Diagnostic correction based on toxicology: A case of combined dichlorvos (DDVP), ethanol, and pesticide co-solvent intoxication.

Organophosphate pesticides (OPs) are commonly encountered toxic substances in poisoning incidents, often associated with severe clinical symptoms. The diagnosis and treatment become more complicated, especially when mixed with other toxic substances, requiring detailed toxicological analysis to identify the involved agents. This report presents a case of mixed poisoning in a 37-year-old male who ingested 250 mL of 40% alcohol and was admitted with impaired consciousness, absent pupillary light reflexes, and severe metabolic acidosis, which did not improve with initial treatment. [Poisoning by hypochlorite-containing disinfectants. A retrospective analysis of 594 cases of poisoning].

Hypochlorite-containing disinfectants or bleaching fluids, if inhaled, may lead to life-threatening poisoning (56% of moderately severe cases, 5% of severe cases) through the immediate liberation of chlorine gas, if they are used together with another cleansing fluid which is very acid. A rough estimate suggests that there must be about 700 cases of such poisonings per year in the Federal Republic of Germany. The characteristic symptom is a respiratory distress syndrome when the liberated chlorine gas is inhaled. Toxic pulmonary oedema and cardiovascular failure can occur if the poisoning take

[Acute intoxication with hydrogen peroxide with air emboli in central nervous system--a case report].

UNLABELLED: 54-year-old woman with brain gas emboli after an accidental ingestion of concentrated hydrogen peroxide was described. Hydrogen peroxide (H2O2) is a water-soluble, caustic liquid. Exposure to concentrated (> 30-35%) hydrogen peroxide may cause cardiorespiratory insufficiency, shock, convulsions, coma, and chemical burns of skin and mucous membranes. Arterial gas embolization in central nervous system is a relatively rare complication. There are three possible mechanisms of gas embolization: persisting patent foramen ovale, pulmonary gas emboli caused by aspiration of hydrogen perox

Effects of Chlorine Addition on Nitrogen Oxide Reduction and Mercury Oxidation over Selective Catalytic Reduction Catalysts.

The effect of chlorine on mercury oxidation and nitrogen oxides (NO x ) reduction over selective catalytic reduction (SCR) catalysts was investigated in this study. Commercial SCR catalysts achieved a high Hg0 oxidation efficiency when Cl2 was sprayed into the flue gas. Results indicated that an appropriate concentration of Cl2 was found to promote NO x reduction and Hg0 oxidation significantly. An optimal concentration of Cl2 (25 ppm) was found to significantly promote NO x reduction and Hg0 oxidation. Moreover, we studied the effects of Cl2 on NO x reduction and Hg0 oxidation over SCR cataly

The role of hyperbaric oxygen in current emergency medical care.

HBO is the definitive treatment for carbon monoxide poisoning, air or gas embolism, and decompression sickness. It is an important adjunct to other treatment modalities in the management of patients with cyanide poisoning, exceptional blood loss (anemia), crush injury, and gas gangrene. In addition, HBO has many investigational uses such as reduction of edema in acute bowel obstruction before surgical intervention. It should be remembered that the best hope for tissue recovery occurs when HBO is initiated within 4 to 6 hours of injury. After this time irreversible muscle and peripheral nerve d

Severe Oleander Poisoning Presenting with Hyperkalaemia and Unusual Electrocardiographic Changes.

BACKGROUND: Hyperkalaemia in oleander (Nerium oleander) poisoning has been associated with a poor prognosis. Different electrocardiographic (ECG) presentations are possible because of vagotonia and hyperkalaemia. METHODS/RESULTS: We report a series of three cases of oleander poisoning in which ECG showed unusual hyperkalaemia features, such as bradyarrhythmia, sinoatrial block, atrioventricular block and junctional rhythm. CONCLUSIONS: If arterial blood gas analysis or laboratory values indicate hyperkalaemia in oleander poisoning, the hyperkalaemia should be treated immediately, even if the

Suggested guide to using lactate gap as a surrogate marker in the diagnosis of ethylene glycol overdose.

BackgroundEthylene glycol (EG) poisoning, if not diagnosed rapidly, can lead to poor patient outcomes. Gas chromatography (GC) is primarily used for EG quantitation which is rarely available, and the turn-around time may be prolonged. Most lactate results from point-of-care (POCT) methods are falsely elevated in EG poisoning compared with automated chemistry analyser results. In combination, the lactate gap (POCT-Automated chemistry) can be used as surrogate marker in just about all laboratories to indicate likely EG toxicity and guide treatment.Case ReportA man presented by ambulance to hospi

A Rare Survival in Celphos Poisoning.

Aluminium phosphide poisoning releases phosphine gas which causes inhibition of cytochrome oxidase, inhibition of electron transport chain and thereby myocardial suppression. It is known to cause various electric abnormalities in the heart from ST-T depression to fatal tachyarrhythmias. Here we present a case of celphos poisoning presenting with both supraventricular tachycardia and ventricular tachycardia.

Methomyl poisoning in dogs: report of 5 cases and literature review.

Methomyl is a widely available carbamate pesticide that acts as an acetylcholinesterase inhibitor. Here, we describe 5 cases of methomyl poisoning in dogs. The main gross and microscopic findings included pulmonary edema and multisystemic congestion and hemorrhages, which were most severe in the respiratory tract. The diagnosis of poisoning was confirmed in all cases by detecting methomyl in gastric contents using gas chromatography-mass spectrometry. Differential diagnoses include poisoning by organophosphates, coumarin rodenticides, and sodium fluoroacetate, which share clinical and patholog

Development of a Rapid Detection Method for Ethylene Glycol and Glycolic Acid in Feline Samples: A Response to Increasing Antifreeze Poisoning Incidents in Korea.

Recently, cases of antifreeze poisoning in companion animals, particularly cats, have surged in the Republic of Korea. Ethylene glycol (EG), the toxic primary component of antifreeze, is metabolized into glycolic acid (GA), leading to severe metabolic acidosis, acute kidney injury, and death. Traditional detection methods, although effective, are often time-consuming owing to complex sample preparation. This study involved a novel analytical method utilizing GC-MS for EG and LC-MS/MS for GA detection, which streamlined the detection process by eliminating the need for derivatization. The metho

Arteriolization of venous blood gases: a clue to the diagnosis of cyanide poisoning.

Prompt diagnosis and treatment of cyanide poisoning is essential for a successful therapeutic outcome. We present a patient with acute cyanide poisoning in whom venous blood gases disclosed an abnormally high pO2. Prompt treatment of the patient with the cyanide antidote, sodium nitrite plus sodium thiosulfate, rapidly reversed the cardiovascular and central nervous system manifestations of cyanide toxicity. The pathogenesis of cyanide-induced blood gas abnormalities and their potential value in the recognition of cyanide intoxication are discussed. Current treatment recommendations for cyanid

Hyperbaric oxygen therapy (HBOT), i. e. breathing pure oxygen at elevated ambient pressure, remains the gold standard of care in treating air or gas embolism and decompression illness. Guidelines are less clear on the value of HBOT in acute management of carbon monoxide (CO) poisoning or clostridial necrosis. To evaluate the evidence of clinical efficacy of HBOT we performed a systematic literature review.
