Radiological Analysis of the Syndesmosis Concept in Metatarsus Primus Varus and Hallux Valgus Deformities Recurrence Prevention.

Hallux valgus (HV) is a common deformity of the foot. Its postoperative recurrence is not uncommon and is closely related to the recurrence of its underlying metatarsus primus varus (MPV) deformity. The syndesmosis procedure uses 1 to 2 intermetatarsal cerclage sutures to realign the first metatarsal and then induces a biological bonding between the 2 metatarsals to prevent the MPV deformity from recurring. This radiological study aimed to assess its effectiveness in long-term MPV and HV deformities recurrence prevention. Ninety-two feet of 51 consecutive patients had syndesmosis procedures th

[Intermediate results following subcapital metatarsal osteotomy in the therapy of hallux valgus with metatarsus primus varus].

71 feet were investigated after subcapital modified osteotomy of the metatarsus according to Kramer for an average follow-up period of 19 months. Mean improvement of the Hallux valgus angle was 10 degrees whereas the intermetatarsal angle was improved by 4 degrees on average. The high degree of satisfaction of patients (83%) with the outcome of this surgical method suggest its application in cases of medium metatarsus primus varus within hallux valgus deformations when the metatarso-phalangeal joint is not affected. In patients aged 50 years and older as well as in cases of MT I varus exceedin

The geriatric bunion: correction of metatarsus primus varus and hallux valgus with the Swanson total joint implant.

The authors have evaluated the long-term results of the Keller/Mayo bunionectomy with replacement of the first metatarsophalangeal joint with a Silastic double-stemmed implant (Swanson design). Examination of 50 preoperative and postoperative radiographs revealed a decrease in the hallux abductus angle of 21.46 +/- 2.66 degrees, and the intermetatarsal angle of 6.40 +/- 0.67 degrees; both of these figures have a statistically significant probability value of 0.05 attached. The mean patient age at time of surgery was 61.31 years; the mean follow-up time was 5.01 months. A literature review of i

Failure after soft-tissue release with tendon transfer for flexible iatrogenic hallux varus: a systematic review.

Hallux valgus is a common forefoot pathology often requiring surgical intervention for symptomatic relief. One complication of hallux valgus correction is flexible hallux varus. Iatrogenic flexible hallux varus often requires surgical repair; however, the most advantageous surgical procedure for repair of iatrogenic flexible hallux varus and their sustainability remains unclear. Therefore, we performed a systematic review to determine the sustainability of soft-tissue release with tendon transfer for the correction of iatrogenic flexible hallux varus. Studies were eligible for inclusion only i

Bilateral congenital hallux varus with metatarsus adductus in a 6-year-old girl: A case report.

BACKGROUND: Congenital hallux varus (CHV) is a rare form of hallux varus deformity, characterized by medial deviation of the first toe at the metatarsophalangeal joint. It may be primary or secondary and presents clinically with pain and asymmetry with footwear. CASE SUMMARY: We documented a case of a 6-year-old girl with bilateral CHV, accompanied by adduction of the toes in the left foot. Clinical diagnosis was made by physical examination and X-ray imaging based on Bleck's classification. Conservative treatment did not show any noticeable improvement, so the child underwent corrective surg

Management of Iatrogenic Hallux Varus with Local Soft Tissue Reconstruction: A Case Series with Mid-Term Outcomes.

Iatrogenic hallux varus is a rare but functionally disabling complication most commonly following hallux valgus surgery. Traditional revision techniques, such as tendon transfer or arthrodesis, may compromise joint mobility or require implants. This study evaluates a biologically autologous alternative-local soft tissue myoligamentoplasty-for the correction of flexible deformities. # Methods A retrospective case series of seven patients (mean age 62 years) with flexible iatrogenic hallux varus following prior hallux valgus surgery was conducted at a single tertiary care center. All patients un

Are sesamoids and proximal phalanx of metatarsus primus varus deformity of hallux valgus feet displaced? A radiological study.

BACKGROUND: The hallux valgus deformity is made up of misaligned first metatarsal, hallux, and sesamoids. Their angular deformities are well-studied, but not their positional displacements. A few available reports claimed the proximal end of the proximal phalanx and sesamoids were not shifted medially along with the first metatarsal head. However, the general observation is otherwise. This study revisits the issue. METHODS: A radiological study of 189 feet with and without the hallux valgus deformity was carried out to analyze the first metatarsal, hallux, and sesamoid positional changes in r

Can the Syndesmosis Procedure Prevent Metatarsus Primus Varus and Hallux Valgus Deformity Recurrence? A 5-Year Prospective Study.

One of the main objectives of hallux valgus surgery is correction of the metatarsus primus varus deformity by osteotomy, arthrodesis, or soft tissue correction. The syndesmosis procedure uses intermetatarsal cerclage sutures to realign the first metatarsal and also induces a syndesmotic bonding between the first and second metatarsals to prevent metatarsus primus varus deformity recurrence. The purpose of the present study was to demonstrate radiologic evidence of the effectiveness of the syndesmosis concept and to identify the incidence and nature of deformity recurrence. A new surgical procedure for iatrogenic hallux varus: reverse transfer of the abductor hallucis tendon: a report of 7 cases.

Iatrogenic hallux varus is a possible complication of hallux valgus surgery following Mc Bride or Scarf osteotomy, with or without Akin osteotomy of the first phalanx. It may also occur following chevron osteotomy or Keller's procedure. One possibility for surgical revision of iatrogenic hallux varus is reconstruction of the lateral stabilising soft-tissue components of the first metatarsophalangeal joint. Until now, only dynamic tendon transfers, possibly combined with interphalangeal fusion, have been described. The aim of our study was to develop a static, anatomic reconstruction procedure.

A method of predicting the degree of functional correction of the metatarsus primus varus with a distal lateral displacement osteotomy in hallux valgus.

Metatarsus primus varus may be an important pathologic component in hallux valgus deformity. The correction of the pathologic intermetatarsal angle M1 M2 is essential for providing an acceptable alignment of the hallux. The distal lateral displacement osteotomy of the first metatarsal (Chevron or Mitchell) provides correction of the intermetatarsal angle M1 M2. The present paper describes a simple method of calculating preoperatively the maximum possible correction through such osteotomies. This method, based on the measurement of the length of the first metatarsal and the width at the osteoto

Gamekeeper's Toe: A Case Series and Review of Surgically Treated Traumatic Hallux Varus Due to Lateral Collateral Ligament Injury.

BACKGROUND: Injuries to the lateral collateral ligament (LCL) of the first metatarsophalangeal (MTP) joint that are treated surgically are rare. We present 3 cases of traumatic LCL injury of the first MTP joint that were treated surgically for a Stener-like lesion preventing healing. Additionally, we present a review of previously published cases. METHODS: Diagnosis was confirmed with magnetic resonance imaging and intraoperative stress tests, which demonstrated instability and LCL disruption with interposition of the adductor hallucis aponeurosis. All cases were managed operatively with sutu

Basal closing wedge osteotomy for correction of hallux valgus and metatarsus primus varus: 10- to 22-year follow-up.

Between 1974 and 1985, 59 patients (83 feet) underwent basal closing wedge osteotomy in combination with a bunionectomy and a lateral soft tissue release for correction of hallux valgus and metatarsus primus varus at this institution. Of the original 59 patients, 42 patients (60 feet) with at least 10 years of follow-up (average, 194 months; range, 144-266 months) were available for this study. Results were analyzed by review of the medical records and plain radiographs, a standardized clinical questionnaire, and physical examination. Of the 60 feet, patients rated outcomes as excellent or goo

Arthroscopically Assisted Reconstruction of Lateral Metatarsosesamoid Suspensory and Lateral Metatarsophalangeal Collateral Ligaments in the Management of Iatrogenic Hallux Varus Deformity.

The medial and lateral metatarsosesamoid suspensory ligaments together with the sesamoid apparatus are the main contributors to the rotational stability of the first metatarsophalangeal joint (MTPJ). Rotational instability of first MTPJ is a major factor in the development of hallux valgus and varus deformities. As the great toe starts to rotate, the muscle pulls across the first MTPJ, causing it to become imbalanced and lead to progressive medial or lateral deviation of the great toe. The gold standard and most reliable option for hallux varus deformity is first MTPJ fusion. However, it resul

Reconstruction of Secondary Type Congenital Hallux Varus with Modification of the Farmer Technique for Cover of Skin Defect: Report of Three Cases.

The reconstruction of hallux varus deformity involves the release of contracted medial structure and realignment of the phalange, leaving a significant skin defect which requires cover. Farmer described proximal based rotational skin flap from the first web space to cover the defect. This technique may compromise the circulation to the flap and risk to the lateral digital vessels. We modified his technique to address these issues. We report a successful reconstruction using the Farmer's technique on one patient and a modified technique on three patients. We used the excess skin from the extra

Revolution or Evolution? The Treatment of Metatarsus Primus Varus with Hallux Valgus: My Experience with Modified Lapidus and Minimally Invasive Surgery.

Minimally invasive surgery has a high learning curve and multiple instructional courses and trainings should be completed by the surgeon prior to patient application. Modified Lapidus also is still a valid procedure selection in patients that may have concomitant medial column pain and/or dysfunction. Procedure selection is not "one-size fits all".

Distal chevron osteotomy with lateral soft tissue release for moderate to severe hallux valgus decided using intraoperative varus stress radiographs.

The purpose of the present study was to investigate the outcomes of distal chevron osteotomy with lateral soft tissue release for moderate to severe hallux valgus. The patients were selected using criteria that included the degree of lateral soft tissue contracture and metatarsocuneiform joint flexibility. The contracture and flexibility were determined from intraoperative varus stress radiographs. From April 2007 to May 2009, 56 feet in 51 consecutive patients with moderate to severe hallux valgus had undergone distal chevron osteotomy with lateral soft tissue release. This was done when the

Influence of Hallux Valgus Varus Deformity on First Metatarsal Sagittal Inclination Assessment: A Comparison Between Weightbearing Radiography and Weightbearing Computed Tomography.

BACKGROUND: Radiographic assessment of the sagittal inclination of the first metatarsal (1M) is essential for evaluating foot disorders. However, 1M varus deviation may influence these measurements. This study aimed to determine whether hallux valgus (HV) varus deformity influences the sagittal radiographic inclination of the 1M by comparing weightbearing radiography (WBR) and weightbearing computed tomography (WBCT) images. METHODS: Eighty-four feet were analyzed, including HV cases with intermetatarsal angle (IMA) >15° and control feet without HV. The sagittal inclination of the 1M, its bas
