Revisional bariatric surgery following sleeve gastrectomy: a meta-analysis comparing Roux-en-Y gastric bypass and one anastomosis gastric bypass.

INTRODUCTION: The number of bariatric operations is increasing each year. Sleeve gastrectomy is the most popular procedure; however, it often requires revision surgery because of insufficient weight loss, weight regain or gastro-oesophageal reflux disease (GORD). The most popular revisional procedures are Roux-en-Y gastric bypass (RYGB) and one anastomosis gastric bypass (OAGB). The primary outcome of this meta-analysis was weight loss after revisional surgery following laparoscopic sleeve gastrectomy and the secondary outcomes were gastro-oesophageal reflux, BMI difference, operative time, bl

Conversion of one-anastomosis gastric bypass to Roux-En-Y gastric bypass: mid-term results from the United Arab Emirates.

BACKGROUND: One-Anastomosis Gastric Bypass (OAGB) is becoming popular, but some patients may need to convert to Roux-en-Y Gastric Bypass (RYGB) due weight-related difficulties or postoperative complications. The data on conversions is currently limited to 30-day or short-term follow-up studies. As such, the objective of this study was to evaluate the indications and mid-term outcomes for OAGB conversions to RYGB at a tertiary referral center in the United Arab Emirates. METHODS: A retrospective analysis was conducted on patients who underwent conversion from OAGB to RYGB between February 2016

Surgical revision of loop ("mini") gastric bypass procedure: multicenter review of complications and conversions to Roux-en-Y gastric bypass.

BACKGROUND: The claim that the "mini"-gastric bypass (MGB) procedure with its loop gastrojejunostomy is safer and equally effective to the Roux-en-Y gastric bypass (RYGB) procedure has been promoted before validation. Rumors of unreported complications and the accuracy of follow-up are additional concerns. This study was undertaken to identify MGB patients who require or required revisional surgery at 5 hospitals within the region of the United States where the MGB procedure originated to assess the claim that revision to RYGB is rarely needed. METHODS: The databases of 5 medical centers were

Cardia Gastric Cancer in the Gastric Pouch 5 Years after Gastric Bypass: A Case Report.

BACKGROUND: Roux-en-Y gastric bypass (RYGB) is one of the most frequently performed bariatric procedures worldwide. Remnant gastric cancer after RYGB is a rare complication. There were about seventeen cases reported in the world. The location of the tumor in these cases occurs mainly in the gastric antrum, followed by the body, then the pylorus and linitis plastica, and the last was fundus of the stomach. To the best of our knowledge, this is the first case that gastric cancer located in the cardia of stomach after RYGB. CASE REPORT: A 68-year-old male patient had chronic esophagitis, bile re

A new approach for treatment of gastro-gastric fistula after gastric bypass.

We report a novel technique for gastro-gastric fistula (GGF) repair. A 44-year-old woman was found to have a fistula between her gastric pouch and bypassed stomach 18 years after Roux-en-Y gastric bypass (RYGBP) for morbid obesity. She underwent an attempted open surgical repair, which was complicated by postoperative abdominal sepsis. An upper gastrointestinal series, abdominal CT scan and upper endoscopy confirmed the diagnosis of failed surgery with recurrent GGF. Under endoscopic and fluoroscopic guidance, two ports were inserted percutaneously into the stomach. The fistula was closed with

Gastro-gastric fistula between pouch and fundus following gastric banding and bypass.

A complication observed with revisional surgery involving a fistula between the former pouch after gastric banding and the fundus is described. A 59-year-old man with BMI 41 kg/m(2) presented for Roux-en-Y gastric bypass (RYGBP). He had previously undergone an open gastric banding operation, with the band removed for obstruction 1 year later. He presented to our hospital with a third incisional hernia which was so large that he suffered from abdominal angina following meals. A RYGBP and a hernia repair with mesh were performed. The postoperative contrast x-ray study disclosed a fistula between

Comparison of one-anastomosis gastric bypass and Roux-en-Y gastric bypass for treatment of obesity: a 5-year study.

BACKGROUND: Roux-en-Y gastric bypass (RYGB) is the gold standard in bariatric surgery. One-anastomosis gastric bypass (OAGB) has been reported to have equivalent or better weight loss, with added advantages of being technically easy, amenable to reintervention/reversal, and offering better food tolerance. OBJECTIVE: This study was undertaken to compare weight loss, metabolic syndrome outcome, complications, and long-term nutritional outcomes between the 2 procedures. SETTINGS: A high-volume, private-practice bariatric surgery center in India. METHOD: This retrospective study is based on pro

One Anastomosis Gastric Bypass-Mini Gastric Bypass with Tailored Biliopancreatic Limb Length Formula Relative to Small Bowel Length: Preliminary Results.

BACKGROUND: One Anastomosis Gastric Bypass-Mini Gastric Bypass (OAGB-MGB) is rapidly gaining popularity and is currently being performed by an increasing number of bariatric surgeons worldwide. However, excessive postoperative weight loss and malnutrition still remain a major concern regarding this procedure. The aim of this observational retrospective study was to investigate whether a tailored biliopancreatic limb (BPL) length relative to small bowel length (SBL) is superior to a fixed BPL length of 200 cm in terms of weight loss results and nutritional deficiencies in morbidly obese patient

Conversion of one-anastomosis gastric bypass to Roux-en-Y gastric bypass: short-term results from a tertiary referral center.

BACKGROUND: One-anastomosis gastric bypass (OAGB), also known as minigastric bypass, is an increasingly popular bariatric surgery option worldwide. While OAGB offers advantage in terms of procedure time and technical ease, revisional operations to correct complications may be necessary. OBJECTIVES: We aimed to describe the indications and perioperative outcomes for OAGB conversions to Roux-en-Y gastric bypass (RYGB) at a single-referral center. SETTING: Academic hospital, Abu Dhabi, United Arab Emirates. METHODS: All patients undergoing conversion from OAGB to RYGB from February 2016 throug

Comparison of nutritional deficiencies after Roux-en-Y gastric bypass and after biliopancreatic diversion with Roux-en-Y gastric bypass.

BACKGROUND: Patients undergoing either Roux-en-Y gastric bypass (RYGBP) or biliopancreatic diversion (BPD) with RYGBP are at risk of developing metabolic sequelae secondary to malabsorption. We compared the differences in nutritional complications between these two bariatric operations. METHODS: A retrospective analysis of a prospective database was done. From June 1994 to December 2001, 243 morbidly obese patients underwent various bariatric procedures at our institution. Of these patients, 79 (BMI 45.6 +/- SD = 4.9) who underwent RYGBP (gastric pouch 15 +/- 5 ml, biliopancreatic limb 60-80

One anastomosis gastric bypass (OAGB): a scoping review.

BACKGROUND: Obesity has become a global health crisis, with bariatric surgery being among the most effective interventions for treatment-resistant obesity. One Anastomosis Gastric Bypass (OAGB) has gained popularity for its technical simplicity and promising outcomes. However, concerns remain regarding long-term complications, especially bile reflux and gastroesophageal reflux disease (GERD). This scoping review aimed to synthesize the current literature on OAGB, focusing on four domains: percentage excess weight loss (%EWL), remission of metabolic and cardiovascular conditions, postoperative

Small Bowel Recurrent Intussusception Status Post Gastric Bypass.

Intussusception is an uncommon problem in the adult population. Interestingly, it is also a rare late complication after gastric bypass. We report the case of a 73-year-old woman with a history of Roux-en-Y gastric bypass (RYGB) in 2004 complicated by jejunal intussusception in 2011 for which a small bowel resection was performed. The patient presented to the emergency department (ED) with intractable abdominal pain and nausea. The diagnosis of intussusception was made by abdominal CT scan. Initial laparoscopic reduction of the small bowel intussusception resulted in recurrence within the firs

Esophagectomy for Cancer After One Anastomosis Gastric Bypass.

One anastomosis gastric bypass (OAGB) is growing in popularity, although it is potentially associated with biliary gastritis and gastroesophageal reflux esophagitis, with a potential rise in esophageal carcinoma. We describe the surgical management of a 53-year-old man with history of OAGB in whom biliary reflux and esophageal adenocarcinoma developed. We performed a minimally invasive Ivor Lewis esophagectomy, resected the sleeved stomach pouch, created a new conduit out of the remnant greater curve of the remnant stomach with blood supply from an intact gastroepiploic artery, and created an

Retrograde intussusception post Roux-en-y gastric bypass.

A 72-year-old female presented to the emergency department with a short history of abdominal pain and vomiting, having undergone laparoscopic Roux-en-y gastric bypass (RYGBP) 3 years earlier. Computed tomography imaging revealed intussusception causing small bowel obstruction, so she was taken emergently to theatre. Intra-operative findings were of a retrograde intussusception at the jejunojejunostomy. Retrograde intussusception in patients post RYGBP has a unique pathophysiology and management.

One-anastomosis gastric bypass with a short limb.

OBJECTIVE: To develop an acceptable method of surgical treatment of patients with obesity grade 1 and 2 in accordance with the following criteria: high safety, no risk or minimal risk of intestinal malabsorption, no limitations for postoperative gastrointestinal examination, no need for organ resection and surgical reversibility in case of necessity. MATERIAL AND METHODS: In accordance with the above-mentioned criteria, we have modified OAGB (one-anastomosis gastric bypass). A gastric tube was formed from a lesser curvature using 33Fr stomach catheter with a length of at least 25 cm. Manual r

Retrograde Intussusception after Roux-en-Y Gastric Bypass.

Retrograde intussusception (RI), although relatively uncommon, has been increasingly seen in adults post Roux-en-Y gastric bypass (RYGB) surgery. The exact mechanism for its occurrence remains unknown but several theories have attributed it to bowel persialtic dysmotility. The increase in bariatric surgery over the last decade has resulted in a proportionate increase in the number of cases of intussusception seen globally. We report a case of RI seven years following RYGB done for morbid obesity.

Weight Development in Children After Gastric Bypass Surgery.

Objective: More and more young obese women get pregnant after undergoing gastric bypass surgery (GBP) but little is known about weight development in their offspring. The first aim of this study was to investigate weight development of children whose mothers have undergone GBP before pregnancy and compare them to age specific reference values in Sweden. Second aim was to study the frequency of small for gestational age (SGA) in this population. Materials and methods: Weight of offspring (38 male and 28 female) where the mother had undergone GBP before pregnancy were studied from birth up to 18

Perforated duodenal ulcers after Roux-Y Gastric Bypass.

Even though the incidence of complicated peptic ulcer disease (PUD) has decreased in the last decades, it remains a condition with a significant mortality. Whilst diagnosis and treatment of PUD in morbidly obese patients can be challenging, patients with excluded segments - such as after Roux-Y Gastric Bypass (RYGB) - present an even greater problem, as the subsequent altered anatomy impedes the common modalities used for diagnostic and therapeutic measures. We report the cases of two patients after RYGB with perforated duodenal ulcers in the intention to highlight problems regarding diagnosis

Shoulder arthroplasty following gastric bypass, do complications follow?

INTRODUCTION: Previous reports have shown an increased risk of complications after arthroplasty in the obese population. It remains unclear if gastric bypass surgery prior to shoulder arthroplasty modifies the complication and failure rate. The purpose of this study is to assess the complication and re-operation rates following shoulder arthroplasty in this population. METHODS: Between 2002 and 2012, 39 shoulders with prior gastric bypass underwent shoulder arthroplasty (3 HA, 16 TSA, 20 RSA). The mean time from the gastric bypass to arthroplasty was 13 years (range, 0.7-32). Shoulders were f
