The Usefulness of Wearable Sensors for Detecting Freezing of Gait in Parkinson's Disease: A Systematic Review.

Background: Freezing of gait (FoG) is one of the most debilitating motor symptoms in Parkinson's disease (PD). It often leads to falls and reduces quality of life due to the risk of injury and loss of independence. Several types of wearable sensors have emerged as promising tools for the detection of FoG in clinical and real-life settings. Objective: The main objective of this systematic review was to critically evaluate the current usability of wearable sensor technologies for FoG detection in PD patients. The focus of the study is on sensor types, sensor combinations, placement on the body a

[Normal aging and Parkinson's disease: deciphering the neurophysiological changes affecting gait].

With age, walking can be impaired, with a high risk of falls. This risk is even greater with Parkinson's disease. This research highlights the fact that, during walking, the need for cognitive resources increases with age and with Parkinson's disease. It also demonstrates that after an intensive exercise program, the demand for resources is reduced. These results are promising for improving the mobility and quality of life of these people.

Upregulation of the parietal cortex improves freezing of gait in Parkinson's disease.

BACKGROUND The posterior parietal cortex (PPC) is a key brain area for visuospatial processing and locomotion. It has been repetitively shown to be involved in the neural correlates of freezing of gait (FOG), a common symptom of Parkinson's disease (PD). However, current neuroimaging modalities do not allow to precisely determine the role of the PPC during real FOG episodes. OBJECTIVES The purpose of this study was to modulate the PPC cortical excitability using repetitive transcranial magnetic stimulation (rTMS) to determine whether the PPC contributes to FOG or compensates for dysfunctional

Cognition affects gait adaptation after split-belt treadmill training in Parkinson's disease.

BACKGROUND: Split-belt treadmill (SBTM) training has been proposed to improve gait symmetry and overall gait performance of patients with Parkinson's disease (PD). OBJECTIVES: To determine whether patient's baseline features affect gait adaptation to SBTM in PD with freezing of gait (FOG). METHODS: Twenty participants with idiopathic PD and treatment-resistant FOG underwent several clinical assessments including the Toronto Cognitive Assessment (TorCA) prior to treadmill training. Velocity of the treadmill was adjusted to over-ground walking speed. During SBTM training, the belt velocity on

Levodopa ONOFF-state freezing of gait: Defining the gait and non-motor phenotype.

BACKGROUND: Freezing in the levodopa-medicated-state (ON-state) is a debilitating feature of Parkinson's disease without treatment options. Studies detailing the distinguishing features between people with freezing of gait that improves with levodopa and those whose freezing continues even on levodopa are lacking. OBJECTIVE: To characterize the gross motor, gait, and non-motor features of this phenotype. METHODS: Instrumented continuous gait was collected in the levodopa-medicated-state in 105 patients: 43 non-freezers (no-FOG), 36 with freezing only OFF-levodopa (OFF-FOG) and 26 with freezi

Cumulative additional information does not improve the neuromuscular control during postural responses to perturbations in postural instability/gait disorders subtype of Parkinson's disease.

BACKGROUND: Postural response impairments in postural instability and gait disorders (PIGD) subtype patients may be attributed to Parkinson's disease (PD)-deterioration in central-set (programing/modulating of central outputs during motor responses). Although additional information improves some PD motor impairments, an unanswered question is whether additional information can benefit postural response in PIGD subtype. OBJECTIVE: To analyze the effect of cumulative additional information on postural responses after perturbation in PIGD and neurologically healthy older adults (CG). METHODS: P

What does evidence tell us about the use of gait robotic devices in patients with multiple sclerosis? A comprehensive systematic review on functional outcomes and clinical recommendations.

INTRODUCTION: There is growing evidence on the efficacy of gait robotic rehabilitation in patients with multiple sclerosis (MS), but most of the studies have focused on gait parameters. Moreover, clear indications on the clinical use of robotics still lack. As part of the CICERONE Italian Consensus on Robotic Rehabilitation, the aim of this systematic review was to investigate the existing evidence concerning the role of lower limb robotic rehabilitation in improving functional recovery in patients with MS. EVIDENCE ACQUISITION: We searched for and systematically reviewed evidence-based studi

Measures of postural control and mobility during dual-tasking as candidate markers of instability in Huntington's disease.

BACKGROUND: Individuals with Huntington's disease (HD) have impairments in performing dual-tasks, however, there is limited information about the effects of changing postural and cognitive demands as well as which measures are best suited as markers of underlying motor-cognitive interference. METHODS: Forty-three individuals with HD and 15 healthy controls (HC) completed single tasks of walking (Timed Up & Go (TUG), 7 m walk), standing (feet together, feet apart and foam surface) and seated cognitive performance (Stroop, Symbol Digit Modalities Test (SDMT), Delis-Kaplan Executive Function Sys

Assessment of Force Production in Parkinson's Disease Subtypes.

Muscle weakness is a secondary motor symptom of Parkinson's disease (PD), especially in the subtype characterized by postural instability and gait difficulty (PIGD). Since the PIGD subtype also presents worse bradykinesia, we hypothesized that it also shows a decreased rate of force development, which is linked to an increased risk of falling in PD. Therefore, we investigated the effects of PD and PD subtypes on a force production profile and correlated the force production outcomes with clinical symptoms for each PD subtype. We assessed three groups of participants: 14 healthy older adults (O

Step capacitive array sensor to trigger stimulation on Functional Electrical Stimulators devices for Drop Foot: Preliminary results.

In this work, a new custom wireless capacitive step sensor and a real-time algorithm are proposed to detect the start and end of the swing phase of the gait to trigger the stimulation in Functional Electrical Stimulator devices (FES) for Drop Foot. For this, an array of capacitive pressure sensors was designed to detect patterns of the gait swing phase through the Heel Center of Pressure (HCOP). The proposed system recognized all the events with an average error of 20.86±0.02[ms] for the heel strike (initial contact) and 27.60±0.03[ms] for the heel-off (final contact) compared with lower-back

Apraxia of gait- or apraxia of postural transitions?

"Apraxia of gait" is not a useful concept and freezing of gait should also not be considered an apraxia. The concept of apraxia may, however, be applied to distortions of postural transitions that can accompany fronto-parietal lesions.

Dual-Task Gait Cognitive-Motor Interference as a Marker of Cognitive Impairment in Parkinson's Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Cognitive impairment in Parkinson's disease (PD-CI) negatively affects mobility, independence, and quality of life. Dual-task (DT) gait testing, which captures cognitive-motor interference (CMI), has emerged as a promising, non-invasive method to detect PD-CI. OBJECTIVE: This systematic review and meta-analysis assessed the sensitivity of DT gait parameters-including stride length, gait speed, cadence, variability, and Timed Up and Go (TUG)-in differentiating PD-CI from cognitively normal PD (PD-NC), and examined subgroup differences between mild cognitive impairment (PD-MCI) and

Abnormal activity in the brainstem affects gait in a neuromusculoskeletal model.

BACKGROUND: The ability to start and stop locomotion in response to different situations is an essential survival strategy in mammals. Mammalian locomotion is controlled by central pattern generators in the spinal cord, which are modulated by higher centers, particularly by the stimulation of the midbrain locomotor region. The midbrain locomotor region consists of the pedunculopontine nucleus and cuneiform nucleus, each having different roles in animals. Optogenetic activation of the cuneiform nucleus increases locomotion activities, whereas that of pedunculopontine nucleus decreases them. In

Towards a better understanding of anticipatory postural adjustments in people with Parkinson's disease.

INTRODUCTION: Previous studies have shown that anticipatory postural adjustments (APAs) are altered in people with Parkinson's disease but its meaning for locomotion is less understood. This study aims to investigate the association between APAs and gait initiation, gait and freezing of gait and how a dynamic postural control challenging training may induce changes in these features. METHODS: Gait initiation was quantified using wearable sensors and subsequent straight walking was assessed via marker-based motion capture. Additionally, turning and FOG-related outcomes were measured with weara
