Situated vulnerabilities and resiliencies associated with lifetime and past 6-month suicide attempt among transgender and gender diverse primary care patients.

Transgender, nonbinary, and gender diverse (TGD) adults experience elevated suicide attempt (SA) risk relative to cisgender adults, yet modifiable vulnerabilities and resiliencies remain understudied. This cross-sectional study examined correlates of lifetime and past 6-month SA among TGD adults receiving primary care. Electronic surveys were completed by 2131 TGD patients at two urban health centers, assessing demographics, gendered contexts, social determinants, interpersonal violence, mental health, and protective factors. Bivariate and multivariable log-Poisson generalized estimating equat

Transphobia in the United Kingdom: a public health crisis.

BACKGROUND: The moral panic surrounding trans, non-binary, and gender diverse (TGD) lives in the United Kingdom (UK) has been incited by high-level political and government actors and exacerbated by pervasive misinformation in social and press media. This hostile environment contributes to increasing interpersonal violence and social exclusion experienced by TGD people. Structural and interpersonal discrimination—conceptualised as minority stress—is understood to elicit physiological and psychological stress responses that predispose TGD individuals to a range of adverse health outcomes, inclu

Healthcare discrimination, minority stress, psychological distress, and quality of life among transgender and gender diverse people in Chile.

BACKGROUND: Transgender and gender-diverse (TGD) individuals experience elevated psychological distress and poorer quality of life (QoL) due to pervasive stigma and discrimination. Minority stress theory posits that distal (e.g., discrimination) and proximal stressors (e.g., internalized stigma) predict adverse mental health outcomes, whereas protective factors such as community connectedness foster resilience. However, empirical data from contexts in the global south, such as Chile, remain scarce. AIM: This study assessed the impact of minority stress and healthcare discrimination on psychol

State racial and LGBTQ+ equality: Moderators of prospective suicide risk pathways among young ethnoracially minoritized lesbian and bisexual women and nonbinary individuals.

Ethnoracially minoritized LGBTQ+ young people demonstrate greater suicide risk than their non-Hispanic White peers, yet limited research exists examining state equality protective factors that can reduce risk, especially among lesbian and bisexual women/nonbinary individuals assigned female at birth (LBWNBI). Guided by a strengths-based approach and integrated theoretical model, we used an observed variable path analysis to examine moderating effects of state racial and LGBTQ+ equality, and interactions between these factors and individual- and interpersonal/community-level protective factors,

[Gender Minority Youth in Pediatric Care: A Survey of Pediatricians].

BACKGROUND: In recent decades, an increasing number of gender-nonconforming children and adolescents have been presenting to healthcare system worldwide. The pediatric practice is often the first point of contact for those affected. To date, it is unclear what challenges and difficulties pediatricians face in caring for gender-variant children and adolescents. METHODS: Qualitative interviews were conducted using a semi-structured guideline to analyze the care situation from the perspective of practicing pediatricians. Sample: 32 German pediatricians in private practice. RESULTS: Pediatrician

Exploring the use of gender-inclusive language amongst health care students and staff in obstetrics and gynaecology.

INTRODUCTION: Gender-inclusive language is increasingly recognised as essential in health care to ensure respectful and equitable care for transgender and gender-diverse individuals. However, the adoption of gender-inclusive language in Obstetrics and Gynaecology (O&G) may vary across generations and hierarchical levels, and the perspectives of students and staff on its use remain underexamined. This study aimed to explore how O&G learners and clinicians understand and use gender-inclusive language. METHODS: An exploratory qualitative study was conducted in a UK teaching hospital's O&G depart

Building a Sexual and Reproductive Health Module for Implementation in Early Psychosis Programs: A Co-creation Process: Création d'un module sur la santé sexuelle et reproductive à mettre en œuvre dans les programmes d'intervention précoce en cas de psychose : un processus de création conjointe.

BACKGROUND: Individuals with psychosis are at risk for adverse sexual and reproductive health (SRH) experiences, and for receiving suboptimal SRH care. Early psychosis intervention (EPI) programs offer an opportunity to address SRH early in the illness course, yet SRH is not currently part of routine EPI care. This project sought to develop a SRH module for women, transgender, and gender-diverse individuals for delivery within EPI care. METHODS: To develop the SRH module, an integrated knowledge translation frame was used, informed by a community-based participatory action research approach.

Caring for Gender Diverse Youth With Duchenne Muscular Dystrophy: A Multisite Case Series.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a progressive neuromuscular condition requiring long-term multidisciplinary care. Endocrine management often includes pubertal induction with testosterone for delayed puberty, which commonly occurs in individuals receiving chronic glucocorticoid therapy. Although gender diversity is increasingly recognized among adolescents, gender identity and gender-affirming care have not been well described among individuals with DMD. METHODS: We conducted a multi-institutional, retrospective case series across pediatric neuromuscular centers in the United

Formative Research to Identify Contributors to Risky Drinking and Adapt an Alcohol Reduction Intervention Among Young Adults Vulnerable to HIV: Qualitative Focus Group Study.

BACKGROUND: Across populations, risky drinking has been demonstrated to increase HIV risk behaviors. This is of special concern for sexually minoritized cisgender men and gender-diverse young adults (aged 18-34 years), who report greater incidence of hazardous drinking (as defined by the Alcohol Use Disorders Identification Test - Consumption criteria) and HIV compared to their heterosexual and/or cisgender peers. OBJECTIVE: This study aimed to examine alcohol perceptions, patterns of use, and the role that anti-LGBTQ+ (lesbian, gay, bisexual, transgender, queer) policies and discrimination p

Self-Sampling for Cervical Cancer Screening in Transmasculine and Nonbinary People.

Nearly all cases of cervical cancer are caused by persistent infection with high-risk human papillomavirus (HPV), and many cases of invasive cancer have been attributed to lack of screening. Transmasculine and nonbinary patients are more likely to be underscreened than cisgender women for a variety of reasons, and high-risk HPV self-sampling has been proposed as a method to increase screening uptake in this population. This narrative review is among the first to assess HPV self-sampling as a cervical cancer screening modality in transmasculine and nonbinary populations. Self-sampling for HPV d

Testing the tripartite influence model among Chinese transgender and gender-diverse adults by gender identity.

Transgender and gender-diverse (TGD) people face elevated risks for eating and body image disturbances relative to their cisgender counterparts. The tripartite influence model has been widely validated in general populations; however, there is limited evidence on its applicability among TGD adults. Existing studies have also often aggregated individuals with different gender identities and have been primarily conducted in Western cultural contexts. Accordingly, the suitability of the tripartite influence model in non-Western cultural contexts (e.g., China) remains unknown. To fill these gaps,

Impact of Anticipated Bias from Healthcare Professionals on Perceived Successful Aging Among Transgender and Gender Nonconforming Older Adults.

PURPOSE: Transgender and gender nonconforming (TGNC) persons have routinely experienced high rates of violence and discrimination. Discrimination in healthcare can affect the ability of TGNC persons to age successfully as it often deters them from seeking care. The purpose of this study was to determine if anticipation of bias from healthcare professionals, as well as other variables, predicted perceived successful aging in a sample of TGNC adults. METHODS: A total of 384 (of the original 1963) participants older than 50 years completed the relevant parts of an 83-item online survey as part o

Health ramifications of antitransgender legislation: Trends in mental distress from 2016 to 2023 in a nationally representative sample of transgender adults.

Laws and policies that target and restrict transgender- and gender-nonconforming (TGNC) individuals are increasing in the United States. Despite limited prior research, studies using nationally representative samples are needed to examine how these policies relate to TGNC mental health. This study used two data sources: the Behavioral Risk Factor Surveillance System for population mental health metrics and the Movement Advancement Project policy tallies. Using pooled annual state-based data from probability-based samples of adults from 2016 to 2023, we examined differences in prevalence and te

Metabolic effects of testosterone therapy in transgender and gender diverse individuals with polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) affects 10% to 13% of individuals assigned female at birth (AFAB), although a greater prevalence is reported in transgender/gender diverse individuals (TGD) (23.8-38.8%). Testosterone therapy (TT) in TGD individuals is linked to worsening cardiovascular risk factors (CVRFs). Whether TT induces more adverse cardiometabolic changes in TGD with PCOS than TGD without PCOS remains uncertain. OBJECTIVE: To compare the effects of TT on CVRF in TGD with and without PCOS. METHODS: We retrospectively reviewed 373 records to identify 62 TGD with PCOS (withes

HIV and Other Sexually Transmitted Infections Among Men and Transgender/Gender Expansive Individuals Who Have Sex with Men in Kazakhstan: Associations with HIV Stigma, Internalized Homophobia, and Community Connectedness.

Since 2010, Kazakhstan has experienced an increase in HIV transmission. Men and transgender/gender-expansive people who have sex with men (MSM and TSM) carry a disproportionate HIV burden. Building on Paine et al. (2021), which linked HIV testing to stigma, internalized homophobia, and sexual- and gender-expansive (SGE) community connectedness-this brief report examined biologically confirmed HIV and other sexually transmitted infections (STIs). SGE connectedness was significantly associated with HIV infection, while HIV stigma and internalized homophobia were significantly associated with STI

Evaluating the Alliance against Violence and Adversity Online Training Program: an online curriculum to support children, women, and gender-diverse people.

BACKGROUND: Gender-based violence (GBV) and adverse childhood experiences (ACEs) are Canadian public health concerns and human rights violations that inequitably impact the health of children, women, and gender-diverse people. The Alliance against Violence and Adversity (AVA) developed an Online Training Platform (AVA Online) to facilitate learning about GBV, ACEs, implementation science, and equity, diversity, inclusion, and accessibility for students, academics, and community partners. This study evaluated the first iteration of AVA Online using mixed-method survey data from platform users.
