The aim of the study was to investigate the prognostic role of extranodal extension (ENE) of lymph node metastasis in adenocarcinoma of the pancreas (PDAC) and papilla [cancer of the papilla of Vater (CPV)]. A PubMed and SCOPUS search from database inception until 5 January 2015 without language restrictions was conducted. Eligible were prospective studies reporting data on prognostic parameters in individuals with PDAC and/or CPV, comparing participants with the presence of ENE (ENE+) with those with intranodal extension (ENE-). Data were summarized using risk ratios for number of deaths/recu

Background: Extranodal extension (ENE) is a critical prognostic factor in head and neck squamous cell carcinoma (HNSCC) and was incorporated into the AJCC eighth-edition staging system. However, the concordance between clinical (cENE) and pathological (pENE) ENE remains poorly understood in real-world practice. Methods: We conducted a retrospective analysis using Taiwan Cancer Registry (TCR) long-form data from 2018 to 2022, focusing on four major HNSCC sites (oral cavity, oropharynx, hypopharynx, and larynx). The diagnostic gap was defined as the difference between pENE and cENE positivity ra

BACKGROUND: Lung adenocarcinoma (LUAD) prognosis is strongly influenced by lymph node (LN) status. While extranodal extension (ENE) is a recognized negative prognostic factor, the International Association for the Study of Lung Cancer (IASLC) has proposed reclassifying ENE from R0 (complete resection) to R1 (incomplete resection). The prognostic significance of intranodal metastasis (INM) remains less defined. This study aims to compare the prognosis of LUAD patients with INM versus ENE to inform potential refinements to the R classification system.
METHODS: This retrospective study enrolled

BACKGROUND AND PURPOSE: This multicenter retrospective study aimed to investigated the prognostic value of unequivocal radiologic extranodal extension (rENE) and the efficacy of chemotherapy for stage T1-2 N1 nasopharyngeal carcinoma (NPC) in the IMRT era.
MATERIALS AND METHODS: We included 1,082 patients treated in 2005-2017 from three centers. rENE was recorded as G1 (coalescent nodal mass comprising ≥ 2 inseparable nodes) or G2 (invading beyond perinodal fat to frankly infiltrate adjacent structures). Multivariable analysis (MVA) evaluated the prognostic value of rENE. The value of chemoth

The necessity of routine carotid sheath removal during neck dissections for head and neck carcinomas, particularly in clinically N0 and N+ necks without extranodal extension (ENE), remains debatable. This prospective study aimed to document the incidence of pathological involvement of the carotid sheath in such cases. A total of 121 patients with oral squamous cell carcinoma underwent neck dissections with the carotid sheath left intact unless infiltration was suspected. Among these, only one patient (0.8%) exhibited carotid sheath involvement, while 120 (99.2%) did not. No regional recurrence

Primary thyroid gland lymphomas are uncommon tumours that occur in the setting of lymphocytic thyroiditis or Hashimoto's disease in almost all cases. Rarely these tumours show extensive plasmacytic differentiation. In these conditions, a distinction between an inflammatory thyroid infiltrate and extramedullary plasmacytoma may be extremely difficult and precise criteria and ancillary techniques are necessary for a correct diagnosis. The authors report a case of mucosa associated lymphoid tissue (MALT) lymphoma of the thyroid gland in a 69-year-old Iranian man with Hashimoto's thyroiditis which

BACKGROUND: Extranodal extension (ENE) of lymph node metastasis is one of the most reliable prognostic indicators for patients with locally advanced oral cancer. Although multiple reports have found a close relationship between immune infiltration of tumors and patient clinical outcomes, its association with ENE is unknown.
METHODS: We identified 234 human papillomavirus-negative (HPV-) oral cavity squamous cell carcinoma (OSCC) patients in The Cancer Genome Atlas and investigated the immune infiltration profiles of primary tumors and their association with survival.
RESULTS: Hierarchical cl

BACKGROUND: Breast cancer related lymphoedema (BCRL) occurs in a substantial proportion of breast cancer survivors and is a major contributor to patients' disability. Regrettably, there are no validated predictive biomarkers, diagnostic tools, and strong evidence-supported therapeutic strategies for BCRL. Here, we provide an integrative characterization of a large series of women with node-positive breast cancers and identify new bona fide predictors of BCRL occurrence.
METHODS: Three hundred thirty-two cases of surgically-treated node-positive breast cancers were retrospectively collected (2

OBJECTIVE: This study aimed to develop the Radiomics-Assembled ENE system (RAIEm), a multimodal preoperative computed tomography (CT) radiomics model, for identifying extranodal extension (ENE) in lung adenocarcinoma (LUAD) and to examine its utility in risk stratification and individualized treatment planning.
METHODS: We retrospectively assembled preoperative thin-slice chest CT scans and postoperative hematoxylin-eosin-stained histology from patients with LUAD. Tumors were segmented automatically using a 3D U-Net with virtual adversarial training. Radiomic features were derived from the in

BACKGROUND AND AIMS: The transmanubrial approach (TMA) provides excellent exposure of the cervical-thoracic border area, allowing for the resection of thoracic outlet structures while preserving the clavicle, avoiding muscle sacrifice, and maintaining full shoulder mobility.
METHODS: This report presents the first instance of TMA being utilized for oligometastatic, extranodal extension-positive lymph node dissection en bloc with partial resection of the common carotid artery and trachea following esophagectomy.

OBJECTIVE: Suggest and validate pre-surgical tomographic evaluation criteria for OCC based on the pathological DOI criteria.
METHODS: A retrospective cohort study was conducted with 80 patients with tongue and floor of mouth tumors, T1 and T2, followed from 2009 to 2015. Patients were initially classified according to the 7th edition of the TNM (TNM-7) using pathological samples and then reclassified based on the updated 8th edition of the TNM (TNM-8) criteria. The same was done radiologically, using extrapolation and adaptation of the pathological criteria to tomographic evaluation. The resu

PURPOSE: Extranodal extension (ENE) increases the risk of recurrence and death in head and neck squamous cell carcinoma (HNSCC) patients and is an indication for treatment escalation. Histopathology forms the mainstay of diagnosing ENE. There is substantial variation in the diagnosis of ENE and related terminology. Harmonising the diagnostic criteria for ENE was identified as a priority by the Head and Neck Consensus Language for Ease of Reproducibility (HN CLEAR) Steering Committee and its global stakeholders.
METHODS: An international working group including 16 head and neck pathologists fr

BACKGROUND: Prognosis of locally advanced squamous cell carcinoma of the head and neck (LA-HNSCC) is poor, particularly in cases with extranodal extension. The Head and Neck Cancer International Group (HNCIG) recently published criteria for imaging-detected extranodal extension (iENE). Here, we aimed to clarify the significance of iENE in induction chemotherapy followed by chemoradiotherapy.
METHODS: We retrospectively investigated patients with LA-HNSCC scheduled to receive induction chemotherapy followed by chemoradiotherapy between 2013 and 2022 at the National Cancer Center Hospital East,

PURPOSE: Radioactive iodine therapy (RAI) ≥100 mCi (3.7 GBq) is commonly recommended for papillary thyroid cancer (PTC) patients with extranodal extension (ENE). However, no study has evaluated whether RAI ≥100 mCi is effective in reducing the recurrence of PTC with ENE.
METHODS: This retrospective cohort study enrolled 191 PTC patients with ENE who underwent total thyroidectomy and RAI. Recurrence according to RAI activity (<100 vs. ≥100 mCi) was compared before and after propensity score matching (PSM) (1:2) using Kaplan-Meier curves and Cox proportional hazards regression models. BACKGROUND: Multiple factors contribute to recurrences in differentiated thyroid cancers (DTC). Though the nodal size and number of positive nodes along with the presence of extranodal extension (ENE) have been mentioned in the present ATA risk stratification, the weightage given for ENE seems inadequate compared to the former two.
METHODOLOGY: Factors predicting recurrences were analysed in this retrospective study of patients with DTC operated in a tertiary care centre. Based on our findings, we propose a modification in the present risk stratification. We have done so by comparing with exi

INTRODUCTION: Diffuse large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma, with extranodal involvement in up to 40% of cases. However, simultaneous involvement of multiple extranodal sites without dominant nodal disease remains very rare and diagnostically challenging.
CASE PRESENTATION: We report a case of a 61-year-old female with a history of hypertension and recurrent anemia, who presented with progressive swelling in the left breast and parotid area, and systemic symptoms. Evaluation with imaging and biopsy confirmed multifocal extranodal DLBCL involving the bil
