of current and ex-smokers with airway disease without COPD, focusing on overlaps in those with and without asthma. These individuals have more respiratory symptoms, worse quality of life, increased exacerbation rates, reduced l

Obstructive sleep apnoea, cigarette smoking and plasma orexin-A in a sleep clinic cohort.

Orexin-A is a neuropeptide involved in the regulation of food intake and the sleep-wake cycle. This study investigated plasma orexin-A levels in a sleep clinic cohort, adjusting for smoking habits, in 76 participants comprising 41 with obstructive sleep apnoea (OSA) (apnoea-hypopnoea index [AHI] 44.1 +/- 19.1 events/h) and 35 without OSA (AHI 6.3 +/- 4.7 events/h). Plasma orexin-A levels were significantly lower in OSA patients (15.0 +/- 4.6 ng/ml) compared with those without OSA (31.4 +/- 6.5 ng/ml). In non-OSA subjects, there was no significant difference between never smokers and ex/current

Smoking shapes the relationship between metabolic biomarkers and femoral-neck bone mineral density: A stratified analysis by smoking status.

INTRODUCTION: Bone mineral density (BMD) is a crucial indicator of bone strength and osteoporosis risk. Smoking is known to negatively impact bone health and related bone metabolism biomarkers. However, if and how smoking status influences the relationship between bone metabolism biomarkers and BMD, particularly measured at the femoral neck, remains poorly understood. The objective of this study was to examine the relationship between bone metabolism biomarkers and BMD stratified by smoking status. METHODS: Data from 4464 healthy individuals were collected from the Qatar Biobank. The sample w

Cigarette smoking is associated with levels of the serotonin transporter in the brain: a [11C]DASB PET Study.

BACKGROUND: Preclinical work suggests that chronic nicotine/tobacco use is associated with reductions in serotonin within the hippocampus, yet no research has yet shown an association of smoking behaviors and alterations in brain serotonin in humans in vivo. METHODS: We therefore analyzed existing [11C]DASB PET data from the Cimbi Database to compare the availability of the serotonin transporter (SERT) in the hippocampus, midbrain (including the raphe), and neocortex of 60 healthy non-smokers, 15 ex-smokers, and 11 current smokers who also provided blood samples for determination of plasma tr

Functional capacity in smoking patients after coronary artery bypass grafting surgery: a quasi-experimental study.

Coronary artery bypass grafting surgery (CABG) is an important approach to treating coronary heart disease. However, patients undergoing open heart surgery are at risk of postoperative complications. Cigarette smoking is one of the preoperative risk factors that may increase postoperative complications. Studies show that early mobilization intervention may reduce these complications and improve functional capacity, but the impact of smoking on early outcomes after CABG has been controversial for the past two decades. This quasi-experimental study explored the effects of early mobilization on f

Preoperative smoking cessation improves carotid endarterectomy outcomes in asymptomatic carotid stenosis patients.

OBJECTIVE: Smoking cessation has been suggested as having the potential to improve the outcomes of carotid endarterectomy (CEA) and mitigate the risk of long-term stroke in patients with asymptomatic carotid stenosis (ACS). This study aims to compare the perioperative and long-term outcomes of CEA in patients with ACS across different smoking status groups. METHODS: All patients receiving an elective CEA for ACS between 2013 and 2023 were identified in the Vascular Quality Initiative (VQI). Patients with an ipsilateral carotid stenosis <70% and those receiving a concomitant coronary artery by

Cigarette Smoke Exposure and Radiographic Pulmonary Vascular Morphology in the Framingham Heart Study.

Rationale: Cigarette smoke exposure is a risk factor for many lung diseases, and histologic studies suggest that tobacco-related vasoconstriction and vessel loss plays a role in the development of emphysema. However, it remains unclear how tobacco affects the pulmonary vasculature in general populations with a typical range of tobacco exposure, and whether these changes are detectable by radiographic methods. Objectives: To determine whether tobacco exposure in a generally healthy population manifests as lower pulmonary blood vessel volumes and vascular pruning on imaging. Methods: A total of

Increased intracellular T helper 1 proinflammatory cytokine production in peripheral blood, bronchoalveolar lavage and intraepithelial T cells of COPD subjects.

The role of T cells in the pathophysiology of chronic obstructive pulmonary disease (COPD) is not yet certain, although varying reports have shown increases in T helper 1 (Th1) and/or Th2 cytokines in peripheral blood and bronchoalveolar lavage (BAL). No studies have examined cytokine production by intraepithelial T cells obtained by bronchial brushing (BB). Intracellular cytokine analysis of T cell subsets from peripheral blood, BAL and BB from smoker and ex-smoker COPD patients, COPD patients receiving inhaled corticosteroids and smoker and non-smoker control subjects was studied using multi

Association of smoking status, duration and amount with the risk of head and neck cancer subtypes: a national population-based study.

Smoking is positively associated with multiple cancer types including head and neck cancer (HNC). We sought to confirm the effect of smoking in HNC and subtypes through big data analysis. All data used in this study originated from the Korean National Health Insurance Service database. We analyzed subjects who had undergone health check-ups in 2009 with follow-up until 2018 (n=10,585,852). We collected data on smoking and other variables that could affect the risk of HNC. The overall incidence of HNC was highest in current smokers (HR: 1.822, 95% CI: 1.729-1.920), followed by ex-smokers (HR: 1

A DNA Methylation Marker, cg05575921 (AHRR), Outperforms Self-Reported Smoking Exposure for Its Association With Cardiovascular Disease Prevalence.

BACKGROUND: The differential DNA methylation (DNAm) of the CpG site cg05575921, within the gene body of AHRR, has been shown to be a robust indicator of tobacco smoking exposure. This study aimed to determine if cg05575921 at AHRR DNAm had a clinically relevant association with prevalent cardiovascular disease (CVD) and compare its predictive performance with self-reported smoking assessments. METHODS: Whole blood cg05575921 (AHRR) DNAm was assessed using a simple droplet digital PCR assay in a cohort of 2784 elderly individuals. DNAm values were used to impute smoking status and its independ

Differences in clinical features between current smokers and former smokers with OSA: a cross-sectional study.

Smoking is both a cause of obstructive sleep apnea (OSA) and is an important reason for its rising prevalence. However, there is a lack of studies predicting smoking cessation specifically in patients with OSA. This study aimed to identify the factors linked to smoking cessation by examining and comparing the clinical characteristics of current smokers and former smokers with OSA. Eligible adults with a diagnosis of OSA and who were smokers (n = 504) were enrolled in the study. Data on demographics, PSG results, and clinical information were collected. Participants were categorized into curren

Prediction of the physiological dead space/tidal volume ratio during anaesthesia/IPPV from simple pre-operative tests.

It can be shown that the assumption of an arbitrary value for VDphys/VT during IPPV can lead to unacceptable degrees of hypo- or hyperventilation. We investigated 33 adult patients scheduled for major non-thoracic surgery, to see if any simple tests could be used to predict VDphys/VT during anaesthesia/IPPV. Fifteen were smokers and 18 non- or ex-smokers. The tests were spirometry, and the single breath tests for CO2 and for N2 (SBT-CO2: SBT-N2). Patients were ventilated during anaesthesia with a Servo Ventilator 900 B, and SBT-CO2 was recorded from a CO2 Analyzer 930. During anaesthesia/IPPV,

Smoking has disruptive effects on the small bowel luminal microbiome.

Tobacco use is the leading preventable cause of cancer, and affects the respiratory, oral, fecal, and duodenal mucosa-associated microbiota. However, the effects of smoking on the duodenal luminal microbiome have not been studied directly. We aimed to compare the duodenal luminal microbiome in never-smokers, current smokers, and ex-smokers who quit ≥ 10 years ago. In a cross-sectional study, current smokers (CS, n = 24) were identified and matched to never-smokers (NS, n = 27) and ex-smokers (XS, n = 27) by age (± 5 years), body mass index (BMI, ± 3 kg/m2), and sex.
