Our previous experience with vertical (nonbanded) gastroplasty proved disappointing because of unsatisfactory maintenance of weight loss. Vertical banded gastroplasty seemed to be an attractive alternative operation because it provided an externally reinforced (banded) stoma that would not enlarge over time. In this study, our aim was to determine prospectively the results of vertical banded gastroplasty used as a primary, weight-reducing procedure in patients with morbid obesity. Seventy consecutive patients with morbid obesity (mean weight, 139 kg), all of whom had obesity-related complicati

Between 1979 and 1982, 162 patients underwent horizontal gastroplasty in which polypropylene mesh (Marlex) was used to support the greater curve channel. There were no deaths and early weight loss was good. The operation was stopped because of late weight gain. Marlex erosion was recognized by endoscopy in 10 (6%) patients between 27 and 60 months after operation. Two other patients with Marlex erosion had the gastroplasty performed elsewhere. The patients' symptoms were abdominal pain, vomiting or weight gain. The Marlex was often difficult to resect because the inflammatory mass was adherent

INTRODUCTION: Endoscopic Sleeve Gastroplasty (ESG) is proven to induce sustained weight loss and improvement of obesity-related comorbidities. ESG is performed using endoscopic full-thickness sutures leading to gastric restriction and shortening. While several studies have described technical variations in ESG and their potential impact on outcomes, the effect of ESG on gastric volume reduction has not yet been quantified. This study aims to objectively measure stomach volume reduction before and after ESG using image-based assessment.
METHODS: Videos of ESG procedures were collected at a sin

The study aimed to evaluate the feasibility of using cone-beam computed tomography (CBCT) for gastric volume assessment before and after endoscopic sleeve gastroplasty (ESG), focusing on patient radiation dose. Ten patients scheduled for ESG were prospectively enrolled. Each patient underwent three CBCT scans under anesthesia: two scans following CO2 insufflation of the gastric lumen-one pre-ESG and one post-ESG-and a third scan post-ESG after gastric distension with Gastrografin. Image quality was evaluated for its adequacy in generating three-dimensional reconstructions and calculating gastr

BACKGROUND: Bariatric weight-loss surgery rates are increasing internationally. Endoscopic sleeve gastroplasty (ESG) is a novel, minimally invasive endoscopic procedure thought to mimic some of the effects of a more common surgery, laparoscopic sleeve gastrectomy (LSG). Patient factors affecting procedural choice are unexplored.
OBJECTIVE: This formative study aimed to determine the preoperative and early postoperative characteristics of adults matched for age, sex, and BMI who chose ESG versus LSG.
METHODS: This prospective cohort study recruited ESG and matched LSG adults in Australia. Pre

The most effective and durable treatment for obesity is bariatric surgery. However, less than 2% of eligible patients who fulfill the criteria for bariatric surgery undergo the procedure. As a result, there is a drive to develop less invasive therapies to combat obesity. Endoscopic bariatric therapies (EBT) for weight loss are important since they are more effective than pharmacological treatments and lifestyle changes and present lower adverse event rates compared to bariatric surgery. Endoscopic sleeve gastroplasty (ESG) is a minimally invasive EBT that involves remodeling of the greater cur

Laparoscopic conversion of vertical banded gastroplasty (VBG) into Roux-en-Y gastric bypass is a technically demanding procedure. This article demonstrates the feasibility of conversion of failed VBG to Roux-en-Y gastric bypass (RYGBP).We have a series of 15 patients who underwent conversion from VBG to RYGBP, and the results were satisfactory regarding the percent excess weight loss and the resolution of the comorbidities, without a significant increase in the complications. In patients with a difficult anatomy below the cardia, dividing the esophagus just above the esophagogastric junction a

A new paradigm in the treatment of obesity and metabolic disease is developing. The global obesity epidemic continues to expand despite the availability of diet and lifestyle counseling, pharmacologic therapy, and weight loss surgery. Endoscopic procedures have the potential to bridge the gap between medical therapy and surgery. Current primary endoscopic bariatric therapies can be classified as restrictive, bypass, space-occupying, or aspiration therapy. Restrictive procedures include the USGI Primary Obesity Surgery Endolumenal procedure, endoscopic sleeve gastroplasty using Apollo OverStitc

OBJECTIVE: To evaluate the safety and feasibility in human subjects of a new transoral restrictive procedure for the treatment of obesity.
METHODS: The protocol was approved by the institutional review boards (IRBs) of both centers involved, and all patients gave informed consent. Patients met established inclusion criteria for bariatric surgery. The TOGa system (Satiety Inc., Palo Alto, CA), a set of transoral endoscopically guided staplers, was used to create a stapled restrictive pouch along the lesser curve of the stomach. Patients were hospitalized overnight for observation and underwent

BACKGROUND: Morbidly obese patients who undergo purely restrictive bariatric operations may fail to maintain satisfactory long-term results. In an attempt to achieve the best possible outcome after restrictive procedures, we have employed preoperative selection criteria and are following this selected patient group over time in order to evaluate long-term success.
MATERIALS AND METHODS: From June 1994 through August 2000, 166 morbidly obese patients underwent various bariatric procedures at our institution. Of these patients, 35 underwent vertical banded gastroplasty (VBG) based on selection

BACKGROUND: A total of 30 consecutive morbidly obese patients, six males and 24 females, who underwent vertical banded gastroplasty (VBG) between January 1992 and December 1994 and were followed up by endoscopy and biopsy were included in this study with the aim to determine the short- and mid-term complications and to investigate alterations in esophageal, gastric, and duodenal mucosa after surgery.
METHODS: All patients underwent endoscopy before operation. Postoperatively, 28 patients were reendoscoped at 6 months, 26 at 12 months, and 22 at 18 months. Biopsies were taken from the lower pa

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is an established endoscopic bariatric therapy. Two main platforms are currently used in clinical practice: the OverStitch Endoscopic Suturing System (OESS) and the triangulation Endomina System (ES). Comparative data regarding their technical performance and suture durability remain limited. This experimental study aimed to compare both systems in terms of usability, safety, and durability of full-thickness sutures.
METHODS: In this prospective comparative animal study, six adult minipigs were randomized to ESG using either OESS or ES (three a

Obesity is a major global health issue, contributing to numerous comorbidities such as type 2 diabetes mellitus, hypertension, and cardiovascular diseases. While bariatric surgery remains the gold standard for treating severe obesity, its invasive nature has led to the development of minimally invasive alternatives. Endoscopic sleeve gastroplasty (ESG) is a promising, minimally invasive procedure for weight loss, offering significant weight reduction without the need for surgical incisions. This editorial evaluates the efficacy and safety of ESG, highlighting its weight loss outcomes, safety p

OBJECTIVE: To assess the outcome of a laparoscopic wedge-fundectomy Collis gastroplasty for a short esophagus during fundoplication and hiatal hernia repair.
BACKGROUND: The Collis gastroplasty provides a surgical solution for a foreshortened esophagus but has been associated with postoperative dysphagia and esophagitis.
METHODS: We identified 150 patients who underwent a Collis gastroplasty from 1998 to 2012, and of these, 85 patients underwent laparoscopic procedures using the wedge-fundectomy technique.
RESULTS: The median age of the 85 patients (42 men/43 women) was 66 years (range, 37-

Patients who have undergone bariatric surgery and present with upper abdominal symptoms pose a diagnostic and management challenge. This is a case report of a 53-year-old lady who presented a number of years after vertical banded gastroplasty with upper abdominal pain and weight gain. Radiological investigation demonstrated a large para-esophageal hernia including the stapled area of the stomach, but with a staple-line dehiscence. She successfully underwent repair of the hiatus hernia and conversion to a Roux-en-Y gastric bypass resulting in resolution of the abdominal pain and weight loss.

BACKGROUND AND AIMS: Diffuse corrosive gastric stricture is a relatively rare entity, and gastric ablative procedures are traditionally recommended for it. We emphasize the importance of preservation of a cicatrized stomach and describe its augmentation using a segment of transverse colon.
CASE REPORT AND PROCEDURE: A young female with a history of corrosive acid ingestion presented to our surgical unit with nonbilious vomiting following meals, consistent weight loss and features of gastric outlet obstruction. A barium study revealed a small-capacity stomach with pyloric stricture. We planned

BACKGROUND: Transoral endoluminal gastroplasty (EG) by the Bard Endocinch device is available for the treatment of gastroesophageal reflux disease (GERD). This study assessed the early (<or=12 months) outcomes in patients undergoing EG performed by one gastroenterologist compared with another set of patients referred by the same gastroenterologist for laparoscopic antireflux surgery (LAS) at a foregut surgery center.
METHODS: From June 2000 to July 2002, 87 consecutive patients cared for by a single gastroenterologist were diagnosed with refractory GERD and underwent either EG (n = 47) or ref

A 46 year old man was operated on for an oesophageal carcinoma. Total oesophagectomy and gastroplasty were carried out. Postoperatively, a fistula developed between the gastroplasty and the right main bronchus. This led to respiratory impairment, with pulmonary infection, pleural effusion and hypoxia; the patient could not remain supine. A cervical oesophagostomy was decided on; to carry it out, a cervical plexus block was preferred to a cervical epidural anaesthesia because of the lesser respiratory and haemodynamic effects of the former technique. The procedure was well supported by the pati

BariClip gastroplasty is an emerging bariatric technique designed to preserve gastric anatomy while providing restrictive weight loss. While preliminary reports have shown promising outcomes, the variety of complications remains incompletely defined. We report the case of a woman in her late 40s with BMI > 35 and a medical history of hypothyroidism, who underwent elective BariClip placement as a primary bariatric intervention. On the second postoperative day, she presented with abdominal discomfort, nausea, and distention. A computed tomography scan revealed inferior slippage of the BariClip a
