BACKGROUND: Intraarticular steroid and local anesthetic injections are often performed for foot pain. Localizing the exact joint acting as a pain generator in foot pain is eminent for optimal and cost-effective treatment. Because of the complex anatomy of the foot with multiple small articulations side by side, this can be challenging. This study's purpose was to determine magnetic resonance imaging (MRI) findings as possible predictive factors for the immediate outcome of intraarticular injections of the foot.
METHODS: All single joint foot injections at our institution from January 2019 to

BACKGROUND: Diabetes-related foot ulcers pose substantial health risks globally, yet the biomechanical intricacies underlying their development remain incompletely understood. This study aimed to evaluate lower limb gait joint coordination variability in individuals with diabetes-related foot ulcers compared to those with diabetes (without diabetes-related foot ulcers) and healthy controls.
METHODS: A total of 99 participants (diabetes-related foot ulcers cases - 16, Diabetes controls - 50, Health controls - 33) compared three self-paced walking trials. Vector coding, a technique quantifying

Hypermobility of the first tarsometatarsal joint can associate with different clinical problems, e.g., hallux valgus, hallux elevatus, hallux rigidus, and transverse metatarsalgia or arthritis of the second metarsocuneiform joint. The first tarsometatarsal joint may also contribute to other foot deformities involving the medial foot column. Osteoarthritis of this joint is one of the causes of medial foot pain. Arthrodesis of the first tarsometatarsal joint (modified Lapidus procedure) is indicated for these conditions, which are resistant to conservative treatment. Lapidus arthrodesis has seve

INTRODUCTION: It has been widely postulated that structural and functional misalignments of the foot, such as flat foot, may cause mechanical deviations of the lower limb during walking. The aim of this study was to investigate the effect of foot orthoses on lower extremity joint moment asymmetry during the stance phase of walking in children with asymptomatic flexible flat feet.
METHODS: Fourteen volunteer male children, clinically diagnosed with flexible flat feet, participated in this study. Subjects completed 12 walking trials at a self-selected walking speed while 3-dimensional kinematic

A non-invasive method, that can be used to describe the underfoot pressure distribution during stance and gait, is pedobarography. This examination helps to describe biomechanics of foot and ankle. It has been used to diagnose foot disorders, assess the disease progression, monitor the progress of rehabilitation and also evaluate the effectivness of undergone surgical treatment. In this article we describe chosen issues of pedobarographic examination in diagnostics and treatment of foot and ankle in adults. We base on our own experience (about 10 thousand examinations) and review of literature

Guillain-Barré syndrome is a serious disorder that occurs when the body's immune system mistakenly attacks the peripheral nervous system. This leads to nerve inflammation and damage that can cause muscle weakness and/or paralysis, including foot drop. Therapy ranges from supportive measures, such as physical therapy, to surgical intervention. It appears from the published data that these patients, generally, have been left to their own devices, because no known cure is available, necessitating reliance on others for their daily activities and ambulation. We present a unique surgical option, wi

BACKGROUND: Metatarsophalangeal joint deformity is associated with skin breakdown and amputation. The aims of this study were to compare intrinsic foot muscle deterioration ratios (ratio of adipose to muscle volume), and physical performance in subjects with diabetic neuropathy to controls, and determine their associations with 1) metatarsophalangeal joint angle and 2) history of foot ulcer.
METHODS: 23 diabetic, neuropathic subjects [59 (SD 10) years] and 12 age-matched controls [57 (SD 14) years] were studied. Radiographs and MRI were used to measure metatarsophalangeal joint angle and intr

The goal of this study was to investigate clinically relevant biomechanical conditions relating to the setup and alignment of knee-ankle-foot orthoses and the influence of these conditions on knee extension moments and orthotic stance control during gait. Knee moments were collected using an instrumented gait laboratory and concurrently a load transducer embedded at the knee-ankle-foot orthosis knee joint of four individuals with poliomyelitis. We found that knee extension moments were not typically produced in late stance-phase of gait. Adding a dorsiflexion stop at the orthotic ankle signifi

We studied the influence of different positions in neighboring joints on strain in the tibial and plantar nerves during ankle and toe movements. Tibial nerve strain at the ankle was measured during ankle dorsiflexion in ten cadavers; plantar nerve strain was measured during toe extension. Tibial nerve strain increased with ankle dorsiflexion (mean increase: 3.9%) and strain was higher when the nervous system was pretensioned by either knee extension or hip flexion (p < or = .011). Strain was even higher when the nerve bed was elongated at both the hip and knee (p < or = .006) before performing

Podiatric physicians often encounter patients with dorsal foot pain related to either an exostosis or a ganglion arising at the junction of the first metatarsal and the cuneiform. Removal of the exostosis or ganglion is routine but may not relieve the pain. Exostosis surgery can result in worsening of pain owing to injury of the deep peroneal nerve. In this retrospective series, ten patients with dorsal foot pain-related exostosis or ganglion underwent measurement of the cutaneous pressure threshold of the skin of the dorsal first web space to determine whether compression of the deep peroneal

The aim of the study was the evaluation of both the foot correction and foot lengthening obtained using the distraction method with osteotomies versus distraction after the transverse tarsal joint resection. Ten patients (10 feet) aged from 5 years to 24 years (average, 10.5 years) were analyzed. Seven of them were treated for severe equinovarus deformity: six of congenital and one of post-traumatic etiology. In three patients, the indication for treatment was foot shortening due to hypoplasia with tibial shortening, combined with foot deformity. Preoperative shortening of the foot ranged from

Pathological ankle conditions alter joint kinematics and contribute to degeneration in adjacent hindfoot and midfoot joints. Accurate in-vivo assessment of these motions is essential for understanding disease progression and guiding clinical interventions. Biplane videoradiography (BVR) is a tool for six-degree-of-freedom in-vivo bone kinematics, but validation against gold-standard radiostereometric analysis (RSA) is needed for each new joint of interest and BVR configuration. This study validated a BVR system using one participant instrumented with tantalum beads in the tibia, fibula, talus,

BACKGROUND: Peritalar subluxation (PTS) contributes to progressive collapsing foot deformity (PCFD), yet its pathophysiology remains unclear. This study analyzed subtalar joint (SJ) dynamics throughout deformity progression.
METHODS: A retrospective review of 153 WBCT scans classified PCFD into four stages: (1) no PTS, (2) PTS without impingement, (3) PTS with sinus tarsi impingement (STI), and (4) PTS with subfibular impingement (SFI). Three-dimensional measurements of middle facet subluxation and incongruence, foot-ankle offset, SJ offset, and lateral process offset were compared between gr

Background: Wrist immobilization is a common clinical intervention for wrist injuries; however, its repercussions on gait parameters and plantar support have not been extensively investigated. Objectives: The purpose of the study was to determine whether wrist immobilization causes alterations in foot pressure and center of gravity, considering hand dominance and visual conditions (eyes open or closed). Methods: The research experiment was conducted using the PodoPrint S4 platform. Basic descriptive statistics were calculated to summarize the variables. Additionally, in the mixed linear model

Multisegmented foot models (MSFMs) are used to capture data of specific regions of the foot instead of representing the foot as a single, rigid segment. It has been documented that different MSFMs do not yield the same joint kinematic data, but there is little information available regarding their use for kinetic analysis. We compared the moment and power at the tibiotalar, midtarsal, and metatarsophalangeal joints of four MSFMs using motion capture data of young adult runners during stance phase of barefoot walking and jogging. Of these models, three were previously validated: the Oxford, Mil

OBJECTIVES: We aimed to clarify the clinical implication of ultrasound (US)-detected foot joint inflammation in tightly controlled patients with rheumatoid arthritis (RA).
METHODS: We evaluated bilateral foot joints (second to fifth metatarsophalangeal joints of forefoot; tarsometatarsal, cuneonavicular and midtarsal joints of midfoot) of 430 RA patients for synovitis using Power Doppler (PD) imaging by US. We made a cross-sectional and a 3-year longitudinal analysis about the associations of US-detected synovitis with clinical, laboratory and radiographic data as well as foot-specific outcom

BACKGROUND: Adolescent DanceSport athletes who regularly dance in high heels have a higher possibility of developing hallux valgus deformity and foot pain. We believe that the occurrence of foot disorders may change the loading on their feet, which thus affects the athletic performance of those adolescents.
METHODS: A total of 63 adolescent DanceSport athletes (16 boys, 47 girls) were included. The plantar pain in the first metatarsophalangeal (1st MTP) joint was evaluated using a questionnaire, and the hallux valgus angle was evaluated using digital photographs (HVAp). The loading values of

BACKGROUND: It is difficult to evaluate the kinematics of flat foot from 2D images, and no definitive methods have so far been established to diagnose flat foot. This study evaluated hindfoot kinetics through the progression of posterior tibial tendon dysfunction (PTTD) in patients with stages II and III PTTD flat foot compared with those in normal patients under dorsiflexion and plantarflexion conditions using 3D computed tomography (CT) reconstruction images.
MATERIALS: CT images were taken of 26 normal and 32 flat feet in neutral, plantarflexion, and dorsiflexion positions of the ankle joi

Charcot-Marie-Tooth disease results in pes cavus or equinocavovarus deformity. An accepted surgical correction is triple arthrodesis of the foot. In a retrospective study of 45 Charcot-Marie-Tooth patients, two cases developed neuropathic ankle joint after triple arthrodesis. This procedure prevents mobility of the tarsal joint and consequently increases stress on the ankle joint. If coupled with decreased joint sensation, it may predispose the ankle to neuroarthropathy. This paper emphasizes joint position sense as an important consideration before triple arthrodesis or any other surgical pro
