The authors reviewed nine children with 10 displaced supracondylar femoral fractures that were treated by closed reduction and percutaneous pinning. The average age of the patients was 8.3 years (range 5-13). After removal of the pins and plaster at 5 weeks, full range of movement was regained within 3 weeks. At late follow-up (average 7.4 years [range 4-10]), seven of the eight patients reviewed (nine fractures) had a satisfactory result. One patient had a valgus deformity of 6 degrees. There was no growth plate arrest and no leg length discrepancy. Intra-articular pin placement was avoided,

Closed reduction with external fixation (CREF) and percutaneous pinning (CRPP) are commonly used surgical interventions to treat distal radius fractures. However, there is no consensus regarding the optimal management of these types of fractures. Therefore, this study aimed to compare these treatments' clinical and radiological outcomes in two subgroups of distal radius fractures. The patients who were ≥ 18 and were referred for the diagnosis of type I and III (Fernandez) distal radius fractures were treated with one of these two methods, which have been evaluated through a retrospective analy

Ping-pong fracture in neonates is a well-known complication. There have been many reports on methods to achieve closed reduction using a suction technique. Here we describe in detail the steps for such a maneuver and show the favorable cosmetic outcome without the need for surgery. At the start of the video, the technique for closed reduction of ping-pong fracture in neonates is presented in detail. The present child was born full term through a normal delivery without complications. After delivery, a ping-pong fracture was noticed in the left fronto-temporo-parietal region (see Video 1). The

Acute haematogenous infection of a closed fractures is rare. A 68-year-old diabetic male sustained a burst fracture of a lumbar vertebra (L2) after a fall onto his back. After 5 days of conservative management, he developed a chest infection and amoxicillin was commenced empirically. However, after 6 days his previously moderate focal L2 back pain had become more severe. Pyrexia and systemic inflammatory markers continued to rise despite administration of antibiotics. Blood cultures and a CT-guided biopsy of L2 both revealed Staphylococcus aureus which was sensitive to flucloxacillin. The pati

This is the first time we encountered a peculiar case of osteomyelitis complicating a closed fracture. The patient was a 38 year-old lady who presented just like any other patient with a closed fracture of the right femur. Intraoperatively we were surprised to find pus from the fracture site. This not only changed the surgical management but altered the subsequent outcome as well.

Thirty-one closed fractures of the lower extremity in diabetics were retrospectively reviewed to determine healing times. There was a prolonged union time overall (163% of expected) and in both insulin and oral hypoglycemic-controlled diabetics (157% and 176% of expected). Displaced fractures showed a prolonged union time, while nondisplaced fractures healed in the normal time period (187% and 96% of expected). Fractures treated by open reduction had a more prolonged healing time than those treated by closed reduction (186% and 142% of expected), but this was primarily related to displacement.

The relationship between bone pain and stress fractures diagnosed by bone scintigraphy was investigated in military recruits during active training. In three patients pain appeared in the site of abnormal uptake 7-14 days after the bone scan in a previously asymptomatic site. One hundred and twenty-four sites of stress fractures were found in 64 patients; 32 (26 per cent) were asymptomatic. In 38 patients (59 per cent) there were multiple stress fractures; 32 (33 per cent) had asymptomatic stress fractures. Fifty-three per cent of the regions with abnormal uptake in the femur were painless, co

A stepwise study of nasal fractures includes an initial investigation that was carried out to discover why so many patients were awaiting rhinoplasty, which led us to find a 30% to 40% failure rate in nasal manipulation operations. We then studied the mechanics of nasal fracture by striking fresh cadaver noses. We found that if the nasal bones were deviated by more than half the bridge width, there was a concomitant C-shape fracture of the bony and cartilaginous septum. Two trials were performed, and patients with nasal deviation after trauma were allocated to either manipulation or manipulati

PURPOSE: Preoperative antibiotic prophylaxis is associated to internal fixation for closed phalangeal and metacarpal fracture, but its effectiveness is not known.
METHODS: In a consecutive series of 119 adult patients undergoing Kirschner-wire fixation for phalangeal or metacarpal fracture, 56.3% (n = 67) received antibiotic prophylaxis and 43.7% (n = 52) did not.
RESULTS: The rate of deep surgical site infection was 1.5% (n = 1) in the group with antibiotic prophylaxis and 1.9% (n = 1) in the group without. Minor skin irritation or infection of the pin tract occurred in 13.4% of cases (n =

This study evaluated reasons for healthcare expenditures both before and after the occurrence of fractures among Medicare beneficiaries. In a previous study we examined healthcare expenditures in the 6 months before and after fractures. The difference-"incremental" expenditures-provides one estimate of the potentially avoidable costs associated with fractures. We constructed a second estimate of the cost burden-"attributable" expenditures-using only those costs recorded in claims with fracture diagnosis codes. Attributable expenditures accounted for only 24% to 60% of incremental expenditures,

Fifty-one cases of supracondylar fracture of the femur with a mean follow-up of 29 months were retrospectively studied. Thirty-one fractures were open; most had major soft tissue damage. Open management was used for 32 of the fractures with good to excellent results in 81%, while a good or excellent outcome was attained in only 42% of the 19 fractures treated nonsurgically. Malunion was observed in seven cases treated by closed methods and in only one case treated by open reduction. The patients treated surgically also had shorter average hospital stays and a better range of motion in the knee

INTRODUCTION: Distal radius fractures (DRFs) are common, with over 14,000 annual cases in Denmark. Most cases are treated non-surgically and may require closed reduction prior to casting. National guidelines for anaesthesia and reduction technique for closed reduction of DRFs are lacking, impacting clinical practices and potentially optimal care. This survey aimed to describe Danish clinical practice for initial treatment and closed reduction of DRFs.
METHODS: Between January and April 2025, the survey was distributed online to the 23 physicians responsible for all 44 emergency departments an

BACKGROUND: The described technique is useful for achieving closed reduction of severely displaced (i.e., Judet Type-III and IV) pediatric radial neck fractures. It is widely agreed that radial neck fractures with angulation of >30° should be reduced. Various maneuvers have been described, but none uniformly achieves complete reduction in severely displaced radial neck fractures (Types III and IV)1-4. The aim of the present technique is to achieve closed reduction in these severely displaced radial neck fractures without surgical instrumentation.
DESCRIPTION: A stepwise approach is described.

BACKGROUND: Despite the low incidence of pilon fractures amongst lower limb injuries, their high impact nature presents difficulties in surgical management and recovery. The high complication rate and long recovery times presents a challenge for surgeons and patients. Current literature is varied, with no universal treatment algorithm. We aim to highlight differences in outcomes and complications between open and closed pilon fractures, and between patients treated by open reduction internal fixation (ORIF) or fine wire fixator (FWF) for open and closed fracture subgroups.
METHODS: This retro

Distal radius fractures are one of the most common orthopedic injuries. After history, physical examination, and diagnostic imaging, treatment begins with closed reduction and immobilization to restore an upper extremity that has both acceptable alignment and stability. Whether for definitive nonoperative management or in preparation for surgical fixation, it is important to understand the principles of closed reduction and immobilization. This article presents a brief review of indications and a technical guide to successful closed reduction and casting for the orthopedic surgeon.
