CONCLUSIONS: Subtotal facial nerve decompression seems effective to prevent further episodes of facial palsy and promote facial nerve recovery for recurrent facial palsy in Melkersson Rosenthal syndrome (MRS). The main inflammatory sites of recurrent facial palsy in MRS may be the mastoid segment, tympanic segment, geniculate ganglion, and labyrinthine segment.
OBJECTIVE: To present our surgical experience in preventing further episodes of facial palsy and improving facial nerve recovery of patients with recurrent facial palsy in MRS.
METHODS: We performed transmastoid subtotal facial nerve

Although Ramsay Hunt syndrome is one of the most important diseases causing peripheral facial palsy, the detailed pathology of the disease in the intratemporal facial nerve remains unclear. The purpose of this study was to increase knowledge of the pathogenesis of the syndrome by means of surgical findings. Between April 1976 and March 1997 we performed subtotal decompression of the facial nerve in 74 patients with severe Ramsay Hunt syndrome. The grade of nerve swelling was assessed using a microscope and recorded in a standardized form. The relationships between nerve swelling, the timing of

BACKGROUND: The relative afferent papillary defect (RAPD) is an important parameter for quantifying the loss of neuronal function in asymmetric optic nerve disease. We wanted to assess its correlation with retinal ganglion cell loss, estimated from visual field defects, using a recently described method.
METHODS: In 36 patients with unilateral optic nerve disease, the RAPD was measured using an automated, computerized infrared pupillometer. The percentage of ganglion cell loss was estimated with clear templates.
RESULTS: We found a highly significant correlation between the RAPD and estimate

Rats with geniculate laminae that run perpendicular to the lines of projection are described. Earlier reports of laminae parallel to the lines of projection were based on mammalian mutants that may be relatively common. In these mutants, the chiasmatic course of axons arising in a patch of retinal ganglion cells is wrongly specified.

Intracranial malignant nerve sheath tumors rarely arise in the central nervous system. They usually arise from the cranial nerves, though rare cases of intraparenchymal lesions have also been reported. We report a case of malignant nerve sheath tumor located in the right middle cranial fossa. Preoperatively, the lesion resembled a meningioma arising from the petrous temporal bone. The lesion was completely excised. Postoperatively, the patient developed righted-sided complete facial nerve palsy. Histopathology and immunohistochemistry revealed the lesion to be a malignant nerve sheath tumor. T

BACKGROUND: Avulsion of the abducens nerve in the setting of geniculate ganglion injury after temporal bone fracture is unreported previously. We discuss clinical assessment and management of a patient with traumatic avulsion of cranial nerve (CN) VI in the setting of an ipsilateral CN VII injury after temporal bone fracture and call attention to this unusual injury.
CASE DESCRIPTION: A 26-year-old man suffered a temporal bone fracture after a motor vehicle accident and developed diplopia and right-sided facial droop. Six weeks after the accident, the patient was readmitted with worsening dip

This article describes the pathophysiology, diagnosis, treatment, and outcomes of primary tumors of the facial nerve. These tumors include facial nerve schwannomas, geniculate ganglion hemangiomas, glomus facialis, and granular cell tumors. Although these tumors are rare, collected data help to form a consensus with regard to optimal treatment methods.

Ramsay Hunt syndrome (RHS) occurs due to reactivation of latent Varicella Zoster Virus (VZV) infection in the geniculate ganglion of the facial nerve. Major clinical symptoms include ipsilateral facial paralysis, otic pain, and herpetic vesicles (rashes) along the nerve with accompanying ear pain. Rarely clinical findings include retrograde transaxonal spread of the virus from the ganglion into the brain parenchyma with developing the encephalitis or multiple cranial nerve involvement. We describe here a patient with both RHS along with complicating brainstem encephalitis developed due to the

Cerebrospinal fluid (CSF) fistulae originating from the fallopian canal of the facial nerve is hypothesized to arise due to atypical patterns of subarachnoid space extension into the geniculate ganglion or more distal regions along the intratemporal course of the facial nerve, but its pathogenesis remains poorly understood. Although a rare etiology of CSF fistulae of the temporal bone, there are significant clinical ramifications due to the risk of recurrent meningitis, difficulty in identifying the anatomic location of the CSF leak, and technical challenges associated with surgical repair. We

Varicella-zoster infections commonly are seen in clinical practice; however, Ramsay Hunt syndrome caused by reactivation of latent varicella-zoster virus in the geniculate ganglion is rare. We report the case of a 30-year-old woman who developed Ramsay Hunt syndrome and also review the characteristic features of this disease.

The innervation of the rat external auditory meatus has contributions from several nerves. Fluoro-gold, a retrograde neuronal tracer, was used to determine the relative contributions from neurons in cranial and spinal ganglia, and to distinguish any difference in the sensory and motor innervation between the outer cartilaginous and inner bony portions. The following ganglia were examined: the trigeminal, geniculate, glossopharyngeal, vagal, superior cervical, and dorsal root (C2-C4) ganglia. All selected ganglia demonstrated innervation of the external ear canal, particularly the trigeminal, g

OBJECTIVES: To find out if the ratio of facial nerve to facial canal diameter plays any role in the etiopathogenesis and grade of Bell's palsy.
PATIENTS AND METHODS: Twenty-nine consecutive patients (16 females, 13 males) diagnosed with unilateral Bell's palsy were enrolled into the study. At admission, 5 patients were grade V, 11 were grade IV, 11 were grade III and 2 were grade II. The grade of Bell's palsy was documented by House-Brackmann facial nerve grading system at admission, 15th day, 1st month and 3th month. Temporal MRI and CT scans were obtained at the time of diagnosis. Diameter

OBJECTIVE: To evaluate the risk of the facial nerve injury during operations for chronic otitis media without cholesteatoma by analysis of the intraoperative findings of the facial canal dehiscence.
METHODS: We retrospectively reviewed operative findings of 152 patients who underwent tympanoplasty with mastoidectomy for chronic otitis media. We examined every segment of the facial canal from the geniculate ganglion to the mastoid segment. Facial canal dehiscence was confirmed by palpation with a Rosen pick after inspection with a surgical microscope.
RESULTS: The rate of facial canal dehisce

Ramsay hunt syndrome[1] is a varicella zoster virus infection of the geniculate ganglion of the facial nerve. It is typically associated with a red rash and blister (inflamed vesicles or tiny water filled sacks in the skin) in or around the ear and eardrum and sometimes on the roof of the mouth or tongue. Corticosteroid, oral acyclovir, and anticonvulsant are used for treatment of this. In addition to this sympathetic neural blockade via stellate ganglion block is used to prevent facial nerve damage and relieve symptoms. We present a case of Ramsay hunt syndrome in which pain and symptoms are

The middle cranial fossa approach to lesions of the geniculate ganglion and internal auditory canal preserves cochlear function and affords access to the lateral internal auditory canal. The labyrinthine portion of the facial nerve tends to course near the basal turn of the cochlea, just beneath the middle cranial fossa floor, and is usually dissected in this approach. To determine the distance from the labyrinthine portion of the facial nerve to the basal turn of the cochlea, measurements were obtained in the temporal bones of 24 subjects (48 ears) 9 to 76 years of age. These subjects had no

The authors describe a rare coexistence of Ramsay Hunt syndrome with glossopharyngeal zoster in a 67-year-old patient. A trial of systematization of the nomenclature of Ramsay Hunt syndrome is suggested, on the basis of certain anatomophysiological data. It is concluded that in zoster involvement of the sensory elements in the geniculate ganglion and in otic ganglion should be accompanied by taste sensitivity disturbances, but often the patients fail to notice these disturbances and routine taste testing shows also no such tase impairment. Routine use of electrogustometry is postulated since t
