Bulletin of the Hospital for Joint Disease (2013)2022eng
Avascular necrosis (AVN) and subsequent fragmentation of the proximal pole of the scaphoid following fracture is a challenging problem to treat. Multiple treatment methods have been described, although they have been shown to have varying degrees of success and are associated with donor site morbidities. This case report demonstrates a technique and the excellent radiographic and clinical outcome at 8 months postoperatively for reconstruction of the proximal pole of the scaphoid using an ipsilateral proximal pole of the hamate autograft.

Background: This study aimed to investigate the relationship between postoperative clinical results and long-term morphological changes in patients with carpal tunnel syndrome (CTS) as observed on magnetic resonance imaging (MRI) before and after open carpal tunnel release (OCTR). Methods: We retrospectively analysed data for 28 hands that had undergone OCTR with at least 24 months of follow-up data. Two-point discrimination (2PD) test results were examined for the first three fingers, as were the distal motor latency (DML) and sensory conduction velocity (SCV) of the median nerve. We also cal

We report the usefulness of hook of hamate pull test (HHPT), described by Wright et al. in 2010, along with therapeutic outcomes of hook of hamate fractures. Eleven patients (two with fresh fractures and nine with nonunion) were studied. The fractures were diagnosed with HHPT for recently encountered 4 patients and a definitive diagnosis was made by a carpal canal view and a computed tomography (CT) scan. Treatment involved cast immobilization for one fresh fracture case, and bone fragment removal immediately over the hook for ten others. HHPT was positive in all the four cases. Union was achi

OBJECTIVE: The aim of this study was to demonstrate a new robotic technique for the fixation of the fracture of the hook of hamate.
METHODS: In this retrospective study, six patients who had undergone treatment of the hook of hamate fracture with a new technique using robot navigation were included. Patients were evaluated postoperatively with flexion - extension and radial-ulnar range of wrist motion, and were measured using a goniometer. Grip strength and pinch strength were measured and compared with the contralateral wrist at the final follow-up. The Visual Analog Scale (VAS) and the Mayo

Background: Treatment of proximal interphalangeal joint (PIPJ) fracture-dislocations is difficult given the potential long-term complications of the involved finger and entire hand. Several surgical methods have been utilized for management of these injuries, none of which have shown consistently favorable results. The purpose of this systematic review of the literature is to report the post-operative outcomes of multiple treatment modalities for PIPJ fracture-dislocations in various studies. Methods: A literature review of PubMed and EMBASE databases was performed for all articles on PIPJ fra

Ulnar-sided wrist injuries are common in sports that require repeated pronosupination, wrist radial/ulnar deviation, axial loading, and gripping equipment. Common anatomic structures affected include the triangular fibrocartilage complex, extensor carpi ulnaris tendon, distal radioulnar and ulnocarpal joints, and hamate bone. Presenting symptoms include pain with activity, swelling, possible snapping or clicking, and reproduction of symptoms with provocative maneuvers. Imaging may confirm or rule out pathologies, but abnormal findings also may present in asymptomatic athletes. Initial treatmen

We report the treatment of three patients with stage 3 Kienböck disease that had advanced after radial shortening (n = 2) or lateral wedge osteotomy (n = 1). We shortened the capitate and fused the capitate and hamate, and applied a vascularised bone graft taken from the distal dorsal radius to the lunate. This was successful in all three patients.

The hypothenar hammer syndrome is an uncommon but underestimated lesion of the cubital artery caused by repetitive trauma at the level of the hamate bone. It characteristically occurs in patients with a history of manual work as metal workers, carpenters and motor mechanics. We present a case of a patient who developed this syndrome following intensive use of a dig. Clinical finding, diagnosis and treatment are discussed.

BACKGROUND: One of most accepted principles for treating Kienböck's disease before wrist degeneration settles in is to decompress the lunate by an osteotomy. Several osteotomies have been proposed since 1935. However, they are based on biomechanical hypotheses that are sometimes conflicting: This study compares the decompression effect of radius transverse shortening, radius lateral closing and medial closing wedge osteotomies, capitate shortening - with and without hamate shortening - and a Camembert-type radius wedge osteotomy with and without ulnar head shortening according to Sennwald.

During routine anatomical dissection in the hypothenar region of the left hand of a 64-year-old female cadaver, a number of variant structures were observed. The most prominent finding in our case was a supernumerary muscle hitherto unknown in the anatomical literature. This variant muscle had a muscular body formed by the connection of two deeply situated muscular bellies--medial and lateral. The lateral belly originated from the flexor retinaculum, the medial one--from the hamate bone. The common muscular body inserted to the antero-lateral surface of the base of the fifth proximal phalanx.

RATIONALE: Pan-carpal dissociation is very rare injury and there is little information as to diagnosis, treatment, and prognosis of this injury.
PATIENT CONCERNS: A 35-year-man presented to our hospital with severe pain and swelling of the left wrist and forearm after slipping and falling while riding a motorcycle.
DIAGNOSIS: The wrist simple radiographs demonstrated unrecognizable severe fracture-dislocation of the carpal bones concomitant with fractures of the radioulnar shaft. Three-dimensional computed tomography revealed a capitate fracture-dislocation, as well as hamate dislocation, lu

Background: The excision of the hook of the hamate is an accepted modality for the treatment of hook of hamate fractures. Three surgical approaches to the hook of hamate have been described in literature. This includes two palmar approaches namely the Guyon canal approach and the carpal tunnel approach, and the lateral approach. The aim of this article is to compare the outcomes of the carpal tunnel approach and the lateral approach. Methods: Twenty-four patients with hook of hamate fractures were treated by excision of the hook of hamate. The hook of hamate was approached via the carpal tunne

BACKGROUND: A fracture of the hook of the hamate is a common injury affecting professional baseball players. The treatment for these fractures ranges from nonoperative immobilization to excision of the fragment.
PURPOSE/HYPOTHESIS: The purpose was to report the results of surgical treatment of hook of hamate fractures in professional baseball players and determine which factors are associated with return to sport (RTS) and time to RTS. The hypothesis was that there is a high rate of RTS in professional baseball players after surgical treatment of the hook of the hamate fracture with no signif

A 65-year-old male presented with an acute nondisplaced hook of hamate fracture while lifting a suitcase. Conservative management was employed, and he was treated with a thumb spica cast. At the most recent follow-up, he demonstrated resolution of pain and restoration of motion. CT imaging confirmed osseous union. High nonunion rates following conservative management with short arm casting without thumb immobilization for hook of hamate fractures may be in part due to inadequate immobilization. Here, use of a thumb spica cast resulted in successful osseous union following an acute hook of hama

Thirty-three patients with pisiform area pain have been treated. Of these, the pisiform bone was excised in 21 patients who had intractable pisiform bone pain that was not relieved by the conservative methods of immobilization, local steroid injection, and anti-inflammatory medication. The indications for excision were painful union or nonunion of pisiform fractures, pisiform-triquetrial arthritis, and tendonitis of the flexor carpi ulnaris (FCU) insertion. Wrist strength and mobility was maintained by doing a subperiosteal dissection and removal of the pisiform bone. This technique preserves

Scaphoid proximal pole fractures remain a surgical challenge because of high propensity for nonunion, osteonecrosis, and ultimately carpal collapse. Options for management of nonsalvageable proximal pole fractures include non-vascularized bone grafts, vascularized pedicled bone grafts, free vascularized bone flaps, and rib cartilage grafts. The proximal pole of the hamate can also serve as a replacement arthroplasty in the setting of proximal pole scaphoid nonunions with collapse, bone loss, and/or osteonecrosis. This novel graft addresses shortcomings of other graft choices by providing a loc

Background: The purpose of this study is to determine the effects of proximal hamate transfer for proximal pole scaphoid reconstruction upon carpal kinematics. Methods: Eight fresh-frozen cadaveric wrists underwent evaluation of their radiocarpal and midcarpal motion after proximal hamate osteotomy. A wrist simulator was used to apply cyclical tension to the flexor carpi ulnaris (FCU), flexor carpi radialis (FCR), extensor carpi ulnaris (ECU), and extensor carpi radialis longus and brevis stitched together (ECR). Kinematic motion was captured using Moiré Phase Tracking 3-dimensional motion-tra

Fractures of the hook of the hamate are rare injuries of the hand. They are caused by direct trauma to the palm, however there are some cases without history of trauma. While physical examination is often suspicious of fracture, standard X-ray films can fail to demonstrate the fracture. Bone scans show an increased uptake in the ulnar region of the wrist. The carpal tunnel view or computed tomography confirm the diagnosis. Operative treatment is recommended. Simple excision of the hook, bone grafting or internal fixation are possible procedures. The diagnostic problems are demonstrated in two

This article documents the outcome of treatment of intraosseous ganglia and simple bone cysts of the carpal bones by curettage and injectable calcium phosphate bone cement (CPC) grafting. The patients consisted of five men and three women. One had a cystic lesion in the scaphoid, one in the hamate, and five in the lunate. Curettage of the lesions was performed, and CPC was injected into the cavity. Five patients were diagnosed with a ganglion and three with a simple bone cyst. Among the five patients with wrist pain, the pain disappeared completely in four. Radiographs showed apparent partial
