INTRODUCTION: Trendelenburg position (TP) is used to transport gaseous emboli away from the cerebral region during cardiac surgery. However, TP effectiveness has not been fully considered when combined with varying the cardiopulmonary bypass (CPB) flow. This study simulated the supine and TP at different pump flows and assessed the trapped emboli and embolic load entering the aortic arch branch arteries (AABA).
METHODS: A computational fluid dynamics (CFD) approach used a centrally cannulated adult patient-specific aorta model replicating a CPB circuit. Air emboli of 0.1 mm, 0.5 mm, and 1.0 m

BACKGROUND: During caesarean section mothers can be in different positions. Theatre tables could be tilted laterally, upwards, downwards or flexed and wedges or cushions could be used. There is no consensus on the best positioning at present.
OBJECTIVES: We assessed all available data on positioning of the mother to determine if there is an ideal position during caesarean section that would improve outcomes.
SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (September 2009), PubMed (1966 to 14 September 2009) and manually searched the references of re

The altered vestibular signaling and somatosensory unloading of microgravity result in sensory reweighting and adaptation to conflicting sensory inputs. Aftereffects of these adaptive changes are evident postflight as impairments in behaviors such as balance and gait. Microgravity also induces fluid shifts toward the head and an upward shift of the brain within the skull; these changes are well-replicated in strict head-down tilt bed rest (HDBR), a spaceflight analog environment. Artificial gravity (AG) is a potential countermeasure to mitigate these effects of microgravity. A previous study d

Gastroesophageal reflux is a latent factor that may cause esophagitis, esophageal stenosis, and aspiration pneumonia through the regurgitation of the gastric fluid contents. For laparoscopic surgery, posture-changing and pneumoperitoneum operations are conducted to develop the visual field. However, few studies have examined the influence of these operations on gastroesophageal reflux. In this experiment using 10 Beagles, 10 mL of contrast medium was administered into the stomach, and the dogs were placed in the Trendelenburg position with 10-degree tilting. Pneumoperitoneum treatment with car

Background The cardiovascular system is strongly dependent on the gravitational environment. Gravitational changes cause mechanical fluid shifts and, in turn, autonomic effectors influence systemic circulation and cardiac control. We implemented a tilt paradigm to (1) investigate the acute hemodynamic response across a range of directions of the gravitational vector, and (2) to generate specific dose-response relationships of this gravitational dependency. Methods and Results Twelve male subjects were tilted from 45° head-up tilt to 45° head-down tilt in 15° increments, in both supine and pron

During orthostatic hypotension we evaluated whether presyncopal symptoms relate to a reduced brain oxygenation. Nine subjects performed 50 degrees head-up tilt for 1 h and eight subjects were followed during 2 h of supine rest and during 1 h of 10 degrees head-down tilt. Cerebral perfusion was assessed by transcranial Doppler determined middle cerebral artery blood velocity (MCA vmean), while brain blood oxygenation was assessed by near-infrared spectrophotometry determined concentration changes for oxygenated (delta HbO2) and deoxygenated haemoglobin and brain cell oxygenation by the oxidized

BACKGROUND AND OBJECTIVE: The Doppler-derived echocardiographic TEI-index, defined as the sum of the left ventricular isovolumic contraction and isovolumic relaxation times divided by ejection time, quantifies combined systolic and diastolic ventricular functions. The index has been proposed to be independent of arterial pressure and heart rate, implying a broad clinical usefulness. However, it is unclear whether the index is preload independent. We assessed whether and to what degree the TEI-index is altered by left ventricular loading conditions, and the feasibility of measurement by transoe

This review addresses tilt-testing methodology by searching the literature which reports timing of asystole and loss of consciousness (LOC). Despite the Italian protocol being the most widely adopted, its stipulations are not always followed to the letter of the European Society of Cardiology guidelines. The discrepancies permit reassessment of the incidence of asystole when tilt-down is early, impending syncope, compared with late, established LOC. Asystole is uncommon with early tilt down and diminishes with increasing age. However, if LOC is established as test-end, asystole is more common,

The effect of the Trendelenburg position on systemic and pulmonary hemodynamics in critically ill patients is not generally appreciated. This study examined the hemodynamoc effect of 15-20 degrees head-down tilt in 61 normotensive and 15 hypotensive patients with acute cardiac illness or sepsis. In normotensive patients, the head-down tilt increased the preload of both right and left ventricles, increased cardiac output slightly, decreased systemic vascular resistance, and did not change the mean arterial pressure. This effect was probably mediated by baroreceptor stimulation. In hypotensive p

STUDY DESIGN: Trial outcome.
OBJECTIVES: To demonstrate the relationship between circulation and breathing with orthostasis in a tetraplegic man.
SETTING: A long-term care unit at a Veterans Administration Hospital, USA.
METHOD: A tetraplegic patient with a history of orthostatic hypotension was monitored for blood flow at the level of the supraorbital artery, for breathing by nasal air flow, and for pulse oxymetry on a tilt table at 0, 45 and 0 degrees of head elevation.
RESULTS: Tilting up caused a coincidental reduction in blood flow and enhanced breathing. These effects were coincident

The NO-dependent mechanism of system hemodynamics reactions comprising increased system blood flow (cardiac output) and blood pressure to head-down tilting at 6, 12 and 18 degrees was studied in anaesthetized rats. Tilting of the rats with inhibited NO synthesis was shown to aggravate hypertension and elevate total peripheral resistance (TPR). Therefore, it can be concluded that NO-dependent vasodilation is involved in regulation of the arterial tone during tilting.

Due to anomalous anatomy, pyeloplasty with a horseshoe kidney (HSK) presents a technical challenge. Previously, we reported a technique of intraperitoneal laparoscopic surgery in Trendelenburg position (ILSTP) for pyelolithotomy, pyeloplasty, and heminephrectomy in patients with HSKs. To demonstrate the procedure, the case of a 29-year-old female with HSK and left hydronephrosis who underwent pyeloplasty using transmesenteric ILSTP is presented. Magnetic resonance confirmed HSK with left-sided hydronephrosis. After opening a window in the mesentery and dissecting the renal pelvis and left uret

Syncope is a very common problem during childhood presenting to emergency departments. It generates a great deal of anxiety among children and their parents because of the fear that syncope are at risk for sudden death. Neurocardiogenic (vaso-vagal) syncope are the most common, while cardiac type are rare. The vaso-vagal syncope are secondary to irregularities in an autonomic reflex resulting in an abnormal relaxation of the blood vessels. This is non-life threatening form of syncope in differ cardiac type that can be very dangerous. In cardiac syncope causes can be secondary to obstruction to

Changes in the gravitational vector by postural changes or weightlessness induce fluid shifts, impacting ocular hemodynamics and regional pressures. This investigation explores the impact of changes in the direction of the gravitational vector on intraocular pressure (IOP), mean arterial pressure at eye level (MAPeye), and ocular perfusion pressure (OPP), which is critical for ocular health. Thirteen subjects underwent 360° of tilt (including both prone and supine positions) at 15° increments. At each angle, steady-state IOP and MAPeye were measured, and OPP calculated as MAPeye - IOP. Patients with primary open angle glaucoma have decreased outflow through the trabecular meshwork of the eye, resulting in increased intraocular pressure (IOP). It is known that the Trendelenburg position causes increased IOP, but there are no current guidelines for monitoring and treating patients with glaucoma undergoing surgical procedures while in the Trendelenburg position. We describe a case of successful intraoperative management of increased IOP in a patient with glaucoma undergoing robotic radical prostatectomy while in steep Trendelenburg position.

OBJECTIVES: We propose a novel technique for peritonsillar abscess (PTA) drainage in which the patient is lying in the Trendelenburg position. We provide evidence that this novel technique is relatively safe and effective in PTA drainage.
METHODS: We queried otolaryngology training programs in regard to techniques of PTA drainage after receiving Institutional Review Board approval. Respondents were asked to rate their own level of success in draining PTAs, as well as the perceived satisfaction and comfort of the patient. These were rated on a scale of 1 (never successful or comfortable) to 5
