Management strategies for treatment of the trapped fourth ventricle.

BACKGROUND: An isolated or trapped fourth ventricle (TFV) is an occasional, serious sequela of hemorrhagic, infectious, or inflammatory conditions of the central nervous system. The TFV usually occurs after successful shunting of the lateral ventricles. It may be heralded by delayed clinical deterioration after an initial period of symptomatic improvement. The typical clinical findings suggest an expanding posterior fossa mass lesion. Surgical treatments include CSF diversionary procedures as well as open and endoscopic approaches. Complications related to the treatment of the TFV are common a

Albendazole and praziquantel treatment in neurocysticercosis of the fourth ventricle.

The purpose of this study was to determine the therapeutic efficacy of albendazole and praziquantel administration in the treatment of neurocysticercosis of the fourth ventricle. The authors report the results obtained in 10 patients with cystic neurocysticercosis of the fourth ventricle who were treated with albendazole at a dosage of 15 mg/kg/day for 2 weeks. Because of the failure of albendazole treatment, two of the patients received an additional course of praziquantel at a dosage of 100 mg/kg/day for 2 weeks. A total of 16 courses of albendazole and two courses of praziquantel were admin

[Progressive dysautonomia in hemangioblastoma in the region of the fourth ventricle].

Tumors of the posterior fossa presenting orthostatic hypotension are rare and only nine cases have been reported so far. The locations of almost all these tumors were near the fourth ventricle and three of them were hemangioblastoma. A case of a tumor of the fourth ventricle showing autonomic disturbances mainly composed of orthostatic hypotension is reported. A 42-year-old male was admitted to the Department of Neurology of Chiba University Hospital on June 25th, 1981 because of three years' history of autonomic disturbances including orthostatic syncope, impotence, urinary disturbance and bo

Meningioma of the Fourth Ventricle of the Brain: A Case Report.

Meningiomas constitute a significant proportion of primary intracranial tumors; however, their occurrence within the brain's ventricles is exceptionally uncommon. This report details the case of a 24-year-old man who presented with six months of diplopia. Diagnostic imaging revealed a mass in the fourth ventricle, causing obstructive hydrocephalus. The patient underwent a lateral ventricular puncture for drainage, followed by surgical removal of the tumor. Histopathological examination identified the mass as a fibrous World Health Organization grade 1 meningioma. Symptoms were significantly re

Histological characterization of an ependymoma in the fourth ventricle of a cat.

A tumour occupying the fourth ventricle in a 3-year-old cat was removed surgically and characterized as a tanycytic ependymoma on the basis of histological features of low cellularity, inconspicuous perivascular pseudorosettes and fascicular architecture. Immunohistochemical analysis of sections revealed that the neoplastic cells were immunoreactive for glial fibrillary acidic protein (GFAP), vimentin and S-100. The histological and immunohistochemical findings were similar to those of human tanycytic ependymoma, a subclassification of ependymoma not previously described in domestic species.

Anatomical basis for the lateral approach to the fourth ventricle.

This study focused on the posterior inferior cerebellar artery bifurcation and branching patterns in the fissures around the fourth ventricle. The vertebral arteries in 25 unfixed human cerebellum were cannulated and injected with polyester colored resin. The suboccipital surface of the cerebellum was exposed and the cisterna magna main landmarks localized. The average distance was 12.6 mm between the tonsillovermian notches and 21.8 mm between the inferior tips of the tonsils. The mean vertical distance between these horizontal planes was 14.5 mm. The posterior inferior cerebellar artery was

Vestibular and oculomotor disturbances in pathology of the fourth ventricle.

Positional nystagmus of the central type and gaze paretic (gaze evoked) nystagmus have been reported in pathology of the posterior fossa. It has also been reported that positional nystagmus of the central type is only infrequently accompanied by vertigo and nausea. In this paper five cases are discussed which showed positional nystagmus of the central type and/or gaze paretic nystagmus. All cases showed severe vertigo, nausea and vomiting in the provocative head position. In four out of five cases the pathology was found to be in the area of the IVth ventricle. On the basis of recent neurophys

Isolated myxopapillary ependymoma of the fourth ventricle: illustrative case.

BACKGROUND: A myxopapillary ependymoma (MPE) is a distinctive ependymoma variant that arises exclusively in the conus medullaris or the filum terminale. There are few case reports of intracranial occurrences, with even rarer occurrences in the fourth ventricle. OBSERVATIONS: A 31-year-old female patient presented with difficulty in maintaining her balance and headaches of 3 months' duration. Brain magnetic resonance imaging showed a fourth ventricle mass with obstructive hydrocephalus. Surgical removal of the tumor was performed. The biopsy of the tumor revealed the histological feature of MP

[Fourth ventricle meningiomas. A case report and literature review].

Intraventricular meningiomas are rare and account for approximately 0.5 to 3% of all meningiomas and 9.8 to 14% of all intraventricular tumors. Most rarely, intraventricular meningomas occur in the third and fourth ventricles. The article reviews the literature devoted to meningiomas of a rare localization, in the fourth ventricle. On the basis of published surgical procedures and neuroimaging data, we divided posterior cranial fossa meningiomas into tumors completely located in the fourth ventricle cavity and those with a partial intraventricular component, which are not associated with any s

Disorders affecting the fourth ventricle: etiology and clinical correlates.

OBJECTIVE: The fourth ventricle encompasses many vital structures including the brainstem as its floor and the cerebellum as its lateral wall and roof. Therefore, lesions affecting the fourth ventricle may present as cerebellar or brainstem manifestations. Herein, we presented our experience in the diagnosis of disorders affecting the fourth ventricle during the past 15 years. STUDY DESIGN: Retrospective study. SETTING: University hospital. SUBJECTS: From September 1995 to August 2010, 24,838 patients with vertigo/dizziness visited the clinic. Of them, 13 patients (0.5%) had tumor/stroke af

Late dissemination of fourth ventricle ependymoma: a case report.

Dissemination of benign ependymoma is relatively rare, especially when a local tumor is under control. The majority of the failures take place within a few years after surgery. However, we experienced a case in which multiple spinal cord dissemination was found 13 years after resection of the fourth ventricle ependymoma without a local failure. A 38-year-old man had undergone a subtotal resection of the fourth ventricle ependymoma and radiation therapy to the posterior fossa when he was 25 years old. Follow-up MR imaging repeated once every one or two years detected no recurrence until he bega

Posterior inferior cerebellar artery aneurysm in the fourth ventricle.

We report a patient with a ruptured aneurysm of the choroidal branch of the right posterior inferior cerebellar artery (PICA), lying in and causing an isolated haemorrhage in the fourth ventricle. MRI on the first day after bleeding revealed an abnormal vessel in the fourth ventricle, which was surrounded by a mass of intermediate signal on T1- and T2-weighted images. The aneurysm was clipped via partial splitting of the lower vermis and opening the inferior medullary velum. A postoperative angiogram confirmed complete obliteration of the aneurysm. With PICA aneurysms the rate of intraventricu

Isolated fourth ventricle in neurocysticercosis: pathophysiology, diagnosis, and treatment.

Four cases of isolated fourth ventricle in neurocysticerocosis are reported (three after ventriculoperitoneal shunting, and one after hydrocephalus without shunting). Diagnosis was made based on computed tomography (CT) and/or CT cisternogram/ventriculogram. Three patients underwent fourth ventricle drainage (two died and the third remains well). The fourth patient refused surgical treatment and continues to have severe ataxia. Diagnosis, pathophysiology, and treatment of this entity in neurocysticercosis are discussed.

[Disproportionately large communicating fourth ventricle--report of 2 cases].

A term of 'disproportionately large, communicating fourth ventricle' (DLCFV) was first proposed by in Harwood-Nash in 1980. It is somewhat different from the well known clinical entity of 'isolated or trapped fourth ventricle', because of apparent patency of aqueductal canal. Two cases of typical DLCFV encountered in our clinic were described. First patient was a 24 year old man in whom this condition developed following operations for lumber disc and second patient was 22 year old woman in whom the disease developed after subarachnoid hemorrhage. In both cases, main symptoms were attributable
