Conservative care for patients with osteoporotic vertebral compression fractures.

As the aging population grows, vertebral compression fractures are becoming an important source of pain and dysfunction. Management can be complex, because care may require multiple treatment modalities and the treatment plan must be tailored to the individual's pain, functional limitations, and goals. Treatment options usually involve a combination of medications, bracing, and physical therapy. This article reviews current recommendations for managing vertebral compression fractures. Indications, complications, and treatment options, including vertebral augmentation, are discussed.

Classification of vertebral compression fractures in the osteoporotic spine.

OBJECTIVE: The purpose of this study was to clarify the relationship between initial radiographs of osteoporotic vertebral compression fractures (VCFs) and clinical results. METHODS: Of the 135 VCFs in the elderly, 73 consecutive patients (84 vertebrae) were reviewed retrospectively. All patients were treated without rigid immobilization. The subjects consisted of 15 men and 58 women. The mean age was 75.0 years with a range from 61 to 91 years. The early radiographic features were classified into five types based on lateral radiographs. Radiographic results during follow-up were evaluated ac

Percutaneous vertebroplasty for malignant compression fractures with epidural involvement.

PURPOSE: To evaluate safety and effectiveness of performance of percutaneous vertebroplasty (PVP) in patients with malignant compression fractures and involvement of the epidural space. MATERIALS AND METHODS: PVP was performed in 50 patients with metastatic disease or multiple myeloma between June 1998 and April 2002. Twenty-five women (mean age, 62.3 years; range, 38-85 years) and 25 men (mean age, 63.1 years; range, 37-92 years) were included. Cases were retrospectively reviewed. Patients who had undergone cross-sectional imaging were classified into three groups. First group had no epidura

Percutaneous vertebroplasty for severe osteoporotic vertebral body compression fractures.

PURPOSE: To determine the efficacy of percutaneous vertebroplasty in treating severe vertebral body compression fractures, or vertebra plana, in patients with osteoporosis. MATERIALS AND METHODS: In 155 patients, 310 percutaneous vertebroplasties were performed during 25 months and 15 days. Of these, 37 patients (27 women, 10 men; mean age, 73.6 years) underwent 48 vertebroplasties for severe osteoporotic vertebral body compression fractures. The fractures were defined as vertebrae that have collapsed to less than one-third of their original height. Imaging and clinical features were analyzed

Treatment of fractures about hip prostheses with compression plating.

Between 1984 and 1986, 10 patients who sustained a femur fracture about a hip prosthesis were treated with open reduction and internal fixation using compression plating. Nine of the 10 fractures healed in an average time of 5 months. Other than one nonunion, no significant complications were noted. Follow-up study, ranging from 13 to 44 months (average, 26 months), was obtained on all 10 patients. One had a loose femoral component, which was attributed to inadequate stem size. None of the other patients had either clinical or radiographic evidence of loosening. Based on Harris hip scores, the

Clinical profile of acute vertebral compression fractures in osteoporosis.

Thirty patients with acute, painful vertebral compression fracture were studied. Certain vertebrae, namely T8, T12, L1 and L4, were more likely to fracture. The majority occurred spontaneously (46%) or after trivial strain (36%). Of the former, 30% occurred whilst in bed. Radiation to the flanks and anteriorly was common (66%) but leg radiation was uncommon (6%). Associated symptoms noted were: nausea (26%), abdominal pain (20%), chest pain (13%). Straining exacerbated pain in only 60%. The position of comfort was lying flat (43%) or sitting (36%) but 16% found standing or walking most comfort

A meta-analysis of the secondary fractures for osteoporotic vertebral compression fractures after percutaneous vertebroplasty.

To identify the risk factors of the secondary fractures for osteoporotic vertebral compression fractures (OVCFs) after percutaneous vertebroplasty (PVP).We conducted a search of relevant articles using Cochrane Library, PubMed, Medline, Science Direct, Embase, the Web of Science and other databases. The time range we retrieved from establishment of the electronic database to November 2017. Gray studies were found in the references of included literature reports. STATA version 11.0 (Stata Corporation, College Station, Texas) was used to analyze the pooled data.Fourteen studies involving 1910 pa

Acupoint Application for Constipation in Thoracolumbar Compression Fracture.

Thoracolumbar compression fractures (TLFs) are common spinal injuries. Due to prolonged bed rest, pain-induced stress, and other factors, the incidence of constipation in affected patients exceeds 70%, which hinders rehabilitation. This study describes the operational procedures of acupoint application for the treatment of such constipation. To do this, acupoints were selected, including Tianshu (ST25), Daheng (SP15), Zusanli (ST36), Shangjuxu (ST37), and Zhigou (TE6), Chinese medicinal herbs such as Rheum officinale were ground into a paste for application, with a frequency of 1x a day, 6 h p

Scrotum Pain Leading to the Diagnosis of a Compression Fracture.

A 45-year-old male presented to the emergency department with an acute onset of severe lumbosacral pain and scrotum pain. Despite initial differential diagnoses, including testicular torsion, perineal necrotizing fasciitis, and acute epididymitis, subsequent imaging revealed a burst fracture of the 12th thoracic vertebra. This case highlights the importance of considering vertebral compression fractures in the differential diagnosis of acute scrotum pain, especially in patients with atypical presentations.

Percutaneous vertebroplasty for osteoporotic vertebral compression fracture.

BACKGROUND: Percutaneous vertebroplasty remains widely used to treat osteoporotic vertebral fractures although our 2015 Cochrane review did not support its role in routine practice. OBJECTIVES: To update the available evidence of the benefits and harms of vertebroplasty for treatment of osteoporotic vertebral fractures. SEARCH METHODS: We updated the search of CENTRAL, MEDLINE and Embase and trial registries to 15 November 2017. SELECTION CRITERIA: We included randomised and quasi-randomised controlled trials (RCTs) of adults with painful osteoporotic vertebral fractures, comparing vertebro

Intramedullary compression device for proximal ulna fracture.

Proximal ulna fractures account for 20% of all proximal forearm fractures. Many treatment options are available for such fractures, such as cast immobilisation, plate and screw fixation, tension band wiring and intramedullary screw fixation, depending on the fracture pattern. Due to the subcutaneous nature of the proximal forearm, it is vulnerable to open injuries over the dorsal aspect of the proximal ulna. This may in turn prove challenging, as it is critical to obtain adequate soft tissue coverage to reduce the risk of implant exposure and bony infections. We herein describe a patient with

Prevention and treatment of multiple osteoporotic compression fracture.

Korea's demographic profile is undergoing tremendous change as the country rapidly ages at one of the fastest rates in the world. Indeed, the country is expected to become an "aged society" in 2018 when the proportion of elderly is estimated to reach 14.3% of the total population. With the notable increase in the number of elderly individuals, the incidence of osteoporotic fractures will also likely increase. Osteoporosis is a systemic musculoskeletal disease that is characterized by the decreased bone quantity and the abnormalities of the microstructures. There are both conservative and surgi

Current status of vertebroplasty for osteoporotic compression fracture.

Vertebral compression fracture is the most common complication of osteoporosis. It may result in persistent severe pain and limited mobility, and significantly impact the quality of life. Conservative therapy using external bracing, bed rest and analgesics is necessary for pain control in these patients. However, some patients may experience protracted or ongoing pain even with these measures. Surgical treatment is indicated when conservative treatment fails, or in patients with spinal instability or neurologic deficit. Elderly patients often have comorbilities, and because of osteoporosis, hi

[Osteoporotic vertebral fracture with spinal cord compression].

Osteoporotic vertebral fractures with neurologic complications are rarely reported in the literature. We report a case of a 66-year-old woman who developed an osteoporotic fracture of T6 followed one month later by spinal cord compression without aseptic vertebral osteonecrosis. Diagnosis is based on magnetic resonance imaging and examination of vertebral biopsy specimens. After reviewing the literature, we stress some features: the osteoporotic vertebral fractures does not have any particularity compared to the uncomplicated form; the neurological complication occurs progressively and belated

[Diagnosis of uncomplicated compression fractures of the spine in children].

During the first three days after trauma a pronounced hypotonia of muscles independent of the localization of the vertebral column was observed. Two patients of mathematical estimation of the clinoid form of the vertebra were proposed aimed at objectivization of the roentgenological information. In 90% of cases the methods proved to be effective. Gamma scintigraphic examination of the vertebral column was carried out in 101 patients. The results obtained have shown the method to be highly sensitive and objective and to distinguish the normal state, disease and trauma of the vertebral column
