Intralipid emulsion treatment as an antidote in lipophilic drug intoxications.

Intravenous lipid emulsion (ILE) is a lifesaving treatment of lipophilic drug intoxications. Not only does ILE have demonstrable efficacy as an antidote to local anesthetic toxicity, it is also effective in lipophilic drug intoxications. Our case series involved 10 patients with ingestion of different types of lipophilic drugs. Intravenous lipid emulsion treatment improved Glasgow Coma Scale or blood pressure and pulse rate or both according to the drug type. Complications were observed in 2 patients (minimal change pancreatitis and probable ILE treatment-related fat infiltration in lungs). In

Bridging the Gap in the Clinical Application of Advanced Parenteral Nutrition Formulations: From Efficacy to Real-World Challenges.

Parenteral nutrition (PN) represents a critical therapeutic approach for patients unable to meet nutritional needs through oral or enteral routes. In recent years, the development of advanced PN formulations has accelerated, driven by improvements in lipid emulsion design, compound amino acid and micronutrient preparations, and multichamber bag (MCB) technologies. These innovations not only enhance safety, stability, and metabolic efficiency but also expand the capacity of PN to address increasingly complex clinical scenarios. Novel lipid emulsions derived from mixed-oil systems, structured tr

Metabolic fuel selection in rainbow trout: coping with intralipid infusion.

The impact of hyperlipidemia on fuel selection has never been investigated in fish. This study quantifies how intralipid administration affects i) in vivo mobilization of lipids (lipolytic rate: Ra glycerol) and carbohydrates (hepatic glucose production: Ra glucose) in rainbow trout and ii) key proteins involved in the regulation of fuel metabolism that could explain changes in glycerol and glucose kinetics. Results show that intralipid triples lipolytic rate (from 2.5 ± 0.5 to 7.8 ± 1.1 µmol glycerol kg-1·min-1) and inhibits glucose production by 36% (from 7.3 ± 0.9 to 4.7 ± 0.4 µmol kg-1·min

Depletion and enrichment of phytosterols in soybean oil lipid emulsions directly associate with serum markers of cholestasis in preterm parenteral nutrition-fed pigs.

BACKGROUND: Clinical reports show a positive correlation between phytosterol concentrations and severity of cholestatic liver disease markers in infants during long-term administration of parenteral lipid emulsions. Establishing a causal link between phytosterols and cholestasis has been complicated by confounding factors of lipid emulsion load, fatty acid composition, and vitamin E in many of these studies. The goal of this study is to determine whether altering the phytosterol concentration within a common soybean oil-based emulsion will alter the onset and severity of cholestasis in parente

Lipid Emulsion, More Than Reversing Bupivacaine Cardiotoxicity: Potential Organ Protection.

Efforts to develop a treatment for bupivacaine cardiotoxicity led to the discovery that Intralipid, a popular brand of intravenous lipid emulsion, could be used not only as an effective treatment for anesthetic-induced cardiac arrest, but also as a means of reversing many other toxicities. Contradictory data exist regarding the mechanism of action of lipid emulsion, a combination of fatty acids traditionally used in parenteral nutrition. Some researchers attribute the effects to lipophilicity and the individual characteristics of the lipids, while other data demonstrate a direct empowering mec

Ibrutinib-Induced Ventricular Electrical Storm Successfully Managed with Veno-Arterial ECMO and Intralipid Administration: A Rare Case Report.

We report a 55-year-old men patient with a primitive central nervous system non-Hodgkin lymphoma B cell (LNH PNSLC), treated with chemotherapy rituximab, methotrexate, and ibrutinib (first treatment) who developed a refractory ventricular arrhythmic storm two hours after the ibrutinib intake. Indeed, ibrutinib could be associated with severe and occasionally fatal cardiac events. The swift emergence of a ventricular electrical storm with cardiac arrest demanded the prompt initiation of veno-arterial extracorporeal membrane oxygenation to effectively navigate this critically ill patient toward

Diazepam adsorption to infusion sets and plastic syringes.

The adsorption of diazepam to infusion sets and plastic syringes was studied. Infusion solutions consisting of diazepam injection (Valium) in glucose 5.5%, or diazepam emulsion in a lipid emulsion (Intralipid 10%) were infused through two different infusion sets (Transcodan L-74 and Cutter IL). It was found that, when an infusion solution with a low diazepam concentration (0.04 mg/ml) was infused slowly (4 ml/h), the diazepam adsorption was more than 80%. At a higher diazepam concentration (0.1 mg/ml) and increased infusion rate (20ml/h) the adsorption decreased. Diazepam injection in glucose

Intravenous lipid emulsion therapy in 2 dogs and 2 cats with vitamin D toxicosis.

Vitamin D toxicosis can lead to severe and prolonged hypercalcemia resulting in multi-organ damage and even death. Current treatment often involves prolonged hospitalization and may require medications with potential for adverse effects. The objective of this case series was to describe reductions in serum ionized calcium concentrations following intravenous lipid emulsion therapy in vitamin D toxicosis. Two dogs and 2 cats with vitamin D toxicosis were treated with intravenous lipid emulsion therapy in addition to standard treatment regimens. Ionized hypercalcemia was lower following intraven

Accidental acute lidocaine toxicity in 6 pet rabbits from multiple institutions and treatment with intravenous lipid emulsion.

OBJECTIVE: To describe the signs, treatments, and outcomes of rabbits with lidocaine toxicoses after receiving accidental overdose with IV lidocaine. ANIMALS: 6 client-owned rabbits with lidocaine toxicosis following accidental overdoses of 2% lidocaine administered intravenously as a constant rate infusion (> 100 µg/kg/min) or bolus (> 2 mg/kg) were identified through medical record searches at 4 institutions, covering cases from July 18, 1974, to September 15, 2024, depending on each institution's available records. Rabbits were excluded if they received lower doses or did not subsequently

Critically ill patients and fat emulsions.

In critically ill patients, lipid tolerance may be impaired. In ICU, patients are usually given TPN including fat emulsions. If sedation with propofol is given simultaneously the fat load increases and may exceed the maximal clearing capacity of exogenous fat emulsion unless the administration of the parenteral fat is adjusted to take account of the lipid contained in propofol. An accumulation of fat may result in the so-called 'fat overload syndrome'. Close monitoring of serum triglyceride concentration is therefore recommended to avoid fat overload.

A historical perspective and review of the safety of lipid emulsion in pregnancy.

Total parenteral nutrition for pregnant patients is essential to maintain or restore optimal nutritional status for both the mother and fetus when adequate protein and caloric intake by the oral or enteral route is not feasible. This report reviews the safety of intravenous lipid emulsions in total parenteral nutrition programs for pregnant patients. Controversy regarding the adverse effects of intravenous lipid emulsion when administered to pregnant patients is based on early reports of the use of cottonseed oil lipid emulsions. The adverse effects reported included infarction of the placenta

Physical Compatibility of Propofol-Sufentanil Mixtures.

BACKGROUND: Combined infusions of propofol and sufentanil preparations are frequently used in clinical practice to induce anesthesia and analgesia. However, the stability of propofol emulsions can be affected by dilution with another preparation, sometimes leading to particle coalescence and enlargement. Such unwanted effects can lead to fat embolism syndrome after intravenous application. This study describes the physical stability of 5 commercially available propofol preparations mixed with sufentanil citrate solutions. METHODS: Two common markers of emulsion stability were used in this stu

Intrathecal bupivacaine and morphine toxicity leading to transient hypotension and delayed status epilepticus.

BACKGROUND: Local anesthetic systemic toxicity characteristically occurs after inadvertent intravascular injection of local anesthetics; however, it is unclear if similar symptoms arise after intrathecal adminstration. Intrathecal use of local anesthetics for chronic pain is increasing and carries a potential risk of toxicity. Experience with the presenting symptoms and appropriate treatment for intrathecal local anesthetic toxicity is limited. CASE STUDY: A 74-year-old woman with an intrathecal bupivacaine/morphine pump developed lower extremity sensory neuropathy followed by obtundation, hy

A nanometer lipid emulsion, lipid nano-sphere (LNS), as a parenteral drug carrier for passive drug targeting.

We attempted to develop an artificial lipoprotein-like particle, lipid nano-sphere (LNS), incorporating dexamethasone palmitate (DMP). LNS is 25-50 nm in diameter and is composed of soybean oil and egg lecithin. Potential drug carriers were compared with a conventional fat emulsion for intravenous nutrition, lipid microsphere (LM, d=200-300 nm), which is already used clinically. LM easily entered reticuloendothelial systems, such as the liver, and was rapidly cleared from the circulation. However, LNS showed much higher plasma levels of DMP after intravenous administration to rats and recovere

Hazards of parenteral treatment: do particles count?

After prolonged parenteral nutrition a 12 month old infant died with pulmonary hypertension and granulomatous pulmonary arteritis. A review of necropsy findings in 41 infants who had been fed parenterally showed that two of these also had pulmonary artery granulomata, while none of 32 control patients who died from sudden infant death syndrome had similar findings. Particulate contaminants have been implicated in the pathogenesis of such lesions and these were quantified in amino acid/dextrose solutions and fat emulsions using automated particle counting and optical microscope counting respect

Use of Mixed-Oil Fat Emulsion to Improve Intestinal Failure-Associated Liver Disease in Long-Term Home Parenteral Nutrition: A Case Report.

Home parenteral nutrition (HPN) is a life-saving therapy for patients who are not able to use their gastrointestinal tract. There are a number of complications associated with HPN, including metabolic bone disease, intestinal failure-associated liver disease (IFALD), and catheter-related bloodstream infections. We present a case of a 32-year-old HPN patient who initially developed biopsy-proven IFALD (total bilirubin, 2.4 mg/dL) while on long-term HPN. His HPN was initiated due to myopathic intestinal dysmotility and pseudo-obstruction when he was 15 years old. Because of his IFALD, the intrav

Propofol emulsification in Intralipid and SMOFlipid: A promising alternative in response to future shortages.

INTRODUCTION: The Covid-19 pandemic has intensified shortages in various pharmaceutical products, notably injectable propofol in lipid emulsion form. Its demand surged sharply due to its critical role in intubating patients with respiratory distress during the pandemic, exposing vulnerabilities in the supply chain for this essential product. OBJECTIVES: This project aims to develop an alternative formulation to commercially available propofol products and to evaluate its stability through a detailed study. METHODS: Two lipid emulsions commonly used for intravenous nutrition, Intralipid 20% a
