Transurethral extraction of ureteral stone guided by television fluoroscopy.

In 45 patients with ureteral calculi 2 to 8 mm in diameter, attempts were made to remove the stones transurethrally, using a Dormia extractor and television fluoroscopic monitoring. The procedure was successful in 34 patients. The success rate was 74% when the stones were 5 mm or less and 68% with the larger stones. In 10 patients when instrumental extraction failed, this was followed immediately by open surgery, for removal of stone. The reasons for failure of the transurethral procedure are presented. Great care in performance and preparedness for open surgery are always required, especially

Recommendations From the Blue Ribbon Panel on Fluoroscopy Safety.

There are many challenges associated with the safe use of fluoroscopy. These challenges include but are not limited to highly variable regulatory requirements, scope of practice concerns, inconsistent education and training, and lack of staff empowerment. Challenges are further compounded by the increasing use of fluoroscopy across a wide range of medical specialties. To facilitate consensus on how to address the issues, the ACR convened the multidisciplinary Blue Ribbon Panel on Fluoroscopy Safety (BRP-FS), with 32 organizations represented. The goal of the BRP-FS is to establish multi- and i

Fluoroscopy-Guided Electrode Placement for Porcine Sacral Neuromodulation Studies.

Sacral neuromodulation is increasingly utilized clinically to treat conditions such as overactive bladder, urinary incontinence, and various pelvic pain disorders. Given their anatomical similarities to humans, porcine models have become prevalent in neuromodulation device research. This study introduces a validated fluoroscopy-guided implantation protocol for precise needle-based electrode placement onto porcine sacral nerves. "Blind" targeting through the posterior foramen results in an unreliable nerve-electrode interface. To address this challenge, we performed a detailed dissection and mi

FREQUENCY OF AORTIC MISCANNULATION DURING FLUOROSCOPY-FREE ENDOVASCULAR NAVIGATION.

BACKGROUND: Aortic wire cannulation is sometimes performed without the assistance of fluoroscopy for expediency, but the associated risk of miscannulation has not been quantified. We used human cadavers and an electronic endovascular simulator to characterize guidewire misplacement rates during attempted fluoroscopy-free cannulation of the descending thoracic aorta. METHODS: Aortic cannulation was done in perfused human cadavers (n=10, age 81±13 years) and in their vascular anatomies loaded into the Mentice VISTG5 simulator to validate the method. Trauma Computerized Tomography Angiography sc

Vaginal stenosis treatment using computed tomography and fluoroscopy guidance.

INTRODUCTION: Vaginal stenosis is a common complication following construction of a neovagina with vascularized myocutaneous flaps. This is primarily because of inconsistent or inappropriate vaginal dilator use. Image-guided recanalization, especially for obstructed genitourinary tracts, is an emerging idea in interventional radiology. Although multiple surgical techniques have been reported to treat vaginal agenesis or obstruction, the idea of image-guided recanalization of vaginal stenosis is a relatively new management strategy for vaginal stenosis. CASE: We present a challenging case of a

Safety and efficacy of large balloon dilatation under fluoroscopy.

The maximum diameter of the balloon used for balloon dilatation(BD) of esophagogastric anastomotic stricture (EAS) is generally 20 millimeters. This study aimed to evaluate the safety and efficacy of BD under fluoroscopy, using balloons with a diameter of 25-30 millimeters. We retrospectively analyzed the data of patients with benign EAS treated by large BD (balloon diameter, 25-30 mm) under fluoroscopy. The Cox proportional hazards model (PHM) was used to identify the factors associated with stricture-free survival. The results show that a total of 127 patients were included in this study, an

Errors in Measuring Glenohumeral Arthrokinematics With 2-Dimensional Fluoroscopy.

Two-dimensional fluoroscopic imaging allows measurement of small magnitude humeral head translations that are prone to errors due to optical distortion, out-of-plane imaging, repeated manual identification of landmarks, and magnification. This article presents results from in vivo and in vitro fluoroscopy-based experiments that measure the errors and variability in estimating the humeral head translated position in true scapular plane and axillary views. The errors were expressed as bias and accuracy. The variability with repeated digitization was calculated using the intraclass correlation co

The posterolateral approach for fluoroscopy-guided tibiotalar joint injection.

OBJECTIVE: To report a novel technique for tibiotalar joint injection that utilizes a posterolateral approach, including indications and technical considerations. METHODS: The posterolateral approach for tibiotalar injection is similar to that used in posterior subtalar joint injections. Using this technique, the tibiotalar joint space is accessed by directing the needle anterosuperiorly beneath the fibula until the posterior aspect of the talar dome has been reached. A retrospective review was conducted of all posterolateral approach tibiotalar joint injections at our institution. RESULTS:

Laparoscopic Removal of a Retroperitoneal Hysteroscopic Microinsert Using Fluoroscopy.

Perforation during placement of hysteroscopic microinserts for permanent sterilization occurs in approximately .9% to 2.6% of women undergoing the procedure. Most of the time perforation results in intraperitoneal placement of the hysteroscopic microinsert requiring laparoscopy or laparotomy for removal of the device. Herein we present a case of hysteroscopic microinsert perforation with subsequent retroperitoneal identification of the device. This is the first such case to our knowledge of retroperitoneal identification and retrieval of a perforated device that required real-time fluoroscopy

Assessment of Lumbar Spine Instability Using C-Arm Fluoroscopy.

A 47-year-old woman was referred to physical therapy with a diagnosis of lumbar radiculopathy. Weight-bearing flexion/extension radiographs showed no change in a 13-mm (at L5-S1) spondylolisthesis measured with a neutral posture. Physical therapy with a focus on flexion-biased stabilization exercises was initiated. After failing to improve after 6 weeks, her referring physician ordered magnetic resonance imaging, which revealed a 6-mm spondylolisthesis in a supine position. Additionally, the physical therapist performed an anterior stability test of L5 on S1 under C-arm fluoroscopy, which demo

Iliosacral screw insertion using CT-3D-fluoroscopy matching navigation.

BACKGROUND: Percutaneous iliosacral screw insertion requires substantial experience and detailed anatomical knowledge to find the proper entry point and trajectory even with the use of a navigation system. Our hypothesis was that three-dimensional (3D) fluoroscopic navigation combined with a preoperative computed tomography (CT)-based plan could enable surgeons to perform safe and reliable iliosacral screw insertion. The purpose of the current study is two-fold: (1) to demonstrate the navigation accuracy for sacral fractures and sacroiliac dislocations on widely displaced cadaveric pelves; and

Cervical lung lobe herniation in dogs identified by fluoroscopy.

This study aimed to determine the frequency of cervical lung lobe herniation (CLLH) in dogs evaluated fluoroscopically and to identify associated characteristics. Reports of diagnostic procedures and patient summaries from 2008 to 2010 were reviewed retrospectively. Signalment, body weight, duration of cough, presence of heart murmur and airway collapse, and radiographic findings were compared between dogs with and without CLLH. Of the 121 dogs that were examined, CLLH occurred in 85 (70%). The extra-thoracic trachea kinked during herniation in 33 (39%) dogs with CLLH. Collapse of the intra-th

Fluoroscopy-guided, transcervical, selective salpingography and fallopian tube recanalisation.

This study is a retrospective review of the transcervical fluoroscopy-guided fallopian tube recanalisation (FTR) procedures done in a multi-ethnic south-east Asian population, over 9 years. A total of 100 patients with infertility and documented proximal tubal obstruction (PTO) were referred for FTR. On-table hysterosalpingography under sedation demonstrated true PTO in 96 patients. At selective-salpingography, the PTO cleared in 16 patients; 78 required FTR and two had fimbrial blockage. The technical success rate of FTR was 86.8% and the post-FTR pregnancy rate was 36.84% at a mean follow-up

Tracking intraoperative fluoroscopy utilization reduces radiation exposure during ureteroscopy.

PURPOSE: Recent studies have demonstrated deleterious effects of ionizing radiation from diagnostic and therapeutic imaging procedures. One of the barriers to minimizing patient exposure is physician awareness. We prospectively studied whether providing surgeons with feedback on their fluoroscopy utilization would affect intraoperative fluoroscopy times. MATERIALS AND METHODS: In 2007, we prospectively began to track fluoroscopy usage for all urology cases. Nine months later, surgeons started to receive periodic reports with their mean fluoroscopy time compared with their peers. We reviewed a

3D visualization and open planning platform in virtual fluoroscopy.

Typically virtual fluoroscopy systems display the tracked instruments as a projected shadow on a number of 2D x-ray images completely missing the depth information of the third dimension. This paper describes an extra tool for 3D reconstruction in virtual fluoroscopy which is useful to clarify the position of instruments or anatomy and can be used in planning and assessing surgical procedure without further x-ray images. Two examples are given: displaced subtrochanteric fracture and slipped upper femoral epiphysis is presented.
