Effect of G suit type on cognitive performance.

BACKGROUND: Sustained acceleration protection ensembles are being developed to help pilots of high performance aircraft endure high G exposures for longer periods of time. It has been assumed that better G endurance confers better pilot task performance. This premise was studied on the Armstrong Laboratory Dynamic Environment Simulator centrifuge. METHOD: Human subjects repeatedly endured prolonged high-G simulated aerial combat on a centrifuge to the point of loss of vision or physical exhaustion. Some profiles included over 20 exposures to +9 Gz. While enduring the G exposures, subjects tra

Perfusion of the visual cortex during pressure breathing at different high-G stress profiles.

The effects of pressure breathing for G protection (PBG) on perfusion of the visual cortex were studied in a subject during various high-G stress profiles. Blood flow velocity was measured in the posterior cerebral artery using a transcranial Doppler (TCD) ultrasound instrument. The G profiles examined included gradual and rapid onset rates. Mean cerebral blood flow velocity (MCBFV) declined with increasing +Gz with G-suit protection alone. The MCBFV increased in direct proportion with increase in +Gz acceleration with PBG. The mediating mechanisms for the effects of PBG may include improved g

Utility of military anti-shock trousers (MAST) in anaphylactic shock--a case report.

A 49-year-old man with severe anaphylactic hypotension deteriorated and developed an unmeasurable blood pressure despite vigorous intravenous (IV) fluid and epinephrine administration. Application of the lower body compression garment known as MAST (Military Anti-Shock Trousers) improved the patient's blood pressure and helped to stabilize it until additional fluids and vasopressors were administered; he survived without sequellae. We propose that augmentation of preload and afterload by MAST is a useful adjunct in severely hypotensive anaphylactic patients who do not respond initially to IV f

Venous return and the pneumatic antishock garment in hypovolemic baboons.

The pneumatic antishock garment is widely used in trauma patients because it compresses small veins and should augment venous return. If the garment does augment venous return, it should probably be used in all injured patients in shock, regardless of injuries and regardless of anticipated transport time. On the other hand, the garment could distort and narrow large retroperitoneal and abdominal veins and impede venous return. We quantitated venous return in normovolemic and hypovolemic baboons. The garment both augmented and impeded venous return, the two effects counteracting each other. We

MAS trousers improperly applied causing a compartment syndrome in lower-extremity trauma.

A case is presented in which the leg segments of MAS trousers were deflated without deflating the abdominal segment, causing the trousers to act as a venous tourniquet. As a result bleeding and edema secondary to a pertrochanteric hip fracture were aggravated, causing a compartment syndrome in the thigh; fasciotomy became necessary. MAS trousers must be inflated in proper sequence, the leg segments first and then the abdominal portion. In deflation, the order is reversed. Only if this sequence is followed can MAS trousers be used safely.

Metabolic acidosis with pneumatic trousers in hypovolemic dogs.

To evaluate the possibility of anaerobic metabolism occurring beneath pneumatic trousers because of the decreased circulation, 20 mongrel dogs were evaluated for central and femoral vein levels of lactic acid, potassium and central pH changes when the trouser pressure was both above and below systolic arterial pressure. The results indicate that if the systolic pressure is less than trouser pressure, there is a significant increase in both potassium and lactic acid levels. There was a drop in the central pH accompanying these metabolic changes. When the pneumatic trouser pressure was less than

G-suited for prevention of syncope in patients with vasovagal syncope: a pilot study.

BACKGROUND: Vasovagal syncope (VVS) represents by far the most common cause of syncope as it is diagnosed in around 50% of all patients that come to an emergency department. Although VVS is not fatal, it can cause an injury. Even serious injuries are not common, but there are reports of serious injuries of up to 5%. There are no current studies that demonstrate the effectiveness of any treatment. Past studies found that an Anti-Gravity suit (G-suit) can increase blood pressure and has been reported to prevent orthostatic hypotension effectively in patients with diabetes. It is possible that th

A systematic review of pneumatic compression for treatment of chronic venous insufficiency and venous ulcers.

INTRODUCTION: As part of a reconsideration of coverage policy, the Centers for Medicare and Medicaid Services requested a systematic review of the evidence on the use of pneumatic compression devices in the home environment for treatment of chronic venous insufficiency (CVI) and venous ulcers. METHODS: Articles were found with a systematic literature search of MEDLINE, EMBASE, and AMED (Allied and Complementary Medicine) databases, hand searches of reference lists, and suggestions of experts. RESULTS: Eight trials that met the inclusion criteria, including several randomized control trials,

Premature ventricular contractions during +Gz with and without pressure breathing and extended coverage anti-G suit.

BACKGROUND: High +Gz is known to provoke cardiac dysrhythmias. Pressure breathing during G (PBG) and extended coverage anti-G suits (ECGS) are used to enhance +Gz-endurance and reduce fatigue during high +Gz flying. It is not known whether PBG in combination with ECGS increases the risk for premature ventricular contractions (PVC). HYPOTHESIS: PBG in combination with ECGS increases the risk of PVCs during high +Gz-loads. METHODS: Retrospective data were obtained from 14 subjects exposed to three different simulated aerial combat sorties each, in the centrifuge with a standard anti-G suit ens

The effect of mechanical compression on chronic hand edema after burn injury: a preliminary report.

Chronic hand edema after wound healing is a troublesome condition to treat in patients with burns. Stagnant edema can cause fibrosis, which impedes rehabilitation and may lead to deformity. Although favorable results have been reported with mechanical compression used in acute injuries, no literature was found on the effects of compression for the treatment of chronic hand-burn edema. Five male patients with nine chronically edematous burned hands were subject to mechanical compression at 55 mm Hg pressure. A single-cell unit was used for a 30-minute treatment at 4:1 treatment ratio. Goniometr

[Amniotic embolism with favourable outcome].

A 31-year-old multiparous pregnant woman forced into labour with oxytocin, suffered a sudden circulatory collapse and, after the birth of her child, a defibrination syndrome with uterine haemorrhage. In the absence of a local cause, either placental or cervico-uterine, a diagnosis of amniotic fluid embolism was made. The time course was favourable, mainly thanks to the absence of acute pulmonary cardiogenic oedema and to the use of an anti-shock G suit. The authors point out the lack of actual in vivo paraclinical means for confirming the diagnosis.

A review of the risks and benefits of differing prophylaxis regimens for the treatment of deep venous thrombosis and pulmonary embolism in neurosurgery.

BACKGROUND: Annually, 2 million people in the United States develop deep venous thrombosis (DVT), and nearly 100,000 sustain fatal pulmonary emboli. Prophylaxis against DVT/pulmonary embolism (PE) is a critical issue, and options include elastic stockings, intermittent pneumatic compression stockings, low-dose unfractionated heparin (5000 U every 8-12 hours), and low molecular-weight heparin (ie, enoxaparin and dalteparin). The risks and benefits associated with different prophylaxis regimens used in the prevention of DVT and PE in neurosurgical procedures were analyzed. METHODS: Neurosurgica

Pulmonary air embolism.

Pulmonary air embolism is a well-known consequence of surgery, trauma, diving, and aviation. This article reviews the physiological effects, means of detection and methods of prevention and treatment of pulmonary air embolism. The primary physiological effects are elevated pulmonary artery pressures, increased ventilation-perfusion inhomogeneity, and right ventricular failure. The degree of physiological impairment depends on the volume of gas entrained, the rate of entrainment, the type of gas entrained, and the position of the patient when the embolism occurs. Pneumatic sequential compression reduces the risk of deep vein thrombosis in stroke patients.

OBJECTIVE: To determine if pneumatic sequential compression devices (SCDs) combined with subcutaneous heparin and antiembolic hose reduce the risk of deep vein thrombosis (DVT) and pulmonary embolism (PE) in stroke patients. BACKGROUND: DVTs and PEs are serious complications among hospitalized stroke patients. Subcutaneous heparin and SCDs have both been used to prevent DVT. It is not known if SCDs combined with subcutaneous heparin can improve the protection afforded by heparin alone. METHODS: The study group was comprised of nonhemorrhagic stroke patients admitted to the neurology service

Intrathoracic airstream temperatures during acute expansions of thoracic blood volume.

1. To determine the validity of employing intrathoracic heat flux as a reflection of changes in bronchial blood flow, we used a thermal probe to record airstream temperatures within the tracheobronchial tree in five normal and five asthmatic subjects during isocapnic hyperventilation challenges with and without inflation of the lower limb bladders of a pressure suit. 2. During hyperpnoea, airstream temperatures fell progressively in both subject groups. When blood volume was acutely shifted from the legs into the thorax via anti-shock trousers, airstream temperatures within the tracheobronchia

A safe, pre-fabricated, immediate postoperative prosthetic limb system for rehabilitation of below-knee amputations.

Thirty-eight below-knee amputees were treated with a commercially available pneumatic prosthetic limb system, applied immediately postoperatively, that would allow immediate weight bearing following lower extremity amputation. Thirty-four limbs were amputated for peripheral vascular insufficiency and four were amputated due to trauma. Weight bearing was initiated an average of 4.7 days following surgery and patients were discharged with a non-removable temporary prosthesis at an average of 8.9 days. Thirty-three healed and were uneventfully fit with standard below-knee prosthetic limbs. Three
