Characteristics of electroencephalographic changes induced by different hypnotics in elderly patients: a narrative review.

Aging is associated with widespread structural and functional changes in the brain including reduced neural plasticity, slower information processing, and impaired network integration. These age-related alterations influence the brain's response to anesthetic agents, particularly electroencephalography (EEG) activity. This narrative review summarizes the characteristic EEG features induced by commonly used hypnotic agents such as propofol, inhaled anesthetics, dexmedetomidine, ketamine, and remimazolam in elderly patients and examines how aging modulates these responses. With increasing age, E

Precision Sedation for Gastrointestinal Endoscopy in Older Adults: Brain Vulnerability and Recovery.

Older adults undergoing gastrointestinal endoscopy are particularly susceptible to oversedation, hypotension, hypoventilation, hypoxemia, and delayed recovery because physiological and cognitive reserve decline with age. Precision sedation provides a practical approach to these problems by tailoring drug selection, dose titration, monitoring, and supportive measures to the individual patient and the requirements of the procedure. Propofol remains widely used because of its rapid onset and predictable recovery, although cardiorespiratory depression remains dose dependent. Remimazolam and ciprof

[Clinical practice progress and prospects of "brain enhanced recovery after surgery" in elderly patients].

The concept of "brain enhanced recovery after surgery (Brain-ERAS)" has emerged against the backdrop of population aging, a renewed recognition of the profound consequences of postoperative cerebral complications, the growing accumulation of evidence-based clinical data, and the continuous refinement of the enhanced recovery after surgery (ERAS) paradigm. It represents a multimodal intervention strategy centered on postoperative cognitive recovery and neuroprotection in elderly surgical patients. In this article, we systematically review the latest advances in the clinical application of Brain

Some clinical aspects of geriatric surgery.

The relative frequency of surgical problems in the aged is discussed and some statistics based on experience are given. General rules in the management of the aged are suggested and the situation is analysed with stress on biological age and past influences. The increased importance in surgery of fluid loss as well as shock is discussed. A regimen for fluid replacement therapy is suggested. Problems in history-taking and the investigation of the elderly surgical patient are debated, and the value of ice as an anaesthetic agent for geriatric patients is discussed. Points about postoperative car

Spinal anesthesia in the percutaneous fixation of fragility fractures of the pelvis.

INTRODUCTION: The number of geriatric patients with a pelvic ring fracture is rising and minimal invasive fixation techniques are increasingly popular. The patient characteristics of these fragile patients are similar to those of patients with a proximal femur fracture. In the field of proximal femur fracture surgery spinal anesthesia is a very commonly used anesthetic technique in this more fragile patient population. METHODS: All patients were treated between January 2022 and May 2022 in the Amsterdam UMC location AMC in The Netherlands. The operations were performed by a surgeon who specia

Anesthesia for the elderly: selected topics.

PURPOSE OF REVIEW: With the graying of the Western population, there is a continuous increase in the proportion of elderly patients undergoing surgical procedures. Geriatric anesthesia is emerging from a 'subspecialty' to the mainstream of today's anesthesia and perioperative care. Much has been written on anesthesia for the elderly, but this review will concentrate on selected topics related to elderly care that represent current unresolved and pertinent issues for the care of the elderly surgical patient. RECENT FINDINGS: Postoperative cognitive dysfunction, cardiac diastolic dysfunction an

Comparison of Effects of PELD and Fenestration in the Treatment of Geriatric Lumbar Lateral Recess Stenosis.

PURPOSE: Although degenerative lumbar spinal stenosis (LSS) is increasingly being diagnosed in older people, there is much uncertainty about the appropriate operative treatment options. The objective of this study was to compare the outcome of percutaneous endoscopic lumbar decompression (PELD) versus fenestration for lumbar lateral recess stenosis (LRS) in geriatric patients over 75 years old. MATERIALS AND METHODS: This prospective controlled study was performed on 46 consecutive over aged patients with lateral recess stenosis who underwent either PELD or fenestration. Clinical data were re

Pro-Con Debate: Electroencephalography-Guided Anesthesia for Reducing Postoperative Delirium.

Postoperative delirium (POD) has significant implications on morbidity, mortality, and health care expenditures. Monitoring electroencephalography (EEG) to adjust anesthetic management has gained interest as a strategy to mitigate POD. In this Pro-Con commentary article, the pro side supports the use of EEG to reduce POD, citing an empiric reduction in POD with processed EEG (pEEG)-guided general anesthesia found in several studies and recent meta-analysis. The Electroencephalography Guidance of Anesthesia to Alleviate Geriatric Syndromes (ENGAGES) trial is the exception to this, and issues wi

Cold temperatures, hot risks: perioperative hypothermia in geriatric patients - a narrative review.

Aging adversely impacts thermoregulatory function, thereby increasing the risk of intraoperative hypothermia. Age-associated alterations-including diminished thermal perception, impaired autonomic responsiveness, reduced thermogenic capacity due to sarcopenia, and decreased cardiovascular adaptability, exacerbate the vulnerability to hypothermia. Concomitant comorbidities and polypharmacy further compromise thermal homeostasis in geriatric patients. Anesthetic agents compound this risk by lowering the thresholds for vasoconstriction and shivering and attenuating the magnitude of thermal respon

PURPOSE: To investigate the effect of esketamine on peripheral vascular resistance in elderly patients during anesthesia induction. PATIENTS AND METHODS: Eighty elderly patients undergoing elective surgery were randomly assigned to two groups (n=40 each): the esketamine group (Group A) received esketamine 0.3 mg/kg, while the control group (Group B) received an equal volume of normal saline. Both groups received the same standard anesthesia induction (etomidate, sufentanil, rocuronium). Anesthetic Techniques and Postoperative Cognitive Dysfunction in Older Adults: Current Evidence and Perioperative Strategies.

Background and Objectives: With the rising number of geriatric surgical patients, postoperative cognitive dysfunction (POCD) has become a major concern, linked to impairments in memory, attention, and executive function. POCD increases morbidity, prolongs hospitalization, and diminishes quality of life. This review examines the mechanisms underlying POCD, with emphasis on neuroinflammation, blood-brain barrier (BBB) disruption, and oxidative stress, and evaluates the impact of anesthetic techniques on cognitive outcomes in the elderly. Materials and Methods: This narrative review used a target

Cohort profile: PeRiOperative sTress risk assEssment and Clinical decision cohorT (PROTECT), a multi-center observational study based on real-world data.

BACKGROUND: Global surgical volumes continue to rise, yet postoperative morbidity and mortality remain substantial, particularly among geriatric patients. The scarcity of multi-center prospective perioperative cohorts with active follow-up and large-scale, high-quality data limits the understanding of risk profiles and hinders individualized perioperative management in this population. To address this gap, the PeRiOperative sTress risk assEssment and Clinical decision cohorT (PROTECT) was established by creating a dedicated perioperative data platform for geriatric patients. This cohort profil

The Effects of Switching from Sevoflurane to Short-Term Desflurane prior to the End of General Anesthesia on Patient Emergence and Recovery: A Randomized Controlled Trial.

While sevoflurane and desflurane have been regarded as inhalation agents providing rapid induction and emergence, previous studies demonstrated the superiority of desflurane-anesthesia compared to sevoflurane-anesthesia in the postoperative recovery in obese and geriatric patients. We investigated whether a short-term switch of sevoflurane to desflurane at the end of sevoflurane-anesthesia enhances patient postoperative recovery profile in non-obese patients. We randomly divide patients undergoing elective surgery (n = 60) into two groups: sevoflurane-anesthesia group (Group-S, n = 30) and sev

Preoperative assessment of the older surgical patient: honing in on geriatric syndromes.

Nearly 50% of Americans will have an operation after the age of 65 years. Traditional preoperative anesthesia consultations capture only some of the information needed to identify older patients (defined as ≥65 years of age) undergoing elective surgery who are at increased risk for postoperative complications, prolonged hospital stays, and delayed or hampered functional recovery. As a catalyst to this review, we compared traditional risk scores (eg, cardiac-focused) to geriatric-specific risk measures from two older female patients seen in our preoperative clinic who were scheduled for electiv

Incidence and predictors of postoperative cognitive dysfunction among elderly elective surgical patients at selected governmental hospitals in Southern Ethiopia, 2023 GC: a multicenter prospective single arm cohort study.

INTRODUCTION: The geriatric population is the fastest-growing age group and accounts for one-third of surgical patients. A growing body of evidence reveals that postoperative cognitive dysfunction (POCD) is the most common complication among older surgical patients. However, there is a wide variation in incidence and independent predictors globally. Furthermore, there is a lack of evidence in sub-Saharan Africa, including Ethiopia. OBJECTIVE: To assess the incidence and predictors of postoperative cognitive dysfunction among geriatric patients undergoing elective surgery in selected government

Effects of Anesthetic Technique on Outcomes After Hip Fracture Surgery in Geriatric Patients: A Retrospective Observational Study.

BACKGROUND: Hip fractures in older adults almost always require surgery under general or neuraxial anesthesia. While the influence of anesthetic technique on mortality has been studied extensively, its relationship with resource-related outcomes such as hospital length of stay is less well characterized, particularly in high-volume trauma settings. OBJECTIVE: To determine whether anesthetic technique is associated with hospital length of stay in geriatric patients undergoing hip fracture surgery (primary objective). Secondary objectives were to compare ICU admission and length of stay, and po

The effects of early femoral nerve block intervention on preoperative pain management and incidence of postoperative delirium geriatric patients undergoing trochanteric femur fracture surgery: A randomized controlled trial.

BACKGROUND: Hip fracture is a common clinical problem which causes severe pain in geriatric patients. However, severe pain following fracture may bring on mental disorders and delirium. A neuroinflammatory response with IL-6 and IL-8 has been shown to be associated with the pathophysiology of delirium. In this study, our primary hypothesis is that preoperative femoral nerve block (FNB) intervention in geriatric patients will more effectively attenuate pain following trochanteric femur fracture than the preoperative paracetamol application. Our secondary hypothesis is that interleukin levels (I

Full Endoscopic Transforaminal Decompression Surgery for Symptomatic Lumbar Spinal Stenosis in Geriatric Patients.

OBJECTIVE: Recently, full endoscopic transforaminal decompression surgery (FETDS) under local anesthesia has been used to treat symptomatic lumbar spinal stenosis (SLSS) in selective patients. Considering the minimal injury associated with this technique, it has a potential advantage for geriatric patients. In addition, combining thorough preoperative assessments and preparations with this technique to achieve a safer and more effective outcome for geriatric patients with SLSS is necessary. METHODS: Between June 2015 and September 2016, after thorough preoperative assessments and preparations

A comparison of ultrasound-guided erector spinae plane block and epidural anesthesia for postoperative recovery in elderly individuals following laparoscopic gastrectomy: a randomized controlled trial.

BACKGROUND: Erector spinae plane block (EspB) is increasingly being applied for laparoscopic radical gastrectomy as an analgesic supplement to general anesthesia. The aim of this study was to evaluate the analgesic efficacy and postoperative recovery outcomes of EspB and epidural anesthesia (EDA) in elderly patients (above 65 years-of-age) undergoing laparoscopic gastrectomy in order to determine the precise multimodal analgesia (PMA) that promotes postoperative recovery. METHODS: In total, 102 patients assigned for elective laparoscopic gastrectomy were classified into three groups: preoperat
