Treatment of acute injuries of the hand Abstract. Injuries to the hand are a common presentation in primary care units. When accurately assessed, many open injuries may be handled in the emergency department without referral to a hand surgery specialist. We would like to give some recommendations on how to treat the most frequent injuries like lesions to the nail and nailbed, fingertip amputation as well as burns, infections and bites. But first, we highlight the different methods of local anesthesia and discuss the use of a tourniquet or vasoconstriction with adrenalin - WALANT - instead.

Injuries of the hand and wrist are frequently encountered in radiology. Avulsions of the hand and wrist are a heterogeneous group of injuries, but they often have a characteristic imaging appearance that relates to the intricate bone and soft-tissue anatomy and the mechanism of injury. The imaging appearance and this intricate form and function dictate treatment of hand and wrist avulsions. This article reviews frequently and infrequently encountered avulsion injuries and describes abnormalities that may mimic the imaging appearance of avulsions. Specifically discussed entities include the Ben

The authors retrospectively reviewed their experience with bilevel positive airway pressure (BiPAP) to treat respiratory insufficiency in pediatric patients over a 5-year period. After excluding patients on chronic home BiPAP and those in whom BiPAP was used to facilitate tracheal extubation (because there were no pre-BiPAP values on which to judge its efficacy), the study cohort included 45 patients (1.5 to 22 years) in whom BiPAP was used for acute respiratory insufficiency. The primary indication for BiPAP was a primary pulmonary parenchymal process in 29 patients and postoperative atelecta

High pressure injection of oil-based substances can cause devastating injuries of the hand and the necessity for urgent surgical debridement has been well established. We present three cases of injection injury caused by vaccines used in the fish farming industry. Patients presented with pain, swelling and lymphangitis. The marked vascular changes often associated with such injuries were absent. All patients were treated with intravenous antibiotics combined with early surgical debridement and irrigation. We report full recovery in all three patients.

Sports-related injuries of the wrist and hand are commonly encountered in clinical practice. Athletic injuries create special challenges for the orthopedic surgeon and radiologist, as these patients expect to recover quickly and return to the same athletic endeavors that caused their original injuries. A thorough understanding of the mechanism of injury and imaging findings are necessary for accurate diagnosis and effective therapy. This knowledge allows the clinician to effectively educate the athlete concerning the risks, complications, and prognosis of the injury and its treatment. This art

A review of dog bite injuries referred to one surgeon over a 12 month period demonstrated a significant association between a delay in referral and a prolonged period of subsequent in-patient treatment. Two patients sustained injuries when delivering material through a letter-box.

Successful treatments of musculoskeletal injuries in the pediatric population demand a thorough understanding of the basic anatomy and its biomechanics, and the physiology of growth and development of the immature skeleton. In addition, good treatment outcomes rely on the treating physician being an effective teacher to the young athlete and the patient's parents, coaches, and trainers. At the same time, the physician must be a good student in learning the nature of the patient's sports and each patient's athletic ability and aspirations. Most pediatric hand and wrist injuries can be treated n

To permit a smooth entry into the water, platform divers assume a special position with their hands. An older method consists of hitting the water with both fists closed while holding one thumb in the palm of the other hand. In a newer technique fingers and wrists are hyperextended, the forearms pronated, and the hands overlapped. In this series of 21 divers, the injuries included 2 fractures and 5 sprains treated by conservative methods. Two divers required surgery: one with loose bodies in the radiocarpal articulation, the other with chronic rupture of the trapeziometacarpal ligament and syn

A total of 25 cases of acute injuries caused by a meat mincer are presented. Of these, 20 were children. The pattern of injury and the extent of damage to individual digits of the hand are defined. Due to the mutilating nature of the injury, attempts at replantation or revascularization failed in all but two of the ischaemic digits. Emphasis must be placed on the safety and prevention of these dreadful accidents.

Airless gun injection injuries of the hand are a surgical emergency. The injection of paint or paint solvents are the most pernicious, whereas grease or diesel fuel injections have a better prognosis for the victim. Tissue necrosis and subsequent morbidity results from the diffusion of the noxious substance. Solvents and paint generate the most severe inflammatory response. Prompt major surgical treatment can minimize morbidity.

Severe damage to functionally important structures of the hand as a direct result of megavoltage radiation may be seen. The appropriate treatment of these uncommon acute and chronic radiation changes consists in principle or excision and skin flap covering. Such treatment will limit the functional disability of the hand. In addition, the severe late consequences of radiokeratosis and squamous cell carcinoma can be prevented.

Electrical injury results in serious damage or amputation of the upper extremity in a significant percentage of survivors. Fourteen of sixteen patients with electrical injuries followed over a two and a half year period had major hand wounds. Careful wound assessment, management, and closure combined with appropriate reconstructive technics now available provide the greatest potential for restoration of satisfactory quality of life. Because these injuries are often of an industrial nature, occupational vigilance and prevention remain essential.

Although carpal tunnel syndrome associated with injury to the wrist is common, it is possible to overlook symptoms of median nerve compression caused by an ascending tenosynovitis secondary to trauma distal to the wrist. One should look for these symptoms in such patients who complain of pain and weakness of the hand, and release the carpal tunnel if nerve compression is suspected.

The powerful forces involved in the function of motorized lawnmowers can cause serious injuries. The study reported was based on a series of 81 cases of injury, that were treated at University Hospital, Umeå. Direct trauma to hand and foot was the most common injury mechanism, though high energy expulsion of objects from the machine was not uncommon. A common cause of direct trauma to hand or foot was attempting to clear grass or other matter from the vicinity of the rotating blades. Hand injuries were most common and occurred predominantly in men, whereas women and children accounted for a gr

BACKGROUND: Most hand injuries in children can be managed non-operatively and are associated with excellent outcomes. Whilst the majority of our patients are discharged to the care of hand therapists, there is no literature to support this protocol. Our aim was to ensure this is safe and effective practice.
MATERIAL AND METHODS: We conducted a retrospective analysis of all patients referred to our paediatric hand trauma clinic for closed injuries over a four month period between December 2014 and March 2015. Data related to demographics, injury pattern and clinical outcomes was recorded and a

The extensor apparatus of the hand is a complex system consisting of extrinsic and intrinsic muscles, which in combination enable the individual extension of the fingers. Extensor tendon injuries of the hand are frequent injuries and the operative or conservative treatment options are determined by the localization and involvement of osseus structures. For an optimal outcome of the treatment of extensor tendon injuries, correct diagnostics and a consistent hand aftercare are absolutely essential. The crucial decision making regarding the further procedure starts with the initial patient treatm

Ligamentous injuries in the hand and wrist are often underdiagnosed and can present with significant functional limitations if there is untimely recognition of injury. Adequate repair or reconstruction is critical in restoring joint stability and mobility. The purpose of this review is to provide an overview of the metacarpophalangeal joint, scapholunate interosseous ligament (SLIL), and non-SLIL carpal ligament anatomy, diagnosis, imaging, treatment consideration and options, as well as surgical techniques encompassing repair, reconstruction, and fusion.
