[Surgical access for thoracic and intrathoracic goiter].

OBJECTIVE: To evaluate the effectiveness of surgical approach for resection of retrosternal and intrathoracic goiter. MATERIAL AND METHODS: There were 33 patients who underwent surgery for retrosternal (31) and intrathoracic goiter (2) through various surgical approaches. For retrosternal goiter, a Farabeuf hook was used to remove a large retrosternal component of tumor. RESULTS: In 28 cases (84.8%), substernal goiter was resected through cervical collar incision. In 4 cases, a Farabeuf hook was used to extract retrosternal part of the goiter to the neck and avoid sternotomy. In one case, a

Extended cervical approach for retrosternal multinodular goiter.

OBJECTIVE: Partial or total sternotomy is required for 10% of retrosternal goiter. This study reviewed our experience with an extended cervicotomic approach as an alternative surgical solution for retrosternal goiter. METHODS: A retrospective study was performed on patients who underwent partial or total thyroidectomy for retrosternal goiter between 2014 and 2019 at a tertiary medical centre. Data on clinical, radiologic, and pathologic factors were analysed. Peri- and postoperative outcomes were compared between extended and standard cervical approaches to predict the need for an extended ce

Massive retropharyngeal goiter: Case presentation and literature review of a rare entity.

Retropharyngeal goiters are uncommon entities characterized by the presence of thyroid tissue in the retropharyngeal space. We present the case of an 89-year-old female with a history of dysphagia and hoarseness of voice. Imaging revealed a massive goiter extending into the retropharyngeal and retrosternal spaces. The patient underwent a total thyroidectomy with transcervical removal of the substernal component. Our analysis of 13 documented cases (14 including ours) of retropharyngeal multinodular goiters reveals a range of presentations with dysphagia, dysphonia, and dyspnea being the most c

Case Report of a Massive Retropharyngeal Goiter Resulting in Laryngeal Compression.

This is a rare presentation of a morbidly obese male with a massive retropharyngeal goiter causing laryngeal compressive symptoms and unique airway management challenges. Flexible laryngoscopy revealed a retropharyngeal mass circumferentially compressing the oropharynx and supraglottis and preventing the visualization of the glottis. Awake tracheostomy was performed before total thyroidectomy. Airway compression from a goiter typically results from substernal extension, which usually does not cause difficulty with endotracheal intubation. Extensive retropharyngeal extension can cause supraglot

Bilateral phrenic nerve palsy associated with benign thyroid goiter.

Phrenic nerve palsy secondary to benign thyroid enlargement is a previously unreported complication. Large goiters, particularly substernal, may impinge upon adjacent structures, often leading to significant symptoms such as dysphagia or dyspnea due to airway compression. The phrenic nerve may be stretched by a large goiter along its course in the neck, but the more likely site of injury is the point at which it enters the thoracic cavity adjacent to the first rib. Such an injury, caused by compression, may go unrecognized if unilateral, as symptoms would be uncommon. However, bilateral phreni

[Surgical treatment of large goiters extending into the retrosternal space].

The authors describe the characteristics of primary and secondary goitres reaching as far as the retrosternal space. 154 operations of retrosternal goitres during a five-year period account for 16.7% of all operations of the thyroid gland. The great majority (97.4%) of operations was made from a cervical approach. The authors express also their opinion on sternotomies in these operations; the reasons comprise also extensive goitres which cannot be removed across the upper thoracic aperture where the orientation on the position of major blood vessels is poor, when local invasion of the tumour i

Innovative trends in surgery: a robotic approach to retrotracheal goitre.

The retrotracheal form is a rare variant of retrosternal goitre. The incidence is low, representing approximately 4% of all retrosternal goitres. The traditional approach to the treatment of this type of pathology is the cervical approach combined with high thoracotomy. Recently, however, new methods and technologies have begun to emerge that also open new possibilities for the surgical management of these conditions. The aim of this case report was to present the potential of a video-robotic approach in the treatment of retrotracheal goitre. The authors have successfully used this innovative

A rare presternal goiter: presentation and literature review.

Presternal goiters are incredibly rare presentations worldwide; very little is known about their genesis despite many hypotheses and even fewer cases are seen in our medical literature. The commonest pathology for presternal goiters is papillary carcinoma of the thyroid (PCT), typically presenting as a solitary cystic cervical mass. Differential diagnoses are vast causing diagnostic difficulty, prompting crucial pre-operative radiological investigations and pathological confirmation. Curative management in all cases of malignant presternal goiters is total thyroidectomy with a modified radical

The "forgotten" goiter after total thyroidectomy.

INTRODUCTION: "Forgotten" goiter is an extremely rare disease which is defined as a mediastinal thyroid mass found after total thyroidectomy. PRESENTATION OF CASE: We report two cases with forgotten goiter. One underwent total thyroidectomy due to thyroid papillary cancer and TSH level was in normal range one month after surgery. The thyroid scintigraphy scan revealed mediastinal thyroid mass. The second case underwent total thyroidectomy due to Graves' disease and TSH level was low after surgery. At postoperative seventh year, patients were admitted to our Endocrinology Division due to persi

Multilayered Insights into Poorly Differentiated, BRAFV600E-Positive, Thyroid Carcinoma in a Rapidly Developing Goiter with Retrosternal Extension: From En "Y" Cervicotomy to SPECT/CT-Positive Lung Metastases.

Poorly differentiated thyroid malignancy, a rare histological type of aggressive thyroid malignancy with associated difficulties and gaps in its histological and molecular characterization, might lead to challenging clinical presentations that require a prompt multimodal approach. This case study involved a 56-year-old, non-smoking male with a rapidly developing goiter (within 2-3 months) in association with mild, non-specific neck compressive symptoms. His medical history was irrelevant. A voluminous goiter with substernal and posterior extension up to the vertebral bodies was detected using

Resection of a giant bilateral retrovascular intrathoracic goiter causing severe upper airway obstruction, 2 years after subtotal thyroidectomy: a case report and review of the literature.

The intrathoracic (or substernal) goiter is more often benign; but it can be malignant in 2-22% of patients. There is history of prior thyroid surgery in 10% to more than 30% of patients. Intrathoracic goiters cause adjacent structure compression more frequently than the cervical goiters, due to the limited space of the thoracic cage. Compression of trachea, oesophagus, vascular and neural structures may cause dyspnoea, dysphagia, superior vena cava syndrome, subclavian vein thrombosis, hoarseness, and Horner's syndrome. There is usually progressive deterioration, but acute exacerbation may oc

Goiter with severe respiratory compromise: evaluation and treatment.

Goiter with major respiratory compromise is uncommon but troublesome. Evaluation and treatment of this condition are controversial. Of a total of 2,908 goiters operated on over a 17-year period, 58 cases with this particular complication were studied retrospectively to define optimal management. Twenty-two patients had severe or acute dyspnea, and four of them required immediate tracheal intubation. Thirty-six patients had chronic dyspnea without cyanosis. Carcinoma was present in these two groups in 50% and 11% of patients, respectively. Results of our retrospective study are as follows: long

Approach to management of the patient with primary or secondary intrathoracic goiter.

Intrathoracic (substernal) goiter, depending on definition, is seen in up to 45% of all patients operated for goiter. It can either be primary (ectopic thyroid tissue detached from a cervical thyroid mass), which is very rare (1%), or (more commonly) secondary, where a portion of the goiter extends retrosternally. There is no consensus on diagnostic or therapeutic management, partly because many are asymptomatic. Classification involves functional characterization with serum TSH and morphological characterization with diagnostic imaging and cytology to rule out malignancy, which is not more co

Technical review by the Brazilian Society of Surgical Oncology and the Brazilian Society of Head and Neck Surgery on hierarchical coding in thyroid surgery.

The Brazilian Society of Surgical Oncology and the Brazilian Society of Head and Neck Surgery developed a technical consensus on hierarchical coding in thyroid surgery, considering the role of the Brazilian Hierarchical Classification of Medical Procedures in determining appropriate codes. A panel of 40 specialists - recognized academic and clinical leaders from across Brazil - assessed the applicability of procedure codes for common thyroid surgeries and prepared the current consensus. Deliberations were conducted via electronic voting among 37 participants, and consensus was defined as agree

A prospective evaluation of recurrent laryngeal nerve paralysis during thyroidectomy.

HYPOTHESIS: Recurrent laryngeal nerve paralysis after thyroidectomy can be unrecognized without routine laryngoscopy, and patients have a good potential for recovery during follow-up. DESIGN: A prospective evaluation of vocal cord function before and after thyroidectomy. Periodic vocal cord assessment was performed until recovery of cord function. Persistent cord palsy for longer than 12 months after the operation was regarded as permanent. SETTING: A university hospital with about 150 thyroid operations performed by 1 surgical team per year. PATIENTS: From January 1, 1995, to April 30, 199
