Non-traumatic Atypical Peri-implant Femoral Fracture at the Distal Screw after Short Femoral Nail Fixation for a Pertrochanteric Fracture.

This is the second report of an atypical peri-implant femoral fracture occurring at the distal screw after intramedullary nail fixation of a pertrochanteric fracture. A 94-year-old Japanese female with a 5-year history of alendronate intake presented with prodromal pain in her right thigh after intramedullary nail fixation. Plain radiographs showed an incomplete noncomminuted transverse fracture at the distal screw, suggesting an atypical peri-implant femoral fracture. The pathogenesis of an atypical peri-implant femoral fracture could be a combined systemic bone metabolism disorder and repeti

Distal femoral fractures: A comparison between single lateral plate fixation and a combined femoral nail and plate fixation.

OBJECTIVES: Distal femoral fractures occur in patients with osteoporotic bone and also as a consequence of high energy trauma. The recognised treatment options include closed or open reduction of the fracture and fixation using a locking plate or a femoral nail. Both these fixation modalities have some drawbacks. There is a risk of metalwork failure with single lateral locking plates and limited distal fixation with intramedullary nails. Since January 2018, we started using augmented fixation of distal femoral fractures using a combination of a retrograde femoral nail and a lateral locking pla

A qualitative study of patients' experience of recovery after a distal femoral fracture.

PURPOSE: This qualitative study was conducted as part of a feasibility study for TrAFFix, (ISRCTN92089567), a randomised controlled trial that will compare two surgical interventions used to fix distal femoral fractures. Our aim was to understand patients' experiences of treatment and the early phase of recovery after a distal femoral fracture. While, much is known about the experience of recovery from hip fracture, little is known about whether patients with other lower limb fragility fractures experience the same concerns and challenges. MATERIALS AND METHODS: Semi-structured interviews wer

The effect of percutaneous pin fixation in the treatment of distal femoral physeal fractures.

Distal femoral physeal fractures in children have a high incidence of physeal arrest, occurring in a mean of 40% of cases. The underlying nature of the distal femoral physis may be the primary cause, but other factors have been postulated to contribute to the formation of a physeal bar. The purpose of this study was to assess the significance of contributing factors to physeal bar formation, in particular the use of percutaneous pins across the physis. We reviewed 55 patients with a median age of ten years (3 to 13), who had sustained displaced distal femoral physeal fractures. Most (40 of 55)

Distal femoral replacement in periprosthetic fracture around total knee arthroplasty.

INTRODUCTION: Total knee arthroplasty is a common orthopaedic procedure in the UK; consequently, revision surgery and periprosthetic fractures are increasing in incidence. Strategies for management of these cases include non-operative strategies, internal plate fixation and revision of the distal femoral component. One under-reported practice is to perform distal femoral replacement in cases with poor distal femoral bone stock. MATERIALS AND METHODS: The department's electronic database was searched for all patients undergoing revision of total knee arthroplasty. From these, all patients havi

Trifocal femur fracture with intracapsular femoral neck, open diaphyseal, and distal complete articular fractures.

We describe a trifocal femur injury with intracapsular femoral neck fracture, diaphyseal fracture with bone loss, and distal complete articular (AO/OTA C type) fracture, an injury rarely described in the literature. Surgical management utilized a not-yet-reported implant combination: screw-side plate device for the intracapsular femoral neck, retrograde nail for the diaphysis, and lag screws plus mini fragment buttress plating for the distal fracture. The patient had uneventful fracture union with no changes in alignment. Given the rarity and complexity of this injury, there is little consensu

Interprosthetic femoral fractures managed with modern distal femoral locking plates: 10 years' experience at a UK major trauma centre.

INTRODUCTION: Interprosthetic femoral fractures (IFF) are rare but increasing with an ageing population. Operative management is challenging and there is currently a paucity of outcome data in literature. The purpose of this study was to evaluate outcomes of IFFs managed with modern distal femoral locking plates, in a larger sample size than previously published. METHOD: This retrospective study reviewed 49 closed IFFs in 48 patients at a major trauma center from 2009 to 2019 occurring between previous total hip arthroplasty (n=38), hemi hip arthroplasty (n=3), dynamic hip screw (n=6) or ceph

Open Reduction vs Distal Femoral Replacement Arthroplasty for Comminuted Distal Femur Fractures in the Patients 70 Years and Older.

BACKGROUND: The ideal management of distal femur fractures in the elderly is unclear. Acute arthroplasty has the theoretical advantage of earlier mobilization. We examined the outcomes of patients 70 years and older who underwent open reduction internal fixation (ORIF) vs distal femoral replacement (DFR) for comminuted, intra-articular distal femur fractures. METHODS: A retrospective review of patients with AO/OTA classification 33C distal femur fractures treated with either ORIF or DFR was performed. Outcomes including all-cause reoperation, length of stay, fracture union, postoperative comp

Removal of bent intramedullary nail in diaphyseal segmental femoral fracture with intra-articular distal femoral fracture after below-knee amputation.

Refracture accompanied by nail bending in healed or unhealed femoral fractures is a rare complication, and its removal is challenging intraoperatively. A man in his mid-20s presented to the emergency department with a deformed thigh with a puncture wound in a below-knee amputee limb following a road traffic accident, and radiographs revealed a diaphyseal segmental femur fracture with an intra-articular distal femur fracture with a bent femur nail in situ. Attempts to remove a bent femoral nail frequently fail non-invasively and may cause iatrogenic damage. Cutting the bent femur nail using a c

Management of Distal Femoral Periprosthetic Fractures after Total Knee Arthroplasty: Fixation versus Revision Reconstruction.

The incidence of periprosthetic distal femur fractures after total knee arthroplasty (TKA) is increasing in parallel with the increasing number of primary TKA procedures being performed. This number will continue to rise going forward. Treatment decisions depend on patient factors, fracture characteristics, and implant stability. It is important to have a thorough understanding of the available modalities for treatment, including their indications, advantages, and disadvantages, to effectively manage these injuries and optimize outcomes. Nonoperative treatment is indicated in select cases but

Prophylactic femoral neck fixation in distal femur fractures: evaluating protection against ipsilateral hip fracture.

PURPOSE: Distal femur fractures occur in older, osteoporotic patients and carry a measurable risk of subsequent ipsilateral hip fracture. Prophylactic femoral neck screw placement is believed by many to mitigate this risk, but clinical evidence is limited. This study evaluates the association between prophylactic femoral neck fixation after distal femur fracture and future ipsilateral hip fracture. METHODS: A retrospective cohort study was conducted at a single Level 1 tertiary trauma center between 2018 and 2024. Adult patients (≥ 18 years) with AO 33A/B/C fractures and Su type 1-3 periprost

Dual Plating for Periprosthetic Distal Femoral Fractures Following Total Knee Arthroplasty: A Systematic Review.

AIM: The demand for total knee arthroplasty (TKA) is projected to double over the next decade; consequently, the incidence of periprosthetic fractures associated with TKA is also rising. One of the most frequently used constructs is a single lateral locking plate, chosen for its ease of application and familiarity. However, single lateral plating has been associated with a high incidence of nonunion, complications, and revision. Augmenting the lateral plate construct with a medial plate decreases the likelihood of failure, offering stronger fixation for low periprosthetic fractures. The aim of

Medial helical versus straight lateral plating of distal femoral fractures-a biomechanical comparative study.

BACKGROUND: Distal femoral fractures are commonly treated with lateral straight plates. However, the lateral approach may not always be desirable, and 180°-helical plates may be an alternative. AIM: To investigate the biomechanical competence of 180°-helical plating versus standard straight lateral plating of unstable fractures at the distal femur. METHODS: Twelve left artificial femora were instrumented with a 15-hole Locking Compression Plate-Distal Femur, using either 180°-helical plates (group 1) or conventional straight lateral plates (group 2). An unstable distal femoral fracture AO/OT

Fixation of supraglenoid tubercle fractures using distal femoral locking plates in three Warmblood horses.

Three horses that were presented with supraglenoid tubercle fractures were treated with open reduction and internal fixation using distal femoral locking plates (DFLP). Placing the DFLP caudal to the scapular spine in order to preserve the suprascapular nerve led to a stable fixation, however, it resulted in infraspinatus muscle atrophy and mild scapulohumeral joint instability (case 1). Placing the DFLP cranial to the scapular spine and under the suprascapular nerve resulted in a stable fixation, however, it resulted in severe atrophy of the supraspinatus and infraspinatus muscles and scapulo

Dual vs. single plating in distal femoral fractures: a systematic review and meta-analysis.

BACKGROUND: Distal femoral fractures, comprising 0.4% of all fractures, present significant management challenges due to their complex anatomy and associated complications. While single plating (SP) is commonly used, its biomechanical limitations in complex fractures have prompted interest in dual plating (DP), which may offer superior stability. This systematic review and meta-analysis aimed to compare clinical outcomes between single and dual plating techniques in distal femoral fractures. METHODS: A comprehensive literature search was conducted in PubMed, Scopus, and Web of Science up to M
