Granulomatous mastitis with erythema nodosum: A breast cancer mimicking entity.

We present the case of a 42-year-old Hispanic patient who consulted with a left breast mass that showed clinical and imaging signs of breast cancer. During preprocedural examination before needle biopsy, the patient was found to have bilateral, purplish-brown skin lesions on her lower legs, suggestive of erythema nodosum. This clinical finding raised the diagnostic suspicion of granulomatous mastitis, which was later confirmed by histopathology. Granulomatous mastitis is a rare, nonmalignant entity that should be considered in patients of childbearing age who present with a breast mass. The co

Idiopathic Granulomatous Lobular Mastitis: A Mimicker of Inflammatory Breast Cancer.

BACKGROUND: Idiopathic granulomatous lobular mastitis (IGLM) is a rare, chronic inflammatory breast disease without a known etiology. Even though the current literature proposes several treatment strategies, there is no universal consensus for long-term management. CASE PRESENTATION: A 43-year-old White woman (gravida 5, para 4) presented with a 2-week history of right lower outer quadrant breast tenderness, heaviness, warmth, and redness. Mammography and ultrasound were concerning for inflammatory breast cancer. Biopsies returned as granulomatous mastitis without malignancy. After 8 months o

Clinicopathological features of granulomatous lobular mastitis and mammary duct ectasia.

Granulomatous lobular mastitis (GLM) and mammary duct ectasia (MDE) are inflammatory diseases. However, only a limited number of studies have focused on characterizing their clinicopathological features. The aim of the present study was to investigate the etiology, clinicopathological characteristics and diagnosis of GLM and MDE. The clinical information and treatment of 118 female patients with pathologically-proven GLM or MDE were retrospectively analyzed in the present study. A total of 29 cases had GLM, 77 had MDE and 12 had GLM accompanied by MDE. GLM tends to occur in patients who have h

Antimicrobial Treatment Options for Granulomatous Mastitis Caused by Corynebacterium Species.

Corynebacterium species are increasingly recognized as important pathogens in granulomatous mastitis. Currently, there are no published treatment protocols for Corynebacterium breast infections. This study describes antimicrobial treatment options in the context of other management strategies used for granulomatous mastitis. Corynebacterium spp. isolated from breast tissue and aspirate samples stored from 2002 to 2013 were identified and determined to the species level using matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS), 16S RNA sequencing, and rpo

Erythema nodosum associated with granulomatous mastitis: report of two cases.

Granulomatous mastitis is a rare breast disease characterized by chronic inflammation. Clinical presentation of granulomatous mastitis usually mimics malignancy or infection. Coincidence of granulomatous mastitis and erythema nodosum is a quite rare feature. In this paper, we aimed to present two young women with granulomatous mastitis accompanied by erythema nodosum and successfully treated with corticosteroids. Granulomatous mastitis should be kept in the mind for differential diagnosis of erythema nodosum.

Granulomatous Lobular Mastitis Following Drug-Induced Galactorrhea and Blunt Trauma.

We report a single case of chronic granulomatous lobular mastitis following metoclopramide-related galactorrhea and a blunt trauma in a young parous woman who underwent two conservative operations before becoming symptom-free. We have found only two other literature cases associated with hyperprolactinemia, and our case could be another of this etiologic group. The absence of well-formed granulomas in the first histology specimen in the present case was misleading; it was reinterpreted as granulomatous mastitis only after the second specimen was examined. Reinterpretation was based on the lobu

Cystic Neutrophilic and Granulomatous Dermatitis as the Cutaneous Counterpart of Cystic Neutrophilic and Granulomatous Mastitis: A Case Series.

Cystic neutrophilic granulomatous mastitis (CNGM) has a distinctive morphology characterized by small cystic spaces with a concentric rim of primarily neutrophils and epithelioid histiocytes. These cystic spaces have a granular basophilic quality; a Gram stain discloses gram-positive bacilli compatible with a Corynebacterium species with the most common isolate being Corynebacterium kroppenstedtii. We identified 16 cases of Corynebacterium related cystic neutrophilic granulomatous dermatitis (CNGD) within our dermatopathology database. The histology in all cases was reproducible being in the c

When it is not cancer: Cystic neutrophilic granulomatous mastitis masquerading as carcinoma.

Cystic neutrophilic granulomatous mastitis (CNGM) is a rare inflammatory breast disease that can clinically and radiologically mimic breast carcinoma. Accurate histological diagnosis is essential to prevent unnecessary treatment and guide appropriate therapeutic management. A 33-year-old female patient presented with a painful, firm nodule in the left breast, in the absence of typical inflammatory clinical findings. Breast ultrasound revealed an ill-defined, hypoechoic nodule measuring 17mm×22mm, with peripheral acoustic shadowing, classified as BI-RADS 4B. A core needle biopsy was performed.

The diagnostic dilemma of idiopathic granulomatous mastitis with an emphasis on histopathologic findings.

BACKGROUND: Idiopathic granulomatous mastitis (IGM) is a rare benign inflammatory breast condition with diverse clinical and radiologic features, often misdiagnosed as malignancy prior to sampling. Therefore, histological findings are one of the most essential bases of diagnosis. METHODS: We retrospectively searched the histopathologic database of the Cancer Institute of Imam Hospital for IGM cases and collected clinical and radiological data. After ruling out other causes of granulomatous mastitis, the histopathologic features of each case were evaluated. The pattern of inflammation was class

Investigation of multi-infections and breast disease comorbidities in granulomatous mastitis.

INTRODUCTION: Granulomatous mastitis (GM) is an inflammatory breast disease typically caused by infection, posing diagnostic challenges. It can coexist with other breast disorders or multiple infections, which have been vaguely discussed. This study investigates the incidence of multi-infection and breast disease comorbidities in GM. METHOD: The study enroled 63 females who had a confirmed diagnosis of GM. Laboratory investigations and bacterial cultures had been conducted for all the cases. The patients had undergone ultrasonography examination utilizing the LOGIQ E9 system. Core needle biop

The Use of Intralesional Corticosteroids in Idiopathic Granulomatous Mastitis: A Systematic Review.

Idiopathic granulomatous mastitis (IGM) is a debilitating, chronic, inflammatory condition of the breast. Several studies have emerged evaluating intralesional steroid (ILS) injection and topical steroid administration as a treatment for IGM. However, there is a dearth of international consensuses with regards to the management of IGM. Therefore, we have systematically reviewed the effectiveness of ILS in the management of IGM. A systematic search was conducted in PubMed and Cochrane Library databases, the Google Scholar website and by citation searching up to June 15th, 2023. Eight articles w

Case report on gynecomastia with ipsilateral recurrence of granulomatous lobular mastitis.

This study describes a rare case of male granulomatous lobular mastitis (GLM) with recurrence in different sites on the ipsilateral side. A 48-year-old male patient presented with no previous history of breast-related disease, and physical examination suggested a mass in the right breast. Ultrasonography revealed a cyst and infection in the right breast. No obvious abnormality was found in laboratory analysis, and a core needle biopsy revealed GLM. Incision and drainage were applied to the right lesion, and symptoms resolved within a few weeks. At 2 years following this initial presentation, t

Management of Granulomatous Lobular Mastitis and Risk Factors Associated with Recurrence.

BACKGROUND: Granulomatous lobular mastitis (GM), an inflammatory breast lesion with unknown etiology, is prone to recur. There is no global consensus on the optimal treatment at present. This study was conducted to show our step-by-step systemic procedure and discuss the recurrence risk factors of GM. METHODS: The medical record database was retrospectively searched for patients with GM treated in our hospital between January 2015 and September 2021. Patients were divided into non-recurrence group (group A) and recurrence group (group B). Demographic and clinical characteristics, treatment an

An Unusual Cause of Oligoarthritis and Erythema Nodosum: Idiopathic Granulomatous Mastitis.

Idiopathic granulomatous mastitis (IGM) is an unusual benign inflammatory disease of breast. Breast cancer mimics IGM both radiologically and clinically. However, IGM is a benign disease and awareness of such an entity prevents unnecessary surgical procedures. Although its etiology is unknown, it may be an autoimmune disease. There are few patients reported in the literature presenting with reactive arthritis and/or erythema nodosum accompanying IGM of breast. Granulomatous mastitis should be considered as a possible underlying cause of arthritis and erythema nodosum. In this article, we repor

Idiopathic granulomatous mastitis. A report of three cases and review of the literature.

We report three cases of idiopathic granulomatous mastitis that occurred in women of reproductive age (range, 25 to 36 years). These patients presented with breast masses of 2.5, 7, and 10 cm. One patient also had erythema nodosum. On biopsy specimens, the characteristic histopathologic features of granulomatous inflammation, centered on mammary lobules, were present in each case. In only one case was a specific diagnosis of granulomatous mastitis rendered based on the results of the biopsy; this patient received oral prednisone with prompt shrinkage of the breast mass to 20% of its original s

A Case of Cystic Neutrophilic Granulomatous Mastitis in a Young Breastfeeding Woman.

Cystic neutrophilic granulomatous mastitis (CNGM) is a rare, benign inflammatory breast condition with an unclear etiology. Proposed theories include infection and autoimmune conditions. CNGM often mimics infectious mastitis and inflammatory breast carcinoma, making diagnosis a challenge, especially in breastfeeding patients. Here, we present the case of a 29-year-old breastfeeding woman who developed a painful, ulcerating left breast mass initially suspected to be mastitis or malignancy. Imaging revealed a mixed solid and cystic lesion, and biopsy confirmed CNGM. Her symptoms improved with ce
