Clinical features and treatment strategies for vertex epidural hematoma: a systematic review and meta-analysis from individual participant data.

Vertex epidural hematoma (VEDH) is a rare extradural hematoma and often misdiagnosed because of its variety of clinical symptoms and characteristic location. Determining optimal timing and technique for VEDH surgery is difficult because of its midline location and atypical clinical course. This study aims to understand the clinical manifestations and current treatment strategies for VEDH. We searched the published literature regarding VEDH through PubMed and Google Scholar, and individual patient data (IPD) were obtained from the eligible articles. A systematic review and IPD meta-analysis wer

Early electroencephalogram does not reliably differentiate outcomes in post-hypoxic myoclonus.

OBJECTIVE: Prognostication in patients with post-hypoxic brain injury remains difficult; yet, clinicians are commonly asked to guide decisions regarding withdrawal of life support. We aimed to assess whether electroencephalogram (EEG) is a useful tool in predicting neurological outcome in patients with post-hypoxic myoclonus (PHM). DESIGN AND SETTING: This study was conducted as part of an internal hospital audit assessing therapeutic hypothermia in patients with hypoxic cardiac arrest. PARTICIPANTS: We identified 20 patients with PHM and evaluated their initial routine EEG. MAIN OUTCOME ME

Association between neurofilament light chain concentrations and outcomes in patients with moderate to severe traumatic brain injury: a systematic review and meta-analysis.

BACKGROUND: Moderate to severe traumatic brain injury (TBI) is associated with high rates of mortality and long-term disability. Accurate biomarkers are needed to predict longterm neurological outcomes and guide decision-making early after TBI. Neurofilament light chain (NfL), a structural protein of neurons, has emerged as a promising candidate, but its association with outcomes in this population remains uncertain. METHODS: We conducted a systematic review and meta-analysis to assess the association between blood or cerebrospinal fluid NfL concentrations and outcomes in adults with moderate

Epidemiology of head injuries in pedestrian-motor vehicle accidents.

Despite efforts of many countries to improve pedestrian safety, international reports show an upward trend in pedestrian-motor vehicle accidents. Although the most common cause of death of pedestrians is head injuries, there is a lack of knowledge on the epidemiology and characteristics of head injury in terms of the Glasgow Outcome Scale to be used for prevention. However, this study aimed to determine the epidemiology of pedestrian-motor vehicle accidents, the characteristics of head injury, and differences in the Glasgow Outcome Scale in terms of gender. In this retrospective analytical stu

Long-term prediction of traumatic brain injury outcome using the CBI-M framework.

BACKGROUND: Long-term functional and vocational outcomes after traumatic brain injury (TBI) remain difficult to predict, limiting prognostic counselling and rehabilitation planning. We aimed to evaluate the Clinical-Biomarker-Imaging-Modifier (CBI-M) framework for predicting functional and employment outcomes up to 10 years following TBI. METHODS: Retrospective cohort study of 2529 patients with TBI admitted for inpatient rehabilitation (1984-2024), contributing 5424 observations across 1, 2, 5 and 10 years post-injury. Prediction models used logistic regression with temporal validation, inco

Comparing survival and outcomes in isolated versus polytrauma-associated TBI: a retrospective cohort study.

BACKGROUND: One in four patients with moderate to severe traumatic brain injury (TBI) also has other body district injuries (OBD). The impact of OBD on mortality and disability is debated. This study compared outcomes of TBI patients with polytrauma (p-TBI) versus isolated TBI (alone-TBI) and identified outcome determinants, focusing on survival time and prognosis. METHODS: This single-center, retrospective cohort study analyzed 193 patients with moderate to severe TBI admitted to an ICU from 2011 to 2012. Patients were divided into p-TBI and alone-TBI groups. Decompressive hemicraniectomy after aneurysmal subarachnoid hemorrhage-justifiable in light of long-term outcome?

PURPOSE: Decompressive hemicraniectomy (DHC) is a potentially lifesaving procedure in refractory intracranial hypertension, which can prevent death from brainstem herniation but may cause survival in a disabled state. The spectrum of indications is expanding, and we present long-term results in a series of patients suffering from aneurysmal subarachnoid hemorrhage (SAH). METHODS: We performed a retrospective analysis of previously registered data including all patients treated for SAH between 2010 and 2018 in a single institution. Patients treated with decompressive hemicraniectomy due to ref

Incidence and effects of increased cerebral blood flow velocity after severe head injury: a transcranial Doppler ultrasound study II. Effect of vasospasm and hyperemia on outcome.

Cerebral blood flow velocities were measured in 50 severely head injured (Glasgow Coma Scale (GCS) 8 or less) patients using transcranial Doppler ultrasonography (TCD). Abnormally high TCD velocities were recorded in 35 patients; in 20 this was deemed to be due to vasospasm and in the other 15 to hyperemia. Patients who developed hyperemia also had the highest intracranial pressure (ICP) and the lowest cerebral perfusion pressure (CPP) of the three groups. Outcome was assessed at six months after injury using the Glasgow outcome scale and the disability rating scale. In the normal velocity gro
