A critical analysis of the literature regarding surgical approach and outcome for adult low-grade isthmic spondylolisthesis.

OBJECTIVE: A systematic review of the radiographic and clinical outcomes of adult patients undergoing surgery for low-grade isthmic spondylolisthesis was performed to determine whether conclusions could be made regarding the optimal choice of surgically managing adult low-grade isthmic spondylolisthesis. METHODS: We tabulated the radiographic and clinical outcomes of patients who underwent a posterior procedure alone, an anterior procedure alone, or a combined anterior and posterior procedure. We also evaluated the influence of covariates such as laminectomy, spinal internal fixation, smoking

Developing an approach for assigning GRADE levels in a systematic overview of reviews of diagnostic test accuracy using general principles identified from current GRADE guidelines: A case study.

Existing guidelines on overviews of reviews and umbrella reviews recommend an assessment of the certainty of evidence, but provide limited guidance on 'how to' apply the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) to such a complex evidence synthesis. We share our experience of developing a 'general principles' approach to applying GRADE to a complex overview of reviews. The approach was developed in an iterative and exploratory manner during the planning and conduct of an overview of reviews of a novel molecular imaging technique for the staging of prostate canc

An explainable deep learning approach for automated detection and grading of diabetic retinopathy from fundus images.

Diabetic Retinopathy (DR) is a leading cause of preventable blindness, and it is important to accurately detect it as early as possible, as well as to measure the severity to provide early clinical indicators. Fundus image assessment in the manual mode is subjective and labor-intensive, and is hard to scale, which has encouraged automated approaches. Current deep learning methods are usually based on either convolutional neural networks (CNNs) or transformer-based ones, focusing on local lesion features or global retina context separately. In addition, relational dependencies among lesions tha

A Chemical Glycoproteomic Approach Decrypts Urinary Extracellular Vesicle Surfaceome for Prostate Cancer Diagnosis and Grading.

Extracellular vesicle (EV) surface proteins serve as the fundamental signaling mediators, determining the communication function of EVs. The selective and in-depth exploration of EV surface proteome (surfaceome) is highly compelling but remains technically challenging due to the unique physicochemical properties of surface proteins. These hurdles are further compounded by the inherent heterogeneity of EVs and the low abundance of surface targets in biofluids. Herein, we developed a chemical glycoproteomic strategy, EVscope (EV surface comprehensive one-pot proteomic exploration), for comprehen

A novel approach to partial reduction of high-grade spondylolisthesis with offset rods - A report of two cases.

High-grade spondylolisthesis (HGS) remains a difficult entity to treat, given the high rate of complications following surgical correction of such a spinal deformity. Reduction of spondylolisthesis may be associated with traction injury to the L5 nerve root due to stretching. We report on two cases of HGS where a novel surgical technique of offset rods was used for partial reduction of spondylolisthesis thereby reducing L5 nerve root traction. Both cases had more than 50% correction of translation with good correction of slip angle and satisfactory clinical outcomes were achieved at the final

Systematic approach to managing vernal keratoconjunctivitis in clinical practice: Severity grading system and a treatment algorithm.

Vernal keratoconjunctivitis is an ocular allergy that is common in the pediatric age group. It is often chronic, severe, and nonresponsive to the available treatment options. Management of these children is difficult and often a dilemma for the practitioner. There is a need to simplify and standardize its management. To achieve this goal, we require a grading system to judge the severity of inflammation and an algorithm to select the appropriate medications. This article provides a simple and practically useful grading system and a stepladder algorithm for systematic treatment of these patient

Low-Grade Chronic Inflammation and Atherosclerotic Cardiovascular Risk: The Beneficial Role of Specific Nutritional Approaches.

Healthy nutrition is pivotal for the prevention of atherosclerotic cardiovascular disease (ASCVD). In addition to traditional risk factors, other components, like low-grade chronic inflammation, play a pathogenetic role in ASCVD. Research efforts aim to identify low-grade chronic inflammation in patients with established ASCVD and in subjects with low ASCVD risk, and to validate effective nutritional strategies. Here, current nutritional approaches to counteract low-grade chronic inflammation related to ASCVD and possibly mitigated ASCVD risk are discussed, along with related molecular pathway

An enumerative pre-processing approach for retinopathy severity grading using an interpretable classifier: a comparative study.

BACKGROUND: Diabetic retinopathy (DR) is a serious eye complication that results in permanent vision damage. As the number of patients suffering from DR increases, so does the delay in treatment for DR diagnosis. To bridge this gap, an efficient DR screening system that assists clinicians is required. Although many artificial intelligence (AI) screening systems have been deployed in recent years, accuracy remains a metric that can be improved. METHODS: An enumerative pre-processing approach is implemented in the deep learning model to attain better accuracies for DR severity grading. The prop

Remimazolam for sedation: A protocol for a systematic review with meta-analysis, trial sequential analysis, and GRADE approach.

BACKGROUND: Procedural sedation aims to facilitate a successful diagnostic or therapeutic procedure. The pharmacokinetic properties and pharmacodynamic effects need to be taken into consideration when choosing the ideal sedative. Midazolam and propofol are frequently employed. However, they are associated with respiratory depression with increasing dosage. Also, midazolam has a potentially unpredictable pharmacodynamic response and propofol may cause hypotension and injection site pain. Remimazolam may provide a superior alternative due to its rapid pharmacodynamic profile and insignificant ci

Approach to the diagnosis and treatment of high-grade B-cell lymphomas with MYC and BCL2 and/or BCL6 rearrangements.

High-grade B-cell lymphomas (HGBLs) with MYC and BCL2 and/or BCL6 rearrangements, so-called "double-hit" lymphomas (HGBL-DH), are aggressive lymphomas that form a separate provisional entity in the 2016 revised World Health Organization Classification of Lymphoid Tumors. Fluorescence in situ hybridization (FISH) will be required to identify HGBL-DH and will reclassify a subset of diffuse large B-cell lymphomas (DLBCLs) and HGBLs with features intermediate between DLBCL and Burkitt lymphoma into this new category. Identifying patients with HGBL-DH is important because it may change clinical man

"See and treat" approach is appropriate in women with high-grade lesions on either cervical cytology or colposcopy.

This study was undertaken to evaluate the overtreatment rate of women with abnormal cervical cytology undergoing colposcopy followed by loop electrosurgical excision procedure (LEEP), the so-called "see and treat" approach. Overtreatment was defined as LEEP specimens containing cervical intraepithelial neoplasia (CIN) 1 or less. In this study, medical records of 192 women with abnormal Pap smears undergoing the "see and treat" approach in Chiang Mai University Hospital between October 2008 and October 2010 were reviewed. The preceding Pap smears were as follows: 124 (64.6%) with high-grade squ

Multigene testing to guide clinical adjuvant decisions in breast cancer: An overview focused on the assessment of the quality of evidence with the grading of recommendations assessment, development and evaluation (GRADE) approach.

Multigene tests have emerged as valuable tools in guiding adjuvant chemotherapy decisions for patients with ER-positive/HER2-negative early breast cancer. This study applied the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach to assess the quality of evidence supporting the clinical utility of these tests. We focused on OncotypeDX® and MammaPrint®, the two tests evaluated in prospective randomized trials. The analysis was structured around the clinical question of whether these tests should be recommended for patients with ER-positive, HER2-negative, lymph no

Italian adaptation of the European guidelines for breast cancer diagnosis, staging, and preoperative planning: a GRADE-ADOLOPMENT approach.

An Italian guideline panel issued the national recommendations for breast cancer diagnosis, staging, and preoperative planning. The panel employed the ADOLOPMENT process to adopt or adapt the guidelines developed by the European Commission Initiative on Breast Cancer (ECIBC). This process utilises the Grading of Recommendations Assessment, Development and Evaluation evidence-to-decision framework. Hereby, we present 15 prioritised recommendations from the second chapter of the Italian guidelines. All of the recommendations as originally developed by the ECIBC were adopted.
