Fosfomycin in the treatment of bacterial infections: summary of clinical trials in Japan.

The Japan Research Committee of Fosfomycin was organized in the fall of 1972 to promote the basic and clinical studies on fosfomycin. First of all, a subcommittee of fosfomycin consisting of a limited number of members was organized to establish the methods of determination on its antibacterial activity and its concentration in the biological fluid, and the most applicable methods were devised. The clinical trials on its oral form in a small scale were commenced from spring in 1973, and then gradually expanded to almost all of Japan. The clinical trials on its parenteral intravenous form were

Fosfomycin Resistance in Bacteria Isolated from Companion Animals (Dogs and Cats).

Fosfomycin is an old antibacterial agent, which is currently used mainly in human medicine, in uncomplicated Urinary Tract Infections (UTIs). The purpose of this review is to investigate the presence and the characteristics of Fosfomycin resistance in bacteria isolated from canine or feline samples, estimate the possible causes of the dissemination of associated strains in pets, and underline the requirements of prospective relevant studies. Preferred Reporting Items for Systematic Reviews (PRISMA) guidelines were used for the search of current literature in two databases. A total of 33 articl

[Sensitivity to fosfomycin of multiresistant serotype 012 Pseudomonas aeruginosa. Multicenter study].

In a multicentric study including 214 strains of Pseudomonas aeruginosa, we observed that serotype O12 is more frequently sensitive to fosfomycine than other serotypes strains (72% vs 13.2%, p < 0.000001). Time killing experiments demonstrated that combination of ceftazidime-fosfomycine and amikacine-fosfomycine have usually bactericidal and synergistic activities and could be used for the treatment of multiresistant Pa O12 infections. However, an antagonism between beta-lactams and fosfomycin in disk diffusion needs an adequate concentration of both antibiotics in the site of infection.

Fosfomycin-meropenem synergistic combination against NDM carbapenemase-producing Klebsiella pneumoniae strains.

Carbapenemase-producing Enterobacteriaceae (CPE) are an increasing threat to global public health. Treatment of CPE isolates, like New Delhi metallo-β-lactamase (NDM), is limited and often necessitates combination therapies. The aim of this study was to evaluate the synergistic meropenem/fosfomycin combination against K.pneumoniae-producing NDM isolates. Fosfomycin/meropenem, fosfomycin/colistin and meropenem/colistin were tested alone and in combination, using e-test and time-kill assay against 20 clinical carbapenemase-producing K. pneumonia (CPKp NDM) isolates collected from September 2022

Fosfomycin: In vitro efficacy against multidrug-resistant isolates beyond urinary isolates.

OBJECTIVES: Fosfomycin (FOF) is a bactericidal antimicrobial agent active against a range of Gram-negative bacteria, including multidrug-resistant (MDR) and metallo-β-lactamase (MBL)-producing Enterobacteriaceae. However, data are scarce regarding use of the drug beyond urinary tract infections (UTIs). METHODS: In this study, susceptibility rates to FOF among 290 MDR Enterobacteriaceae isolates were analysed by gradient and disk diffusion tests and the results were compared with agar dilution according to the Clinical and Laboratory Standards Institute (CLSI). Minimum inhibitory concentration

Formulation, development and characterization of fosfomycin tromethamine pessaries for the treatment of vaginal infections.

Background The present study investigated the formulation, development, and characterization of Fosfomycin pessaries for the treatment of genitourinary tract infections. To achieve higher local concentration and to avoid the systemic exposure of the body towards Fosfomycin, vaginal pessaries have been developed in this research work. Methodology The Fosfomycin pessaries were prepared using the hand rolling method. After the formulation of the Fosfomycin pessaries, pre-formulation studies were conducted to evaluate the drug-excipient compatibility, followed by in vivo studies. Results The pre-f

Vascular Graft Impregnation with a Fosfomycin/Oritavancin Combination to Prevent Early Infection.

Background/Objectives: Vascular graft infections (VGIs) represent a life-threatening complication, occurring in 0.2-6% of patients following aortic prosthetic placements. Historically, the primary focus for reducing VGIs has been on prevention. Currently, antimicrobial grafts are not available on the market. This study aimed to evaluate the efficacy of combining two antibiotics, fosfomycin and oritavancin, impregnated into the commercially available GelweaveTM vascular graft as a prophylactic alternative against the most commonly implicated bacteria responsible for VGI. Methods: The antimicrob

The Potential Role of Fosfomycin in Neonatal Sepsis Caused by Multidrug-Resistant Bacteria.

The broad-spectrum activity of fosfomycin, including against multidrug-resistant (MDR) strains, has led to renewed interest in its use in recent years. Neonatal sepsis remains a substantial cause of morbidity and mortality at a global level, with evidence that MDR bacteria play an increasing role. The evidence for use of fosfomycin in neonatal subjects is limited. We summarise current knowledge of the pharmacokinetics and clinical outcomes for the use of fosfomycin in neonatal sepsis and issues specific to neonatal physiology. While fosfomycin has a broad range of coverage, we evaluate the ext

Fosfomycin: An Alternative Therapy for the Treatment of UTI Amidst Escalating Antimicrobial Resistance.

INTRODUCTION: Urinary tract infections (UTIs) are the most commonly encountered infectious diseases. The current study was undertaken with a dual purpose, to provide an insight into the current scenario of the microorganisms causing UTI, their antimicrobial sensitivity patterns and also try and evaluate the activity of fosfomycin against E. coli, both ESBL producers as well as non-producers. MATERIALS AND METHODS: The study was conducted prospectively in the Department of Microbiology of a tertiary care hospital from January to June 2014. A total of 358 isolates from the urinary samples of th

[Fosfomycin--its significance for treatment of diseases due to multidrug-resistant bacteria].

Fosfomycin is a bactericidal phosphonic acid derivative, which engages by inhibiting pyruvyltransferase at an early stage in the peptidoglycan synthesis. It shows a broad spectrum of activity that includes many multidrug-resistant gram-negative and gram-positive bacteria. Fosfomycin is active against most strains of Pseudomonas aeruginosa and several multidrug-resistant Enterobacteriaceae, e.g., Escherichia coli strains expressing extended spectrum beta-lactamases (ESBL) and Klebsiella pneumoniae strains with decreased susceptibilities to carbapenems. Most methicillin-resistant Staphylococcus

Is fosfomycin a potential treatment alternative for multidrug-resistant gram-negative prostatitis?

BACKGROUND: Multidrug-resistant gram-negative bacterial (MDR-GNB) infections of the prostate are an increasing problem worldwide, particularly complicating transrectal ultrasound (TRUS)-guided prostate biopsy. Fluoroquinolone-based regimens, once the mainstay of many protocols, are increasingly ineffective. Fosfomycin has reasonable in vitro and urinary activity (minimum inhibitory concentration breakpoint ≤64 µg/mL) against MDR-GNB, but its prostatic penetration has been uncertain, so it has not been widely recommended for the prophylaxis or treatment of MDR-GNB prostatitis. METHODS: In a pr
