Regenerative Treatment of Middle Ear Mucosa With Topical Retinoid Administration.

OBJECTIVE: Dysfunction of the middle ear can lead to hearing loss and the development of diseases such as cholesteatoma. Although radical treatment for cholesteatoma involves complete removal of the lesion, incomplete regeneration of the middle ear mucosa may contribute to hearing loss and recurrence of the cholesteatoma. This study aimed to evaluate the regenerative potential of topical retinoid therapy for damaged middle ear mucosa. METHODS: We used a guinea pig model to investigate the regenerative effects of topical retinoids on middle ear mucosal damage. Retinoids, which are known to pro

Novel therapeutic approaches for cutaneous T cell lymphomas.

Introduction: Cutaneous T-cell lymphoma (CTCL) is a rare non-Hodgkin's lymphoma, characterized by malignant T cells infiltrating the skin. CTCL exhibits vast heterogeneity which complicates diagnosis and therapeutic strategies. Current CTCL treatment includes skin-directed therapies (such as topical corticosteroid, topical mechlorethamine, topical bexarotene, ultraviolet phototherapy and localized radiotherapy), total skin electron beam therapy and systemic therapies. Elucidation of molecular and signaling pathways underlying CTCL pathogenesis leads to identification of innovative and personal

Co-delivery of methotrexate and nicotinamide by cerosomes for topical psoriasis treatment with enhanced efficacy.

Psoriasis is an immune-mediated skin disorder that affects populations worldwide. Methotrexate (MTX) is a cytotoxic drug with powerful anti-proliferative and anti-inflammatory effects that has gained prominence in treating inflammatory diseases including psoriasis. However, low solubility and side effects through oral administration hinder its systemic application. In this study, we developed a novel niosomes based on ceramide (cerosomes) to co-deliver MTX and nicotinamide (NIC), i.e., MTX/NIC cerosomes, for topically treating psoriasis with the aim to enhancing the efficacy and reducing the t

[Acerbin for treatment of indurative cellulite in patients with venous trophic ulcers].

Acute indurative cellulite can impair regeneration processes in patients with venous trophic ulcers. Wide range of antimicrobial agents is used for its management, while their role and place remain disputable. The choice of adequate local therapy plays a key role in total success. Conventional topical drugs and anticeptics in most cases appear to be ineffective or, besides antimicrobial action, can inhibit reparative processes. Combined formulation Acerbin exhibits not only antimicrobial properties, but stimulates granulation and epithelization, as well, and resolves inflammation in adjacent t

Discovery of a novel and highly selective JAK3 inhibitor as a potent hair growth promoter.

JAK-STAT signalling pathway inhibitors have emerged as promising therapeutic agents for the treatment of hair loss. Among different JAK isoforms, JAK3 has become an ideal target for drug discovery because it only regulates a narrow spectrum of γc cytokines. Here, we report the discovery of MJ04, a novel and highly selective 3-pyrimidinylazaindole based JAK3 inhibitor, as a potential hair growth promoter with an IC50 of 2.03 nM. During in vivo efficacy assays, topical application of MJ04 on DHT-challenged AGA and athymic nude mice resulted in early onset of hair regrowth. Furthermore, MJ04 sign

Topical rose bengal: pre-clinical evaluation of pharmacokinetics and safety.

BACKGROUND AND OBJECTIVES: Rose bengal (RB) is a potent photosensitizer that has largely been overlooked as a potential photodynamic therapy (PDT) agent. In this study, the feasibility of topical delivery of RB to the epidermis has been evaluated. STUDY DESIGN/MATERIALS AND METHODS: Topical formulations of RB were assessed on murine and rabbit skin for pharmacokinetic properties, cutaneous toxicity, and photosensitization. RESULTS: Hydrophilic formulations (<or=1% RB) exhibited rapid, selective, uniform delivery to the epidermis, with no significant acute cutaneous toxicity in normal skin. I

Topical recombinant human Nerve growth factor (rh-NGF) is neuroprotective to retinal ganglion cells by targeting secondary degeneration.

Optic neuropathy is a major cause of irreversible blindness worldwide, and no effective treatment is currently available. Secondary degeneration is believed to be the major contributor to retinal ganglion cell (RGC) death, the endpoint of optic neuropathy. Partial optic nerve transection (pONT) is an established model of optic neuropathy. Although the mechanisms of primary and secondary degeneration have been delineated in this model, until now how this is influenced by therapy is not well-understood. In this article, we describe a clinically translatable topical, neuroprotective treatment (re

Effects of astaxanthin on dinitrofluorobenzene-induced contact dermatitis in mice.

Astaxanthin (AST) is known to exhibit antioxidative and antitumor properties, therefore, the present study investigated its other potential medical applications. AST was observed to exhibit anti‑allergic and anti‑inflammatory effects in a dinitrofluorobenzene (DNFB)‑induced contact dermatitis (CD) mouse model and RBL‑2H3 cell lines. The topical application of AST effectively inhibited the enlargement of ear thickness and increase in weight, which occurred following repeated application of DNFB. Furthermore, topical application of different concentrations of AST inhibited inflammatory hyperplas

The use of biodegradable microneedle patches to increase penetration of topical steroid for prurigo nodularis.

BACKGROUND: The stratum corneum is an almost impermeable barrier. Recently, microneedles have been used to increase drug delivery passing the stratum corneum by incorporating the drug within the microneedle or by coating the surface of the microneedle with the drug. OBJECTIVE: This study was performed to investigate whether applying a biodegradable microneedle patch after topical steroid application increases penetration of the steroid in vitro, as well as treatment efficacy in patients with prurigo nodularis. MATERIALS & METHODS: In vitro penetration of topical steroids after biodegradable

Anti-inflammatory, analgesic, and anti-immune-mediated polyarthritis activities of topically applied polarity fractions of Clematis florida var. Plena and their underlying mechanisms.

BACKGROUND: Clematis florida var. Plena (C. florida) holds significant potential as a topical treatment for immune-mediated polyarthritis (IMPA); however, its active fractions and mechanisms of action remain poorly understood. Given the chemical complexity of herbal remedies, this study adopted a bioactivity- and mechanism-driven approach, focusing on the effects of polarity-based fractions rather than isolated compounds. METHODS: C. florida was extracted with 70% ethanol and sequentially partitioned into petroleum ether (PE), ethyl acetate (EA), n-butanol (BU), and water (WA) fractions. Phyt

Efinaconazole in Onychomycosis.

Onychomycosis is a fungal nail infection that causes nail discoloration, nail plate thickening, and onycholysis. Efinaconazole 10% topical solution is an FDA-approved treatment for onychomycosis patients aged 6 years and above. The drug functions as an antifungal by disrupting ergosterol synthesis in the fungal cell membrane. It exhibits higher in vitro activity against dermatophytes than other available antifungals such as ciclopirox and itraconazole, and the activity is comparable to amorolfine and terbinafine. Efinaconazole also provides enhanced nail penetration compared with other topical

Antigastrolesive, gastric antisecretory, diarrheagenic and mucus-stimulating effects in rats following topically applied rioprostil, a synthetic prostaglandin E1 analog.

Prostaglandins may have many biological actions including hypotensive and antipeptic ulcer activity. The purpose of this investigation was to determine if the primary alcohol prostaglandin E1 analog rioprostil1 prevents ethanol-induced gastric lesions (antigastrolesive activity), inhibits gastric acid secretion (antisecretory activity), or causes diarrhea in rats when administered topically, and to compare these responses to the effect of rioprostil following enteral (oral or intraduodenal) administration. Rioprostil exhibited antigastrolesive activity in rats when administered either orally o

Current treatments for atopic dermatitis in Japan.

The goal of a treatment regimen for atopic dermatitis is to reach and maintain a state where the patient exhibits mild symptoms or an absence of symptoms, and the patient should not experience disturbance during daily activities. The basis of a treatment regimen for atopic dermatitis is topical therapy, and currently there exist topical corticosteroids, tacrolimus and delgocitinib. Using these, proactive therapy is performed as maintenance therapy after remission induction therapy. However, in cases of moderate to severe atopic dermatitis, topical drugs alone cannot induce remission and system

Host defense peptides for use as potential therapeutics.

The ongoing emergence of bacterial strains resistant to multiple classes of antimicrobial agents continually exerts pressure on researchers to develop novel molecules and novel approaches to combat these bacterial pathogens. One such approach is the use of topical agents to prevent or treat infections of exposed and accessible tissue. These surfaces are primarily protected in humans by innate immunity, and a significant part of this system consists of the activities of host-defense peptides. Recently, these molecules have been demonstrated to exhibit a wide range of defense-targeted activities

A Rationally Designed Multifunctional Antibiotic for the Treatment of Drug-Resistant Acne.

Acne is a multifactorial skin disease, underpinned by colonization of Propionibacterium acnes and inflammation. The emergence of resistant P. acnes strains has affected the current acne treatment algorithm. This setback served as an impetus for rationally designing a library of next-generation antibiotics that exhibit a bactericidal effect on resistant P. acnes and exert an immunomodulatory function to reduce inflammation. In silico screening showed that one of the molecules, VCD-004, exhibits improved mode of binding to bacterial DNA gyrase. VCD-004 shows high potency against clinical isolate

Clindamycin: A Comprehensive Status Report with Emphasis on Use in Dermatology.

Clindamycin is a lincosamide antibiotic that has been used as a topical, oral, or injectable formulation for over five decades. It exhibits a narrow spectrum of microbiologic activity, primarily against gram-positive and anaerobic bacteria. In dermatology, clindamycin has been used primarily as a topical agent, usually for the treatment of acne vulgaris. Despite questions surrounding antibiotic resistance and/or its relative contribution to antibiotic treatment efficacy, a large body of data support the therapeutic value of topical clindamycin for acne vulgaris. As a systemic agent, clindamyci
