Findings on MR sleep studies as biomarkers to predict outcome of genioglossus advancement in the treatment of obstructive sleep apnea in children and young adults.

OBJECTIVE: Genioglossus advancement, an operation to pull the tongue anteriorly, is a treatment of obstructive sleep apnea (OSA) secondary to glossoptosis. MRI predictors to identify which children will benefit from genioglossus advancement would be helpful for planning. We reviewed imaging findings on MR sleep studies as biomarkers to predict success or failure of genioglossus advancement in the treatment of OSA in children and young adults. MATERIALS AND METHODS: Twenty-eight patients who had undergone genioglossus advancement and preoperative MRI were identified. For each subject, genioglo

Ossicular fusion and cholesteatoma in auriculo-condylar syndrome: in vivo evidence of arrest of embryogenesis.

Auriculo-condylar syndrome (ACS) is a rare condition affecting first branchial arch structures. The types of hearing loss and temporal bone findings in ACS have not been reported. We describe a 14-year-old male with constricted pinnae, mandibular dysostosis, glossoptosis, a high-arched palate, hearing loss, and cholesteatoma. Computed tomography imaging demonstrated malleoincudal joint ankylosis. The fused malleoincudal complex was removed during mastoidectomy for cholesteatoma. Electron microscopy and histopathology of the joint suggested the fusion was congenital. This is the first report of

Brainstem dysfunction: a possible neuroembryological pathogenesis of isolated Pierre Robin sequence.

UNLABELLED: Pierre Robin sequence (posterior U-shape cleft palate, glossoptosis, retrognathia) (PRS) is a frequent and heterogeneous neonatal condition of obscure origin. We show here that orodigestive and cardiorespiratory functional disorders are very frequent in PRS and that these functional disorders, as well as anatomical and embryological data, argue for the involvement of brainstem dysfunction in the pathogenesis of some cases of isolated PRS. A total of 66 infants consecutively admitted for isolated PRS were followed-up with observations and investigations focused on their orodigestive

Glossopexy as an alternative to aortopexy in infants with repaired esophageal atresia and upper airway obstruction.

BACKGROUND/PURPOSE: Clinical manifestations of airway obstruction in infants with repaired esophageal atresia or tracheoesophageal fistula (EA/TEF) are attributed conventionally to tracheomalacia. In the current study, the authors tested the hypothesis that a retrodisplacement of the tongue (glossoptosis), by causing a functional upper airway obstruction (obstructive apnea/hypopnea), may play a role in the pathogenesis of the respiratory problems. METHODS: The records of 5 infants with repaired EA/TEF and respiratory symptoms treated by glossopexy, which serves to anchor the tongue forward, w

Auriculo-condylar syndrome.

The auriculo-condylar syndrome is caused by abnormalities of the first and second pharyngeal arches during embryonic development. Its inheritance follows the autosomal dominant pattern. Both familial and individual cases are reported in the literature. The syndrome is characterized by wide phenotypic variation, with affected individuals expressing clinical signs of variable severity due to variable expressivity of the responsible genes. Clinical signs of the syndrome include auricular malformation, hypoplasia of the mandibular condyles, anomalies of the temporomandibular joints, malocclusion,

[How I treat : airway obstruction in children with sequence of Pierre Robin].

Pierre Robin sequence (PRS) is described as a triad of micrognathia, glossoptosis and cleft palate. This sequence may be isolated or associated with a genetic syndrome in half of the cases. Main complications affect respiratory function and the upper digestive tract. Respiratory obstruction can arise as soon as in the first hours of life and may be life-threatening if no early appropriate management is implemented. PRS is phenotypically variable and the required treatment varies accordingly: some patients will be relieved with prone positioning alone while others will benefit from nasopharynge

Lehman syndrome: a new syndrome for pierre robin sequence.

Lehman syndrome, or lateral meningocele syndrome, is characterized by facial dysmorphism, multiple lateral meningoceles, and skeletal abnormalities. Only nine cases have been described. We present a case of a 2-year-old boy presenting with micrognathia, glossoptosis, and hypertelorism as well as associated severe obstructive sleep apnea. He was submitted to bilateral mandibular distraction with external nonresorbable devices to correct Pierre Robin sequence (PRS). Later, multiple lateral meningoceles were identified, and a diagnosis of Lehman syndrome was made. Lehman syndrome must be consider

Magnetic resonance sleep studies in the evaluation of children with obstructive sleep apnea.

Magnetic resonance (MR) sleep studies have been shown to be a useful tool in the evaluation and surgical planning of children with persistent obstructive sleep apnea. MR sleep studies include sequences to evaluate both static anatomic and well as dynamic motion information about the airway. The combination of the information about anatomic abnormalties and patterns of collapse can help in surgical decision making. The most common identified entities include recurrent enlargement of the adenoid tonsils, enlargement of the lingual tonsils, abnormal soft palate, glossoptosis, and hypopharyngeal c

Hypomandibular faciocranial dysostosis in consanguineous parents revealed by ultrasound prenatal diagnosis.

We report here the fourth case of hypomandibular faciocranial dysostosis (HFD). The diagnosis was made at birth on the association of severe retrognathia, microstomia, severe hypoglossia with glossoptosis, persistent buccopharyngeal membrane, median cleft palate, bifid uvula, down-slanting palpebral fissures, short nose with anteverted nares, laryngeal hypoplasia, and low-set ears. A severe microstomia and micrognathia were detected by ultrasound at 31 weeks of gestation. Interestingly, even though the present case exhibits many facial dysmorphic features characteristic of HFD, craniosynostosi

Lacrimal drainage anomalies in Pierre Robin sequence.

Pierre-Robin sequence consists of a classical triad of micrognathia, glossoptosis, and upper airway obstruction. The common ocular manifestations include congenital glaucoma, high myopia, maculopathy, and retinal detachment. Except for a mention of a nasolacrimal duct obstruction without many details, lacrimal drainage anomalies have not been reported earlier to the best of the authors' knowledge. The present case describes several lacrimal drainage anomalies including supernumerary puncta, canalicular wall hypoplasia, grossly dilated nasolacrimal duct, and complex congenital nasolacrimal duct

A Challenge in Perioperative Anesthetic Management: A Case Report of an Infant With Concurrent Ullrich Congenital Muscular Dystrophy and Pierre Robin Sequence.

Ullrich congenital muscular dystrophy (UCMD) is a severe form of progressive congenital muscular dystrophy, and it presents with severe hypotonia, recurrent respiratory infections, respiratory failure, and problematic airway management secondary to congenital anatomical abnormalities and high aspiration risk. Pierre Robin sequence (PRS) is a rare congenital syndrome presenting at birth with a triad of micrognathia, glossoptosis, and chronic upper airway obstruction leading to known difficult airway management and opioid sensitivity. Perioperative management of both UCMD and PRS requires carefu

Bilateral complex odontomas: a rare complication of external mandibular distraction in the neonate.

BACKGROUND: The authors present an unusual complication of external mandibular distraction in a child with bilateral complex odontomas in the posterior mandible. CLINICAL REPORT: The patient is a 4-year-old girl with a history of micrognathia, glossoptosis, and respiratory distress (Pierre-Robin sequence) treated with neonatal external mandibular distraction. She was referred to our service with bilateral amorphic radio-opaque masses in the body of the mandible. CONCLUSIONS: The etiology of odontomas is uncertain, but different theories exist, such as local trauma, infection, family history,

Auriculo-condylar syndrome is associated with highly variable ear and mandibular defects in multiple kindreds.

We report on 20 individuals in 4 kindreds with auriculo-condylar syndrome (ACS), an autosomal dominant disorder characterized by congenital auricular clefts, mandibular condyle hypoplasia, temporomandibular joint (TMJ) abnormalities, micrognathia, microstomia, and a round facial appearance with prominent cheeks. Affected individuals have varying degrees of glossoptosis, respiratory distress, masticatory abnormalities, and malocclusion. Data from these families and those previously described suggest that this is a unique disorder with widely variable expression, including lack of obvious extern

Tongue lip adhesion (TLA) in the management of airway obstruction and feeding in Pierre Robin sequence, a case report.

INTRODUCTION AND IMPORTANCE: Pierre Robin Sequence (PRS) is a rare and complex condition that often necessitates the collaboration of a full medical team from various disciplines to save the lives of babies with this genetic mutation, characterized by three clinical manifestations: glossoptosis, micrognathia, and cleft palate. Treatment primarily involves freeing upper airway obstructions and enhancing nutrition to allow the babies to lead a normal life. The lip-tongue adhesion procedure has been identified in medical literature as the recommended approach to addressing the issues associated w

Complications associated with neonatal mandibular distraction osteogenesis in the treatment of Robin sequence.

BACKGROUND: Robin sequence (RS) is defined as a triad of retrognathia, glossoptosis, and airway obstruction. Although several studies have reported on the efficacy of mandibular distraction, the risks associated with this operation remain unclear. An outcomes analysis focusing on complications is reported here. METHODS: A 7-year retrospective review of all patients with RS treated with mandibular distraction was performed. Recorded variables included associated medical comorbidities, improvement in apnea/hypopnea index, need for tracheostomy, repeat distraction, and complications. Complicatio

A case of Pierre Robin syndrome in a child with no soft palate and complications from pneumonia in Bangladesh.

KEY CLINICAL MESSAGE: Children with Pierre Robin syndrome (PRS) often have trouble breathing and eating as soon as they are born. If conservative therapy fails to alleviate airway obstruction, surgical surgery may be considered. Patients with PRS require multidisciplinary approaches for treatment. ABSTRACT: Pierre Robin syndrome is a common craniofacial abnormality that causes glossoptosis and blockage of the upper airway. This renders it difficult to feed, which leads to severe malnutrition. This condition is also often marked by an absence of a soft palate. We mention a newborn with Pierre

Cervical Stenosis in Non-Syndromic Pierre Robin Sequence.

Pierre Robin sequence is a triad of micrognathia, glossoptosis, and airway obstruction that has reported associations with numerous birth defects and anomalies including craniocervical pathologies. Given the number of airway assessments and neck manipulations these neonates experience, undetected cervical instability can result in detrimental consequences. The authors present a case of a neonate with non-syndromic Pierre Robin sequence planned for mandibular distraction osteogenesis that developed abnormal motor activity following intubation. The patient was eventually diagnosed with cervical

The child with mandibular hypoplasia (Treacher Collins syndrome, Pierre Robin sequence, hemifacial microsomia, etc) presents the otolaryngologist and anesthesiologist with considerable problems when direct laryngoscopy and/or endotracheal intubation is attempted. In addition to the small mandible, several other features of these patients contribute to the difficult laryngoscopy: macroglossia, glossoptosis, trismus related to temporomandibular joint abnormalities, and prominent maxilla or maxillary incisors. Most of the techniques that have been described for laryngoscopy/intubation in problem
