Triple arthrodesis for adult acquired flatfoot.

The primary goal of triple arthrodesis for stage III and IV adult acquired flatfoot is to obtain a well-aligned plantigrade foot that will support the ankle in optimal alignment. Ancillary procedures including posterior muscle group lengthening, medial displacement calcaneal osteotomy, medial column stabilization, peroneus brevis tenotomy, or transfer and harvest of regional bone graft are often necessary to achieve adequate realignment. Image intensification is helpful in confirming optimal realignment before fixation. Results of triple arthrodesis are enhanced with adequate preparation of jo

Medial arch orthosis for paediatric flatfoot.

PURPOSE: To evaluate any correlation between various foot angles and their respective American Orthopaedic Foot and Ankle Society (AOFAS) scores for pain, and the effectiveness of a medial arch orthosis. METHODS: 81 children with bilateral symptomatic flatfoot were randomised into orthosis (n=55) and control (n=26) groups. The orthosis group consisted of 33 male and 22 female patients aged 36 to 204 (mean, 99) months and they were given a medial arch support. The control group consisted of 15 male and 11 female patients aged 36 to 192 (mean, 100) months and they were managed with analgesics.

Subtalar arthroereisis in pediatric flatfoot reconstruction.

Pediatric and juvenile flatfoot is a common problem in childhood, present in one in nine children. The morphologic characteristics of this condition are heel valgus and flattening of the medial longitudinal arch. Other characteristics are usually observed, such as supination and abduction of the forefoot, tightening of the Achilles tendon, and hypertonia of the peroneal muscles. Most children with flatfoot will undergo spontaneous correction or become asymptomatic; those that are symptomatic require treatment. Subtalar arthroereisis, often combined with Achilles tendon lengthening, is a simple

Reconstructive options for failed flatfoot surgery.

Failure of flatfoot reconstruction can occur in many ways. Revision surgery must address the residual deformity if one exists, restore strength and balance to the foot and ankle, and address persistent joint pain. Assessment of the talometatarsal axis, hindfoot alignment, and medial column insufficiency is critical in selection of the appropriate surgical procedure. Arthrodesis of joints that are degenerative and unstable will ensure that the symptom relief will mirror radiographic results.

Coronal Plane Alignment of the Medial Column in Isolated Hallux Valgus, Isolated Flatfoot, and Combined Hallux Valgus-Flatfoot.

BACKGROUND: To provide improved treatment for hallux valgus (HV), we sought to understand more about the pathophysiologic connection between flatfoot deformity and HV by comparing coronal plane alignment of the medial column of the foot for patients with isolated HV, isolated flatfoot, and combined HV-flatfoot vs controls. METHODS: This study retrospectively assessed a consecutive series of 33 patients with combined symptomatic and radiographic HV and flatfoot, 33 isolated symptomatic HV, 33 isolated symptomatic flatfoot, and 33 controls. The medial column alignment was assessed in the corona

[Special flatfoot pathologies of the hindfoot and midfoot].

Flatfoot pathologies in adolescents and adults are among the most common deformities of the hindfoot and midfoot. The etiology is multifactorial and often difficult to precisely classify in individual cases, which is why orthotic insoles and foot exercises often remain the only conservative therapeutic measure; however, there are typical clinical presentations that, while not frequently encountered in everyday practice, can lead to misinterpretation and the missed opportunity for important therapeutic interventions. Therefore, this article discusses the Müller-Weiss syndrome, tarsal coalition

Evaluation and Management of Adolescents With a Stiff Flatfoot.

While flatfeet are normal in children, persistence into adolescence with associated pain or asymmetry warrants additional evaluation. Rigidity of a flatfoot deformity, whether a clinical report or evident on examination, should raise suspicion for pathology. The differential diagnosis includes tarsal coalition, neurogenic planovalgus, and peroneal spasticity. History must include pointed inquiry into birth and neurologic histories to probe for a source of central spasticity. Examination must include standing assessment of hindfoot and midfoot alignment. Hindfoot rigidity may be assessed by the

Anatomic and Biomechanical Considerations of Flatfoot Deformity.

In this article, the authors present essential anatomy in the presence of the postural flat foot. There is a recognition of static versus dynamic stabilizers of the foot. In the continuum of the collapsed foot, there is an exploration of anatomic and pathologic changes. Providing this fundamental knowledge will allow the reader to appreciate the disease process to establish a prognosis and consider treatment alternatives.

Comparative assessment of flatfoot in preschool children.

BACKGROUND: The increase or reduction of the medial longitudinal arch (MLA) can affect essential functions in the foot biomechanics with long-term health consequences. The aim of the present study was to determine the height of the MLA in children from 3 to 6 years of age through five methods of footprint analysis for the diagnosis of flat foot. METHODS: A cross-sectional observational study was conducted with 367 children aged 3 to 6 years in a municipality of the State of Mexico. We conducted a complete postural evaluation, plantoscopy by Viladot classification, and footprint analysis calcu

A cross-disciplinary approach to understanding flatfoot.

As form follows function, pedal anatomy is embedded in a history of evolution. This literature review seeks to further the understanding of physiologic and pathologic flatfoot through cross-disciplinary research of expired and extant members of the Homininae subfamily. Archaeological, anthropological, paleontological, and ontogenetic evidence presents multiple biomechanical similarities and anatomical parallels between flatfooted hominins and humans. Recognizing an evolutionary pattern in flatfoot pathologic disorders enhances anticipation and effective treatment.

[Hindfoot valgus. Diagnosis and therapy of flatfoot].

The clinical finding of flatfoot is characterized by a flattening of the medial longitudinal arch and valgus deformity of the hindfoot. The differential diagnosis of flatfoot is the physiological, flexible, contracted flatfoot, which occurs as a congenital or acquired deformity. Congenital flatfoot deformity requires early intensive therapy, while a flexible flatfoot in children has a good prognosis and conservative treatment usually leads to a stable and sufficient load-bearing foot. Severe flatfoot in children can be corrected successfully by simple, minimally invasive procedures. In adults

Minimizing the role of fusion in the rigid flatfoot.

The goals of surgery for the rigid flatfoot are to achieve a painless, stable, functional plantigrade foot. Although triple arthrodesis affords predictable correction and pain relief, the long-term sequelae of extended hindfoot fusions include arthritis and often the need for further, more extensive fusion procedures. We propose that satisfactory results can be achieved in the rigid flatfoot by limiting fusion to joints that are arthritic, and correcting associated deformity with osteotomy and soft tissue reconstruction.

Comparison of radiological measures for diagnosing flatfoot.

BACKGROUND: In the Taiwanese military, flatfoot is indicated by a calcaneal-fifth metatarsal angle (arch angle) ≥165°. However, the arch angle is not always easily defined. PURPOSE: To assess correlations between the arch angle and other radiographic measures and thus identify an alternative radiographic measure for diagnosing flatfoot. MATERIAL AND METHODS: Eighty-seven male Taiwanese military recruits were studied (median age 22 years, interquartile range 20-23 years). Lateral, weight-bearing radiographs were taken. Five radiographic measurements, including the calcaneal-fifth metatarsal a

Characteristics of adult flatfoot in the United States.

Many factors have been suggested to cause flatfoot deformity. The purpose of this study was to identify risk factors for flatfoot deformity, which itself can be a causative factor for other foot and ankle pathologies. The National Health Interview Survey (Podiatry Supplement) from 1990 was analyzed to determine associations of various demographic factors and other foot and ankle pathologies with self-reported flatfoot deformity. We found statistically significant (P <or= .05) associations of flatfoot with age, male gender, BMI, white-collar occupation, veteran status, bunion, hammertoe, callus

Clinicoradiographic assessment of flexible flatfoot in children.

BACKGROUND: Radiographic assessment is still used to evaluate flexible flatfoot in children. METHODS: To find a set of radiologic parameters for assessing this disease, we studied 53 children aged 10 to 14 years. The degree of plantar collapse was measured by Viladot's classification (grades 0-4). The degree of valgus deformity measured in the heel in a standing position, the presence of painful points, and functional limitation during daily-living activities were also reported. The children underwent standard radiography of the foot under load. On the dorsoplantar view, the talocalcaneal, ha
