Wrist and hand degloved injuries salvaged with reverse radial forearm flap in the CoVid-19 era.

Reverse radial forearm flap is a quick and reliable flap, providing a well-vascularized tissue for coverage of hand defects. It is based on the reverse flow of the radial artery and the only drawback is the proximal ligation of the vessel. Two cases with complex dorsal hand defects were referred with delay to our Hand Unit, due to the CoVid-19 pandemic restrictions. In both cases deep structures of the wrist and fingers were exposed and the wounds were infected. Surgical debridement, skin coverage and reconstruction were decided in a staged approach: The reverse radial forearm flap was first p

Missed hand and forearm injuries in multiple trauma patients: An analysis from the TraumaRegister DGU®.

PURPOSE: Multiple trauma patients have a high risk of missed injuries. The main point of our study was to provide new epidemiological data on hand and forearm injuries in multiple trauma with a focus on those that were missed. Therefore, we used the database of the TraumaRegister DGU®. METHODS: In this study, we evaluated anonymous data from 139931 patients aged 1-100 years with multiple trauma in the TraumaRegister DGU® of the German Society for Trauma Surgery from 2007 to 2017. Patients with hand and forearm injuries documented during hospital stay were identified and analyzed. We included

Injuries of the hand and forearm in young children caused by steam roller presses in laundries.

Fourteen children (median age 43 months, range 14-82 months; 7 girls and 7 boys) were treated for mangle injuries (one hot steam, and 13 cold roller presses) to the hand and forearm between 1996-2002 at the Department of Hand Surgery, Malmö, Sweden. All children had unilateral skin damage with avulsion or necrosis of skin in nine. Seven of the 14 had signs of compartment and carpal tunnel syndromes and three had fractures (phalanges or hamate bone). Initial treatment included fasciotomy, decompression, and skin revision with split skin grafts and later further skin cover, including one pedicle

The serious full-length forearm injury - a case report and literature review.

BACKGROUND: Among upper limb injuries, carpal bone fractures and dislocation, Essex-Lopresti injury, and the terrible triad injury of the elbow are serious and relatively rare injuries. These injuries require surgical intervention. The surgical method is difficult, and the treatment effect is poor. These injuries have not been described in the same limb in the literature. CASE PRESENTATION: A 21-year-old male patient fell from a height in our institution and sustained multiple injuries, including carpal bone fracture-dislocation, Essex-Lopresti injury, and the terrible triad injury of the elb

Recovery of nerve injury-induced alexia for Braille using forearm anaesthesia.

Nerve injuries in the upper extremity may severely affect hand function. Cutaneous forearm anaesthesia has been shown to improve hand sensation in nerve-injured patients. A blind man who lost his Braille reading capability after an axillary plexus injury was treated with temporary cutaneous forearm anaesthesia. After treatment sensory functions of the hand improved and the patient regained his Braille reading capability. The mechanism behind the improvement is likely unmasking of inhibited or silent neurons, but after repeated treatment sessions at increasing intervals the improvement has rema

Revascularization of a Circumferential Hand and Forearm Degloving Injury Using an Arteriovenous Shunt.

Major upper-extremity degloving injuries with distal dysvascularity are rare and challenging surgical problems. When these degloving injuries occur over nonexpendable regions, such as the glabrous skin of the palm and digits, revascularization or replantation may be the treatment of choice. Because the degloved skin flap is often separated in the suprafascial plane, direct arterial repair may be impossible. We present a rare case of circumferential degloving of the hand and forearm with distal dysvascularity, treated successfully with revascularization with arterial anastomosis, venous anastom

Pedicled Breast Flap for Soft Tissue Coverage of a Forearm Blast Injury.

This article presents the case of a 35-year-old woman who sustained a shotgun blast injury to the left forearm and chest wall causing significant soft tissue loss of the extensor compartment. The patient suffered a Gustilo IIIB open radial shaft fracture requiring orthopedic stabilization and plastic surgery intervention. As a result, the patient eventually was reconstructed with the use of a pedicled breast flap. Because of the patient's macromastia and her large forearm wound and morbid obesity, an individualized approach was developed such that a breast flap was designed because of its prox

Awake Fibreoptic Intubation for Forearm Injury in a Patient with Occipito-Cervical Fixator.

A 23-year-old male patient with occipitocervical fixator was scheduled for surgery due to injury to the right forearm. The patient's thyromental distance was 5 cm, mouth opening grade II, sternomental distance 10 cm and Mallampati score 4. Loss of extension and rotation movements of the head was assessed as difficult intubation criteria. Anaesthetic procedures are almost always difficult in patients with occipitocervical fixation; the limited cervical extension complicated both intubation and ventilation. In this report, application of general anaesthesia using awake fibreoptic bronchoscopic i

Major League Baseball Pitchers Experienced Increased Flexor Tendon and Forearm Injuries 2 Years After Pitch Clock Implementation.

PURPOSE: To evaluate the incidence of pitcher injuries 2 years after implementation of the Major League Baseball (MLB) pitch clock. METHODS: Injury data were collected for the 2021, 2022, 2023, and 2024 MLB seasons using the fangraphs.com injury database. The incidence rate ratio was calculated to compare the injury rate for the 2024 season with that for each of the 2021, 2022, and 2023 seasons. Data were pooled for the pre- and post-pitch clock seasons and were compared in similar fashion to the individual seasons using the incidence rate ratio. The z test for proportions was used to determi

Ultrasound-Guided Forearm Nerve Blocks: A Novel Application for Pain Control in Adult Patients with Digit Injuries.

Phalanx fractures and interphalangeal joint dislocations commonly present to the emergency department. Although these orthopedic injuries are not complex, the four-point digital block used for anesthesia during the reduction can be painful. Additionally, cases requiring prolonged manipulation or consultation for adequate reduction may require repeat blockade. This case series reports four patients presenting after mechanical injuries resulting in phalanx fracture or interphalangeal joint dislocations. These patients received an ultrasound-guided peripheral nerve block of the forearm with succe

Shock attenuation of various protective devices for prevention of fall-related injuries of the forearm/hand complex.

BACKGROUND: Attenuation of the peak impact force is essential in any protective devices for prevention of fall-related injuries. HYPOTHESIS: Common wrist guards have limited effectiveness because of the multifaceted nature of wrist injury mechanisms, and other modalities may provide enhanced shock-absorbing functions. STUDY DESIGN: Controlled laboratory study. METHODS: A free-fall device was constructed using a mechanical surrogate to simulate falling impact. At 4 different falling heights, 5 different hand conditions were tested: bare hand, a generic-brand wrist guard, a Sorbothane glove,

Immediate closure of traumatic upper arm and forearm injuries with the latissimus dorsi island myocutaneous pedicle flap.

This paper reports the authors' experience with latissimus dorsi island pedicle flaps in the acute treatment of massive arm injuries. Seven patients with upper arm injuries and four patients with forearm injuries were treated with latissimus dorsi pedicle flaps. All cases involved massive soft-tissue loss and open fractures. Primary healing of wounds occurred without complications in 10 of 11 patients; the eleventh developed a wound infection. There were no instances of flap loss or vascular complications. This report compares and discusses surgical management options and details the importanc

Avulsion injuries of the male external genitalia: classification and reconstruction with the customised radial forearm free flap.

Between 1995 and 2003, 36 men underwent penile reconstruction for avulsion injuries with radial forearm free flap. Classification was according to location of the native urethra at presentation. In type I injury the meatus was at the corporal stump. In type II injury there was loss of corpora with preservation of the crura leaving the meatus near the pubic symphysis. In type III injuries there was a perineal urethrostomy. In type IV injuries urinary diversion via a supra-pubic catheter was required. In some instances, the flap was designed obliquely over the forearm rather than vertically to g

Risk of anterior interosseous nerve injury during forearm surgery: a cadaveric study.

PURPOSE: The anterior interosseous nerve (AIN) is susceptible to injury during forearm surgery, particularly open reduction and internal fixation (ORIF) of radius fractures. This study aimed to analyze the anatomical relationships between the AIN and the radius to identify regions most vulnerable to iatrogenic injury. METHODS: A cadaveric study was conducted on ten fresh, non-embalmed forearms. Standardized dissections were performed to assess the course of the AIN, its motor branches, and their proximity to bony landmarks. Measurements were taken using a graduated ruler, with reference to th

[Translated article] Paresthesias on the lateral face of the forearm: A nervous injury that is difficult to recognize.

The lateral antebrachial cutaneous nerve (LACN) is the sensory branch of the musculocutaneous nerve and usually innervates the lateral aspect of the forearm. Isolated lesions are rare, of varied aetiology and generally underdiagnosed. We present a retrospective descriptive study of electromyography performed at the General University Hospital of Castellón in the last 20 years with isolated NCAL lesion. We identified 11 cases (8 men and 3 women), average age 44 years (15-73 years). 73% were referred from traumatology. Only one patient was correctly guided in the application. 63.6% of cases note
