Decision model to evaluate the cost of clinical events associated with switching from apixaban to rivaroxaban among patients with non-valvular atrial fibrillation in the United States and Germany.

AIMS: Direct-acting oral anticoagulants (DOACs) have emerged as the preferred treatment for nonvalvular atrial fibrillation (NVAF). However, evidence concerning the economic outcomes of DOAC switching remains limited. This study aimed to assess the economic outcomes of DOAC switching in the US and Germany, two countries with a high AF prevalence and DOAC utilization. METHODS: A decision model was developed to assess the incidence and cost of stroke/systemic embolism (SE) and major bleeding (MB) associated with switching from apixaban to rivaroxaban in patients with NVAF. The model compared tw

Free but declining: the paradox of contraceptive uptake in Burkina Faso.

INTRODUCTION: In 2020, Burkina Faso expanded its national user fee exemption policy to include family planning services, with the objective of reducing financial barriers and increasing uptake of modern contraceptive methods. This study assesses the national and subnational trends in modern contraceptive uptake before and after the user fee exemption policy amongst women of reproductive age (WRA) in Burkina Faso. METHODS: To achieve this objective, we collected routine health facility and demographic data spanning the pre-intervention period (2016-2018) and the post-intervention period (2021-

Severity of depression in prepaid and fee-for-service general medical and mental health specialty practices.

This study compares severity of depression for patients of general medical clinicians, psychiatrists, and nonphysician therapists receiving prepaid or fee-for-service care. Cross-sectional severity comparisons were conducted among 715 outpatients with current major depression or dysthymia, by independent assessment. Severity was assessed by counts of current and lifetime depressive symptoms, prognostic and treatment response indicators, and global measures of psychological and physical sickness. Patients of psychiatrists were the most psychologically ill, patients of nonphysician therapists we

Utilization and access to metered-dose inhalers and sustainable alternatives among older adults in the United States: a population-based study.

BACKGROUND: Attaining the Paris Agreement goal of limiting global temperature rise to below 1.5-2 °C requires the healthcare sector to achieve net-zero emissions by 2050. This necessitates adopting diverse strategies, including replacing carbon-intensive metered-dose inhalers (MDIs) with more environmentally sustainable dry powder inhalers (DPIs) and soft mist inhalers (SMIs). While several European countries have successfully made this transition, patterns of MDI use and barriers to adopting sustainable alternatives in the United States remain poorly understood. Thus, we assessed inhaler util

Hospital at Home services: An inventory of fee-for-service payments to inform Medicare reimbursement.

BACKGROUND: Hospital at Home (HaH) is a growing model of care with proven patient benefits. However, for the types of services required to provide an episode of HaH, full Medicare reimbursement is traditionally paid only if care is provided in inpatient facilities. DESIGN: This project identifies HaH services that could be reimbursable under Medicare to inform episodic care within fee-for-service (FFS) Medicare. SETTING: All data are derived from acute services provided from the Mount Sinai HaH program between 2014 and 2017 as part of a Center for Medicare and Medicaid Innovation (CMMI) demo

Declining Utilization Patterns of Percutaneous Adhesiolysis Procedures in the Fee-For-Service (FFS) Medicare Population.

BACKGROUND: Percutaneous epidural adhesiolysis is a minimally invasive therapeutic modality used in the treatment of patients with chronic low back and lower extremity pain, often recalcitrant to other modalities including epidural injections and surgical interventions. While the initial utilization since its introduction and development of appropriate Current Procedural Terminology (CPT) codes increased up until 2008, but since 2009, there has been a significant decline in utilization of these procedures in the Medicare population. These procedures declined by 53.2% at an annual rate of 10.3%

Is Percutaneous Adhesiolysis Effective in Managing Chronic Low Back and Lower Extremity Pain in Post-surgery Syndrome: a Systematic Review and Meta-analysis.

PURPOSE OF REVIEW: The growing prevalence of spinal pain in the USA continues to produce substantial economic impact and strain on health-related quality of life. Percutaneous adhesiolysis is utilized for recalcitrant, resistant conditions involving spinal pain when epidural injections have failed to provide adequate improvement, especially low back and lower extremity pain, specifically in post-lumbar surgery syndrome. Despite multiple publications and systematic reviews, the debate continues in reference to effectiveness, safety, appropriate utilization, and medical necessity of percutaneous

Spillover Effects of Medicare Advantage on Traditional Medicare Beneficiaries With Prostate Cancer.

INTRODUCTION: Medicare Advantage (MA) managed care plans, now chosen by 51% of Medicare beneficiaries, are incentivized to constrain healthcare spending and utilization, a shift in financial incentives compared to Traditional Medicare's fee-for-service payment model. Beyond its primary beneficiaries, MA's mechanisms to constrain utilization may impact Traditional Medicare beneficiaries with prostate cancer through "spillover" effects on physician behavior. METHODS: From a 20% sample of Medicare claims, we identified patients diagnosed with prostate cancer from 2016 to 2019. We calculated MA p

Differences in the Measurement of Comorbidities Based on ICD-10-CM Coding Definitions Using Medicare Advantage Encounter Data and Fee-For-Service Claims.

BACKGROUND: Medicare Advantage (MA) encounter data became available for research in 2019; data quality concerns remain. OBJECTIVES: We evaluated the consistency of ICD-10-CM comorbidity coding between MA and Fee-For-Service (FFS) data. METHODS: We used round 7 (2017) of the National Health and Aging Trends Study (NHATS) linked to Medicare enrollment, MA encounter, and FFS claims (2016-2017). We included participants continuously enrolled in MA or FFS for 1 year before round 7. Comorbidities were identified using ICD-10-CM codes from the Gagne combined comorbidity index. Insurers' Acquisition Of Ambulatory Surgery Centers Was Not Associated With Changes In Performance, 2013-21.

Vertical integration between insurers and providers is increasingly common in US health care markets, yet there is little evidence on its impacts. This study examined the association between UnitedHealthcare's acquisition of ambulatory surgery centers (ASCs) and performance and pricing for arthroscopy and colonoscopy procedures. Using 100 percent Medicare fee-for-service claims data from the period 2013-21, we applied a difference-in-differences approach to assess changes in ASC size, procedure volume, and quality of care, as well as patient mix. We analyzed 2023 data from the Transparency in

Updated Analysis of Decline of 16.8% in Utilization of Interventional Pain Management Techniques Among Traditional (Fee-for-Service) Medicare Beneficiaries from 2019 to 2024.

BACKGROUND: In recent years, rising costs associated with managing spinal pain and other musculoskeletal disorders have been well documented. Prior to the COVID-19 pandemic, the use of interventional techniques to manage spinal pain and other musculoskeletal disorders had steadily increased. However, the pandemic disrupted chronic pain management, including interventional procedures and opioid use, reflecting a broader reduction in healthcare services. OBJECTIVES: To provide an updated assessment of interventional technique utilization for chronic pain management in the U.S. Medicare populati

End-of-Life Care for Older Adults With Blood Cancers With Medicare Advantage Versus Medicare Fee-For-Service Insurance.

PURPOSE: Adults with hematologic malignancies (HMs) often experience suboptimal end-of-life (EOL) care, with patients from minoritized racial/ethnic groups at even greater risk. It is unclear whether these disparities are partly driven by modifiable factors such as insurance. METHODS: Using the Centers for Medicare and Medicaid Services database, we compared the quality of EOL care as defined by hospice use, high-intensity health care utilization, and advance care planning between Medicare advantage (MA) and Medicare fee-for service (FFS) insurance among patients with HM 66 years and older wh

Health System Expansion and Changes in Medicare Beneficiary Utilization of Safety Net Providers.

BACKGROUND: Evidence is limited on insured patients' use of safety net providers as vertically integrated health systems spread throughout the United States. OBJECTIVES: To examine whether market-level health system penetration is associated with: (1) switches in Medicare beneficiaries' usual source of primary care from federally qualified health centers (FQHCs) to health systems; and (2) FQHCs' overall Medicare patient and visit volume. RESEARCH DESIGN: Beneficiary-level discrete-time survival analysis and market-level linear regression analysis using Medicare fee-for-service claims data fr

Association Between a Bundled Payment Program for Lower Extremity Joint Replacement and Patient Outcomes Among Medicare Advantage Beneficiaries.

IMPORTANCE: Much of the evidence for bundled payments has been drawn from models in the traditional Medicare program. Although private insurers are increasingly offering bundled payment programs, it is not known whether they are associated with changes in episode spending and quality. OBJECTIVE: To evaluate whether a voluntary bundled payment program offered by a national Medicare Advantage insurer was associated with changes in episode spending or quality of care for beneficiaries receiving lower extremity joint replacement (LEJR) surgery. DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional

Billed advance care planning visits may be occurring among older adults with high risk of mortality.

CONTEXT: Advance care planning (ACP) is a process that helps people prepare to make decisions about their future medical care. OBJECTIVES: We sought to understand who was received billed ACP visits and measure the association with health care utilization, cost, and mortality. METHODS: We used a randomly sampled 20 % cohort of Medicare fee-for-service (FFS) beneficiaries' files to conduct a retrospective cohort study. Beneficiaries with a billed ACP visit were matched to controls using a 2-stage propensity score matching process that included assigning a pseudo-ACP visit date for controls. Ou

The opportunity for health plans to improve quality and reduce costs by embracing primary care medical homes.

BACKGROUND: The large and growing costs of healthcare will continue to burden all payers in the nation's healthcare system-not only the states that are struggling to meet Medicaid costs and the federal government, but also the private health plans that serve commercial, Medicare Advantage, and Medicaid beneficiaries. Cost will increasingly become a concern as millions more people become newly insured as a result of the Patient Protection and Affordable Care Act (ACA). Primary care delivery through patient-centered medical homes (PCMHs) and other coordinated-care models have improved care and r

Opioid Analgesics in Georgia Medicaid: Trends in Potential Inappropriate Prescribing Practices by Demographic Characteristics, 2009-2014.

BACKGROUND: There has been a dramatic rise in the number of opioid prescriptions and opioid overdose deaths in the United States over the past 15 years. Misuse and abuse of opioids is also a growing public health concern in the United States. Medicaid enrollees are more likely to be prescribed opioids and are at higher risk of prescription drug overdose compared with non-Medicaid populations. Despite rising opioid drug overdose deaths in Georgia, prevalence of indicators for potential inappropriate prescribing practices has not been examined to date. OBJECTIVE: To examine trends in the genera
