[Facial trauma and injury in children].

Facial traumas are common in children but often unconsidered. Facial injury is responsible of impressive bleeding because of the rich vascularization of the face; this bleeding is often underestimated because of the immediate arterial vasoconstriction that is very strong for children. The blood volume is 80ml/kg for a newborn, with a total of 250ml, reaching 70ml/kg at one year of age. The evaluation must be rigorously performed due to the risk of a sudden decompensation. Regarding the wounds, the primary repair must be performed directly neat or optimal in case of damaged tissues. The rule is

Intracranial aerocele in facial injury.

Aspects of intracranial aerocele are described and exemplified by two cases which presented to the Plastic Surgery Unit. Austin Hospital, Melbourne, as radiological findings incidental to facial injury. The first case illustrated the occasional problem of identifying the site of dural tear; a fracture could not be identified on X-ray film or at operation, and this can make subsequent dural repair more difficult. The second case was associated with florid clinical signs of facial fractures which were supported by X-ray findings. Both patients underwent surgery for the primary injury, and receiv

Iatrogenic facial nerve injury: the role of facial nerve monitoring.

Intraoperative facial nerve monitoring has emerged as a powerful tool for facilitation of surgery involving the facial nerve. This tool must be properly applied and maintained, however, in order to avoid untoward results and to maximize its potential benefits. Facial nerve monitoring is best suited for prevention of iatrogenic injury. Once injury has become established, its value may be severely limited, especially for moderate to severe injury, in which visual assessment of injury is still the most effective means to determine the need for repair. The most valuable derivative of facial nerve

Facial dog bite injuries in children: treatment and outcome assessment.

Dog bite injuries to a child's face are not an infrequent occurrence. They may require primary and revisional surgery. All result in permanent facial scars. This report describes the treatment and outcomes of dog bites of the face, scalp, and neck based on a case series of 107 children over a 10-year period. The average children's age was 5.9 years. In cases where the dog was identified (95%), it was known to the victim and their family. The events leading to the dog bite were categorized as provoked (77%) in the majority of the cases. The majority of wounds could be closed primarily without a s

Facial injuries in Scotland 2001-2009: epidemiological and sociodemographic determinants.

To our knowledge, sociodemographic determinants of facial injuries in children have not previously been reported in Scotland. We analysed the pattern, time trends, and key sociodemographic determinants of facial injuries in children and adolescents using Scottish morbidity records for inpatient care (2001-2009) which were retrieved from the Information Services Division of National Health Services (NHS) Scotland. We calculated annual incidences by age, sex, health board, Scottish Index of Multiple Deprivation (SIMD), and mechanism of injury. A Poisson regression analysis model was used to inco

Is tracheostomy suitable for securing airway after facial firearm injuries?

BACKGROUND: Head and neck region have many vital structures, and facial firearm injuries (FFIs) more likely lead to life-threatening situations than other body injuies. These injuries have high potential of airway compromise associated with significant morbidity and mortality. METHODS: We describe an 11-year-old boy who had received tracheostomy after a FFI complicated with pneumothorax and subcutaneous emphysema 8 hours after the procedure. The patient was treated at the Department of Emergency and Critical Care, Gazi University School of Medicine, Turkey. RESULTS: The patient was discharge

Facial trauma: a case of potentially life-threatening distracting injuries.

We report the case of a 60-year-old cyclist with multiple facial lacerations. Reassessment of the mechanism of injury and presenting clinical features of a change in voice, odynophagia, and a graze in the anterior neck triggered concern. Fibreoptic nasoendoscopy showed a large haematoma of the supraglottic airway. Scans confirmed laryngeal injuries, which were managed conservatively as an inpatient without prophylactic tracheostomy. This report emphasises the importance of a high index of suspicion in patients with such facial injuries and other subtle signs, and highlights the need for carefu

Facial fractures and related injuries: a ten-year retrospective analysis.

A retrospective analysis of 828 patients with significant midface or mandibular fractures was undertaken to illustrate the multisystem nature of traumatic injuries associated with fracture of the facial skeleton, covering the period from 1985 to 1994. Special emphasis was placed on determining associated injuries sustained as well as epidemiological information. The experience presented differs from other large series in the literature in that the predominant mechanism of injury is motor vehicle accidents (67%) rather than assaults. Of the patients reviewed, 89% sustained significant associate

Unusual dental injuries following facial fractures: report of three cases.

We report 3 cases of unusual dental injuries following facial fractures. The first patient sustained intrusion of a maxillary incisor into the nasal cavity following a mandibular fracture. The tooth dislocated into the pharynx and was found lodged in the piriform fossa during surgery. The second patient sustained intrusion of molars into the maxillary sinus following maxillary and mandibular fractures. His treatment was delayed due to life-threatening hemorrhage. The third case involved ingestion of multiple avulsed teeth into the alimentary tract following severe maxillofacial fractures. Alth

Are head, neck and facial injuries markers of domestic violence?

Few data exist regarding the relationship between the location of injuries and the presence of domestic violence. This study of 127 people at an inner-city hospital emergency department found that most patients had head, neck and facial injuries. Although only 23 percent of patients with such injuries were victims of domestic violence, 94.4 percent of victims of domestic violence had head, neck and facial injuries. The study results indicate that head, neck and facial injuries could be markers of domestic violence.

[Iatrogenic injuries of the facial nerve in the mastoid region].

During the past ten years nine patients were referred to our hospital with lesions of the VIIth nerve. These lesions were inflicted during operation on account of chronic otitis media. One patient recovered after conservative treatment. Based on the EMG examination and clinical picture, eight patients were operated. In one who had an extensive lesion it did not prove possible to find the proximal stump and the patient improved after Normann-Dott's operation. In the remaining seven subjects also severe damage was involved: five times complete severing of the nerve occurred, twice partial severi

Head and facial injuries after low-speed motor-cycle accidents.

In the United Kingdom there appears to be little available information concerning the prevalence and the patterns of head and of facial injuries sustained in road traffic accidents by motorcyclists wearing protective helmets. The present preliminary survey was of 56 accidents in urban traffic. 50 per cent of surviving motor-cyclists incurred head injuries. Soft tissue facial wounds were sustained by 38 per cent and fractures of facial bone and teeth by 11 per cent of the injured. The majority of the accidents occurred at speeds of 30 m.p.h. or less to riders of machines of small cubic capacity
