Biochemical markers of bone metabolism and bone mineral density in patients with type 2 diabetic foot ulcer.

Type 2 diabetic foot ulcer (DFU) is closely linked to disorders of bone metabolism and abnormal bone mineral density (BMD), while serum 25-hydroxyvitamin D [25(OH)D], the gold standard for evaluating Vit D nutritional status, acts as an independent protective factor for DFU and is implicated in skeletal health regulation in diabetic patients. This cross-sectional study aimed to investigate biochemical markers of bone metabolism and BMD in patients with type 2 diabetic foot ulcer, and to identify risk factors for diabetic foot and accompanying gangrene. A total of 400 patients with type 2 diabe

[Foot and ankle tumours : Part II: Malignant bone tumours and soft tissue tumours of the foot with differential diagnostic hints].

CLINICAL ISSUE: Both benign and malignant tumours are encountered in the foot and ankle. Due to their rarity, however, diagnosis is often uncertain. Usual criteria such as tumour size, invasiveness or pain fail to differentiate benign from malignant neoplasias. STANDARD RADIOLOGICAL METHODS: Plain radiography and-due to the complex foot anatomy-CT are important in the diagnostic evaluation of primary bone tumours. In the case of soft tissue tumours, ultrasonography is used to identify ganglion cysts. Tumour evaluation and staging are then performed using MRI. Nuclear imaging comes into play w

Screening for Low Bone Mineral Density Using Opportunistic Foot Radiographs.

BACKGROUND: The gold standard for low bone mineral density (BMD) screening is dual-energy X-ray absorptiometry (DXA) scanning, which has declined in use in recent years for a variety of reasons. This decline warrants alternative screening mechanisms. This study aimed to determine if opportunistic foot radiographs can serve as a screening tool for predicting low BMD as measured by DXA scan. METHODS: The fifth metatarsal index (5MI), a novel measurement made using the oblique foot radiograph to evaluate cortical thickness, was calculated for patients 50 years and older who underwent a DXA scan

[Etiopathogenetic features of bone metabolism in patients with diabetes mellitus and Charcot foot].

Diabetic neuropathy is one of the most common diabetes mellitus complications associated with mediocalcinosis of the lower extremities, a significant decrease in feet bone mineral density, and a high incidence of cardiovascular disease. In most cases, calcium-phosphorus metabolism changes occur in patients with diabetic neuroarthropathy, or Charcot foot, when we can observe feet local osteoporosis, which in 90% of cases associated with a vessel's calcification of the lower extremities in the majority of diabetes population. A large number of studies presented literature have demonstrated that

[Effectiveness of transverse tibial bone transport in treatment of diabetic foot ulcer].

OBJECTIVE: To evaluate the effectiveness of transverse tibial bone transport in treatment of diabetic foot ulcer. METHODS: Between June 2014 and December 2017, 17 patients with diabetic foot ulcer were treated. There were 11 males and 6 females, with a median age of 57 years (range, 46-72 years). The duration of diabetes was 2.4-32.0 years (mean, 16.0 years). According to the Wagner grading criteria, 7 cases were rated as grade 2, 8 cases as grade 3, and 2 cases as grade 4. The CT angiography (CTA) showed the arterial occlusion or stenosis of varying degrees below knee joint. All cases were t

A review of bone metabolism and developments in medical treatment of the diabetic Charcot foot.

Charcot foot is a rare but severe, and possibly limb-threatening, complication to neuropathy and diabetes mellitus. The current treatment consists of long-term off-loading, and has a large negative impact on the patient's life. Much research has gone into understanding the condition and its biochemical mechanisms, however, the underlying pathogenesis of a Charcot foot is not yet fully understood. In the recent decades several key advances in our understanding of the Charcot foot have been made, both in regards to the changes in bone metabolism and structure an acute Charcot foot can cause, and

Markers of Local Inflammation and Bone Resorption in the Acute Diabetic Charcot Foot.

OBJECTIVE: Due to the localized nature of Charcot foot, systemically altered levels of inflammation markers can be difficult to measure. The aim of this study was to investigate whether it is possible to detect an arteriovenous (A-V) flux in any locally produced inflammatory biomarkers from an acute Charcot foot by comparing local and systemic measurements. METHODS: We included patients with acute diabetic Charcot foot. Blood was sampled from the vena saphena magna on the distal part of the crus bilaterally as well as from the arteria radialis. To minimize the A-V shunting effect, the feet we

Bone SPECT/CT of the Spine, Foot, and Ankle: Evaluation of Surgical Patients.

Radionuclide bone scanning has been used routinely in the evaluation of bone pathology for decades. The greatest strength of the procedure is its extreme sensitivity for bone metabolism, allowing it to distinguish between active and inactive bony abnormalities. The downside of this reliance on abnormal bone turnover is the relative lack of anatomical detail compared with ever-improving CT and MRI technology. Fusion imaging using SPECT/CT (SCT), PET/CT, and PET/MRI offers an opportunity to combine the sensitivity of nuclear medicine examinations with the anatomical detail of CT and MRI. This fu

Combined two foot flaps with iliac bone graft for reconstruction of the thumb.

UNLABELLED: The purpose of this report was to retrospectively review the results of reconstruction of the thumb by use of combined two foot flaps with an iliac bone graft. From 2009 to 2014, nine patients with traumatic amputation of the thumb had their thumbs reconstructed. The two flaps were based on one pedicle. All flaps survived completely. Patients were followed for a mean of 15.6 months (range, 6-35 months). The appearance of the reconstructed thumb was comparable to a normal one, except for one thumb which required debulking. The appearance of the nail was satisfactory without deformit

Bone graft from the distal medial tibia in foot and ankle surgery.

UNLABELLED: Autogenous bone graft is the ideal substrate for primary and revisional foot and ankle surgery. The distal medial tibia is an easily accessible site that provides both cancellous and corticocancellous autograft. Thirty cases of distal tibial bone graft with a minimum follow-up of 1 year are presented. There was an 86% union rate, and there were no stress fractures, persistent pain, or reported nerve injuries at the donor site. An efficient and versatile technique for harvesting both cancellous bone graft and corticocancellous graft is described. The use of distal tibial bone graft

MRI evaluation of bone marrow changes in the diabetic foot: a practical approach.

One of the most important roles of magnetic resonance (MR) in imaging of the diabetic foot is to differentiate between the common and often comorbid pathologies that present with abnormal bone marrow signal. The primary diagnostic challenges in this setting are to distinguish osteomyelitis from reactive bone marrow edema, neuroarthropathy from osteomyelitis, and the sterile from the superinfected neuropathic joint. Whereas both osteomyelitis and reactive marrow edema share increased T2 signal, osteomyelitis is confirmed by T1 hypointensity in the bone marrow and reactive edema demonstrates iso

Is this bone infected or not? Differentiating neuro-osteoarthropathy from osteomyelitis in the diabetic foot.

Osteomyelitis (bone infection) and neuro-osteoarthropathy (Charcot arthropathy) are limb-threatening complications of diabetic neuropathy with very different therapies. Distinguishing between them may be difficult, but it is important. In Charcot arthropathy, noninfectious soft tissue inflammation accompanies rapidly progressive destruction, first of joints, then of bone. This occurs in a well-vascularized and severely neuropathic, but nonulcerated, foot. In osteomyelitis, chronic soft tissue ulceration precedes infection of bone, which may be physically exposed. Magnetic resonance imaging and

Tophaceous gout of the navicular bone as a cause of medial inflammatory tumor of the foot.

A case of tophaceous gout of the navicular bone in a 24-year-old woman is reported. Emphasis is placed on the conditions that might have been precipitating, i.e., anorexia nervosa and alcoholism, and the mechanisms by which increased uric acid level may be explained. The main radiographic patterns of tophaceous gout of the foot are recalled to avoid unnecessary surgery in future cases.

Post-traumatic Bone Marrow Edema of the Foot in a Child: A Case Report.

Bone marrow edema in children is an uncommon condition, most often associated with trauma, and typically presents with persistent pain and difficulty walking. Conservative management, including rest and physical therapy, is commonly used. We report the case of an 11-year-old boy with no significant past medical history who sustained a fall while playing, resulting in right foot pain. One week later, due to persistent pain and difficulty walking, he was evaluated by a traumatologist. Plain radiography showed no abnormalities, and the injury was initially managed as a sprain. As symptoms persist
