[Clinico-laboratory parallels in changes in the cerebrospinal fluid and the immune status of patients with early stages of syphilis].

A total of 135 patients with early syphilis were examined, 24 of these with primary seropositive, 31 with secondary new, 70 with secondary relapsing, and 10 with latent early condition. Abnormal shifts were revealed in the cerebrospinal fluid of 51 (37%) patients, in 28 of these Stages I-II shifts, Stages II-III shifts in 10, and Stages III-IV shifts in 13 patients. Clinically the patients with abnormal liquor developed multiple chancres, erosive papules, leukoderma, alopecia, neutrophilia; these patients had suffered from various diseases in the past. Immunologic examinations have detected ma

The CSF accumulator: its role in the central nervous system and implications for advancing hydrocephalus shunt technology.

The 'cerebrospinal fluid (CSF) accumulator' plays an important physiological role in the healthy central nervous system as both a shock absorber and regulator of drainage. In the presence of hydrocephalus, this function is compromised by resistance to the flow of CSF. It is further compromised by the placement of a standard shunt which allows only for one-way directional diversion of the fluid. Complications associated with shunt dependency may be a direct consequence of both the condition, and a failure of existing systems to restore the normal accumulator function. This article reviews the p

Adjustable sockets may improve residual limb fluid volume retention in transtibial prosthesis users.

BACKGROUND: Loss of residual limb volume degrades socket fit and may require accommodation. OBJECTIVES: To examine if either of two accommodation strategies executed during resting, socket release with full socket size return and socket release with partial socket size return, enhanced limb fluid volume retention during subsequent activity. STUDY DESIGN: Two repeated-measures experiments were conducted to assess the effects of socket release on limb fluid volume retention. METHODS: Limb fluid volume was monitored while participants wore a socket with a single adjustable panel. Participants

Non-cardiogenic pulmonary edema after pericardial drainage for cardiac tamponade.

We report a patient who developed adult respiratory distress syndrome following relief of pericardial tamponade. Because of increasing recognition of pulmonary edema in this situation, we recommend gradual removal of pericardial fluid with hemodynamic monitoring to limit the massive fluid shifts which appear to herald this dire complication.

Carbonic anhydrase inhibitors for prevention of space motion sickness: an avenue of investigation.

Space motion sickness (SMS) is an important medical problem facing NASA's space shuttle program. Two theories that have been advanced to explain SMS are the "fluid shift theory" and the "vestibulo-ocular sensory conflict theory". The "fluid shift theory" pre-supposes an active or passive shift of body fluid to the central nervous system (CNS) and vestibulo-auditory mechanisms. In contrast, the "sensory conflict theory" hypothesizes that unfamiliar accelerational and gravitational inputs from the middle ear conflict with visual inputs and lead to SMS. Carbonic anhydrase inhibitors (CAHI) are kn

Mechanisms for hemodynamic instability related to renal replacement therapy: a narrative review.

Hemodynamic instability related to renal replacement therapy (HIRRT) is a frequent complication of all renal replacement therapy (RRT) modalities commonly used in the intensive care unit. HIRRT is associated with increased mortality and may impair kidney recovery. Our current understanding of the physiologic basis for HIRRT comes primarily from studies of end-stage kidney disease patients on maintenance hemodialysis in whom HIRRT is referred to as 'intradialytic hypotension'. Nonetheless, there are many studies that provide additional insights into the underlying mechanisms for HIRRT specifica

Direct visualizations of air flow in the human upper airway using in-vitro models.

A better understanding of airflow characteristics in the upper airway (UA) is crucial in investigating obstructive sleep apnea (OSA), particle sedimentation, drug delivery, and many biomedical problems. Direct visualization of air flow patterns in in-vitro models with realistic anatomical structures is a big challenge. In this study, we constructed unique half-side transparent physical models of normal UA based on realistic anatomical structures. A smoke-wire method was developed to visualize the air flow in UA models directly. The results revealed that the airflow through the pharynx was lami

Much Ado about N…atrium: modelling tissue sodium as a highly sensitive marker of subclinical and localized oedema.

Hypertonic Na+ accumulation in peripheral tissues is a recently described phenomenon: it has been associated with ageing, hypertension, diabetes, chronic kidney disease and heart failure, but its clinical meaning has yet to be determined. This concept conflicts with the classic physiological paradigm of constant balance between salt intake and excretion, and its water-independent nature is still a matter of debate. We developed a theoretical model explaining changes in the chemical composition of tissues as a function of extracellular volume fraction and excess extracellular fluid, i.e. oedema.

Role of the peritoneopleural pressure gradient in the genesis of hepatic hydrothorax. An isotopic study.

Hepatic hydrothorax is defined as the development of significant pleural effusion in a patient with cirrhosis without primary pulmonary or cardiac disease. This complication is seen in 4-10% of patients with cirrhosis. The pleural effusion is a result of a direct passage of ascitic fluid into the pleural cavity through a defect in the diaphragm. We report two patients with posthepatitis cirrhosis presenting with a significant pleural effusion. The peritoneopleural communication was demonstrated by peritoneal scintigraphy. The role of the peritoneopleural pressure gradient is discussed.

[Shifts in subcellular content of blood serum and urine at asthma: animal model, clinic and monitoring data].

We reproduced mouse model of bronchial asthma, evaluated metabolic shifts of serum homeostasis during disease development, and compared these results with the data on metabolic shifts in bronchial asthma patients. In experimental mice pronounced pathological changes in the lungs, considerable lymphocytosis and decrease in neutrophil count were detected. Metabolic shifts were studied using method of laser correlation spectroscopy (LCS) of blood serum and urine. Method of LCS allows determining the dispersion composition of the studied fluid by the relative contribution of particle components in

Fluid volume kinetics of 20% albumin.

AIMS: A population kinetic model was developed for the body fluid shifts occurring when 20% albumin is given by intravenous infusion. The aim was to study whether its efficacy to expand the plasma volume is impaired after major surgery. METHODS: An intravenous infusion of 3 mL/kg 20% albumin over 30 minutes was given to 15 volunteers and to 15 patients on the 1st day after major open abdominal surgery. Blood samples and urine were collected during 5 hours. Mixed-effect modelling software was used to develop a fluid volume kinetic model, using blood haemoglobin and urine excretion the estimate

Fluid Overload.

Most critically ill patients experience external or internal fluid shifts and hemodynamic instability. In response to these changes, intravenous fluids are frequently administered. However, rapid losses of administered fluids from circulation and the indirect link between the short-lived plasma volume expansion and end points frequently result in transient responses to fluid therapy. Therefore, fluid overload is a common finding in intensive care units. The authors consider the evidence of harm associated with fluid overload and the physiologic processes that lead to fluid accumulation in crit

Long-duration bed rest as an analog to microgravity.

Long-duration bed rest is widely employed to simulate the effects of microgravity on various physiological systems, especially for studies of bone, muscle, and the cardiovascular system. This microgravity analog is also extensively used to develop and test countermeasures to microgravity-altered adaptations to Earth gravity. Initial investigations of bone loss used horizontal bed rest with the view that this model represented the closest approximation to inactivity and minimization of hydrostatic effects, but all Earth-based analogs must contend with the constant force of gravity by adjustment

Perioperative Fluid Management.

The medical complexity of the geriatric patients has been steadily rising. Still, as outcomes of surgical procedures in the elderly are improving, centers are pushing boundaries. There is also a growing appreciation of the importance of perioperative fluid management on postoperative outcomes, especially in the elderly. Optimal fluid management in this cohort is challenging due to the combination of age-related physiological changes in organ function, increased comorbid burden, and larger fluid shifts during more complex surgical procedures. The current state-of-the-art approach to fluid manag

Carotid Blood Flow-Guided Fluid Therapy in Cytoreductive Surgery With Hyperthermic Intraperitoneal Chemotherapy: A Case Report.

Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy is associated with significant volume shifts and requires meticulous hemodynamic management. The conventional and arbitrary "liberal" and "restrictive" fluid regimens are now being challenged. With increasing recognition of the need to individualize perioperative fluid therapy, dynamic assessment of fluid status and hemodynamic response to administration of fluids have become vital. Herein, we describe a case in which point of care ultrasound of the carotid artery and derived parameters were used to guide intraoperative fluid

Factors predisposing to worsening of sleep apnea in response to fluid overload in men.

OBJECTIVES: Obstructive sleep apnea (OSA) is highly prevalent in patients with fluid-retaining conditions. Using bioimpedance measurements, previous studies have shown that the greater the amount of fluid redistributed from the legs to the neck overnight, the greater the severity of OSA. Our objective was to investigate factors that predispose the development or worsening of OSA in response to experimental fluid overload. METHODS: Fifteen normotensive and non-obese adult men with and without OSA underwent polysomnography (PSG) during which normal saline was infused intravenously at a minimal

Biomechanical adaptation of the bone-periodontal ligament (PDL)-tooth fibrous joint as a consequence of disease.

In this study, an in vivo ligature-induced periodontitis rat model was used to investigate temporal changes to the solid and fluid phases of the joint by correlating shifts in joint biomechanics to adaptive changes in soft and hard tissue morphology and functional space. After 6 and 12 weeks of ligation, coronal regions showed a significant decrease in alveolar crest height, increased expression of TNF-α, and degradation of attachment fibers as indicated by decreased collagen birefringence. Cyclical compression to peak loads of 5-15N at speeds of 0.2-2.0mm/min followed by load relaxation tests

Comparison of hemodynamic and volume responses to different levels of lower body suction and head-up tilt.

Orthostatic challenge such as lower body suction (LBNP) or head-up tilt (HUT) induces marked shifts of blood from the cardiopulmonary into lower extremity vascular compartments. As a reaction to central hypovolemia, counterregulating mechanisms take place immediately, including central/peripheral circulatory responses and neurohumoral responses. Compensatory answers are exerted via autonomic reflexes, ie, cardiopulmonary and arterial baroreceptor mechanisms. The sympathetic nervous system increases heart rate and peripheral resistance to adjust the cardiovascular system to the stress situation
