Foundations for the Successful Establishment of a Multidisciplinary Orthoplastic Surgery Team.

Orthoplastic surgery, a multidisciplinary approach to addressing complex, traumatic reconstruction, has grown significantly since its emergence over three decades ago. Its success in improving both functional and reconstructive outcomes has led to an increased demand for orthoplastic surgeons and the development of dedicated limb salvage programs at institutions around the world. This review outlines the development of a multidisciplinary orthoplastic surgery program at a Level 1 trauma center in the United States, which serves as a major tertiary referral hub for complex extremity trauma and

Geological and Technical Foundations of Offshore CO2 Storage in Depleted Reservoirs.

Depleted offshore oil and gas reservoirs are compelling carbon dioxide (CO2) storage targets because their traps and seals are field-proven and their geology is unusually well constrained by decades of wells, cores, and 3D seismic. Offshore deployment also reduces onshore land-use conflicts and can repurpose platforms, pipelines, and selected wells after CO2-service qualification, trimming costs and schedules. Technically, prior depletion creates pressure headroom; most candidate intervals lie ≥800-1000 m where CO2 is supercritical, and many reservoirs (e.g., Miocene sandstones) offer high inj

Imaging-Based Parallel Seismic Test for In-Service Bridge Pile Foundations.

The conventional parallel seismic test (PST) is a widely used and highly reliable method for determining lengths of in-service bridge pile foundations. However, it relies solely on the manual interpretation of first-arrival in seismic records and often fails to detect, characterize, or geometrically define internal defects within the piles. To overcome these limitations and enable the intuitive identification of internal defects and damage within piles, this study introduces an elastic reverse time migration (ERTM) imaging algorithm based on the spectral element method, achieving high-resoluti

Meditation: Philosophical Foundations and its Role in Managing Common Noncommunicable Diseases.

Meditation is a mind-body practice with many methods and variations. Initially practiced for self-realization, meditation has recently been recognized for its health benefits, with modern research exploring its medical applications. Mindfulness and transcendental meditation are the two major meditation types that are widely researched. Globally, the common noncommunicable diseases (NCDs), cardiovascular diseases, cancer, chronic respiratory conditions, and type 2 diabetes mellitus account for over 80% of all premature NCD deaths, the majority of which occur in low- and middle-income countries.

Toward high-selectivity CO2 photoelectroreduction: mechanistic foundations, recent advances and challenges.

Photoelectrochemical carbon dioxide reduction reaction (PEC CO2RR) is a promising strategy for converting CO2 into high-value chemicals that contribute to carbon neutrality. However, CO2 reduction is often accompanied by various competitive reaction pathways in the actual reaction process, which may generate a variety of products. The selective regulation of different products not only directly affects the yield and separation cost of the target product, but also influences the energy efficiency and economic feasibility of the whole process. Improving product selectivity is essential for incre

Biomarker Validation in NPC1: Foundations for Clinical Trials and Regulatory Alignment.

Niemann-Pick Type C1 (NPC1) disease is a rare, autosomal recessive, neurovisceral lysosomal storage disorder caused by mutations in the NPC1. This condition leads to defective intracellular cholesterol and lipid trafficking, resulting in the accumulation of unesterified cholesterol and glycosphingolipids in late endosomes and lysosomes. Clinically, NPC1 manifests with a heterogeneous spectrum of progressive neurological symptoms, including ataxia, vertical supranuclear gaze palsy, dysarthria, cognitive decline, and dystonia, often accompanied by systemic signs such as hepatosplenomegaly and ne

Narrative Review of the Theoretical-Methodological Foundations of the TREINI Program.

Scientific knowledge has advanced in the implementation of safe and beneficial interventions for children and adolescents with cerebral palsy (CP). Although the importance of interdisciplinary interventions that integrate all components of the International Classification of Functioning, Disability and Health (ICF) into family-centered practices is widely recognized, this approach is not yet widely adopted. Instead, many programs remain focused on isolated domains. This study presents the theoretical and methodological foundation of TREINI, an interdisciplinary and family-centered program deve

Transcription Regulatory Sequences in Coronaviruses: Mechanistic Foundations for Antivirals and Vaccines.

Coronaviruses employ a distinctive discontinuous transcription mechanism to generate a nested set of subgenomic mRNAs (sgRNAs) from a single large positive-strand RNA genome. Transcription regulatory sequences (TRSs)-comprising a leader TRS (TRS-L) near the 5' end and multiple body TRSs (TRS-Bs) upstream of downstream open reading frames-serve as the primary cis-acting signals that direct polymerase pausing and template switching during negative-strand synthesis. The COVID-19 pandemic spurred intensive investigation of SARS-CoV-2 gene expression, yielding high-resolution transcriptome maps, ge

Regulatory T cell therapies: biological foundations, engineering strategies, and clinical translation.

Regulatory T cell (Treg) therapy has emerged as a promising strategy to control pathological immune responses in autoimmunity, graft-versus-host disease, and solid organ transplantation. Most clinical studies to date have relied on autologous Tregs expanded ex vivo, an approach that has demonstrated safety and feasibility but remains limited by variable cell quality, restricted scalability, and complex manufacturing requirements. To address these constraints, multiple alternative strategies are being developed, including the induction of regulatory phenotypes in conventional T cells, the engin

Refined utilitarianism in practice: reinterpreting the ethical foundations of the NHS.

The COVID-19 pandemic exposed acute tensions in the United Kingdom’s National Health Service (NHS) between its egalitarian self-image and the utilisation of overtly utilitarian tools such as QALY-based cost-effectiveness and prognosis-driven triage. This paper offers a systematic philosophical diagnosis of those tensions through the lens of refined utilitarianism, a consequentialist theory that grounds moral rules in their long-run welfare effects while requiring genuine social approbation and cultural fit. After reconstructing the classical act-/rule-utilitarian debate and engaging canonical
