Since closed treatment is effective in 80% to 85% of distal femoral fractures, operative intervention is not recommended. Unnecessary surgery produces unwarranted complications. The cast-brace technique should be used in the first week or two after injury to prevent deformities commonly associated with distal femoral fractures. The thigh cast should be carefully molded well over the femoral condyles. Distal femoral traction is preferred to proximal tibial traction to correct fracture deformities. If the closed reduction technique is not satisfactory in the early period after injury, operative

Over a 20-year period, 24 patients were treated for ipsilateral hip and femoral shaft fractures. The majority of the hip fractures were extracapsular. All resulted from high-energy impaction injuries (motor vehicle accidents and falls from over 15 feet), with 25% being open. There were numerous associated injuries, many requiring multiple operations. One-third had an associated knee injury, with 75% occurring in those with extracapsular fractures. The diagnosis and treatment of three hip fractures were delayed, but all healed without avascular necrosis. Patients were treated by traction and va

A total of 110 fractures of the neck of femur were studied in 108 patients 17 to 50 years old. The aims were to analyze why these fractures occur in young adults, and what results can be expected after internal fixation. Severe trauma caused about half of the fractures. In the remaining group chronic and disabling disorders were usually present at the time of injury. Seventeen patients were known to abuse alcohol. Aseptic necrosis with late segmental collapse occurred in 41 per cent of the hips (45/110), comparatively more often in patients who were 45 years or younger. Only 16 of the 73 patie

The records of 65 head injured adults with 68 concomitant femoral fractures were reviewed. All patients were victims of high energy accidents. Forty-three fractures were treated by nonoperative methods. Treatment of 30 fractures with traction alone or with hip spica casts resulted in greater than 10 degrees of anterior angulation in 12 fractures (40%), greater than 5 degrees of valgus angulation in 13 fractures (43%), and shortening of greater than 2 cm in 13 fractures (43%). of eight fractures treated with a cast brace, two had shortening of 2.5 cm. Cast braces offered better fracture alignme

Thirty-eight displaced intracapsular fractures of the femoral neck treated by the Deyerle method of internal fixation have been added to the previously reported series of sixty-three cases from the Walter Reed Army Medical Center Orthopaedic Service. A significant change in the incidence of non-union was found. In the over-all series of 101 patients, the incidence of non-union was 13.8 per cent and the incidence of segmental collapse due to avascular necrosis was 13.8 per cent. The roentgenographic and functional over-all end results were rated excellent in 67.7 per cent and poor in 32.3 per c

The femoral cast-brace offers a new method of treatment for femoral shaft fractures. The cast-brace is especially well suited to fractures below the isthmus, although it can be used to treat those more proximal by giving special attention to detail. The method allows early knee motion without prolonged bed rest. Patients who should not be operated on for any medical reason or whose fracture is not amenable to surgery may be treated with the cast-brace. A series of patients with simple and complicated problems that have been treated with the cast-brace is presented. Comparisons between femoral

OBJECTIVE: A basicervical femoral neck fracture, which is located at the junction between the femoral neck and intertrochanteric region, is a rare type of fracture. The treatment effects for this type of fracture vary. The present retrospective study was performed to evaluate the clinical and radiological outcomes of proximal femoral nail antirotation and illustrate its effect on improving the clinical prognosis of basicervical femoral neck fractures.
METHODS: Fourteen patients with two-part basicervical fractures underwent treatment with proximal femoral nail antirotation.
RESULTS: The trea

OBJECTIVE: To investigate the clinical results of the proximal femoral nail antirotation (PFNA) system in the treatment of unstable intertrochanteric femoral fractures.
METHODS: From September 2006 to September 2009, 90 patients (40 males and 50 females, ranged in age from 64 to 95 years with an average of 73.2 years with unstable intertrochanteric femoral fractures were surgically treated with PFNA. Fifty patients had the fractures in the right hip, and 40 patients had the fractures in the left hip. The fractures were classified according to the AO classification: 11 patients were type A2.1,

Treatment of reverse oblique trochanteric femoral fractures is still challenging. We present the results of our proximal nailing surgery performed for reverse obliquity intertrochanteric fractures using two lag screws and evaluated the quality of the reduction, operative time, complications and functional status of the patients. Fifteen patients with AO/OTA 31 A-A3 fractures were treated by proximal femoral nailing in our trauma centre. The mean Harris hip score was 74.66 (range 65-96) and the mean Barthel activity score was 15.71 (range 12-20). The mean duration of surgery was 48 minutes and

BACKGROUND: Multifocal fractures of the femur are uncommon injuries and present unique management challenges. Combined ipsilateral fractures of the femoral shaft and femoral neck or the femoral shaft and distal femur have been described. The combination of noncontiguous ipsilateral femoral neck, femoral shaft, and distal femoral articular injuries, however, has not been described. The purposes of this report are to document the incidence and characteristics of this injury and to present a treatment rationale.
METHODS: This was a retrospective study conducted at a Level I trauma center.

OBJECTIVES: To validate a novel intraoperative method of quantifying femoral head perfusion in adult patients with femoral neck fractures and to determine whether the lack of a perfusion waveform correlates with the development of osteonecrosis, nonunion, or reoperation.
DESIGN: Prospective cohort.
SETTING: Level 1 trauma center.
PATIENTS/PARTICIPANTS: Nineteen patients with 20 acute femoral neck fractures treated with hip-preserving surgical fixation. All patients underwent intraoperative quantification of femoral head perfusion.
INTERVENTION: Intraoperative quantification of femoral head

Treatment of femoral neck fractures remains a significant clinical challenge. The choice between femoral head replacement and internal fixation must take into account the patient's age, activity level, degree of osteoporosis, and medical condition. If open reduction and internal fixation is chosen, an understanding of the vascular pathophysiology of femoral neck fractures is helpful to insure the best clinical result. Osteonecrosis of the femoral head and non-union of the femoral neck fracture are the two most common complications following fixation of a femoral neck fracture. The vascular dam

Sixteen patients with femoral neck fractures were studied with roentgen-stereophotogrammetric analysis (RSA) and low-field magnetic resonance (MR) imaging in addition to plain roentgenography. In six patients, these results were compared with the results of histopathologic analyses. All fractures were stabilized with two cannulated titanium screws. Evaluation of fracture movement before weight bearing (nine fractures) revealed no or only slight movement (less than 3.4 mm or 4.7 degrees). During weight bearing, two undisplaced fractures were compressed about 5 mm and one 20.3 mm, because of del

The purpose of this study was to investigate the efficacy of femoral nerve blocks in patients with fractures of the neck of the femur. Thirty-six patients with femoral neck fractures attending the accident department over a three month period received femoral blocks from one of the two authors. The effect of the block was assessed both subjectively and objectively. Thirty patients reported a worthwhile reduction in pain following the procedure and this was supported by objective blockade in 29. In all six patients who reported no benefit, the block failed objectively. There were no complicatio

Atypical femoral fractures (AFFs) occurring during the course of osteoporosis treatment usually lead to discontinuation of anti-resorptive (AR) drugs. However, the risk of fracture after an AFF is unknown. We conducted a follow-up study of patients with AFF matched 1:3 for age and gender with patients with a peripheral major osteoporotic fracture (pMOF), in the setting of a fracture liaison service, to investigate the incidence of subsequent low-trauma fractures. Fifty-five patients with AFF (95% women, age [mean ± standard deviation] 75 ± 10 years, 89% exposed to AR drugs), followed for 6.2 ±

Four patients with femoral neck fracture following healed intertrochanteric fracture were evaluated retrospectively. This situation is a rare occurrence with a current literature review documenting only 15 cases. Patient charts and radiographs were retrospectively reviewed to evaluate the period from initial injury to definitive treatment for the femoral neck fracture. Emphasis was placed on associated risk factors and operative techniques. In case 1, the femoral neck fracture appeared to be clearly a traumatic fracture as it occurred 11 years after the intertrochanteric fracture. In cases 2,

BACKGROUND: Fractures through the proximal femur are broadly grouped into intertrochanteric fractures and intracapsular fractures. It is not clear why a patient may sustain an intertrochanteric fracture as compared with an intracapsular fracture. There is an established relationship between relative hip retroversion and the development of osteoarthritis. We postulate retroversion also may be a risk factor for having intracapsular fractures develop.
QUESTIONS/PURPOSES: We looked specifically at the geometry of the hip to analyze the possibility of a relationship between acetabular version, fem

There are no reports in the literature about femoral fractures in adults with congenital femoral deficiency. We report on a femoral fracture in a 60-year-old patient with congenital femoral deficiency. Despite early difficulties in mobilisation and delayed union after an angle-stable plate osteosynthesis, full weight bearing was reached after 8 months. The treatment of fractures in combination with congenital femoral deficiency remains difficult and depends on the extent of the defect, the complexity of the fracture, and associated anomalies. The objective of the often demanding operative trea
