Metabolic Dysregulation, Systemic Inflammation, and Pediatric Obesity-related Asthma.

Obesity-related asthma is a distinct pediatric asthma phenotype. It is associated with higher disease burden, lower pulmonary function, and suboptimal response to current asthma medications. Recent studies have made inroads into elucidating its pathophysiology. Systemic immune responses in obese children with asthma are skewed to a nonatopic T-helper cell type 1 (Th1) pattern that correlates with pulmonary function deficits. The prevalence of metabolic dysregulation is also higher among obese children with asthma than among normal-weight children with asthma. Insulin resistance and dyslipidemi

Impact of a multidisciplinary intervention on resting and exercise respiratory functions in adolescents with obesity.

BACKGROUND: Respiratory function (RF), body composition (BC), and cardiorespiratory fitness (CRF) links remain underexplored during multidisciplinary weight loss interventions (MWLI) in adolescents with obesity. OBJECTIVES: To investigate the impact of a 16-week MWLI on resting and exercise RFs and their associations with BC and CRF. METHODS: RFs were measured in 92 adolescents (13.1 ± 1.19 years, 58%female, BMI 34.31 ± 4.93 kg/m²) by spirometry and at several intensities during cardiopulmonary exercise tests (CPET) before MWLI (T0) and after (T1). BC and CRF were measured by dual X-ray abso

Perioperative Management of One-Lung Ventilation in an Adolescent With Metastatic Osteosarcoma Complicated by Bilateral Pneumothoraces.

Thoracic anesthesia in pediatric patients is challenging due to anatomical and physiological factors that increase susceptibility to hypoxemia during one-lung ventilation (OLV). These complexities are further magnified in children with underlying oncological disease and poor pulmonary reserve. We report anesthetic management of a 16-year-old, 32 kg boy with metastatic osteosarcoma presenting with recurrent bilateral pneumothoraces scheduled for right video-assisted thoracoscopic surgery (VATS) and pleurodesis. The coexistence of bilateral pneumothoraces with compromised pulmonary reserve, with

Hypercapnia in Advanced Chronic Obstructive Pulmonary Disease: A Secondary Analysis of the National Emphysema Treatment Trial.

RATIONALE: Hypercapnia develops in one third of patients with advanced chronic obstructive pulmonary disease (COPD) and is associated with increased morbidity and mortality. Multiple factors in COPD are thought to contribute to the development of hypercapnia including increased carbon dioxide (CO2) production, increased dead space ventilation, and the complex interactions of deranged respiratory system mechanics, inspiratory muscle overload and the ventilatory control center in the brainstem. However, these factors have not previously been systematically analyzed in a large, well-characterized

Lung function change and quantitative computed tomography measurements are associated with prospective frailty.

BACKGROUND: Frailty, a state of reduced physiologic reserve, is associated with comorbid lung disease. Its relationship with lung function change over time and with lung computed tomography (CT) measurements is less clear. METHODS: COPDGene, a multicentre cohort study, enrolled adults aged 45-80 years with a smoking history of a minimum of 10 pack-years. This study included participants with normal baseline 6-min walk distance (6MWD), spirometry at all three site visits and frailty assessment. The primary exposures were baseline quantitative CT measurements (airway wall thickening (Pi10), log

Upright versus supine positioning for breast and lung radiotherapy: a pilot study on patient comfort, experience and lung function.

Delivering radiotherapy in upright body positions (UP) may offer advantages for breast and lung cancer treatments due to physiological/anatomical differences compared to the supine position (SP), as well as potential cost savings when establishing new treatment rooms. This pilot study compared (i) patient experiences and (ii) lung function parameters for upright UP versus SP, among a cohort of patients undergoing conventional (supine) radiotherapy for breast or lung cancer. After ethical approval, 8 breast and 7 lung cancer patients were enrolled in this study. Five research appointments were

Increased propensity for apnea in response to acute elevations in left atrial pressure during sleep in the dog.

Periodic breathing is commonly observed in chronic heart failure (CHF) when pulmonary capillary wedge pressure is abnormally high and there is usually concomitant tachypneic hyperventilation. We hypothesized that acute pulmonary hypertension at pressures encountered in CHF and involving all of the lungs and pulmonary vessels would predispose to apnea/unstable breathing during sleep. We tested this in a chronically instrumented, unanesthetized dog model during non-rapid eye movement (NREM) sleep. Pulmonary hypertension was created by partial occlusion of the left atrium by means of an implanted

Predictors of sleep-disordered breathing and chronic hypoventilation in obese women and men: a cross-sectional observational study.

BACKGROUND AND AIM: Obesity is associated with an increased risk of sleep-disordered breathing (SDB)-obstructive sleep apnoea (OSA) and sleep hypoventilation-and obesity hypoventilation syndrome (OHS). The aim was to assess the impact of obesity on lung volumes and the prevalence of SDB and OHS and to identify predictors of these. METHODS: In a cross-sectional analysis, obese patients (body mass index ≥30 kg/m2) who underwent an in-laboratory sleep study, arterial blood gas analysis and pulmonary function tests between 2018 and 2023 were included. Analysis of variance and multivariate regress

Association of Airflow Obstruction and Left Atrial Conduit Strain in Patients with Known or Suspected Coronary Artery Disease.

PURPOSE: Airflow obstruction (AO) and coronary artery disease (CAD) share smoking as a risk factor. They may be associated with alterations in ventricular and atrial volumes and functions, and global coronary flow reserve (CFR), all of which can be assessed by cardiovascular MR (CMR). The purpose of this study was to evaluate the association between AO severity and CMR parameters in patients with known or suspected CAD. METHODS: Eighty patients with known or suspected CAD who underwent CMR, including cine, stress and rest perfusion, stress and rest coronary sinus flow measurements by phase-co

Effects of individualized PEEP obtained by two different titration methods on postoperative atelectasis in obese patients: study protocol for a randomized controlled trial.

BACKGROUND: The incidence of postoperative pulmonary complications (PPCs) is higher in obese patients undergoing general anesthesia and mechanical ventilation due to the reduction of oxygen reserve, functional residual capacity, and lung compliance. Individualized positive end-expiratory pressure (iPEEP) along with other lung-protective strategies is effective in alleviating postoperative atelectasis. Here, we compared the best static lung compliance (Cstat) titration of iPEEP with electrical impedance tomography (EIT) titration to observe their effects on postoperative atelectasis in obese pa

Prospective study of long-term pulmonary manifestations of mantle irradiation.

Given the high cure rate of patients with Hodgkin's disease, the complications related to therapy take on great significance. Mantle irradiation to the thorax is used in virtually all patients with early stage Hodgkin's disease. Prior studies of patients receiving mantle irradiation demonstrated short-term (up to 24 months) abnormalities of pulmonary function. In the present study, we prospectively studied 13 patients for up to 60 months after irradiation only with serial pulmonary function tests, arterial blood gas tests, diffusing capacity of carbon monoxide, chest radiographs, and ventilati

Surgical treatment for lung cancer patients with poor pulmonary function.

UNLABELLED: This retrospective study was aimed to review the risk factors of postoperative hospital death in lung cancer patients with poor pulmonary reserves. We performed surgery on 30 lung cancer patients (average age: 71 years) with less than 1.0L of preoperative forced expiratory volume in one second (FEV1.0 < 1.0L) between 1982 and 2003. The preoperative FEV1.0 of these 30 patients was 0.81+/-0.1L on average. Six of 20 patients who underwent surgery between 1982 and 1997 died during their postoperative hospital stay (hospital death group). Between 1998 and 2003, 10 patients underwent sur

Investigation of dynamic hyperinflation and its relationship with exercise capacity in children with bronchiectasis.

BACKGROUND AND AIM: Dynamic hyperinflation (DH) is a major contributor to exercise intolerance in patients with obstructive lung diseases. However, it has not been investigated in children with bronchiectasis (BE). We aimed to investigate dynamic ventilatory responses and their influence on functional exercise capacity in children with BE. METHODS: Forty children with BE (mean forced expiratory volume in 1 s [FEV1 ] = 78 ± 19%pred) were included. Six-minute walk test (6MWT) was conducted using Spiropalm 6MWT® for evaluating dynamic ventilatory responses including inspiratory capacity (IC), mi

Ventilatory response to CO2 in patients with snoring, obstructive hypopnoea and obstructive apnoea.

Obstructive sleep apnoea (OSA) is caused by an obstruction of the upper airway. Sufficient sensitivity to CO2 in the respiratory centre is known to be a critical factor for adequate tone in the upper airway muscles. The hypothesis of this study is, therefore, that the ventilatory response to CO2 is reduced in patients with OSA. Twenty-six patients who suffered from snoring, 19 snoring patients with obstructive hypopnoea (OH) and 33 snoring patients with obstructive apnoea (OA), were studied. The control group consisted of 25 subjects from a random sample with no history of snoring or daytime s

Connective tissue disease-associated interstitial lung disease: immune profiling and correlation of immune cells with pulmonary function in SSc-ILD.

BACKGROUND: Connective tissue diseases (CTDs) frequently involve the lungs, with CTD-associated interstitial lung disease (CTD-ILD) representing a leading cause of morbidity and mortality, yet the immunopathological mechanisms remain incompletely defined. This study characterized immune profiles in CTD-ILD and their association with pulmonary function impairment, focusing on systemic sclerosis-associated ILD (SSc-ILD). METHODS: This single-center retrospective study enrolled 217 CTD-ILD patients, 100 healthy controls, and 25 idiopathic pulmonary fibrosis (IPF) patients. Peripheral lymphocyte

Non-Invasive Ventilation Reduces Postoperative Respiratory Failure in Patients Undergoing Bariatric Surgery: A Retrospective Analysis.

Background and Objectives: Postoperative non-invasive ventilation (NIV) has been proposed as an attractive strategy to reduce morbidity in obese subjects undergoing general anaesthesia. The increased body mass index (BMI) correlates with loss of perioperative functional residual capacity, expiratory reserve volume, and total lung capacity. The aim of the current study is to evaluate the efficacy of NIV in a post-anaesthesia care unit (PACU) in reducing post-extubation acute respiratory failure (ARF) after biliointestinal bypass (BIBP) in obese patients. Materials and Methods: A retrospective a
