Fetal breathing and pressures in the trachea and amniotic sac during oligohydramnios in sheep.

Oligohydramnios commonly leads to fetal lung hypoplasia, but the mechanisms are not fully understood. Our aim was to determine, in fetal sheep, the effects of prolonged oligohydramnios on the incidence and amplitude of tracheal pressure fluctuations associated with fetal breathing movements (FBM), on tracheal flow rate during periods of FBM (VtrFBM) and periods of apnea (Vtrapnea), on tracheal pressure relative to amniotic sac pressure, and on amniotic sac pressure relative to atmospheric pressure. In five sheep, oligohydramnios was induced by draining amniotic and allantoic fluids from 107 to

Fetal lung maturity testing: the end of an era.

Respiratory distress syndrome is a major cause of neonatal morbidity and mortality that is most commonly caused by a deficiency in lung surfactant in premature infants. Therefore, laboratory tests were developed to measure the presence and/or concentration of lung surfactant in amniotic fluid in order to estimate maturity of the fetal lung. Although these tests were once widely employed, their utilization by physicians has decreased in recent years. Several studies have shown that demonstration of a mature fetal lung index by antenatal testing does not improve neonatal outcomes. Instead, decre

Amniotic lamellar body count: predicting and distinguishing neonatal respiratory complications in twin pregnancies.

BACKGROUND: Twin pregnancies have a higher rate of preterm births, making precise prediction of neonatal respiratory disorders essential. We herein examined the amniotic lamellar body count (LBC) and found it to be an accurate predictor of respiratory disorders in twin pregnancies. METHODS: Five hundred fourteen amniotic fluid samples, comprising 132 dichorionic twin (DCT) and 125 monochorionic twin (MCT) gestations, were obtained at cesarean section performed at 29 to 38 gestational weeks. Samples were analyzed immediately without centrifugation. RESULTS: There were 26 neonates (5.1%) with

Administration of steroids after 34 weeks of gestation enhances fetal lung maturity profiles.

OBJECTIVE: To estimate the effect of antenatal glucocorticoid administration on fetal lung maturity in pregnancies with known fetal lung immaturity between the 34th and 37th weeks of gestation. STUDY DESIGN: Pregnancies between 34(0/7) and 36(6/7) weeks undergoing amniocentesis to determine fetal lung maturity were targeted. Women with negative results (TDx-FLM-II <45 mg/g) were randomly assigned to intramuscular glucocorticoid injection or no treatment. A repeat TDx-FLM-II test was obtained 1 week after enrollment. RESULTS: Thirty-two women who met inclusion criteria were randomly assigned.

Unilateral cervical spinal cord lesion in a term newborn, associated with ipsilateral diaphragmatic atrophy and pulmonary hypoplasia.

Two major factors can interfere with normal development of the fetal lungs: mechanical constriction resulting in decreased intrathoracic space and reduced or absent fetal breathing movements. We report a unique case in which both factors contribute to the development of lethal pulmonary hypoplasia. This full-term neonate had an isolated unilateral lesion in the cervical spinal cord resulting in ipsilateral neurogenic atrophy of the diaphragm, bilateral pulmonary hypoplasia, and lung immaturity. Evenetration of the atrophied diaphragm decreased intrathoracic space, compromising fetal lung growt

Corticosteroids for the enhancement of fetal lung maturity: impact on the gravida with preeclampsia and the HELLP syndrome.

This study was undertaken to determine maternal impact of corticosteroids administered for the promotion of fetal lung maturity in mothers with the HELLP syndrome. Twenty-seven of 427 women with the HELLP syndrome treated between 1980-1991 received a full course of steroids prior to preterm delivery. This group was compared to 27 matched control patients with the HELLP syndrome who received no corticosteroids. Subjects were matched for maternal age, race, sex of the fetus, and severity of the HELLP syndrome. The antepartum platelet count stabilized or increased in 25 of 27 steroid-treated wome

Pathogenesis of respiratory failure in the preterm infant.

Respiratory failure in the preterm results from not only surfactant deficiency but also the immaturity of a number of other elements that have a structural basis. Airway, alveolar, fluid clearance, and epithelial and endothelial barrier functions also are important to lung function. Immaturities in these lung elements have identifiable adverse consequences for lung function such as pulmonary interstitial emphysema and pulmonary edema. The maturation of each of these elements appears to be achievable by agents such as corticosteroids, and maturation will result in an improved response to surfac

Laryngeal ultrasonography in infants and children: anatomical correlation with fetal preparations.

Normal sonographic anatomy of the larynx in children was established by studying 40 healthy children and 3 fetuses. The normal anatomy is described on transverse sonograms passing at level of the ventricular bands, the true vocal cords and the subglottis and the interpretation of the sonographic aspect of the larynx is given. Ultrasound of the larynx seems to provide a good analysis of cartilaginous and endolaryngeal structures and can also show the dynamic aspect of the larynx. A pathological case is described as an illustration of the potentialities of this new way of imaging the larynx.

The accessory molecule Lgp55 plays a role early in murine fetal thymocyte differentiation.

A rat IgM monoclonal antibody, PA3-795, inhibits the antigen-specific responses of mouse T-cell hybridomas. It recognizes a heavily glycosylated cell-surface protein, designated Lgp55, that is detectable after activation on mature T cells. During fetal life, Lgp55 is found at high levels on newly immigrant thymic T-cell precursors prior to surface expression of other T-lineage molecules. High levels of expression are also found on thymocytes in the outer cortex of adult mice. Thymocytes at later stages of differentiation bear decreasing amounts of surface Lgp55, and none is detectable on "sing

[Aggressive versus conservative management of HELLP syndrome--a status assessment].

The HELLP syndrome is a severe and life-threatening form of preeclampsia associated with typical laboratory findings. The major problems are the fluctuating course of the disease, the unpredictable occurrence of severe maternal complications and the high maternal and perinatal mortality. Time-limited reversal of the laboratory parameters has been observed in 20-40% of cases; however, the majority of patients shows a deterioration of the disease within 1-10 days. As no reliable clinical and laboratory indicators exist, as well as no precisely defined cut-off values in predicting the course and

Fetal endoscopic tracheal occlusion ('Fetendo-PLUG') for congenital diaphragmatic hernia.

Despite recent advances in surgical technique, posthysterotomy preterm labor remains a major determinant of postoperative fetal morbidity and mortality after in utero repair of congenital diaphragmatic hernia (CDH). Temporary fetal tracheal occlusion, or "PLUG" (Plug the Lung Until it Grows), reverses the pulmonary hypoplasia seen in experimental models of CDH and provides an alternative treatment strategy for some fetuses with CDH. Adaptation of current, minimally invasive surgical technology to the PLUG technique allows treatment of CDH without opening the uterus. In this report the authors

Severe urethral obstruction diagnosed at 14 weeks' gestation: variability of outcome with and without drainage.

We present 3 case reports to illustrate the variability of outcome of severe fetal posterior urethral obstruction. Two of the described cases support the view that early in-utero decompression of an obstructed fetal urinary system into the amniotic cavity, in the selected patient, will allow adequate lung development and will prevent the development of severe renal dysplasia. It will not prevent the abdominal wall deformity of the prune belly syndrome. The evidence suggests that to allow maximum time for lung development and to prevent increasing renal dysplasia, drainage should be performed b

Relationship between prenatal survival rate at 70 days of gestation and morphometric parameters of vagina, uterus and placenta in gilts.

Swine uterine capacity affects litter size, and it could be used as a selection parameter of reproductive performance. Although there are some controversial results, evidences show that the catheter penetration length is positively correlated with litter size, and it could be used as a tool for predicting selection methods. The aim of this study was to determine whether there is any association between the prenatal survival rate and placental size at 70 days of gestation, the vaginal length [catheter penetration length during artificial insemination (AI)] and the uterine capacity in a homogene

A physiologically based pharmacokinetic computer model for human pregnancy.

A physiologically based pharmacokinetic (PBPK) model for human pregnancy must incorporate many factors that are not usually encountered in PBPK models of mature animals. Models for pregnancy must include the large changes that take place in the mother, the placenta and the embryo/fetus over the period of pregnancy. The embryo/fetal weight change was modeled using the Gompertz equation for growth which gave a good fit to extensive pooled weight data of the human embryo/fetus from 25 to 300 days of gestation. This equation is based on a growth rate that is proportional to the total weight of the

The effects of betamethasone on allopregnanolone concentrations and brain development in preterm fetal sheep.

The risk of preterm delivery often means that the fetus will be exposed to exogenous synthetic glucocorticoids to accelerate fetal lung maturation, but effects on other organs, particularly the brain, are not understood. The neurosteroid allopregnanolone (AP) is a GABAA receptor agonist that influences fetal brain development and has neuroprotective properties. In this study we determined the impact of maternal glucocorticoid (betamethasone) administration on brain development and AP synthesis in preterm fetal sheep. Pregnant ewes underwent surgery at 105 days gestation for implantation of fet

Fetal-to-fetal kidney transplantation in utero.

Potter sequence consists of various symptoms associated with renal dysplasia. For bilateral renal agenesis, there is no hope of survival. As a novel therapeutic approach for Potter sequence, we develop a unique approach of "transplantation of fetal kidneys from a different species during the fetal stage." In this study, we first validate the approach using allogeneic transplantation. Fetal kidneys with bladders from green fluorescent protein-expressing rats (embryonic day 14.0-16.5) are subcutaneously transplanted into allogeneic rat fetuses in utero (embryonic day 18.0-18.5). After birth, the
