INTRODUCTION: Tobacco dependence, a chronic relapsing condition, requires repeated interventions and multiple attempts to quit.
AREAS COVERED: Strategies for assisting smoking cessation include behavioural counselling and pharmacotherapy. Three drugs are currently used as first-line pharmacotherapy: nicotine replacement therapy (NRT), bupropion and varenicline. Compared to placebo, the drug effect varies from RR = 2.27 for varenicline, to 1.69 for bupropion, and 1.60 for any form of NRT. Cytisine (similar to varenicline) has a RR = 3.98 compared to placebo (two trials). Second-line pharmacoth

BACKGROUND: Nicotine pouches (NPs) are a rapidly growing category of tobacco-free oral nicotine products increasingly available in the community, including pharmacies. Whether NPs primarily function as harm-reduction tools for existing tobacco users or as a novel initiation pathway into nicotine and tobacco use, particularly among youth, remains a critical and unresolved clinical and public health question.
OBJECTIVES: To synthesize current evidence on the pharmacology, harm-reduction potential, and initiation risk of NPs; to compare NPs with FDA-approved nicotine replacement therapy (NRT) an

OBJECTIVE: Opioid misuse is a public health crisis in the United States, leading to high rates of addiction, overdose, disease, and death. There are strategies to reduce the harmful consequences associated with opioid use. Harm reduction strategies (HRS) are part of a broader, comprehensive approach to addressing the opioid crisis. By studying and implementing these strategies, communities can develop a more holistic approach to address the complexities of opioid addiction. How the public perceives HRS can influence their likelihood of being implemented across communities. This study examines

Indigenous people using substances experience health disparities that are compounded by systemic inequities shaped by colonialism. The WHiSE 2.0 study aims to describe substance use patterns and harm reduction approaches among Indigenous people who use drugs in Thunder Bay, Sudbury, and Sault Ste Marie. A community-based research approach was used and guided by Indigenous leadership. Eligible study participants self-identified as Indigenous, lived in the study regions, and used substances within the last 3 months. Participants completed an interviewer-administered 1-hour questionnaire. Descrip

Amid the increase in fatal overdoses in Rhode Island (RI) over the past decade, understanding substance use and harm reduction practices is critical for informing prevention strategies. This work aimed to evaluate trends in substance use behaviors, overdose experiences, and harm reduction practices among people who use non-prescribed substances in RI. In a convenience sample of 673 participants from the 2021-2024 Harm Reduction Surveillance System (HRSS), the most reported substances used in the past 30 days were: alcohol (73%), crack cocaine (72%), cannabis (69%), cocaine (42%), and fentanyl/

Global smoking prevalence remains high, underscoring the need for strategies that complement conventional cessation. Tobacco harm reduction (THR) offers a pragmatic strategy by promoting substitution of combustible cigarettes with substantially less harmful alternatives. Oral nicotine pouches (ONPs) have emerged as a promising option. Modeled on the Scandinavian success of snus, ONPs deliver pharmaceutical-grade nicotine without tobacco leaf or combustion, eliminating thousands of toxicants and lowering exposure to carcinogens such as tobacco-specific nitrosamines to near-undetectable levels.

Harm reduction is a classic Public Health concept to refer to the reduction of the negative effect of drug use/abuse with a focus on justice and human rights, but the tobacco industry has been perverting this concept for years and using it as a tool for its own marketing. This publication details what real harm reduction action on tobacco use would be, when it should be implemented, and what pillars it should be based on. Different methods of reducing the harmful effects of tobacco and nicotine have been tried and tested over time, but the results have been poor; therefore, smoking cessation b

BACKGROUND: The rising rates of drug use-related complications call for a paradigm shift in the care for people who use drugs. While addiction treatment and harm reduction have historically been siloed in the US, co-location of these services in office-based addiction treatment (OBAT) settings offers a more realistic and patient-centered approach. We describe a quality improvement program on integrating harm reduction kits into an urban OBAT clinic.
METHODS: After engaging appropriate stakeholders and delivering clinician and staff trainings on safer use best practices, we developed a clinica

North America continues to witness escalating rates of opioid overdose deaths. Scale-up of existing and innovative life-saving services - such as overdose prevention sites (OPS) as well as sanctioned and unsanctioned supervised consumption sites - is urgently needed. Is there a place for critical theory-informed studies of harm reduction during times of drug policy failures and overdose crisis? There are different approaches to consider from the critical literature, such as those that, for example, interrogate the basic principles of harm reduction or those that critique the lack of pleasure i

In this commentary to the special issue on managed alcohol programs, the necessity of working from a harm reduction orientation when supporting people who experience homelessness is explored. While not all people who experience homelessness have substance use disorders, many respond to experiences of trauma and exclusion through the use of substances, and in many cases this leads to problematic use. In a context where people who are homeless regularly experience the control and regulation of their lives through emergency services, harm reduction approaches provide a welcomed alternative throug

The power and influence of healthcare systems comes largely from the ability to prescribe efficacious medicine. However, medicine can sometimes cause harm rather than bring benefits. Systematically checking patients for the adverse effects of medicines, as listed in manufacturers' literature, would protect patients from iatrogenic harm, but this is rarely undertaken. We argue for the benefits of this approach using the example of the prescription of antipsychotics to older adults. Prescribing antipsychotics to control challenging behaviours associated with dementia is a controversial matter, a

Advocacy by feminists and healthcare providers was essential in passing the 2012 bill legalizing abortion in Uruguay, which was primarily framed in terms of a public health imperative. Prior to legalization, a group of influential physicians had established a "harm reduction" approach consisting of pre- and postabortion counseling with a focus on safer abortion methods. The existence of a network of providers from this initiative facilitated the implementation of abortion services after legalization. Particularities of the Uruguayan model include a strong focus on medical abortion, and a multi
