[Functional outcome of cheilectomy for the treatment of hallux rigidus].

INTRODUCTION: Cheilectomy is performed as treatment for hallux rigidus. Its aim is the relief of pain and improvement of function. Previous pedobarograhic studies have revealed that osteoarthritis of the first metatarsophalangeal joint leads to a lateralisation of peak forces during the stance phase of gait. This occurs since the big toe does not participate in push-off in a sufficient way. The results concerning functional outcome as well as subjective patient satisfaction were assessed in the present study. Plantar pressure distribution analysis was used in order to investigate functional ou

Classification systems for hallux rigidus: a review of the literature.

BACKGROUND: Hallux rigidus (HR) is one of the most common causes of forefoot pain. A number of classification systems have been developed to evaluate HR. These systems are based on either radiological parameters alone or a combination of radiological and clinical features. The purpose of this paper is to critically evaluate HR classification systems by undertaking a detailed review using English language medical and paramedical databases. MATERIALS AND METHODS: The authors identified 18 formal HR classification systems described in the literature, some of which are variations on a theme. RES

Hallux rigidus: A review of the literature and a method of treatment.

Twenty patients with hallux rigidus were evaluated following cheilectomy. The surgical procedure involved resection of proliferative bone at the metatarsophalangeal joint to allow at least 45 degrees of passive dorsiflexion. The follow-up ranged from 7 months to 156 months. Postoperatively, there was minimal progression of the degenrative process. Subjectively, there was uniform patient satisfaction with the procedure.

Medial capsular interpositional arthroplasty for severe hallux rigidus.

BACKGROUND: Multiple surgical options have been described for severe hallux rigidus. One option is capsular interpositional arthroplasty. We report our initial results with a technique using the thicker medial capsule as our interpositional material instead of the dorsal capsule and extensor hallucis brevis (EHB). MATERIALS AND METHODS: Twenty-two patients with grade IV hallux rigidus underwent minimal proximal phalanx resection (modified Keller) with preservation of the flexor hallucis brevis (FHB) insertion and medial capsular interpositional arthroplasty. RESULTS: Postoperative AOFAS hall

Complications Associated with the Surgical Management of Hallux Rigidus.

Hallux rigidus is the most common arthritic pathology of the foot. This review article discusses the pathophysiology and common clinical presentation of hallux rigidus. Furthermore, we discuss multiple classification systems that categorize the arthritic process and guide management. Surgical interventions include cheilectomy, Moberg osteotomy, synthetic cartilage implants, interpositional arthroplasty, and arthrodesis. The common complications are reviewed, and the rates of these complications highlighted. Surgical options for hallux rigidus globally have successful outcomes with low rates of

Open, Arthroscopic, and Percutaneous Cheilectomy for Hallux Rigidus.

Cheilectomy consists of excision of the dorsal exostosis and part of the metatarsal head. It is typically performed for patients in the earlier stages of hallux rigidus presenting with dorsal pain and dorsiflexion stiffness in the absence of through-range symptoms, rest pain, and plantar pain and with negative result on grind test. If joint motion-preserving surgery is appropriate, then cheilectomy is generally considered to be the first-line surgical choice. In addition to the standard open technique, minimally invasive surgery in the form of either percutaneous or arthroscopic surgery is ava

Quantitative review of operative management of hallux rigidus.

INTRODUCTION: Surgical techniques for the management of hallux rigidus include cheilectomy, Keller resection arthroplasty, arthrodesis, Silastic implantation, phalangeal or metatarsal osteotomy, capsular arthroplasty, partial or total joint replacement, interposition arthroplasty. However, the optimal management is controversial. SOURCES OF DATA: We performed a comprehensive search of CINAHL, Embase, Medline and the Cochrane Central Registry of Controlled Trials, from inception of the database to 2 November 2010. Sixty-nine articles published in peer reviewed journals were included in this co

Evaluation of the treatment of hallux rigidus by percutaneous surgery.

This longitudinal prospective study of 22 patients (26 feet) aimed to evaluate the effectiveness of percutaneous surgery in the treatment of hallux rigidus, and to assess patient satisfaction with the result of this surgical approach. Pain levels (VAS), quality of life (SF-12) and clinical stage (AOFAS) were scored prior to surgery and 18 months after surgery. Pain relief was noted in all cases, with mean pain scores falling from 7.44 before surgery to 1.69. Perception of quality of life also improved, while AOFAS scores rose from 58.45 to 92.36. These results suggest that percutaneous treatme

Results after cheilectomy in athletes with hallux rigidus.

From 1987 to 1993, 20 athletes (22 feet) underwent cheilectomy for Regnauld grade I, grade II hallux rigidus. Average age was 31 years (10 men and 12 women); mean follow-up was at 5.1 years. All patients performed high-level sports (judo, track & field, soccer, and skating). Indications for surgery included failure of nonsurgical treatment with persistent pain during sports activities, shoefitting problems, and recurrent bursitis. The aim of our study was to evaluate the results clinically, radiographically, and objectively, using dynamic and static pedodynographic measurements. After a mean f

Dorsal wedge osteotomy in the treatment of hallux rigidus.

The results of 18 dorsal wedge osteotomies in the treatment of hallux rigidus are presented. Patients were retrospectively interviewed and examined, and an assessment was made in a number of key functional areas. Good or excellent results were obtained in 14 or 18 cases at a mean follow-up time of 4 years with only one patient requiring further surgery. Pain (p = 0.00014), activity levels (p = 0.0019), footwear difficulties (p = 0.0016), and joint range of motion (p = 0.0001) were all significantly improved; tiptoeing was the only activity assessed (p = 0.23) for which the improvement was not

Metatarsus primus elevatus and the etiology of hallux rigidus.

One hundred twenty radiographs of the foot were evaluated for hallux abductus angle, lateral talometatarsal angle, and metatarsus primus elevatus position. Mean elevatus position of the first metatarsal above the second metatarsal was approximately 7.0 mm. First ray elevation was a consistent finding in all data groups studied, including controls. The presence of hallux abducto valgus or hallux rigidus did not affect the degree of first ray elevation present. The majority of radiographs studied lacked evidence of joint disease. Therefore, the common practice of evaluating radiographic elevatus

End-stage hallux rigidus: cheilectomy, implant, or arthrodesis?

End-stage arthritis of the first metatarsophalangeal joint (MTPJ) typically results in anexophytic process with marked limitation of motion. Pain may occur from the degenerative process itself and/or the bone spur formation that may become directly inflamed from shoe gear. The best surgical treatment for end-stage arthrosis of the big toe joint continues to be a controversial topic despite hallux rigidus being recognized clinically for more than 100 years. Although joint-sparing procedures are considered, arthrodesis is recommended, as this procedure is definitive and produces predictable resu

Free gracilis interposition arthroplasty for severe hallux rigidus.

Surgery for the management of hallux rigidus aims to relieve pain and improve function. Arthrodesis, though reliable in terms of pain relief may encounter some resistance from patients reluctant to have a fusion. Results of other techniques, such as silastic implants, may give poor long-term results. Excisional arthroplasty and cheilectomy are also routinely used. Interposition arthroplasty has been used with some success in other joints. We describe a technique of autologous interposition arthroplasty using a free ipsilateral gracilis tendon graft for patients with severe hallux rigidus with

Quantitative relationship of first metatarsophalangeal head morphology to hallux rigidus and hallux valgus.

BACKGROUND: Previous qualitative studies have linked first metatarsal head morphology with hallux valgus (HV) and hallux rigidus (HR). This study used a quantitative measurement of 1st MT radius of curvature to assess if HR MT heads were flatter than HV heads. METHODS: Weight bearing foot films were used in HV, HR, and normal patients (no forefoot complaints) to measure the metatarsal head radius of curvature (normalized by dividing the radius of curvature by the first metatarsal length to adjust for magnification and foot size). RESULTS: Radiographs from 299 feet were analyzed (105 normal,
