Sharing the burden of TB/HIV? Costs and financing of public-private partnerships for tuberculosis treatment in South Africa.

OBJECTIVE: To explore the economic costs and sources of financing for different public-private partnership (PPP) arrangements to tuberculosis (TB) provision involving both workplace and non-profit private providers in South Africa. The financing required for the different models from the perspective of the provincial TB programme, provider, and the patient are considered. METHOD: Two models of TB provider partnerships were evaluated, relative to sole public provision: public-private workplace (PWP) and public-private non-government (PNP). The cost analysis was undertaken from a societal persp

[A season of healthcare reforms?].

During the Covid-19 pandemic, the importance of having a National Health Service (NHS) became clear, and the commitment of healthcare workers was expressed with admiration and solidarity. However, contrary to expectations, in the years that followed, there was no provision to allocate a growing share of GDP to this sector, reform community medicine and the training and placement of general practitioners, or introduce financial and regulatory measures to benefit medical and nursing staff. The issue of health and the functioning of the NHS is now a major public concern. This factor, the need to

Trait and state biomarkers for psychiatric disorders: Importance of infrastructure to bridge the gap between basic and clinical research and industry.

To further improve clinical activities in psychiatry by early diagnosis and early intervention with novel mechanism-guided treatments, there is a great need for biomarkers that reflect 'trait' and 'state' in major mental disorders. Stable trait biomarkers would allow early diagnosis, prognosis, and hopefully early intervention in these disorders, while dynamic state markers that reflect symptomatic changes would help to develop treatments that target these molecular mechanisms. However, in the search for such biomarkers, and eventually translating them to the clinic and industry, challenges cu

Logic modelling as hermeneutic praxis: Bringing knowledge systems into view during comprehensive primary health care planning for homelessness in Australia.

Logic modelling is used widely in health promotion planning for complex health and social problems. It is often undertaken collaboratively with stakeholders across sectors that hold and enact different institutional approaches. We use hermeneutic philosophy to explore how knowledge is 'lived' by - and unfolds differently for - cross-sectoral stakeholders during comprehensive primary healthcare service planning. An Organisational Action Research partnership was established with a non-government organisation designing comprehensive primary health care for individuals experiencing homelessness in

What happens when public primary care is ill-prepared to respond to non-communicable diseases: a mixed-method study of diabetes and hypertension care in urban Nepal.

BACKGROUND: With rapid urbanisation in low- and middle-income countries, public primary care is struggling to respond to the growing prevalence of hypertension and diabetes, while the private sector has grown to meet demand. We aimed to understand current use and readiness of public and private primary care diabetes and hypertension services, and how readiness impacts perceptions and practices of urban communities. METHODS: We conducted a convergent mixed-methods study in Nepal, including mapping of diabetes and hypertension services in 134 public and private facilities; participatory social

Effects of pay-for-performance on prescription of hypertension drugs among public and private primary care providers in Sweden.

This study exploits policy reforms in Swedish primary care to examine the effect of pay-for-performance (P4P) on compliance with hypertension drug guidelines among public and private health care providers. Using provider-level outcome data for 2005-2013 from the Swedish Prescription Register, providers in regions using P4P were compared to providers in other regions in a difference-in-differences analysis. The results indicate that P4P improved guideline compliance regarding prescription of angiotensin converting enzyme inhibitors and angiotensin receptor blockers. The effect was mainly driven

The environmental footprint of health care: a global assessment.

BACKGROUND: Health-care services are necessary for sustaining and improving human wellbeing, yet they have an environmental footprint that contributes to environment-related threats to human health. Previous studies have quantified the carbon emissions resulting from health care at a global level. We aimed to provide a global assessment of the wide-ranging environmental impacts of this sector. METHODS: In this multiregional input-output analysis, we evaluated the contribution of health-care sectors in driving environmental damage that in turn puts human health at risk. What determines the costs of repair and rehabilitation of flexor tendon injuries in zone II? A multiple regression analysis of data from southern Sweden.

The epidemiology and costs of repair and rehabilitation of zone II flexor tendon injuries in 135 patients from the southern part of Sweden were analysed. The little finger was most frequently injured (43%), usually with a knife (46%), and 30% of the injuries were work related. Total median costs within the health-care sector for the injuries were SEK 48,500 (1 EURO=9.23 SEK, 4/1/2002). Costs in other sectors were SEK 93,000. Active mobilization or mobilization with rubber band traction increased costs within the health-care sector (SEK 7400 or SEK 6000, respectively) but improved range of move

The plight of chronic filarial lymphoedema patients in choice of health care and health care providers in Pondicherry, India.

Lymphatic filariasis has a wide spectrum of clinical manifestations with asymptomatic parasite carriers at one end and irreversible lymphoedema of extremities at the other. Irreversible lymphoedema of extremities is one of the disabling conditions that drive the affected patients to seek treatment from various systems of medicines and health care providers. This study attempts to map the care seeking pattern and behaviour of patients with chronic filarial lymphoedema of lower limbs in an urban area. Consecutive filarial lymphoedema patients from the VCRC filariasis clinic were recruited for th

Private Practitioners' Perspectives on Their Involvement With the Tuberculosis Control Programme in a Southern Indian State.

BACKGROUND: Public and private health sectors both play a crucial role in the health systems of low- and middle-income countries (LMICs). The tuberculosis (TB) control strategy in India encourages the public sector to actively partner with private practitioners (PPs) to improve the quality of front line service delivery. However, ensuring effective and sustainable involvement of PPs constitutes a major challenge. This paper reports the findings from an empirical study focusing on the perspectives and experiences of PPs towards their involvement in TB control programme in India. METHODS: The s

An introduction to North Carolina Integrated Care for Kids (NC InCK): A model to support whole-child health.

We describe the design of the North Carolina Integrated Care for Kids (NC InCK) model. NC InCK is one of seven nationwide CMMI-funded pediatric health care delivery models that integrate services to promote whole-child health. NC InCK was collaboratively designed by health care systems, the state Medicaid agency, Medicaid managed care organizations, child-serving organizations across multiple sectors, and families. The model uses three key approaches to integrate care: 1) a risk stratification algorithm using data across healthcare, education, and social systems to holistically understand need

[Privatizing Universalism in the Brazilian health system and the Now There Are Specialists Program].

This article reflects on the Now There Are Specialists Program (PATE, acronym in Portuguese) in light of the concept of Privatizing Universalism in the Brazilian health system. The program, presented by the Brazilian Ministry of Health, aims to expand its capacity to provide care and reduce patient waiting lists in the Brazilian Unified National Health System (SUS, acronym in Portuguese). Given the health policies introduced under the PATE, which involve health insurance plans and private hospitals, it is essential to analyze the Program based on the theoretical framework of Privatizing Univer

Demand-Resource Profiles and Job Satisfaction in the Healthcare Sector: A Person-Centered Examination Using Bayesian Informative Hypothesis Testing.

Work characteristics may independently and jointly affect well-being, so that whether job demands deplete or energize employees depends on the resources available in the job. However, contradictory results on their joint effects have emerged so far in the literature. We argue that these inconsistencies can be partially explained by two arguments in the contemporary literature in the field. First, most studies in the job design domain are based on classic variable-centered methodologies which, although informative, are not well suited to investigate complex patterns of interactions among multip
