The value of the occlusion test in decompensating exophorias and intermittent exotropias.

The importance of the occlusion test is analysed in patients who were operated for a decompensating exophoria or an intermittent exotropia. Of particular interest before and after the occlusion test was the number of the following variables: the distant angle, the near angle, the difference between these two (the so-called distant angle-near angle difference), and the angle at right respective left gaze. All patients included in this study had been examined at least three times preoperatively. The occlusion test was performed in all patients by occluding one eye on three consecutive days. A to

Divergence excess and basic exotropia types of intermittent exotropia: a major review. Part 1: prevalence, classification, risk factors, natural history and clinical characteristics.

INTRODUCTION: Intermittent exotropia (IXT) is a common form of strabismus. It is an outward deviation of one eye typically when viewing at distance. Symptoms include, but are not limited to double vision, eyes feeling tired, excessive blinking, and reduced quality of life. Its clinical characteristics are distinctive from other types of strabismus. This paper provides a comprehensive review of prevalence, classification, risk factors, natural history and clinical characteristics of the divergence excess and basic exotropia types of IXT. METHODS: Search strategies involving combination of keyw

[Paying attention to problems and countermeasures in intermittent exotropia surgery].

Intermittent exotropia (IXT) is the most common type of strabismus, with surgical interventions standing as its main therapeutic modality. In recent years, with the rapid development of the strabismus and pediatric ophthalmology subspecialties, surgical correction for IXT has become a routine practice across numerous institutions in China. However, the surgical success rate is not high and tends to decline with longer follow-up periods. In this article, we highlight key misconceptions and challenges in the surgical management of IXT, with focuses on optimizing the timing of surgical interventi

Visual and auditory attention defects in children with intermittent exotropia.

BACKGROUND: Previous studies have shown that children with intermittent exotropia (IXT) have a higher rate of psychiatric abnormalities as they grow up, such as attention deficit. This study explored visual and hearing attention among children with IXT, and evaluated its association with clinical characteristics and cognitive development. METHODS: Forty-nine children with a diagnosis of IXT and 29 children with traditional development were recruited. The Integrated Visual and Auditory Continuous Performance Test (IVA-CPT) was used to measure the subjects' full-scale response control quotient

Exotropia in a pediatric patient with rhabdomyolysis caused by an insect sting.

Various ocular and systemic reactions have been associated with insect sting. However, insect stings have been rarely reported to cause exotropia and diplopia. We encountered exotropia in a 6-year-old child with rhabdomyolysis of the left lower extremities caused by an insect sting. Exotropia and diplopia developed within 1 day after the sting and improved completely 1 week after the onset of symptoms. Clinicians should be aware of the potential for the development of exotropia in patients with insect stings, which requires careful follow-up.

Strabismus surgery for internuclear ophthalmoplegia with exotropia in multiple sclerosis.

BACKGROUND: Internuclear ophthalmoplegia (INO) is a disabling condition affecting up to 40% of patients with multiple sclerosis (MS). Management of bilateral internuclear ophthalmoplegia (BINO) with exotropia in MS has been controversial because of the uncertain course of MS. Diplopia associated with INO severely impacts the patient's quality of life and, therefore, treatment should be considered. METHODS: Three patients (ages 28, 62, and 82 years) who had BINO with exotropia and disabling diplopia secondary to MS underwent bilateral medial rectus resections with either unilateral or bilatera

A comparison of hang-back with conventional recession surgery for exotropia.

BACKGROUND: The hang-back loop suspension surgical technique for rectus muscle recession offers potential advantages over the conventional rectus muscle recession, including better exposure at the site of scleral sutures, shorter procedure duration, and lower risk of scleral perforation. Previous reports suggest that the hang-back technique for lateral rectus recession for exotropia results in poorer surgical success and may require a different surgical dosage. METHODS: We compared strabismus in a nonrandomized series of 55 children with exotropia treated with conventional surgery or hang-bac

Surgical outcomes of intermittent exotropia as a function of strabismic angle.

PURPOSE: To analyze postoperative results of intermittent exotropia as a function of the difference in strabismic angles measured immediately and another time prior to the surgery. METHODS: We reviewed the clinical records of intermittent exotropia patients who received surgery and had differences greater than or equal to 10 prism diopters (PD) between the last preoperative measurement of strabismic angle and another previous measurement. After applying various exclusion criteria, 66 patients were entered into our study. At the last follow-up visit after surgery, we divided postoperative resu

[A study of eight patients with non-paralytic pontine exotropia].

We reported 8 patients with non-paralytic pontine exotropia (NPPE): 7 men and 1 woman aged from 56 to 79 years (mean: 67.8 years). From 1991 to 1994, 402 patients with acute infratentrial cerebrovascular diseases other than subarachnoid hemorrhage were admitted to our hospital within 7 days after the ouset of their stroke. NPPE was observed in 2% of these patients. NPPE was noted on admission in 6 of them, while it developed during the hospital stay in 2 others with paralytic pontine exotropia and bilateral median longitudinal fasciculus (MLF) syndrome, respectively. Augmented lateral rectus muscle recession for treatment of infantile exotropia.

BACKGROUND: The surgical outcome is usually not satisfactory for treatment of primary infantile exotropia (PIE) with large exodeviation angels of more than 50 prism diopters (PD). Here, we evaluate the effectiveness of augmented bilateral lateral rectus muscle recession (ABLRR) in treatment of PIE with large deviation angles. METHODS: A retrospective analysis was performed for 25 patients with PIE who underwent ABLRR. ABLRR was performed using a hemi-hang-back technique. Ocular alignment, ocular motility and binocular vision were evaluated pre- and postoperatively. RESULTS: Fourteen males an

Anomalous extraocular muscles in Crouzon syndrome with V-pattern exotropia.

Strabismus associated with Crouzon syndrome is common and often complex. V-pattern strabismus is most commonly reported in this condition and is mainly thought to be due to an excyclorotation of the orbits and rectus muscle pulleys. We report two cases of children with Crouzon syndrome and V-pattern exotropia who had rectus muscle heterotopy on orbital imaging and were also found intraoperatively to have anomalous extraocular muscles. At the time of surgery, bifid insertion, misdirection, and fibrosis of extraocular muscles were noted. This highlights the various causes of V-pattern strabismus

Characteristics of divergence excess type intermittent exotropia in Asian children.

BACKGROUND: Exotropia is twice as common as esotropia in Asian children, with divergence excess intermittent exotropia contributing more than one half of exotropia cases. In this study, distance-near relationships and ratios of accommodative convergence to accommodation (AC/A) are compared using different measurement methods in Asian children with divergence excess intermittent exotropia. METHODS: Children with intermittent exotropia and a distance deviation exceeding the near by at least 10(Δ) were consecutively recruited. After prism cover test measurements at 6 m and 33 cm, AC/A ratios wer
