Hallux rigidus. Excisional arthroplasty.

The authors feel that capsular interposition arthroplasty can give predictable pain relief in carefully selected individuals with severe (grade III) hallux rigidus. Attention to the relative lengths of the first and second metatarsals, minimal shortening of the proximal phalanx, and use of the dorsal capsule and EHB tendon as an interposition all contribute to good to excellent objective and subjective results. Approximately 30% of patients undergoing this procedure experience some degree of transfer metatarsalgia postoperatively and probably require orthoses for sports. The authors feel that

Idiopathic adult hallux varus.

Adult hallux varus is an uncommon clinical entity usually caused by an inflammatory arthropathy or overcorrection during bunion reconstruction. We present five cases of unexplained spontaneous hallux varus. Clinically, all patients initially were found to have flexible deformities, and no evidence of underlying inflammatory disease or history of trauma. Symptoms were easily relieved with shoe wear modifications. Two of the patients developed more rigid deformities and subsequent medial callusing of the first toe and varus deformities of the lesser toes. These two patients eventually required s

Iatrogenically induced hallux varus.

Hallux varus is not uncommon after hallux valgus surgery. Through understanding of its cause, this deformity may be easily avoided with thorough preoperative evaluation, meticulous and deliberate intraoperative technique, and early postoperative diagnosis. By recognizing these factors, one can significantly reduce the predilection toward hallux varus. Once the diagnosis has been made, aggressive treatment is warranted, in the hope of minimizing the complexity of joint disorientation and surgical repair.

Relationship between medial partite hallux sesamoid and hallux valgus in the general population.

BACKGROUND: An association between the medial partite hallux sesamoid (MPHS) and hallux valgus (HV) has been suggested; however, a causal relationship has not been confirmed. This study aimed to determine their causal relationship using a cross-sectional radiographic survey of a large-scale population cohort covering a wide age group. PATIENTS AND METHODS: The fifth survey of the Research on Osteoarthritis/Osteoporosis against Disability study involved 1997 participants aged 21-95 years who had undergone anteroposterior radiography of bilateral feet. The presence of MPHS, its morphology, and

Hallux Valgus, Not Hallux Abductus Valgus: A Short Report on Historical and Anatomical Terminology.

BACKGROUND: This short report argues that the term "hallux abductus valgus" is anatomically inconsistent and should be replaced by "hallux valgus." Hallux valgus describes a lateral deviation of the great toe with a medial deviation of the first metatarsal. Although the deformity has historically been called "hallux abductus valgus," this creates confusion because the muscle contributing to the lateral deviation is the adductor hallucis, not the abductor hallucis. METHODS: The authors trace this inconsistency back to Carl Hueter's original description in the 19th century. Hueter defined the d

[Pedobarographic evaluation of the foot after Keller's procedure for Hallux valgus and Hallux rigidus].

Hallux valgus and hallux rigidus are common foot deformities. One of possible procedures is Keller's surgery. The purpose of this study is to evaluate the use of pedobarography for postoperative evaluation of the foot. Group. We examined 54 patients aged 48 to 82 (average 60.4) after surgical treatment of Hallux valgus (78 feet) or Hallux Rigidus (15 feet). Methods. Retrospective pedobarographic evaluation 1 to 11 years after surgery was performed using PEL-38 system. Clinical results. All patients had markedly increased pressure under the 2nd and 3rd metatarsal heads comparing to other forefo

Arthrodesis for Hallux Rigidus.

First metatarsophalangeal (MTP) joint fusion, or arthrodesis, has been a cornerstone treatment for advanced joint degeneration since the 1950s, benefiting not only older patients but also younger individuals. The procedure boasts favorable long-term outcomes and a low rate of revision when fusion is successful. However, complications such as interphalangeal arthritis may occur, and nonunion or malunion can necessitate revision. While the first MTP fusion provides significant relief, its applicability varies, underscoring the need for careful patient selection.

Rheumatoid Nodule on the Hallux.

A subcutaneous rheumatoid nodule is a superficial soft tissue lesion that generally affects areas of frequent mechanical contact, such as the extensor aspect of the extremities, occiput, and sacrum. Rheumatoid nodules of the foot are rare, occurring in only 1% of cases. Herein, we present a case of a rheumatoid nodule on the plantar side of the left hallux in a 65-year-old woman with rheumatoid arthritis. The tumor was a 15 × 10 mm, dome-shaped, nontender, elastic hard mass in the plantar aspect of the left hallux. The discovery of rheumatoid nodules provides crucial information for the treatm

[A boy with an abnormal hallux].

An eleven-year-old boy was seen by the general practioner due to a painfull hallux. The diagnosis unguis incarnatus was made. Therefore, a partial nail extraction was performed. Unfortunately, the patient still had complaints of a painful hallux and was forwarded to the surgical outpatient clinic. We saw a round, hard and benign tumor on the lateral side of the nail (picture 1). We performed a partial nail extraction as well. Due to persistence complaints and the abnormal course and X-ray was performed. On the X-ray an exostosis was seen (picture 2). This confirmed the diagnosis: subungual exo

Cheilectomy for Hallux Rigidus.

Hallux rigidus is a degenerative condition leading to arthritis of the first metatarsophalangeal joint. Cheilectomy is a surgical procedure that is used in the treatment of hallux rigidus. It removes dorsal and dorsomedial or dorsolateral osteophytes, primarily relieving the impingement at the first metatarsophalangeal joint, which causes patients pain. The minimally invasive technique has proven to be an excellent technique to remove bony spurs to relieve symptoms with minimal surgical complications and fast recovery time.

The treatment of hallux valgus.

BACKGROUND: Hallux valgus is the commonest forefoot deformity, with an estimated prevalence of 23% to 35%. It causes symptoms on the medial edge of the foot, the sole, and the small toes. Non-operative treatment may alleviate symptoms but does not correct the deformity of the big toe. Surgery is indicated if the pain persists. The correct operation must be selected from a wide variety of available techniques. METHODS: In this article, we selectively review the pertinent literature, including the recommendations of medical societies in Germany and abroad, in the light of our own clinical exper

The pathogenesis of hallux valgus.

The first ray is an inherently unstable axial array that relies on a fine balance between its static (capsule, ligaments, and plantar fascia) and dynamic stabilizers (peroneus longus and small muscles of the foot) to maintain its alignment. In some feet, there is a genetic predisposition for a nonlinear osseous alignment or a laxity of the static stabilizers that disrupts this muscle balance. Poor footwear plays an important role in accelerating the process, but occupation and excessive walking and weight-bearing are unlikely to be notable factors. Many inherent or acquired biomechanical abnor

Turret exostosis of the hallux.

A 22-year-old woman presented with pain on the dorsum of her left big toe. The patient had had an accident 1 year previously, when a heavy object fell on her left big toe. She experienced no pain in the hallux before the injury. Radiographic evaluation revealed a well-circumscribed osseous mass, in close relationship to the underlying dorsal cortex of the proximal phalanx. A 2 x 1.5 x 0.7 cm mass was resected. No communication was found between the undersurface of the bony mass and the medullar space of the phalanx. The histology report confirmed diagnosis of acquired osteochondroma. Removal o

[Pathogenesis of hallux valgus].

In terms of its etiology, the hallux valgus deformity is differentiated into a primary and a secondary form. Widening of the angle between the first and second metatarsals (metatarsus primus varus) is often associated with a hallux valgus deformity (primary hallux valgus). Secondary hallux valgus is caused by changes to the hind- and midfoot. In the presence of a pes planovalgus, lowering of the navicular and talus bones results in a deformation of the hindfoot. This leads to the collapse of the longitudinal arch and pronation of the forefoot (splay-foot). As a result all the tendons that cros

The pathogenesis of hallux rigidus.

The clinical, radiological and pathological features of hallux rigidus affecting nine toes (in seven patients) are described. Characteristic chondral and osteochondral lesions are seen to occur at a specific site on the metatarsal head, and account for the limitation of dorsiflexion but relatively unrestricted plantarflexion typical of hallux rigidus. Radiologically these lesions are often missed because they are mainly cartilaginous and are later obscured by secondary degenerative changes. Histological evidence indicates a traumatic aetiology and a mechanism of injury is suggested.
