A pilot study of balloon dilation in an animal model resulting in cricoid cartilage fracture: implications for the stenotic pediatric airway.

OBJECTIVE: Endoscopic balloon dilation is increasingly popular as primary therapy for infants with subglottic stenosis. We aim to determine the maximum balloon diameter and pressure where no fracture of the cricoid would occur, minimum balloon size and pressures where a gross fracture of the cricoid occurs, and location of these fractures. We tested these objectives by performing balloon dilation in laryngotracheal complexes of eight euthanized adult male New Zealand white rabbits, with airway characteristics similar to a 3- to 9-month-old infant. METHODS: Subglottic airway diameter of each s

Cardiac rupture caused by penetration of the heart by fractured costal cartilage following blunt chest trauma: A case report.

BACKGROUND: Cardiac rupture is rare but has a high mortality rate. Cardiac rupture due to blunt trauma is generally caused by the heart being pinched into the bony thorax. CASE PRESENTATION: A 90-year-old man in shock was transported to the hospital after his precordium was stepped on by a cow. He developed right atrial rupture from penetration of the heart by fractured and calcified costal cartilage, with massive hemothorax, hemorrhagic shock, and respiratory failure. We immediately performed cardiac injury repair. The patient was discharged without sequelae. CONCLUSION: Emergency physician

Increased Osteoblast GαS Promotes Ossification by Suppressing Cartilage and Enhancing Callus Mineralization During Fracture Repair in Mice.

GαS, the stimulatory G protein α-subunit that raises intracellular cAMP levels by activating adenylyl cyclase, plays a vital role in bone development, maintenance, and remodeling. Previously, using transgenic mice overexpressing GαS in osteoblasts (GS-Tg), we demonstrated the influence of osteoblast GαS level on osteogenesis, bone turnover, and skeletal responses to hyperparathyroidism. To further investigate whether alterations in GαS levels affect endochondral bone repair, a postnatal bone regenerative process that recapitulates embryonic bone development, we performed stabilized tibial oste

Isolated comminuted fracture of the cricoid cartilage and narrowing of the airway after a traumatic blunt injury of the neck: a case report.

BACKGROUND: Blunt trauma to the anterior of the neck may compromise the vital structures like major blood vessels, trachea, larynx, pharynx, thyroid, spine, esophagus, and the cricoid. Laryngeal trauma is rare and accounts for 1% of all neck blunt traumas. Cricoid trauma is also very rare and accounts for half of the laryngeal traumas, and the diagnosis is frequently missed. CASE PRESENTATION: A 43-year-old man, with blunt neck trauma after being hardly hit by a crane lifting hook, was referred to the Shahid Beheshti Hospital. The patient complained of dysphonia (hoarseness) and dyspnea. The

Application of One Piece Autologous Rib Cartilage Graft in Hollow Space of Complete Naso-Ethmoid Orbital Fracture.

BACKGROUND: Complete naso-orbital-ethmoid (NOE) complex fractures including hollow space defects are difficult to treat. A proper bone fragment should be maintained for bony rearrangement and reconstruction; however, no guidelines are available for the treatment of the NOE complex. In patients with empty bone fragment from NOE fractures, implantation of autologous rib grafts may be an option for recreating the orbital rim. METHODS: A 57-year-old man with open complete NOE fracture. Computed tomography (CT) revealed complete loss of bone fragments within the NOE area, including the inferior or

Lead exposure inhibits fracture healing and is associated with increased chondrogenesis, delay in cartilage mineralization, and a decrease in osteoprogenitor frequency.

Lead exposure continues to be a significant public health problem. In addition to acute toxicity, Pb has an extremely long half-life in bone. Individuals with past exposure develop increased blood Pb levels during periods of high bone turnover or resorption. Pb is known to affect osteoblasts, osteoclasts, and chondrocytes and has been associated with osteoporosis. However, its effects on skeletal repair have not been studied. We exposed C57/B6 mice to various concentrations of Pb acetate in their drinking water to achieve environmentally relevant blood Pb levels, measured by atomic absorption.

Endochondral ossification in fracture callus during long bone repair: the localisation of 'cavity-lining cells' within the cartilage.

Successful fracture healing typically involves the production of a cartilaginous callus, which is eventually remodelled into new bone. The blood vessels in the advancing front of endochondral ossification are likely to play an important role in the replacement of cartilage with bone within the callus. This was investigated by histology and immunohistochemistry techniques carried out on rabbit tibial osteotomy tissue. Cavities within the cartilage were identified by histology and in many cases, there appeared to be vascular structures within them, identified by the immunolocalisation of the tra

Surgical management of complex non-union in chest wall instability: focus on posterior rib fractures, parasternal cartilage rupture, and costal margin injury.

Non-union of the chest wall is an underrecognized but functionally significant complication of thoracic trauma, particularly when involving posterior ribs, parasternal cartilage, and the costal margin. Complex instability patterns can result in persistent pain, mechanical dysfunction, and impaired respiration. We describe two patients with symptomatic chest wall non-union. Case 1 was a 62-year-old man with posterior non-union of the seventh and eighth ribs and a secondary costal margin rupture with intercostal hernia. He underwent combined posterior rib plating and costal margin reconstruction

The Integrity of Cartilage Hinge in Song 2/3 Lateral Humeral Condylar Fractures in Children: A Retrospective Radiological Study in Two Centers.

OBJECTIVE: For pediatric lateral condylar fractures (LCFs) of the humerus, it is often hard to determine the stability of the fracture based on the Song classification, especially for those categorized as Song stages 2 and 3. This study aims to define the characteristics of cartilage injury and assess the stability of LCFs classified as Song stages 2 and 3 on post-traumatic magnetic resonance imaging (MRI). METHODS: This was a retrospective study based on imaging data, conducted with a short follow-up period. From January 2016 to May 2019, data of all patients with Song 2 and Song 3 LCFs trea

Sequential Exposure to Acute, Intermediate, and Late Synovial Fluid Fracture Hematoma after Ankle Fracture Promotes Cartilage Damage in Vitro Partially Attenuated by Anti Inflammatory Agents.

BackgroundPost-traumatic osteoarthritis (PTOA) following intra-articular fractures (IAFs) remains a common sequela after injury. Cartilage degeneration after injury is influenced by multiple factors, including elevated proinflammatory cytokines produced by synovial fluid fracture hematoma (SFFH). Targeting this inflammatory response may reduce cartilage damage. This study evaluated cartilage injury and inflammatory mediator changes caused by SFFH, assessing whether commercially available anti-inflammatory agents can suppress the inflammatory cascade implicated in PTOA and reduce chondrocyte de

Role of retention of the condylar cartilage in open treatment of intracapsular condylar fractures in growing goats: three-dimensional computed tomographic analysis.

Our aim was to investigate the role of retention of the condylar cartilage in open reduction and internal fixation (ORIF) of intracapsular condylar fractures (ICFs) in growing goats by three-dimensional computed tomographic (CT) analysis. Twelve goats 6 months old were randomly divided into three groups. ICFs were created bilaterally in mandibular condyles and treated with ORIF. On the one side (n=4) the condylar cartilage was removed, and on the other side (n=4) it was retained. Condyles in the control group (n=4) were untouched. CT scans were taken immediately postoperatively, and 3 and 6 mo

Open repair and arthroscopic follow-up of severely delaminated femoral head cartilage associated with traumatic obturator fracture-dislocation of the hip.

This article describes an unusual case of a young adult with traumatic obturator fracture-dislocation of the hip, involving a large femoral head fragment and severe delamination of articular cartilage. The dislocation was irreducible by closed reduction because of interposing soft tissues, including the rectus femoris and iliopsoas muscles, and torn joint capsules, and therefore, open reduction was performed using an anterolateral approach in the lateral decubitus position. The large femoral head fragment was released from the ligamentum teres and fixed to the dislocated femoral head with head
