Focusing by shape change in the lens of the eye: a commentary on Young (1801) 'On the mechanism of the eye'.

In his Bakerian Lecture paper of 1801, Thomas Young provided the best account up to that time of the eye's optical system, including refraction by the cornea and the surfaces of the lens. He built a device, an optometer, for determining the eye's state of focus, making it possible to prescribe appropriate correction lenses. His main contribution, however, was to show that accommodation, the eye's focusing mechanism, was not the result of changes to the curvature of the cornea, nor to the length of the eye, but was due entirely to changes in the shape of the lens, which he described with impres

Induced visual fading of complex images.

Visual stimuli fade from awareness under retinal stabilization or careful fixation, a phenomenon documented by Troxler more than 200 years ago. Research on visual fading during normal visual fixation typically has been restricted to discrete, simple, low-contrast shapes presented peripherally against a uniform or textured background. In four experiments, we document a striking new visual fading effect in which entire photographs of scenes fade to a uniform luminance and hue during normal visual fixation. Critically, this "scene fading" can be induced almost instantaneously by some types of vis

The relationship between fixation disparity and heterophoria.

Twenty-five asymptomatic subjects with normal binocular vision underwent determination of the magnitude and direction of their heterophoria and their fixation disparity. Fixation disparity was measured at a 2-m fixation distance with the phase-difference haploscope under conditions simulating those of casual seeing. When combined with careful attention to symmetrical control of accommodation, this method eliminates several known technical artifacts. Examples of the normal fluctuation of the magnitude and direction of fixation disparity for individual subjects were demonstrated. Whereas only a

[Convergent strabismus in the age of presbyopia (author's transl)].

Based on 15 case reports it can be shown, that the onset of a convergent squint is even possible in the age of presbyopia, independent of the patient's refraction. The clinical picture resembles that of a divergence paralysis or the convergent strabismus of the Bielschowsky type. In 13 cases the decompensation of a previous esophoria may have caused this distance-esodeviation; a divergence paralysis is not probable. In two cases change of orthophoria to esotropia could be observed. Therefore we share the opinion of v. Graefe, that an increased tonicity of the internal recti induced for compens

[Fixation disparity in peripheral and central binocular vision under conditions of prismatic heterophoria (author's transl)].

A critical review of the literature shows that there are controversal interpretations of thephenomenon fixation disparity. The former findings of Ogle and coworkers are ascertained byinvertigator to close the central gap in the fusional patter. Formerly the gap was essentialdue to polarisation techiques. 1. Without central gap fixation disparity decreases, but remainssignificantly different from zero. 2. Diminution or closing of the gap leads to more preciseresults, since the position of the eyes is better controlled. 3. Straining the binocular vision with high prism power cortical tolerances

The effect of chronic one-eye patching on ocular myoclonus.

Ocular myoclonus developed in our 34-year-old patient 4 months after massive brain stem hemorrhage due to eclampsia. On chronic patching of the left eye, the vertical pendular nystagmus in the fixating right eye disappeared, whereas the covered eye was esotropic. While fixating, the left eye showed horizontal pendular nystagmus. With both eyes fixating, the right eye presented markedly vertical pendular nystagmus and the left eye a horizontal pendular nystagmus. It seems that chronic patching of one eye has a beneficial modulating effect on the vertical pendular nystagmus in the fixating eye.

Postural control in dyslexic and non-dyslexic children.

Postural control relies to visual-motion processing (afferent or efferent) and this is thought to be deficient in dyslexics. There is a controversy between clinic and fundamental studies as to the presence of posture abnormalities in dyslexics. To explore further this issue, this study examines posture stability in quite stance in 13 dyslexics (mean age: 13.5 years) and in 13 non-dyslexics (mean age: 13 years). Experiment 1 shows that, similarly to adults and elderly, all children (dyslexics and non-dyslexics), present better stability at near distance (i.e. smaller surface area of the COP, sm

[Case report of osteo-odonto keratoprosthesis (Strampelli) and of Dacron keratoprosthesis (Pintucci)].

BACKGROUND: In severe forms of ocular surface disorders keratoprostheses provide the ultimate possibility to restore vision. They are made of an optical cylinder integrated with a supporting biocompatible or biological haptic. CASE REPORT: We report on two patients with different types of keratoprostheses. An 88-year-old woman with ocular pemphigoid received in 1970 a bilateral osteo-odonto-keratoprosthesis (Strampelli). A 59-year-old man with refractory corneal ulcer after corneal grafting received in 2003 a keratoprosthesis with supporting Dacron tissue (Pintucci). RESULTS: The course 35 y

Gaze holding after anterior-inferior temporal lobectomy.

Eye position-sensitive neurons are found in parietooccipital and anterior-inferior temporal cortex. Putative role of these neurons is to facilitate transformation of reference frame from the retina-fixed to world-fixed coordinates and assure precise action. We assessed the nature of ocular motor disorder in a subject who had selective resection of the right anterior-inferior temporal cortex for the treatment of intractable epilepsy from cortical dysplasia. The gaze was stable when the subject was viewing straight-ahead, but centrally directed drifts in the eye position were seen during eccentr

[Vertical and tortional deviations in early strabismus].

The occlusion of one eye may trigger two types of deviation: 1) Heterophorias: the occluded eye deviates towards a horizontal, vertical or torsional abnormal position of rest. Fusion keeps the eyes straight during binocular fixation. 2) Dissociated deviations, horizontal (DHD), vertical (DVD), torsional (DTD): they are found in infantile strabismus. The deviation without fixation is always smaller than the deviation of the occluded eye. The more typical cases are the ones where the position of rest without fixation is an orthoposition. Normal binocular vision is lacking. Most of the time, an a

Reduced vergence response velocities in dyslexics: a preliminary report.

Dyslexia is a developmental disorder characterized by a reduced ability to recognize or comprehend written or printed words. Two clinically diagnosed dyslexics and two normals served as subjects. Both dyslexic subjects showed reduced static accommodative and vergence ranges compared with normals and numerous regressions and fixation pauses during reading and target tracking tasks. The subjects were then presented with convergent and divergent ramp disparity stimuli ranging in speed from 1.33 to 32.0 deg/s and steps of either 2 or 4 degrees amplitude. The dyslexic subjects showed reduced vergen

Refractive Alterations in Marfan Syndrome: A Narrative Review.

Marfan syndrome (MFS) is a genetic disorder that affects the connective tissue in several systems, with ocular, cardiovascular, and skeletal system manifestations. Its ocular manifestations include ectopia lentis (EL), myopia, astigmatism, and corneal abnormalities. This review examines refractive alterations related to MFS such as EL, microspherophakia, lens coloboma, altered corneal biomechanics (flattening, thinning, and astigmatism), and myopia and their impact on visual acuity. The pathogenesis of these manifestations stems from mutations in the FBN1 gene (encoding fibrillin-1). Moreover,

Cerebellar activation during optokinetic stimulation and saccades.

OBJECTIVE: To investigate the activation pattern of cerebellar structures during small-field optokinetic stimulation (OKN) by functional MRI (fMRI) and compare it with that obtained during voluntary saccades and fixation suppression of OKN. METHODS: Functional images were acquired from oblique transverse slices of eight healthy, right-handed volunteers using a radio frequency-spoiled, single-slice, fast low-angle shot (FLASH) pulse sequence with high-spatial resolution. RESULTS: Horizontal OKN and saccades were associated with bilateral activity in the cerebellar hemispheres (superior semilu

Emmetropisation and accommodation in hypermetropic children before they show signs of squint--a preliminary analysis.

1119 hypermetropic children have been followed from the age of 6 months to 3 1/2 years. Observations are reported on (i) the changes in their refraction and (ii) their accommodation. Children who eventually had either a convergent squint or a microtropia were significantly (i) less likely to have spontaneously reduced their hypermetropia, and (ii) more likely to have problems with their accommodation, than those who had no squint. These abnormalities were demonstrated in both the fixing and the non-fixing eyes. There was no obvious difference between the findings for children who had microtrop

[Antibody titer to Toxoplasma gondii in uveitis of toxoplasmosis and other origin].

The diagnostic value of toxoplasma serology in ocular toxoplasmosis is a controversial issue. Some authors feel that a positive titer indicates nothing more than that the patient had been exposed to Toxoplasma gondii at some stage. Even if in most cases the diagnosis is based on the morphological findings on the fundus, it might sometimes be useful to have an additional serologic evaluation. In a retrospective study we compared the level of antitoxoplasmosis antibodies (measured in a complement fixation test and an immunofluorescence test) in 75 patients with clinically proven ocular toxoplasm

Ocular movements in essential blepharospasm.

In essential blepharospasm histopathologic and electrophysiologic evidence supports the existence of lesions in proximity to brainstem nuclei controlling ocular movements. We studied horizontal ocular movements in eight patients who had been treated previously with surgery or botulinum toxin injection to control essential blepharospasm (mean age, 58 years) and compared these with seven control subjects who did not have blepharospasm (mean age, 68 years). We examined fixation stability, saccades, the vestibulo-ocular reflex, visual enhancement and suppression of the vestibulo-ocular reflex, opt

The lancaster red-green test.

Walter Brackett Lancaster introduced his red-green test in 1939. He felt that any good test of heterophoria should assess the fixation of each eye, simultaneously, without the corrupting influence of a fusion stimulus. His basic concept was to utilize the strength of the cover test-namely, accuracy in measuring ocular deviations-while at the same time devising a way to "measure quickly and accurately the position of the covered eye when the other eye is fixating" in the diagnostic positions of gaze. The Lancaster Red-Green test is a convenient tool for assessing ocular misalignment in patients
