Spontaneous preterm delivery and gestational diabetes: the impact of glycemic control.

OBJECTIVE: Opinions differ whether the rate of spontaneous preterm delivery (sPTD) increases in pregnancies complicated with GDM. We sought to characterize, which factors may influence the rate of sPTD in GDM. METHODS: We conducted a retrospective study with 1,526 GDM patients, all treated at the same center by the same diabetic protocol using self-blood glucose monitoring. The rate of sPTD was compared to that of 10,560 non-diabetic women. Eligibility for the study was limited to women with a singleton pregnancy with spontaneous onset of delivery before 37 weeks of gestation with no history

Benefits and methods of achieving strict glycemic control in the ICU.

Hyperglycemia, a frequent complication in critically ill patients, has been shown to have a negative influence on morbidity and mortality. Many factors contribute to hyperglycemia, including the stress response, diabetes, obesity, advanced age, corticosteroids, sepsis, pancreatitis, and the use of nutrition support. Application of intensive insulin therapy, when compared with conventional glycemic control measures, seems to improve outcomes in the critically ill patient. Therefore, effective insulin therapy along with appropriate nutrition support prescriptions provide a means for the critical

Successful islet transplantation: continued insulin reserve provides long-term glycemic control.

Clinical islet transplantation is gaining acceptance as a potential therapy, particularly for subjects who have labile diabetes or problems with hypoglycemic awareness. The risks of the procedure and long-term outcomes are still not fully known. We have performed 54 islet transplantation procedures on 30 subjects and have detailed follow-up in 17 consecutive Edmonton protocol-treated subjects who attained insulin independence after transplantation of adequate numbers of islets. Subjects were assessed pretransplant and followed prospectively posttransplant for immediate and long-term complicati

Guar gum and glycemic control of pregnant insulin-dependent diabetic patients.

The effect of guar gum preparation on the glycemic control of 12 pregnant insulin-dependent diabetic patients was investigated. The study was performed on inpatients and lasted 3 wk (from week 32 of pregnancy and onward). Weeks 32 and 34 were control periods during which time the patients had their normal diet only. During week 33, the diet was supplemented with guar. The mean blood glucose level of week 33 was 14% lower than that of week 32 (P less than 0.01) but equal to that of week 34. Mean fluctuations of blood glucose during individual days of the 3 wk were similar. The M-value declined

Prescription pattern, glycemic control status, and predictors of poor glycemic control among diabetic patients with comorbid chronic kidney disease in Ethiopia: a facility-based cross-sectional study.

BACKGROUND: Achieving optimal glycemic control is vital for managing diabetes mellitus and preventing its complications, yet it is particularly challenging for individuals with diabetes and concurrent chronic kidney disease. Chronic kidney disease disrupts glucose metabolism and excretion, leading to pronounced and variable blood glucose fluctuations, thereby complicating diabetes management. So far, the intricate impact of chronic kidney disease on the glycemic control status of diabetic patients remains obscure, especially in Sub-Saharan Africa where both diseases pose an escalating burden.

Motor vehicle crashes in diabetic patients with tight glycemic control: a population-based case control analysis.

BACKGROUND: Complications from diabetes mellitus can compromise a driver's ability to safely operate a motor vehicle, yet little is known about whether euglycemia predicts normal driving risks among adults with diabetes. We studied the association between glycosylated hemoglobin (HbA1c) and the risk of a motor vehicle crash using a population-based case control analysis. METHODS AND FINDINGS: We identified consecutive drivers reported to vehicle licensing authorities between January 1, 2005 to January 1, 2007 who had a diagnosis of diabetes mellitus and a HbA1c documented. The risk of a crash

Serum Metabolomics Reveals a Potential Benefit of Methionine in Type 1 Diabetes Patients with Poor Glycemic Control and High Glycemic Variability.

Glycemic variability (GV) in some patients with type 1 diabetes (T1D) remains heterogeneous despite comparable clinical indicators, and whether other factors are involved is yet unknown. Metabolites in the serum indicate a broad effect of GV on cellular metabolism and therefore are more likely to indicate metabolic dysregulation associated with T1D. To compare the metabolomic profiles between high GV (GV-H, coefficient of variation (CV) of glucose ≥ 36%) and low GV (GV-L, CV < 36%) groups and to identify potential GV biomarkers, metabolomics profiling was carried out on serum samples from 17 p

Association of diet composition and meal intervals with short-term glycemic control and glycemic variability among type 2 diabetes mellitus patients.

BACKGROUND: Glycemic variability (GV) is an independent contributor to diabetes related complications beyond glycated hemoglobin. While dietary macronutrient composition influences glycemic responses, the importance of meal timing and nutrient distribution in real world settings remain unclear particularly in Indian diets. METHODS: In this observational study, 47 adults with type 2 diabetes mellitus who completed 10-14 days of continuous glucose monitoring (CGM) were included in the final analysis. The median CGM wear duration was 14(14-14) days and the median CGM analyzed days was 11(10-11)
