Plate fixation of midshaft clavicular fractures: patient-reported outcomes and hardware-related complications.

BACKGROUND: Recent studies report high hardware removal rates after plate fixation of midshaft clavicular fractures. Precontoured clavicle plates may decrease hardware-related complications while improving healing rates and patient-reported outcomes (PROs). METHODS: Using a private-payer national database, we identified 7826 patients who underwent clavicle open reduction and internal fixation (ORIF) in 2007 to 2011. Database patients were tracked for 2 years to assess hardware removal and revision fixation. In addition, we retrospectively identified 73 patients who underwent plate fixation of

A call for fixed standards: a decade of orthognathic surgery, biting into revision rates and metalwork removal.

The removal rate of titanium miniplates following orthognathic surgery varies widely (3.2-27.5%) due to inconsistent study designs and mixed samples. Plate removal is not routinely conducted in the UK. A 2019-2020 meta-analysis reported a 13.4% removal rate, though regular audits are uncommon. This study evaluates whether audit of local plate removal rates against established benchmarks merits encouragement. A retrospective review was conducted of orthognathic surgery at a tertiary centre (2014-2024). Procedures included Le Fort osteotomy, bilateral sagittal split osteotomy (BSSO), bimaxillary

Incidence of hardware-related pain and its effect on functional outcomes after open reduction and internal fixation of ankle fractures.

OBJECTIVES: To document the incidence of late pain and hardware removal after open reduction and internal fixation (ORIF) of ankle fractures. To test the hypothesis that late pain overlying the distal tibial and fibular hardware is associated with poorer functional outcomes. DESIGN: Retrospective review. SETTING: Level II trauma center. PATIENTS: One hundred twenty-six skeletally mature patients undergoing ORIF of unstable malleolar fractures who were followed up for at least six months from injury were included. MAIN OUTCOME MEASUREMENTS: Analog pain score, Short Form-36 Health Survey (SF

Leon Chen's Modified Valsalva Technique (LCMVT) for Retrieval of Displaced Dental Implants: A Clinical Technique Report.

The retrieval of a displaced dental implant from the maxillary sinus-whether through an intraoral lateral window or a transnasal endoscopic route-invariably requires further surgical intervention and a subsequent healing period. For instance, in cases where a lateral window approach was not initially indicated, the accidental displacement of a dental implant into the maxillary sinus often necessitates the creation of such an access, which can lead to patient dissatisfaction. If removal through the lateral window proves unsuccessful, referral to an otolaryngologist may become necessary, and a t

Outcome analysis following removal of locking plate fixation of the proximal humerus.

BACKGROUND: Concerning surgical management experience with locking plates for proximal humeral fractures has been described with promising results. Though, distinct hardware related complaints after fracture union are reported. Information concerning the outcome after removal of hardware from the proximal humerus is lacking and most studies on hardware removal are focused on the lower extremity. Therefore the aim of this study was to analyze the functional short-term outcome following removal of locking plate fixation of the proximal humerus. METHODS: Patients undergoing removal of a locking

Outcomes of subtalar arthroereisis for pediatric and adolescent flexible flatfoot: A systematic review comparing metallic versus bioabsorbable implants.

BACKGROUND: Subtalar arthroereisis is widely used to treat flexible flatfoot in growing patients. Metallic implants have historically been standard, whereas bioabsorbable devices have emerged as an alternative that may reduce implant-related symptoms and avoid elective hardware removal. We systematically compared clinical and radiographic outcomes, complication rates, and patient-reported satisfaction between metallic and bioabsorbable implants. METHODS: We performed a PRISMA-compliant systematic review and meta-analysis. PubMed, Embase, the Cochrane Library, and Web of Science were searched

Treatment of Distal Clavicle Fractures in Adults: Evidence-Based Strategies for Treatment.

» Distal clavicle fractures are less common, accounting for 10% to 30% of all clavicle fractures. » The presence of radiographic nonunion does not correlate with symptomatic nonunion, with a variable percentage of patients necessitating subsequent surgical intervention (20%). » Fracture displacement is associated with a higher risk of nonunion (31%-37%), particularly in Neer type II and V fracture patterns. » Nondisplaced distal clavicle fractures can be treated with nonoperative management (Neer types I, III, and IV). » Surgical treatment is indicated for displaced Neer type II and V fracture

Complications in a Young Adult Attributable to a Retained Pediatric Dynamic Hip Screw.

INTRODUCTION: Orthopedic implants are used for many different conditions in the pediatric population. The literature on hardware removal is controversial and vague. CASE REPORT: We highlight a young adult male who underwent a dynamic hip screw (DHS) due to a motor vehicle accident at 11 years old. He healed the fracture and did well for years. He was lost to follow-up and the hardware was never removed. The patient presented to our facility with a periprosthetic subtrochanteric proximal femur fracture just distal to the retained hardware. The DHS was removed and the fracture fixed with an int

Method of osteotomy fixation and need for removal following bimaxillary orthognathic, osseous genioplasty, and intranasal surgery: a retrospective cohort study.

The purpose of this study was to determine the incidence and causes of fixation hardware removal after bimaxillary orthognathic, osseous genioplasty, and intranasal surgery. A retrospective study was performed, involving subjects with a bimaxillary developmental dentofacial deformity (DFD) and symptomatic chronic obstructive nasal breathing. At a minimum, subjects underwent Le Fort I osteotomy, bilateral sagittal ramus osteotomies (SROs), septoplasty, inferior turbinate reduction, and osseous genioplasty. The primary outcome variable studied was fixation hardware removal. Demographic, anatomic

Bone Loss After Implant Removal.

This case illustrates the complex challenges of managing forearm trauma complicated by bone loss after implant removal. A long-standing radial shaft plate, integrated into cortical bone with overgrowth, had to be removed to treat a new peri-prosthetic fracture. Removal caused massive cortical loss, leaving poor bone stock for fixation. Dual plating was performed to bridge the defect and stabilize the distal radius fracture, but mechanical failure prompted a staged revision. A deliberately delayed second operation allowed time for biological preparation. Definitive reconstruction included corre

Case Series of Comminuted Olecranon Fracture Treated by Plate Fixation; Do We Have to Remove the Plate?

Comminuted olecranon fracture requires surgical intervention. Plate fixation has been performed on the majority of cases. We reviewed the cases of comminuted olecranon fracture in young and middle age treated by plate osteosynthesis and analyzed the functional outcome, complications and ratio and timing of hardware removal. Fifteen cases of comminuted olecranon fractures treated by plate fixation were reviewed. Bone union was achieved in all cases, the average range of motion at the final follow up was -11° in extension, 133° in flexion, 89° in pronation and 88° in supination. Hardware removal

Review of Maxillofacial Hardware Complications and Indications for Salvage.

From 2002 to 2006, more than 117,000 facial fractures were recorded in the U.S. National Trauma Database. These fractures are commonly treated with open reduction and internal fixation. While in place, the hardware facilitates successful bony union. However, when postoperative complications occur, the plates may require removal before bony union. Indications for salvage versus removal of the maxillofacial hardware are not well defined. A literature review was performed to identify instances when hardware may be salvaged. Articles considered for inclusion were found in the PubMed and Web of Sci

Delayed brain and spine migration of a retained SEEG electrode fragment: An unexpected complication.

BACKGROUND: Stereoelectroencephalography (SEEG) is a well-established technique for localizing epileptogenic zones in patients with drug-resistant epilepsy, including children. While considered safe, rare but serious complications can occur. CASE PRESENTATION: We report the case of a girl with tuberous sclerosis complex and drug-resistant epilepsy who underwent SEEG implantation as part of pre-surgical evaluation. Electrode placement was uneventful. However, during SEEG monitoring, the signal was lost on two electrodes, raising suspicion of fracture. During explantation, these electrodes were

Complications related to fracture treatment in HIV patients: a case report.

We present the case report of a 40-year-old woman who was HIV-positive in Highly Active Anti-Retroviral Therapy (HAART) and affected by femural pertrochanteric fracture, which was treated by endomedullary nailing. Two years after the surgical operation, the woman developed an aseptic symptomatic osteolysis around the implant. Hardware removal was resolutive. Aseptic and septic hardware mobilization, hardware removal, and implant decision in HIV patients with pertrochanteric fractures is discussed. The authors suggest close follow-up and prompt hardware removal, as soon as X-rays demonstrate he

A systematic review of volar locking plate removal after distal radius fracture.

BACKGROUND: Indication of volar locking plate (VLP) removal after bony healing of distal radius fracture (DRF) is controversial. Studies with various range of removal rate were reported. The purpose of this systematic review was to investigate the frequency and the reasons of hardware removal over the world. We hypothesized that more frequent VLP removal contribute to better clinical outcomes. METHODS: The authors searched all available literature in the PubMed and EMBASE databases for articles reporting on outcomes of treatment using VLP for DRF. Data collection included hardware removal rat
