Laparoscopic Heller myotomy and fundoplication for achalasia.

OBJECTIVE: The goal of this study was to review the authors' results with laparoscopic cardiomyotomy and partial fundoplication for achalasia. SUMMARY BACKGROUND DATA: Pneumatic dilatation and botulinum toxin (BOTOX) injection of the lower esophageal sphincter largely have replaced cardiomyotomy for treatment of achalasia. After a brief experience with a thoracoscopic approach, the authors elected to perform cardiomyotomy laparoscopically, in combination with a partial fundoplication (anterior or posterior). PATIENTS AND METHODS: Forty patients were treated between July 1992 and November 199

Results of fundoplication in a UK paediatric centre.

Of 55 children (age 3 months to 16 years) who had fundoplication, major complications occurred in nine (16 per cent): paraoesophageal hernia (five cases), prolonged ileus (two cases), recurrent gastro-oesophageal reflux (one case), and accidental perforation (one case). The single most important factor resulting in complications was the omission of crural repair; of seven patients without crural repair, five developed paraoesophageal hernia/recurrence. Four patients required repeat fundoplication for severe recurrent symptoms and one of these developed the unusual complication of pericardiogas

Laparoscopic Nissen Fundoplication: How I Do It?

Surgical treatment of gastroesophageal reflux disease (GERD) aims to control patients' symptoms, improve patients' quality of life, and prevent GERD-related complications (bleeding, esophageal stenosis, Barrett's esophagus, and/or esophageal adenocarcinoma). A careful patient selection and a properly executed operation are key for the success of the procedure. We aimed to describe the operative technique of a laparoscopic Nissen fundoplication, stressing the critical surgical steps we believe should be respected to obtain good surgical outcomes.

Postoperative Gastrointestinal Complaints After Laparoscopic Nissen Fundoplication.

PURPOSE: To investigate the postoperative gastrointestinal complaints and their effects on the satisfaction level of patients after laparoscopic Nissen fundoplication (LNF). MATERIALS AND METHODS: Over a 7-year period, 553 patients who underwent "floppy" LNF were evaluated for preoperative and postoperative complaints. For this purpose, a set of questions derived from gastroesophageal reflux disease-health-related quality-of-life questionnaire (GERD-HRQL) was used. A P-value of <0.05 was considered to show a statistically significant result. RESULTS: The present study included 215 patients w

Does Nissen fundoplication improve deglutition in children?

OBJECTIVES: Anecdotal reports have suggested that silent corticotroph tumours behave in an aggressive fashion; however, clear comparative data with other non-functioning adenomas (NFAs) are lacking. The aims of the study were, first, to review the natural history of those non-functioning pituitary adenomas with positive immunoreactivity for ACTH and secondly, to determine whether this subgroup behave more aggressively than ACTH immunonegative NFAs by means of comparison with existing departmental data. METHODS AND PATIENTS: Twenty-eight patients (16 men, mean age 51.3 years) who underwent tra

Roux-en-Y reconstruction for failed fundoplication.

BACKGROUND: Redo fundoplication has acceptable outcomes in patients with failed previous fundoplications. However, a subset of patients require Roux-en-Y (RNY) reconstruction for symptom relief. AIM: The aim of this study was to demonstrate safety and efficacy of RNY reconstruction for failed fundoplications. METHOD: Retrospective review of data on patients who underwent short-limb RNY gastrojejunostomy (GJ) or esophagojejunostomy (EJ) between the years 2005 and 2007 was performed. RESULTS: Twenty-two patients underwent RNY reconstructions. Fourteen (64%) patients had one, six (27%) patient

Fatal portal thrombosis after laparoscopic Nissen fundoplication.

Portal and mesenteric vein thrombosis is a very uncommon complication of laparoscopic surgery, especially after anti-reflux procedures. We report the case of a twenty-year-old man with a history of alcohol and cocaine consumption. A Nissen fundoplication was performed. The patient received a single 20-mg dose of enoxaparin (Clexane, Aventis Pharma, Spain) two hours before surgery for antithrombotic prophylaxis. On the seventh postoperative day the patient had a portal and mesenteric venous thrombosis, which was confirmed at laparotomy, with both extensive small-intestine necrosis and partial c

Floppy Dor fundoplication after esophagocardiomyotomy for achalasia.

BACKGROUND: When esophagocardiomyotomy (ECM) is performed for achalasia, a complementary antireflux procedure of the surgeon's choice is usually performed to minimize postoperative gastroesophageal reflux. This retrospective analysis describes patients after laparoscopic ECM, most of whom had a modified Dor fundoplication. METHODS: Between 1994 and 2001, 81 patients with achalasia of the esophagus had laparoscopic ECM. We have previously described the use of intraoperative endoscopy to verify completion of ECM in a cohort of 48 patients who had either Toupet fundoplication (n = 25) or floppy

Cardiorespiratory changes during laparoscopic fundoplication in children.

The results of a prospective assessment of cardiorespiratory changes related to anaesthesia and laparoscopic Nissen fundoplication are described in 25 children aged 1.2 to 14.3 years, weighing 9.0 to 64.0 kg. Respiratory disease or oesophagitis were present in 68% cases. During balanced inhalational anaesthesia, hypotension or bradycardia occurred prior to peritoneal insufflation in three cases of reverse Trendelenburg position. During surgery, intra-abdominal pressure was in the 6-10 mmHg range. Transiently, two patients were hypotensive while ten were hypertensive. PETCO2 gradually increased

Laparoscopic Nissen fundoplication: technique and preliminary results.

Between May 1991 and November 1992, 80 consecutive patients with gastro-oesophageal reflux disease underwent laparoscopic Nissen fundoplication. The technique used was exactly the same as for the conventional open approach. There were no deaths but there were four peroperative complications: one gastric perforation, two pleural perforations and one hepatic laceration. Three conversions to laparotomy were necessary, one because of a defective needle holder and two as a result of left hepatic lobe hypertrophy. The duration of operation ranged from 40 to 300 (median 150) min. The median postopera

Adhesion obstruction following Nissen fundoplication in children.

The incidence of postoperative adhesion intestinal obstruction among 156 children who had undergone Nissen fundoplication for intractable gastro-oesophageal reflux was determined. There were 18 episodes of obstruction in 16 patients (10.3 per cent). The mean interval between fundoplication and adhesion obstruction was 10 months (range 10 days-4 years). Additional procedures performed at the original laparotomy substantially increased the risk of developing obstruction. Relaparotomy for adhesion obstruction was required by 21 per cent of patients who had a Ladd's procedure and 12 per cent who h

Robotic Heller myotomy and modified Dor fundoplication.

A 67-year-old female presented with progressive dysphagia to solids and liquids. Imaging was concerning for end-stage achalasia with multiple failed attempts at pneumatic dilation. An outside hospital recommended oesophagectomy, but she came to our clinic for a second opinion. Computed tomography revealed tortuous megaoesophagus measuring up to 7 cm. Barium swallow revealed aperistalsis. Oesophageal manometry revealed an elevated integrated relaxation pressure, 100% failed peristalsis and panoesophageal pressurization in >20% of swallows, consistent with type II achalasia. We recommended a rob

Robotic Fundoplication for Large Paraesophageal Hiatal Hernias.

PURPOSE: Laparoscopic fundoplication is now a cornerstone in the treatment of gastro-esophageal reflux disease (GERD) with sliding hernia. The best outcomes are achieved in those patients who have some response to medical treatment compared to those who do not. Robotic fundoplication is considered a novel approach in treating GERD with large paraesophageal hiatal hernias. Our goal was to examine the feasibility of this technique. METHODS: Seventy patients (23 males and 47 females) with mean age 64 y old (22-92), preoperatively diagnosed with a large paraesophageal hiatal hernia, were treated

Iatrogenic Incarcerated Diaphragmatic Hernia after Nissen Fundoplication.

Postoperative iatrogenic diaphragmatic hernias have been reported sparingly after several thoracic and abdominal surgeries. Although rare, a delay in diagnosis can lead to life-threatening cases of strangulation or perforation, as well as cardiovascular and respiratory insufficiency. This is a case of a 78-year-old female who developed acute obstructive symptoms secondary to herniation of the distal stomach through a defect in the central tendon of the diaphragm. The diaphragmatic defect was presumed to be iatrogenically acquired after Nissen fundoplication one year prior. Other etiologies wer

Fundoplication for laryngopharyngeal reflux despite preoperative dysphagia.

INTRODUCTION Fundoplication for laryngopharyngeal disease with oesophageal dysmotility has led to mixed outcomes. In the presence of preoperative dysphagia and oesophageal dysmotility, this procedure has engendered concern in certain regards. METHODS This paper describes a consecutive series of laryngopharyngeal reflux (LPR) patients with a high frequency of dysmotility. Patients were selected for surgery with 24-hour dual channel pH monitoring, oesophageal manometry and standardised reflux scintigraphy. RESULTS Following careful patient selection, 33 patients underwent fundoplication by lapar

Development of acute pancreatitis after Nissen fundoplication.

Acute pancreatitis subsequent to Nissen fundoplication for gastroesophageal reflux disease (GERD) is an extremely rare adverse event. We describe a pediatric case of acute pancreatitis resulting from superior mesenteric artery syndrome (SMAS) and gas bloat after fundoplication. Gas bloat is one of the known postoperative complications after Nissen fundoplication. Poor nutrition status, which is often associated with severe pediatric GERD, is a risk factor for SMAS. In this case, development of gas bloat and SMAS led to the formation of a closed loop and increased intraluminal pressure of the d

Laryngopharyngeal reflux disease treated with laparoscopic fundoplication.

The objective of this clinical prospective study was to find out if laparoscopic Nissen fundoplication is an effective treatment in laryngopharyngeal reflux disease. Twenty-two consecutive patients, 16 female and 6 male, who were referred to laparoscopic fundoplication and operated by a senior gastroenterology surgeon were included in this study. Their mean age was 41 years (SD 13), and ranged from 18 to 64. The patients selected for surgical treatment had LPRD with several vocal and laryngeal symptoms. The status findings were typical for laryngopharyngeal reflux and the symptoms and signs ha
