Prevalence of gastrointestinal disorders having an impact on tablet levothyroxine absorption: should this formulation still be considered as the first-line therapy?

PURPOSE: In patients with hypothyroidism, levothyroxine (LT4) is the treatment of choice, and tablets are the most commonly prescribed formulation. Despite multiple scenarios being reported in the literature with impaired tablet absorption and likely missed TSH targets, it is yet unclear what the implications are for clinical practice and the role of liquid solution (LS) and soft gel (SG) formulations. We have thus conducted a narrative review evaluating the prevalence within the general population of gastrointestinal disorders impacting tablet LT4 absorption. METHODS: PubMed and Google Schol

The mucosal redox gateway for dietary iron absorption: integrating gastrointestinal mucus, Fe(II)/Fe(III) speciation and ligand-centred valence tautomerism.

Non-haem dietary iron reaches the duodenum as a dynamic mixture of Fe(III) and Fe(II) complexes whose solubility and exchange kinetics are strongly influenced by pH, reductants, proteases, lipases, carbohydrases, and competing ligands. Canonical models place ferrireduction by duodenal cytochrome b and Fe(II) transport by divalent metal transporter 1 at the centre of absorption, but usually treat gastrointestinal mucus as a passive diffusion barrier. Evidence that mucins bind iron and other metal cations suggests a more active role. This review integrates established epithelial transport biolog

Modulatory activity of a bovine hydrolyzed collagen-hydroxyapatite food complex on human primary osteoblasts after simulating its gastrointestinal digestion and absorption.

Introduction: osteoporosis is the most prevalent bone disease and one of the main causes of chronic disability in middle and advanced ages. Conventional pharmacological treatments are still limited, and their prolonged use can cause adverse effects that motivate poor adherence to treatment. Nutritional strategies are traditionally based on supplementing the diet with calcium and vitamin D. Recent studies confirm that the results of this supplementation are significantly improved if it is accompanied by the intake of oral hydrolyzed collagen. Objective: to evaluate the possible in vitro osteoge

Physicochemical characteristics and gastrointestinal absorption behaviors of S-propargyl-cysteine, a potential new drug candidate for cardiovascular protection and antitumor treatment.

1. As a potential new drug candidate for cardiovascular protection and antitumor treatment, the physicochemical properties, gastrointestinal (GI) absorption behaviors and mechanisms of S-propargyl-cysteine (SPRC) were investigated in this study. 2. SPRC exhibited favorable solubility in aqueous media. The log P and log D values were low (≤1.93 ± 0.08). The pKa in the acidic and basic regions was 2.08 ± 0.02 and 8.72 ± 0.03, respectively. The isoelectric point was 5.40 ± 0.02. SPRC was stable in the rat GI fluids, and showed no obvious adsorption and metabolism in the rat GI tract. 3. SPRC disp

Comparative assessment on osteoprotective effects of bone collagen peptides from diverse animal sources: Insights from the characteristics of gastrointestinal digestion and absorption.

BACKGROUND: Although livestock and poultry bone collagen peptides (LBPs) have been reported to exhibit protective effects against osteoporosis, the in vitro osteogenic and in vivo anti-osteoporotic bioactivity of LBPs may vary depending on the animal sources, and the underlying mechanisms remain unclear. The present study aimed to investigate the divergence in the osteoprotective effects of yak (YBPs)/bovine (BBPs)/porcine (PBPs)/chicken (CBPs) bone collagen peptides, and to elucidate how gastrointestinal digestion and absorption characteristics contribute to their distinct osteoprotective mec

A new interpretation of salicylic acid transport across the lipid bilayer: implications of pH-dependent but not carrier-mediated absorption from the gastrointestinal tract.

Transport of several monocarboxylic acids across the lipid bilayer was examined in liposomes consisting of egg yolk phosphatidylcholine and cholesterol. In the presence of inward proton gradient, salicylic acid (SA) was taken up rapidly by liposomes showing overshoot, saturation and competitive inhibition phenomena. These carrier-mediated like profiles of SA uptake can be explained by assuming a very high permeability through the liposomal membrane of protonated SA. Protonated SA in the extraliposomal solution (pH 5.8) was taken up by liposomes rapidly, followed by a redissociation to anion ac

The gastrointestinal tract of the rock hyrax (Procavia habessinica). 2. Fluid flow, production of short chain fatty acids and absorption of water and electrolytes.

In the digestive tract of the rock hyrax (Procavia habessinica), water is absorbed from the small intestine and from the wider part of the connecting colon running between the caecum and the colonic sac. Only little water absorption occurs from these two fermenting chambers. The outflow from the caecum is much higher than from the colonic sac mainly due to the effective absorption in the connecting colon with its very large absorptive area. A significant lower osmolality was found in the caecal contents compared with the contents in the colonic sac. The Na/K ratio was 10.7 and 1.7, respectivel

Optimum roughage proportion in barley-based feedlot cattle diets: total tract nutrient digestibility, rumination, ruminal acidosis, short-chain fatty absorption, and gastrointestinal tract barrier function.

Cattle need physically effective fiber to promote rumination and maintain rumen health, but economics favor the use of low-roughage feedlot diets. The study investigated the optimum barley silage proportion in barley-based finishing diets. Apparent total-tract digestibility (4-d total fecal collection), chewing behavior (6-d video recording), ruminal pH (6-d indwelling pH recording), and fermentation (1 day, sampling 0, 3, 6, 12, and 18 h postfeeding), short-chain fatty acid (SCFA) absorption (washed reticulo-rumen technique), gastrointestinal tract barrier function (marker infusion), and bloo

The gastrointestinal absorption of plutonium and americium in rats and guinea pigs after ingestion of dusts from the former nuclear weapons site at Maralinga: implications for human exposure.

The gastrointestinal absorption of plutonium and americium present in dusts from the Maralinga test sites in South Australia has been measured as an input to dose assessments. The materials studied were from three different areas, designated Taranaki (Q380II), TM100 (10/2) and N. Plume (26). The three dusts were fed to groups of rats, mixed with their normal food. The Taranaki and TM100 dusts were also fed to guinea pigs, as a suspension in water. Expressed as fractional absorption from the gastrointestinal tract (f1), the values obtained for plutonium were 2 x 10(-6) and 8 x 10(-6) for Tarana

Gastrointestinal absorption of a new reversible proton pump inhibitor, YJA-20379-8, and its pharmacokinetics after oral administration in acetic acid-induced gastric ulcer in rats.

The absorption of YJA-20379-8 (3-butyryl-4-[5-(R)-(+)-methylbenzylamino]-8-ethoxy-1,7-naphthyrid ine) from various rat gastrointestinal segments was evaluated using in-situ closed-loops. The pharmacokinetics of the drug were also evaluated after oral administration to rats with acetic acid-induced gastric ulcer (AIURs). The concentrations of YJA-20379-8 in the biological samples were analyzed by HPLC. The absorption of YJA-20379-8 from stomach and jejunum was fast, but approximately 50% of the drug was recovered from each segment at 24 h. The total areas under the plasma concentration-time cur

Exploring Release, Isomerization, and Absorption of Cypermethrin in Pacific Oysters (Crassostrea gigas) with Different Processing Methods during In Vivo Digestion: Insights from a Gastrointestinal Tract Quantitative Tracing Method.

Cypermethrin (CP) is a neurotoxic insecticide found accumulated in oysters, one of the most commonly consumed seafoods, posing potential health risks to the human body. We designed a gastrointestinal tracing method allowing for accurate quantification of the propulsion of chyme and further established the mouse in vivo digestion model to explore the behavior of CP in the digestion of raw, steamed, and roasted oysters. The results showed that bioaccumulation of CP in oysters may be accompanied by the biotransformation of CP. Thermal processing decreased both the CP content in oysters and its bi

The Adaptation of the QV600 LLI Milli-Fluidics System to House Ex Vivo Gastrointestinal Tissue Suitable for Drug Absorption and Permeation Studies, Utilizing MALDI MSI and LC-MS/MS.

Careful formulation of pharmaceuticals for oral delivery is essential to ensure that the optimal amount of the active ingredient reaches its intended site of action. This chapter demonstrates how mass spectrometry can be used in conjunction with ex vivo tissue and an adapted milli-fluidics system to carry out a drug absorption study. MALDI MSI is used to visualize the drug within the small intestine tissue from the absorption experimentation. LC-MS/MS is used to complete a mass balance of the experiment and quantify the amount of drug that has permeated through the tissue.

Stomach absorption of intubated insecticides in fasted mice.

Eight 14C-labeled insecticides representing diverse chemical classes were intubated into fasted mice whose stomachs were ligated at the pylorus. Absorption through the stomach was measured at 3 time intervals over a 60-min period and compared to similar absorption studies in the entire gastrointestinal tract. The percent of stomach absorption (as contrasted to total gastrointestinal absorption) varied from 29% (carbaryl) to 10% (nicotine). Distribution following stomach absorption was found to be similar to that in the gastrointestinal tract.

Dietary fructose and gastrointestinal symptoms: a review.

It has been proposed that fructose may cause or aggravate symptoms in patients with functional gastrointestinal disorders. Fructose is commonly used to sweeten processed foods, and the prevalence of incomplete fructose absorption (25 g, 10%) in healthy subjects is as high as 50%. The only controlled study that has been performed did not demonstrate a higher prevalence of fructose-induced gastrointestinal symptoms or incomplete fructose absorption in patients with functional gastrointestinal disorders. The amount and concentration of fructose used to evaluate absorption by breath testing has va

An analysis of the biopharmaceutical behaviour of proton pump inhibitors with different physicochemical properties.

At present, little information on the biopharmaceutical behaviour of proton pump inhibitors (PPIs) describing their absorption and biodistribution in vivo has been reported because the extreme instability of PPIs in the gastrointestinal environment makes it difficult to analyze such behaviour. In this work, a modified rat in situ intestinal perfusion model was employed to investigate absorption in the gastrointestinal tract and subsequent biodistribution of several PPIs (ilaprazole, esomeprazole and rabeprazole), which have different physicochemical properties. Our data indicated that PPIs exh
